1.Clinical analysis of serum amino acid in patients with liver cirrhosis and liver cancer

Objective To explore the change of serum amino acid serum in patients with liver cirrhosis and liver cancer, and its significance.Methods The serum amino acids of 46 patients with liver cirrhosis and 44 patients with liver cancer were examined with analyzer of Hitachi 835.Results In the case of patients with liver cirrhosis, the levels of glutamic acid, cystine, methionine and tyrosine in serum were significantly increased (P<0.01),while, the level of phenylalanine in serum was obviously increased (P<0.05). When it came to patients with liver cancer, the levels of serum threonine, tyrosine and phenylalanine were significantly increased (P<0.01), the level of serum serine was obviously higher (P<0.05). The level of BCAA/AAA (branched chain amino acids/aromatic amino acid) in the two groups was lower than that in the normal control group (P<0.01). The levels of serum valine and leucine were lower than those in the normal control group (P<0.05).Conclusion Analyzing the levels of serum amino acids has a certain significance for diagnosing and treating patients with liver cirrhosis and liver cancer.
2.Preparation of micro-biosensor and its application in monitoring in vivo change of dopamine.

The self-made high sensitivity and selectivity micro-biosensor was applied to monitor the change of dopamine in cerebral nucleus in rats in vivo. The micro-biosensor was prepared and used to detect dopamine level in vitro and monitor the dynamic change of dopamine in different cerebral nucleus in vivo. The results showed the lowest concentration of dopamine that could be detected by the biosensor was 32.5 nmol/L. Its positive peak was significantly different from that of AA, 5-HTP and E. The biosensor could keep working for monitoring the dopamine concentration in the cerebral tissue for more than 10 h. It was concluded that the microsensor has high sensitivity and selectivity to dopamine and can be used to dynamically monitor the change of dopamine in vivo.

3.Genetic Association Research between Mutation in CYP1A2 Gene and Erxian Soup Response

Objective To investigate the genetic association between mutation in cytochrome P450 1A2 (CYP1A2) gene and Erxian suop response. Methods Forty-eight patients with male climacteric syndrome of both Yin and Yang deficiency received a treatment of erxian suop with three months. The severity of symptoms and responses to Erxian suop were quantizated by the method of Department of Psychology in Istanbul Bosphorus University and Clinical Research Guidance Principle on New Drug of Traditional Chinese Medicine published by China. A single nucleotide polymorphism (SNP) at position 2964 (G-A) of CYP1A2 gene were identified by polymerase chain reaction (PCR) in order to analyse the relationship between Erxian response and CYP1A2 single nucleotide polymorphism. Results Statistical analysis showed there was no significant difference among genetype and genetype distribution of CYP1A2 G2964A between the patients and control group. The genotype of the 16 patients among 32 effective patients was G/G (56.25%). The genotype of the 11 patients among 16 ineffective patients was G/A (68.75%). There was no significant difference in symptoms, physical sign between the two groups at the pre-treat and post-treat. Also the two groups had no significant difference among the score of major symptoms and hyposymptoms. Conclusion The patients with genotype of G/G have higher efficiency, while with genotype of G/A have lower efficiency among patients with CYP1A2 G2964A gene when treated with erxian soup.
4.Effects of children's mentality on mental symptoms during recovery period of ketamine anesthesia

Objective To investigate the effects of children's mentality on mental symptoms during recovery period of ketamine anesthesia. Methods According to the mentality, forty-five children undergoing smalminor surgery were assigned into active group, stable group and fretful group with 15 cases each. Child was anaesthetized by ketamine alone. RP, BP, HR and SpO_2 were recorded at the end of operation, 5,10 and 30 min after operation. Meanwhile the expression, feeling, behavior, talking and activity were observed. Results RR, BP, HR in active group and fretful group were significantly higher than those in stable group. There was a significant difference in the expression, feeling, behavior, talking and activity among three groups. Conclusion Children's mentality has a significant effect on mental symptoms and hemodynamics during recovery period of ketamine anesthesia.
5.Anti-infection value of combined determination of serum levels of procalcitonin, C-reactive protein,prealbumin, white blook cell in the elderly with bacterial infection

