Objective To evaluate the outcomes after implementing gestational diabetes mellitus(GDM) specialist practice model. Methods GDM specialist practice model contained four modules(first visit,follow-up visit,hospital-ization,postpartum),eight aspects of practice contents which included specialist evaluation,diet & exercise survey, diet & exercise prescription,GDM health guidance,weight management during pregnancy. Pregnancy outcomes were compared before and after GDM specialist practice. Results GDM specialist practice model was able to signifi-cantly decrease insulin utilization rate,reduce number of hospitalized patients due to poor blood sugar control,short-en length of hospitalization. Conclusion GDM specialist practice model can effectively save medical costs,and GDM specialist nurses play an important role in management of women with GDM.

Objective To explore feasibility of laparoscopic hepatectomy for hepatic hemangioma.Methods Twelve patients were treated by laparoscopic hepatectomy, including left lateral lobectomy in 5 cases and local liver resection in 7 cases. Three cases of hepatic hemangioma associated with gallbladder stone were performed cholecystectomy synchronously. Results Laparoscopic procedures were successfully performed in all 12 cases. The mean operative time was 155 min. The mean blood loss was 230 mL. The mean postoperative hospital stay was 8 days. The pospostoperative recovery was smooth except that 1 case had pulmonary infection. During a follow-up of 6-20 months for 12 cases,there were no recurrence. Conclusion Laparoscopic hepatectomy for hepatic hemangioma is safe and feasibile with good effect under the condition of strict indication selection and experienced surgeons operating.

In surgery operations, wound should be cleaned with warm sterilized saline solution. In order to reach rapidly warming the washing solution from the room temperature during the surgery, we designed a thermostatic medical infusion pump. The present paper mainly presents researches on the two temperature control methods in the standby mode and in the flushing mode of the system. In the standby mode, the traditional proportional-integral-derivative (PID) control algorithm was adopted. In the flushing mode, dynamic characteristics of the system was changed in real time, which made the thermostatic control process more complex, and the fitted control function combined with the PID control algorithm was adopted in this mode. The temperature control parameters were adjusted in real time according to the initial temperature and the flow rate of the washing solution to obtain a constant temperature of the washing solution, no matter how the initial temperature and the flow rate are changed. The experiment results showed that this kind of control system performed well with a high accuracy.

The angiotensinogen(AGT) expression and angiotensin Ⅱ (AngⅡ ) secretion levels in cultured SD rat mesangial cells were determined. High glucose up-regulated AGT mRNA(0. 29±0.07 vs 0. 20±0. 05,P< 0.05)and protein(0.66±0.23 vs 0.37±0. 15,P<0.05) expression and Ang Ⅱ secretion [(9.85±2.08 vs 7.50± 1. 51) pg/ml,P<0. 05]levels, which were down-regulated by pyrrolidine dithiocarbamate( PDTC) treatment via inhibiting NF-κB activity.

Objective　To Compare the anti hypertensive effects of felodipine and amlodipine on 24 hours blood pressure in patients with essential hypertensin. Method A Radomized， single blind and contro11ed study was perfo rmed and blood pressuree was measured by 24 hours ambulatory blood pressure monitoring. Results Felodipine and amlodipine signiflcantly reduced both s ysto1ic and diasto1ic casual b1ood preessure when compared with placebo and no differences were observed between the two drugs． Felodipine and amlodipine reduce 24h mean，daytime mean ambulatory blood pressure．The rapid rise in blood pressure i n the morning hours was well contro11ed by the two drugs. Conclusion:Felodipine and amlodipine used once daily could cont rol 24 h blood pressure effectively in patients of essential hypertension．

Objective:To study the function of transcription factor 4 (TCF4) and to prepare TCF4 polyclonal antibody.Methods:pET-41/TCF4 was transformed into E.coli BL21(DE3) and induced by IPTG.The purified GST-TCF4 fusion protein was applied to immunize rabbit to produce antiserum. The specificity of the affinity of purified anti-TCF4 antibody was examined by Western blotting analysis of the eukaryotic expressed products of TCF4. Dig-labeled probe and antibody against TCF4 were used to examine the expression of TCF4 in Saos-2 cells under mechanical stretch. Results:Western blotting showed that the antibody could bind to TCF4 specifically. The expression of TCF4 mRNA and protein were significantly increased in Saos-2 cells under mechanical stretch. Conclusion: TCF4 antibody has been prepared successfully.

