1.Transplantation of individualized cultured autologous melanocytes for the treatment of vitiligo

Objective To evaluate the therapeutic effect of transplantation of autologous melanocytes cultured with individualized medium in vitiligo. Methods Donor skin was obtained by suction blisters from a normally pigmented area of the abdomen of 155 patients with vitiligo. The roof of the blisters was clipped and digested with trypsin, then the suspension of epidermal cells and melanocytes were cultured in Hu16 medium.The cell division time (DOT) and melanin content of cultured melanocytes were measured followed by the adjustment of concentration of fetal calf serum, cytokines and cAMP elevating agents based on the DOT,melanin content and morphology of melanocytes for the individualized culture of melanocytes. After 2 - 5 passages, melanocytes were harvested and inoculated into ultrapluse CO2 laser-denuded lesions. All patients were followed up for at least 6 months. Results One hundred and fifty-five vitiligo patients with 204 lesions were treated with transplantation of autologous melanocytes. Of the 155 patients, 119 received 1 session of transplantation, 36 received 2 to 4 session of transplantation. Cells were expanded by 50 - 80 times in vitro after individualized culture. Repigmentation was more than 50% in 84.8% of these lesions, more than 90% in 52.94% of the lesions. A homogeneous skin color was obtained in repigmented skin, and no scarring or other side effects were observed. No influence was noted on the outcome of transplantation for sex, age, course of disease or lesion size of patients. Segmental vitiligo showed better response than vitiligo vulgaris: the effective rate and cure rate were 93.62% and 65.96% respectively for segmental vitiligo, 82.16% and 49.04% respectively for vitiligo vulgaris. Lesions located on the arms and legs (not including elbows and knees) showed the best response, with a cure rate of 73.08%, whereas acral sites were the most difficult area to repigment, with a cure rate of just 25.93%. Conclusions Individualized culture can significantly increase the success rate of melanocyte culture and expanding times of melanocytes. Transplantation of cultured autologous melanocytes is an effective modality deserving clinical application in the treatment of stable vitiligo, with the advantage of treating large depigmented area with melanocytes from a small donor site.
2.Relationship between the efficacy of autologous cultured melanocyte transplantation and serum levels of interleukin-17 and FoxP3 in patients with vitiligo

Objective To investigate the relationship between the efficacy of autologous cultured melanocyte transplantation and serum levels of interleukin-17 (IL-17) and FoxP3 in patients with vitiligo.Methods Forty patients with stable vitiligo vulgaris were included in this study,and received autologous cultured melanocyte transplantation.Six months after the transplantation,treatment efficacy was evaluated,and patients were classified into the successfully treated group (n =25) and unsuccessfully treated group (n =15).Peripheral blood was collected from all the patients before the transplantation,and enzyme-linked immunosorbent assay (ELISA) was performed to measure the serum levels of IL-17 and FoxP3.Statistical analysis was done using two independent samples t-test with the SPSS software (version 17.0).Results The successfully treated patients showed lower serum levels of IL-17 ((15.29 ± 7.86) vs.(43.88 ± 13.02) ng/L,P < 0.05),but higher serum levels of FoxP3 ((6.08 ± 2.03) vs.(3.37 ± 1.81) ng/L,P < 0.05) than the unsuccessfully treated patients.Conclusion The increased serum IL-17 and decreased serum FoxP3 may contribute to the failure of autologous cultured melanocyte transplantation in patients with vitiligo.
3.Effects of Fructus Ligustri Lucidi on the tyrosinase activity and melanogenesis of cultured human melanocytes

Objective To study the effects of tyrosol and oleanolic acid, two monomers of Fructus Ligustri Lucidi on the proliferation, tyrosinase activity and melanogenesis of cultured human melanocytes. Methods The cell proliferation was detected by MTT assay, the tyrosinase activity by dopa oxidase assay, the melanin content by NaOH - assay, and the mRNA level of tyrosinase ( TYR ) and tyrosinase related protein-1 (TRP-1) by RT-PCR. Results Incubation of tyrosol with cultured human melanocytes for 72 hours resulted in a significant stimulation of the tyrosinase activity and melanogenesis in a dose dependent manner. Compared with those in the cells without any treatment, the TYR and TRP-1 mRNA levels in the tyrosol -treated cells increased by 39.10% and 99.26%, respectively. 4.Analysis of disease control rate and its influencing factors in patients with progressive non-segmental vitiligo after combined treatment with compound betamethasone injection