Objective To evaluate the clinical significance of the combined determination of serum procalcitonin (PCT),C reactive protein(CRP),prealbumin(PA)and white blood cell(WBC) in curative effect and prognosis of elderly patients with bacterial infection.Methods Serum PCT,CRP,PA and WBC levels and their dynamic changes of 70 patients with bacterial infection from intensive care unit(ICU)were measured.The patients with bacterial infection were classified into serious infection group and local infection group.And the comparison with virus infection group was made.Results The levels of PCT(18.00±3.20)μg/L and CRP(86.17±10.64)mg/L in serious infection group and local infection group[PCT(2.03±0.46)μg/L,CRP(50.24±7.45)mg/L] were increased.The contents of PA were reduced[(3.214±0.88)mg/L,(12.89± 1.03)mg/L].The levels of WBC were increased mildly[(8.89±1.23) × 109/L,(6.54±0.87) × 109/L].The differences were statistically significant between bacterial infection groups(serious infection group and local infection group) and virus infection group [PCT (0.21 ±0.06)μg/L,CRP (5.21 ±0.84) mg/L,PA (20.14 ±2.57)mg/L,WBC (5.30 ± 0.93) × 109/L] (P< 0.01).Serum PCT,CRP.PA and WBC showed significant differences between serious infection group and local infection group(P<0.01).After antibiotic treatment,the levels of PCT (0.5 ± 0.08) μg/L,CRP (10.32 ± 1.65) mg/L and WBC (6.30± 0.91) × 109/L in bacterial infection group were reduced obviously and the concentration of PA (18.19±2.66)mg/L was elevated compared with before treatment [PCT(11.61±8.27)μg/L,CRP (71.80±20.09)mg/L,WBC(7.95± 1.59) × 109/L,PA(7.08±4.87)mg/L] (P<0.01).Correlative analysis was performed between PCT and others indicators before and after antibiotic treatment in bacterial infection group and virus infection group.There was positive correlation between PCT and CRP,WBC before(r=0.586,0.445) and after treatment (r=0.688,0.463) in bacterial infection group.PCT and WBC(r=0.432) were positively correlated after treatment,while no correlation existed between PCT and others indexes in virus infection group(r<0.306).When comparing sensitivity and specificity of PCT and CRP for diagnosis of bacterial infection disease,the sensitivity (95.7%) of PCT was similar to that of CRP.The specificity of PCT(86.7%) was remarkably higher than that of CRP(73.3%).Conclusions The combination of serum PCT,CRP.PA and WBC is valuable for early diagnosis of bacterial infection disease.Monitoring PCT level dynamically would assist an evaluation of curative effect and disease outcome of elderly patients.
6.Practice and lessons learnt from building the“surgeon qualification admittance system”

An introduction to the practice of introducing the“ surgeon qualification admittance system”in the hospital and a description of its management framework,basic principles,rules of classification for surgical operations,application and approval procedure,and supportive management regulations.The authors hold that this system has normalized behavior of medical activities,clarified operation authorities of medical doctors at various levels,and intensified the awareness of risk exposure.Therefore this system can safeguard the quality and safety of surgical operations and its efficiency to a great extent,in addition to better building the team and backup forces.
7.Relationship between Anemia,Iron Deficiency and Helicobacter Pylori Infection in Children

Objective To investigate the prevalence features of helicobacter pylori (Hp) infection,anemia and iron deficiency in a po-pulation of Wuhan children with 2 to 6 years old,and the relationship between Hp infection and anemia,iron deficiency in the children.Methods Randomly taking 95 children who had taken tests in our hospital's check-up centre in 2008 as the study objects,2 kinds of exa-mination were employed to detect Hp infection.Serum levels of Hp-IgG were measured by enzyme linked immunosorbent assay (ELISA) methods to evaluate past infection.The 14C urea breath test (14C-UBT) was conducted to obtain information of the presence of current/active Hp infection.In the morning 3 mL fasting venous blood was collected to determine the serum levels of Hp-IgG antibodies and ferritin.Hemoglobin values were determined with a hemoglobinometer.Serum levels of C-reactive protein (CRP) were tested in order to determine whether the children had evidence of current inflammation or infection.In addition,demographic information such as age and gender of the children and information about their use of antibiotics within the prior month were recorded.All cases were divided into 2 groups including the Hp infection group and non-Hp infection group according to laboratory examinations,then the Logistic regression was applied to analyze the relationship between Hp infection and anemia,iron deficiency.The Kappa identity test was taked to compare the 2 measures.Results Of the 95 children,18.9% were anemic and 36.8% were iron deficient.Forty percent of the cohort had Hp-IgG antibodies,74.4% tested positive by the UBT.Presence of Hp-IgG emerged as a significant risk factor for anemia,iron deficiency in adjusted analysis controlling for demographic factors,current inflammation,and antibiotic use.Conclusions Findings from different measure of Hp may reflect different stages of infection,with UBT results reflecting an earlier stage of infection,and presence of Hp-IgG reflecting established Hp infection associated with anemia,iron deficiency.
8.The Role of Androgen in High-Salt Induced Hypertension in Rats

