OBJECTIVETo review the literature from 1954-2005 on acupuncture-moxibustion treatment of digestive system diseases and objectively reflect the acupuncture-moxibustion spectrum of digestive system diseases by literature metrology and assessment, so as to better exert the advantages of acupuncture-moxibustion medicine.

METHODSA statistical analysis was made by the method of metrological analysis according to the data from "China modern acupuncture-moxibustion information databank".

RESULTS AND CONCLUSIONThe acupuncture-moxibustion spectrum of digestive system diseases of 91 kinds include 11 kinds of medical records; the effective diseases of acupuncture and moxibustion include hiccup, abdominal pain, diarrhea, constipation, vomiting, intestinal pseudo-obstruction, hemorrhoids. The determination of the disease spectrum of acupuncture and moxibustion needs further study.

OBJECTIVETo study the impact of Shangjuxu (ST 37) with acupuncture on each zang fu organ in human body so as to provide the objective evidence of the efficacy induced by acupuncture on gastrointestinal system and other organs.

METHODSShangjuxu (ST 37) was punctured bilaterally. The biological energy information medical diagnosis apparatus was applied to observe the changes in the energy in every part of human body before acupuncture and 10 min, 1 h and 24 h after acupuncture separately.

RESULTSAcupuncture on this point not only adjusted the digestive functions in esophagus, stomach, rectum and other parts, but also affected the functions of other organs, such as lung, uterus/prostate gland, thymus and spines, resulting in either rising or falling of the energy. The maximal peak value of the change happened 1 h after acupuncture.

CONCLUSION(1) Shangjuxu (ST 37) works mainly on rectum in the adjustment of large intestine functions and induces quite large impacts also on the energy changes of esophagus and stomach. (2) The energy changes in other organs, such as thymus, lung, reproductive organ and spine reveal that the actual indications of ST 37 may exceed to the currently-recognized range. (3) The peak value of the effect produced by acupuncture presents in 1 h after needling.

Photodynamic therapy (PDT) was first used for the treatment of esophageal cancer in early 1980s, Since then, numerous applications have been reported for its use in gastrointestinal tract including Barrett's esophagus, gastric, duodenal, biliary, pancreatic and colorectal lesions. PDT in gastroenterology has made tremendous progress over the last decade but its clear role is yet to be proved. Now, there is an increasing need for less invasive methods of treatment in patients with pre-malignant disease, early cancer or those who are unfit for surgery. It is one of a number of ablative techniques currently under investigation and appears to have a number of potential advantages over other forms of treatment in the alimentary tract. The development of newer potent, highly efficient photosensitizers, as well as endoscopic imaging techniques and light delivery systems, are continuing to expand the clinical uses of PDT. As data from additional clinical trials become available, we will gain a clearer perspective of where PDT fits in the treatment of cancers.

Adult stem cells are multipotent and self-renewing cells that contain several functions; i) migration and homing potential: stem cells can migrate to injured and inflamed tissues. ii) differentiation potential: stem cells which migrated to injured tissues can be differentiated into multiple cell types for repairing and regenerating the tissues. iii) immunomodulatory properties: stem cells, especially mesenchymal stem cells can suppress immune system such as inflammation. All those characteristics might be useful for the treatment of the digestive tract diseases which are complex and encompass a broad spectrum of different pathogenesis. Preclinical stem cell therapy showed some promising results, especially in liver failure, pancreatitis, sepsis, and inflammatory bowel disease. If we can understand more about the mechanism of stem cell action, stem cell therapy can become a promising alternative treatment for refractory digestive disease in the near future. In this review, we summarized current preclinical experiences in diseases of the digestive tract using stem cells.

Building the clinical therapeutic evaluation system by combing the evaluation given by doctors and patients can form a more comprehensive and objective evaluation system. A literature search on the practice of evidence-based evaluation was conducted in key biomedical databases, i.e. PubMed, Excerpt Medica Database, China Biology Medicine disc and China National Knowledge Infrastructure. However, no relevant study on the subjects of interest was identified. Therefore, drawing on the principles of narrative medicine and expert opinion from systems of Chinese medicine and Western medicine, we propose to develop and pilot-test a novel evidence-based medical record format that captures the perspectives of both patients and doctors in a clinical trial. Further, we seek to evaluate a strategic therapeutic approach that integrates the wisdom of Chinese medicine with the scientific basis of Western medicine in the treatment of digestive system disorders. Evaluation of therapeutic efficacy of remedies under the system of Chinese medicine is an imperative ongoing research. The present study intends to identify a novel approach to assess the synergistic benefits achievable from an integrated therapeutic approach combining Chinese and Western system of medicine to treat digestive system disorders.

Professor QIU Mao-Iiang is a famous Chinese physician of acupuncture and Chinese medicine in modern time and is one of representative figure of Chengijang acupuncture schools. The theory of "three benefits" of acupuncture for diseases of disgestive system is established according to pathogenesis. He holds that acupuncture is favorable in removing the stagnated liver qi, reinforcing the functions of the spleen and the stomach in treatment of gastroduodenal ulcer. For problems due to spleen deficiency, reinforcing method should be given to the main points, Back-shu points and Yuan-primary points, while reducing method should be given to Front-mu points. Deficiency type of chronic gastritis is much more seen than excessive type. Therefore, the treating principle of acupuncture shoujd be tonifying the spleen and dispeling the cold. Removing the liver qi and regulating qi circulation can be applied in phases. And for special cases, method of tonifying the stomach yin should be used. For gastroptosis, reinforcing qi in the middle jiao should be applied generally. For those with qi stagnation, qi in middle jiao should be regulated. For retention of food and dampness, treatment should focus on removing phlegm and dampness. And the following points can be selected to lift the stomach by long needle, Juque (CV 14), Liangmen (ST 21) and the acupoints around stomach. For treatment of diarrhea, toward the three pathological factors of dampness, heat and cold, treatment of removing dampness, clearing heat and dispelling cold, especially the first one should be applied, and others are selected by differentiation of syndromes. And for gallstone, treating principle should be established according to the theory of qi stagnation, disharmony between the gallbladder and the sto-mach as well as obstruction by substantial pathogens. And regulation of qi, relieving pain, regulating the functions of the gallbladder and the stomach should be applied so as to removing calculus. Professor QIU Mao-Iiang 's theory of "three benefits" of acupuncture for diseases of digestive system is formed on the analysis of the primary and secondary pathologies, the degree of symptoms, progress of the diseases, characteristics of the acupuncture techniques and aims of the treatment. Thus, his theory has great significance in the clinic.

The incidence of inflammatory bowel disease in China is rising on a yearly basis, followed by an increased number of patients who require surgery and those who suffer from maneuver postoperative complications. Surgical treatment is important in the management of patients with inflammatory bowel diseases. Management and prevention of postoperative complications is also a key factor to the success of surgical treatment if it is not more important than the surgical procedure itself. In this article, the most recent literatures in this field will be reviewed combined with our own clinical experiences. The types, risk factors, preoperative prevention strategies as well as postoperative management of surgical complications of patients with inflammatory bowel disease will be discussed.

Liver dysfunction by various reasons can cause metabolic disorders of a variety of nutrients and malnutrition, while malnutrition affects the recovery of liver function. Clinically patients undergoing gastrointestinal surgery are often accompanied with malnutrition, and these patients with liver dysfunction are easy to develop nutrient metabolic disorders. Therefore, improving the nutritional status of patients with liver dysfunction can improve the success rate of surgery and postoperative recovery. This article reviews the nutritional support in patients with liver dysfunction undergoing gastrointestinal surgery.
