2.Expression and Significance of NF-?B and PKC in Human Bladder Carcinoma

Objective To study the correlation of NF-?B expression with the grade, the stage, the recurrence, the metastasis and PKC expression of bladder transitional cell carcinoma. Methods The expression of NF-?B and PKC was examined with SP immunohistochemical method in 54 cases of paraffin-embedded bladder transitional cell carcinomas tissues and 13 cases of normal bladder tissues. Results The positive expression rate of NF-?B P65 and PKC in bladder transitional cell carcinomas was 88 9% and 81 5% respectively. The expression rate of NF-?B decreased along with pathological grade and clinical stage increased, while its expression increased along with the rate of recurrence and metastasis elevated. There was a positive relationship between NF-?B and PKC expressions. Conclussion NF-?B may play a role at early stage of bladder transitional cell carcinoma. It probably serves as an index to evaluate the malignant extent and progression of bladder transitional cell carcinoma. PKC probably takes part in the activation process of NF-?B.
3.Combination of transcatheter arterial chemoembiolization and antitumor drugs in the treatment of Hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is a common world wide disease.Transcatheter arterial chemoembolization(TACE)has been the first choice for the unresectable midadvanced ones.However, TACE has its own limitation such as high recurrence rate and many complications, what's more, recurrence induced by neovascularization which is stimulated by activation of vascular endothelial growth factors promoted by ischemia and hypoxia in local tissues after the embolization always reduce the effect of treatment.And combination of TACE and anti-angiogenesis therapy is capable to make up the shortage of each other and reduce the recurrence rate, which has been a hot issue for non-surgical treatment of HCC.
4.Application of laparoscopy in acute abdominal emergency

Objective To evaluate clinical effects of laparoscopy in acute abdominal emergency. Methods A retrospective analysis was made on clinical date of 146 cases of acute abdominal emergency from January 2001 to August 2005,including 86 cases of acute cholecystitis,45 cases of acute appendicitis,6 cases of closed abdominal trauma,5 cases of perforation of gastroduodenal ulcer,3 cases of pyosalpinx,and 1 case of adhesive intestinal obstruction. Results Out of the 146 cases,134 cases(91.8%) were successfully diagnosed and treated under laparcoscope,including 7 cases which could not be confirmatively diagnosed preoperatively.Conversions to open surgery were required in 12 cases(8.2%).No complications or fatal cases were noted.(Conclusions Laparoscopic) technique is safe,reliable,and micro-invasive in the diagnosis and treatment of acute abdominal emergency.

6.Cognitive Function in Patients With Myasthenia Gravis

Objective: To study the cognitive function of patients with myasthenia gravis.Methods: Cognitive function of 35 patients with general myasthenia gravis and 31 patients with Eye-Muscles myasthenia gravis,and 35 control subjects was examined with cognitive measures of Recall of Auditory Verbal Learning Test,Symbol Digit Modalities Test,Trail Making Test,Logic Memory Test,Stroop Color Words Test(A、B 、C),Verbal fluency Test and Boston Naming Test.Results: MG patients performed worse than the control subjects on several cognitive measures.Conclusion: There is disorder of cognitive function in the patients with myasthenia gravis,revealing that there may exist deficiency in the central neuroal system of the patients with myasthenia gravis.
7.Bioartificial liver system for treatment of acute liver failure in canines and the effect on serum endotoxin

Objective To evaluate the effect of a bioartificial liver (BAL) system for treatment of acute liver failure (ALF) in canines and its relationship to serum endotoxin. Methods ALF was induced by end-side portocaval shunt combined with common bile duct ligation and transection. Ten ALF canines were distributed to BAL group (n=5), or to a control group (n=5). Each BAL circulation lasted 5h. Serum endotoxin, alanine aminotransferase (ALT) and total bilirubin (TB) before establishment of ALF model, pre-circulation and post-circulation were determined. Results Serum endotoxin level was 0.284 EU/ml, 0.526 EU/ml and 0.416 EU/ml before establishment of ALF model, and pre-circulation and post-circulation in BAL group, respectively. 8.Clinical observation and psychological intervention of early postoperative inhalation of ambroxol hydrochloride combined with intermittent inhalation of sputum for postoperative expectoration of lung cancer