Objective To study the effects of high salt intake on blood pressure and renin-angiotensin in male rats with different plasm androgen levels.Methods Thirty male Wistar rats were randomized to sham(n=10)or operated(n=10),or castration(n=10),or testosterone replacement after castarion(26.7 mg/kg,n=10)and fed with 8% NaCl for 8 weeks.Tall arterial pressure were recorded before,4 and 8 weeks after experiment.Serum PRA,plasma angiotensin Ⅱ(Ang Ⅱ)and testosterone(T)were determined by radioimmunoassey respectively. 9.Application of interleukin 18 in tuberculous pleurisy

Objective To investigate the expression of interleukin-18 in tuberculous pleurisy and its clinical significance by measuring the tuberculous pleurisy pleural effusion in patients with interleukin-18 levels.Methods The clinical data of 20 patients with primary diagnosis of tuberculous pleurisy and 20 patients with non-tuberculous pleurisy (including pneumonia,purulence,etc.) from December 2016 to June 2017 were retrospectively analyzed in the Department of Thoracic Surgery,Dongfang Hospital of Lianyungang City.Immunoassay was used to determine the level of interleukin-18 in pleural effusion of patients with tuberculous pleurisy and non-tuberculous pleurisy and adenosine deaminase by enzymatic method.Interleukin-18 levels and adenosine deaminase levels were compared between the two groups.Measurement data were expressed as ((x) ± s).The difference between the two groups using t test.Results The level of pleural effusion interleukin-18 in patients at admission was (520.5 ± 42.6) pg/ml and that in non-tuberculous pleurisy patients was (174.5 ± 12.7) pg/ml (P < 0.05).Tuberculous pleurisy decreased pleural effusion interleukin-18 levels to (425.5 ± 27.5) pg/ml,(368.4 ± 21.7) pg/ml,(208.9 ± 15.9) pg/ml.Interleukin-18 in pleural effusion was significantly lower than that at admission (P < 0.05) after 7 days of treatment.Conclusion Interleukin-18 production may be the cause of tuberculous pleurisy,may be used as an early diagnosis of inflammatory pleural effusion and observe the therapeutic effect of the immune indicators.
10.Development of the measurement method for MLC small field output factor in intensity modulated radiation therapy (IMRT)

Objective To develop the methods for using 0.015 cc pinpoint chambers, 0.007 cc miniature chambers and diode detector to measure Multi-leaf collimator (MLC) small field in IMRT.Methods MAX4000 and Unidos electrometers were connected with different types of small chambers and diode detectors.MLC shaped fields of10 cm×10 cm, 6 cm×6 cm, 4 cm×4 cm, 3 cm×3 cm, 2 cm× 2 cm were defined at 100 cm SSD.The field sizes for the Varian accelerator were defined by the tertiary MLC, while the secondary jaws were kept at 10 cm × 10 cm field, with the monitor units of 250 MU.Each field was measured three times to obtain the average value.The readings of all small fields were normalized to 10 cm × 10 cm field values for comparison of measured and published output factors.Results The relative deviations of the MLC small field output factors from the published outputs are 1.0% , 1.7% , 1.5% and 2.4%, respectively, for Unidos electrometer connected with 0.015 cc pinpoint chamber;0.2%, 0.8%, 0.8% and 1.4%, respectively, for Unidos electrometer connected with 0.007 cc miniature chamber;and 0.1%, 0.5%, 0.5% and 0.9%, respectively, for MAX4000 electrometer connected with 0.007 cc miniature chamber.Conclusions The 0.015 cc chamber-measured MLC output factors for 3 cm × 3 cm and 2 cm × 2 cm fields are excellent.As required by IAEA, the relative deviations of the measured output factor from the published output factor are within ± 2% for 2 cm × 2 cm fields and ± 3% for larger fields.The results measured using 0.007 cc chamber are better than those measured using 0.015 cc chamber.The measured results using the diode detector, normalized to the 10 cm × 10 cm field, are consistent with the minimum requirements and excellent when being normalized to the 4 cm × 4 cm field.For dosimetric consideration, MLC small field output factor should be measured using small chamber and diode detector.The method is accurate and reliable, therefore, all measured output factors for MLC small fields should be input into radiation treatment plan system.