Objective:To investigate the disease control rate and its influencing factors in patients with progressive non-segmental vitiligo after combined treatment with systemic compound betamethasone injection (CBI) .Methods:A retrospective analysis was conducted on the patients with progressive non-segmental vitiligo, who visited and were treated with CBI in the Department of Dermatology, Hangzhou Third People′s Hospital from October 2022 to April 2023. The disease control rate was analyzed after 3-month treatment. Effects of clinical factors such as disease onset characteristics, duration, disease condition and treatment methods on the disease control rate were analyzed. Chi-square test was used for comparisons of enumeration data between groups, and logistic regression analysis was conducted to analyze factors influencing the efficacy.Results:A total of 145 progressive non-segmental vitiligo patients treated with CBI were collected, including 56 males and 89 females, aged 14 - 67 (35.43 ± 11.54) years. Among the 18 patients having received an intramuscular injection of CBI, 10 (55.6%) obtained stable condition; among the 105 having received 2 injections of CBI, 60 (59.0%) obtained stable condition; among the 22 having received 3 or 4 injections of CBI, 12 (54.5%) obtained stable condition. The overall disease control rate after treatment was 57.9% (84 cases). The disease control rate was significantly higher in the patients with the lesional area < 1% body surface area (BSA) (42/47, 89.4%) than in those with the lesional area ≥ 1% BSA (42/98, 42.9%; P < 0.001), significantly higher in the patients with disease duration ≤ 2 years (32/41, 78.0%) than in those with disease duration > 2 years (52/104, 50.0%; P < 0.05), and significantly higher in the patients treated with CBI combined with phototherapy (33/44, 75.0%) than in those treated with CBI alone (21/44, 47.7%; χ2 = 6.90, P = 0.009), but significantly lower in the patients with special clinical markers (Koebner phenomenon, trichrome vitiligo, confetti-like depigmentation, inflammatory vitiligo, etc., 4/21, 19.9%) than in those without special clinical markers (80/124, 64.5%, P < 0.001). Among the patients with the lesional area ≥ 1% BSA and receiving 2 injections of CBI, the disease control rate was also significantly higher in the patients treated with CBI combined with phototherapy (21/36, 58.3%) than in those treated with CBI alone (12/37, 32.4%; χ2 = 4.94, P = 0.026). There was no significant difference in the disease control rate after the treatment between the patients with first-onset and reccurrent vitiligo, between those with and without predisposing factors, between those with and without family history, among those with different vitiligo disease activity scores, among those with different number of injections, as well as among those with different treatment intervals (all P > 0.05). Multivariate logistic regression analysis showed that the lesional area ( OR = 8.11, 95% CI: 2.74 - 24.04), disease duration ( OR = 0.26, 95% CI: 0.07 - 0.99), having or not having special clinical markers ( OR = 6.37, 95% CI: 1.72 - 23.57), and whether or not receiving combined phototherapy ( OR = 0.34, 95% CI: 0.15 - 0.77) were factors influencing the efficacy (all P < 0.05) . Conclusion:CBI may be suitable for the treatment of mild to moderate progressive vitiligo, especially for patients with lesional area < 1% BSA, while not for those with lesional area > 5% BSA, and combining phototherapy may improve the control rate of progressive vitiligo.
5.Impact of pre-pregnancy body mass index on baby's physical growth and nutritional status.

OBJECTIVE:
To explore the impact of pre-pregnancy body mass index on baby's physical growth and nutritional status.
METHODS:
A total of 491 pairs of mother-infant were divided into 3 groups according to mother's pre-pregnancy body mass index (BMI): a pre-pregnancy low BMI group (BMI<18.5 kg/m², n=93), a pre-pregnancy normal BMI group (18.5 kg/m² ≤ BMI<24.0 kg/m², n=326), and a pre-pregnancy high BMI group (BMI ≥ 24.0 kg/m², n=72). Analysis of variance of repeated measurement data and the median percentage methods were used to compare the physical growth and nutritional status of babies in different groups.
RESULTS:
Baby's weight in the high BMI group were higher than that in the normal BMI and the low BMI group (F=3.958, P=0.020). The incidence of malnutrition in the low BMI group showed a tendency to decline along with the months (χ²=5.611, P=0.018), the incidence of overweight and obesity in the high and the normal BMI groups displayed a tendency to decline along with the months (χ²=18.773, 53.248, all P<0.001). Baby in the low BMI group had higher incidence of malnutrition while baby in the high BMI group had higher incidence of overweight and obesity.
CONCLUSION
Pregnancy BMI was correlated with the growth of baby. Too high or too low prepregnancy BMI exerts harmful effect on baby's weight and nutritional status. Medical workers should strengthen the education on women's pre-pregnancy to remind them keeping BMI at normal level.

6.Bioinformatics analysis of differentially expressed miRNAs and their target genes in growth hormone-secreting pituitary adenoma

Objective:To explore the biological functions of differentially expressed miRNAs and their target genes, and their regulatory relations in growth hormone (GH)-secreting pituitary adenoma.Methods:Four databases (miRDB, miRwalk, Targetscan7.2 and Starbase) were used to predict the target genes of differentially expressed miRNAs in GH-secreting pituitary adenoma, and GO, KEGG and protein-protein interaction network (PPI) analyses were performed on the target genes. Meaningful hub target genes were screened out and compared with mRNA sequencing results from the database; and then, a visual miRNA-target gene interaction network was constructed.Results:Totally, 113 up-regulated miRNAs target genes and 128 down-regulated miRNAs target genes were obtained with the protein interaction degree≥20. Further intersection was conducted to obtain 13 hub target genes; these genes were mainly involved in ubiquitin mediated proteolysis pathway. The regulatory miRNAs of the hub target genes were miR-15b, miR-365, miR-32-3p, and miR-486-5p.Conclusion:A miRNAs-hub target gene mutual regulation network closely related to GH-secreting pituitary adenoma is constructed with bioinformatics analysis tools, and some potential proteins and pathways related to the pathogenesis and pathological process of GH-secreting pituitary adenoma are discovered.