Objective To investigate the effect of combined inhalation of ambroxol hydrochloride, intermittent tapping and sputum drainage and psychological intervention on postoperative lung cancer. Methods 68 cases of lung cancer patients treated in our hospital from January 2015 to March 2017 were selected, the control group received oxygen aerosol inhalation of ambroxol hydrochloride and intermittent beating back sputum at the early stage of the operation. The study group was treated with psychological intervention on the basis of the treatment of the control group. The postoperative pulmonary complications in two groups of patients with lung cancer were recorded. The data were input into SPSS statistical software and analyzed. Results The incidence rate of atelectasis and pulmonary infection in the study group (5.88%, 8.82%) were higher than those in the control group (The incidence of atelectasis was 32.35%, and the incidence of pulmonary infection was 38.24%) was significantly reduced, the difference was statistically significant (P<0.05). Conclusion On the basis of providing oxygen aerosol inhalation of ambroxol hydrochloride and intermittent beating back sputum for early postoperative lung cancer patients, the clinical prognosis can be significantly improved if psychological intervention is used.
9.Evidence-based study on rehabilitation program and empirical study of patients with anterior cruciate ligament reconstruction

Objective:To construct the rehabilitation strategy of anterior cruciate ligament reconstruction based on the evidenced method and evaluate the effectiveness in clinic.Methods:The rehabilitation strategy was firstly built based on evidenced method. And then 80 patients from Department of Sports Medicine and Adult Reconstructive Surgery, Nanjing University Medical School Affiliated Drum Tower Hospital from January to August, 2019 were randomly and equably sent to experiment group and control group. Patients in experiment group or control group were accepted the evidenced method or regular method as treatment strategy, respectively. The visual analogue scale (VAS), thigh circumference, motion range of knee joint and Lysholm score were recorded before and after surgery.Results:Six papers met the inclusion criterion and were been screen out from 1 535 papers, and then the evidenced rehabilitation strategy of anterior cruciate ligament reconstruction was made. The VAS scores in 12 and 24 h were 1(0-2), 1(1-3) in experiment group and 2(1-3), 3(1-3)in control group, the differences were significant between two groups( Z values were 4.955, 6.489, P<0.001). The motion ranges of knee joint in 2, 4, 6, 12 weeks after operation were (61.6±5.1)°, (85.3±6.6)°, (114.6±5.2)°, (127.6±3.4)° in experiment group and(54.9±4.6)°, (71.0±8.8)°, (105.1±5.9)°, (115.8±4.0) °in control group, the differences were significant between two groups( t values were 6.209-14.138, P<0.001). The Lysholm scores at 6,8,12 weeks after operation were 58.8±3.7, 71.1±3.1, 84.3±3.7 in experiment group and 56.0±3.4, 65.4±4.3, 75.6±3.0 in control group, the differences were significant between two groups( t values were 3.511, 6.867, 11.439, P<0.001). Conclusions:This evidenced rehabilitation strategy of anterior cruciate ligament reconstruction in this study was helpful to alleviating the pain, improving motion range of knee joint after anterior cruciate ligament reconstruction.
10.Orbital fat flap for treating sunken upper eyelid in middle-aged and elder patients.

OBJECTIVETo investigate the effect and its indication of orbital fat flap for treating sunken upper eyelid in middle-aged and elder patients.
METHODSThe upper middle orbital fat was transferred to the sunken area as a flat flap to correct Sunken upper eyelid at grade I, II, III. The depression depth was graded as I (<0.5 cm), as II (0.5-1.0 cm), as III ( >1.0 cm). From April 2011 to April 2012, 66 cases were treated and 49 cases were followed up for 12-15 months.
RESULTSAll patients with sunken upper eyelid were successfully treated using this procedure and no complications were encountered. Fat absorption was not observed. The effective rate was 90.9% (20/22) for grade I , 31.0% (18/58) for grade II and 0 for grade III (0/18), showing significant difference between any of two groups (P<0.05).
CONCLUSIONSThe orbital fat flap is effective for grade I Sunken upper eyelid with long- term effect.