OBJECTIVE:To establish a HPLC method for a simultaneous determination of moxifloxacin hydrochloride and ephedrine hydrochloride in compound moxifloxacin nasal drops.METHODS:The separation was performed on VP-ODS chromatographic column with column temperature at 30℃. The mobile phase consisted of phosphate buffer solution (which contained 0.02mol?L-1 sodium dihydrogen phosphate and 0.3% triethylamine, adjust pH to 3.0 by phosphoric acid) - methanol(60∶40) with flow rate at 1.0mL?min-1, detection wavelength at 214nm and sample size at 20?L. RESULTS: The linear ranges of moxifloxacin hydrochloride and ephedrine hydrochloride were 0.1～0.8(r=0.999 7,n=5) and 0.125～1.000?g (r=0.999 9, n=5), respectively,and their average recoveries were 98.94% (RSD=0.89%) and 99.67%(RSD=1.26%),respectively.CONCLUSION: The technique is simple, rapid, accurate, and reproducible, and it can be used as the quantity control of compound moxifloxacin nasal drops.

Objective:To analyze the effects of essential medicine system on revenue structures at primary med-ical institutions. Methods:Data on revenue status and structures from 58 township health centers in Shanxi Province was collected to analyze changes and trends. Results:Along with growing government investments in primary medical institutions, the essential medicine system has fundamentally realized full coverage. The percentage of essential medi-cine revenue to pharmaceutical revenue rose from 20 . 45% in 2009 to 97 . 03% in 2013 , though the percentage of pharmaceutical revenue to total revenue dropped by 18 . 43%. The percentage of fiscal subsidy revenue in total reve-nue rose from 25 . 77% in 2009 to 54 . 16% in 2013 , though the percentage of business revenue to total revenue dropped in general. Conclusions:The essential medicine system requires the government’s financial support. Essen-tial medicine lists and primary financial aid policies should be further improved;doctor reimbursement and incentive mechanisms should be improved as well and unreasonable doctor service pricing and reimbursement mechanisms should be reformed.

Objective To investigate the clinical efficacy and value of percutaneous transluminal angioplasty (PTA) in treating arteriosclerosis obliterans (ASO) of lower extremity that shows no visualization of its vascular outflow tract. Methods A total of 19 patients with ASO of lower extremities showing no outflow tract visualization were included in this study. The patients included 11 males and 8 females with a median age of 66 years (48-79 years. The main clinical symptom was rest pain of the leg in all patients; 10 cases had different degrees of foot ulcer and gangrene. Preoperative multi-slice CT angiography and DSA examination were performed in all patents. After PTA, the patients were followed up for 12 months, the changes of the clinical symptoms and signs were recorded. The claudication distance, toe skin temperature, ankle-brachial index (ABI), toe-brachial index (TBI), and CT angiography or color Doppler examination were separately performed before and one, 3, 6 and 12 months after PTA. Results In the 19 patients (38 diseased limbs in total), ASO with no visualization of vascular outflow tract was detected in 54 arteries, 3%), posterior tibial artery (n=18, 33.3%), peroneal artery (n=9, 16.7%) and popliteal artery (n=2, 3.7%). The technical success rate of limb PTA was 89.5%, while the technical success rate of single artery PTA was 85.2%. After PTA, the skin temperature of all the diseased limbs that had been successfully treated was obviously improved, the pain was significantly relieved or disappeared. One, 3, 6 and 12 months after PTA, the claudication distance, toe skin temperature, ABI and TBI were strikingly improved when compared with those determined before PTA (P<0.05). Twelve months after PTA the vascular restenosis rate was 39.5% (15/38), the limb vascular patency rate was 55.3% (21/38), and the limb salvage rate was 81.6%(31/38). Conclusion For the treatment of lower extremities arterial obliterans with obstructed outflow tract, PTA is safe and effective in short-term period. Although its long-term restenosis rate is higher, this technique can effectively control the progress of the disease, relieve the clinical symptoms, and help improve the limb salvage rate of ischemic limbs.
