1.Different courses of topical tacrolimus 0.1% ointment for facial corticosteroid-dependent dermatitis: a clinical observation

Objective To evaluate the clinical efficacy and safety of different courses of topical tacrolimus 0.1% ointment in facial corticosteroid-dependent dermatitis and to observe the rebound in patients after treatment with these regimens.Methods A total of 104 patients with facial corticosteroid-dependent dermatitis were randomly divided into 3 groups to be treated with topical tacrolimus 0.1% ointment twice daily for 4,8 and 16 weeks respectively.The patients were followed up every 2 weeks within the early 4 weeks of treatment and every 4 weeks thereafter.The rebound phenomena was observed in patients on week 4 after the withdrawal of tacrolimus.Results Finally,90 patients completed this trial,including 32 patients in the 4-week group,29 patients in the 8-week group and 29 patients in the 16-week group.No significant differences were observed between the 4-,8- and 16-week groups in the total reponse rate (75.00%,82.76%,86.21%,respectively,x2 =1.35,P > 0.05).The rebound rate in the 16-week group significantly differed from that in the 4- and 8-week group (20.69% vs.46.88% and 41.38%,both P< 0.05),while no statistical difference was noted between the 4- and 8-week groups.Local burning and itching were reported in 31.73% of these patients,and all of these irritant reactions occurred within the first week of treatment.Conclusions Topical tacrolimus 0.1% ointment is safe and effective for the treatment of facial corticosteroid-dependent dermatitis.The total response rate does not increase with the extended treatment course,and 4 weeks of treatment is enough for the marked and stable improvement of facial corticosteroid-dependent dermatitis,but the rebound rate is likely to be reduced by extended treatment course.
2.Study on Photo-protective Mechanisms of Hydroxychloroquine and Epigallocatechin Gallat on Keratinocytes Damaged from Ultraviolet-B Irradiation

Objective To investigate the photo-protective mechanisms of hydroxychloroquine and epigallocatechin gallate (EGCG) on HaCaT cells damaged from UVB irradiation. Methods Subconfluent HaCaT cells were irradiated with different doses of UVB irradiation and treated with the above listed agents. The mRNA expression levels of p53, p21, c-fos and GADPH genes were evaluated by reverse transcription-polymerase chain reaction (RT-PCR). Results UVB irradiation induced mRNA expression of p53, p21 and c-fos in cultured HaCaT cells, which were alleviated by hydroxychloroquine and EGCG treatment in UVB irradiation group. Conclusions The photo-protective effects of hydroxychloroquine and EGCG on HaCaT cells by UVB irradiation might be related to inhibition of the expression of p53,p21 and c-fos genes.
3.Research on the care informed standardization for stroke patients and its effect survey

Objective To research the care informed standardization based on the situation of incomprehensive content informed and low compliance satisfaction for stroke patients.Methods The questionnaire of stroke patients' care informed by self-designed in 2008 was using to survey 100 cases of stroke patients admitted to hospital as a control group,including the content,the way,the advice and satisfaction of this survey.The patients admitted from July 2009 to July 2011 were selected as experimental group and treated with standardized training of nurses and information and surveyed with their satisfaction for nursing.Results All the subjects were admitted in hospital primitively and the mean stay in hospital was more than 15 days.General conditions - age,gender and degree of educationwere not found differently beteeen the two groups ( P >0.05 ).Satisfaction degree was(3.22 ±0.65)in experimental group,and(2.81 ±0.73)in control group,respectively,the different was significant (t =4.18,P <0.01 ).In experimental group,the admitted nursing notification score was(22.70 + 2.23),therapy nursing notification was(21.86 ± 3.51 ),healthy instruction score was (21.66 ±4.22),discharged nursing information score was(23.20 ±3.40) ; in control group,those score were ( 15.23 ± 3.50),( 14.08 ± 4.64 ),( 13.42 ± 4.20) and ( 19.45 ± 5.70),respectively; the differences were significantly(t =18.00,13.37,13.85,5.65,respectively; P <0.01).Conclusions Research and implementation on the care informed normative for stroke patients can improve the quality of care,patient satisfaction and promote health education standards orderly and efficiently.
4.Construction and Clinical Application of the Novel Theory of'Stress-Based Breast Cancer Prevention & Treatment'

Stress is a non-specific adaptive response of organisms to internal and external stimuli,which helps maintain the homeostasis of the body's internal environment.However,when the stress response exceeds the body's adaptation threshold,a variety of diseases will be induced.The morbidity and mortality of breast cancer rank the highest among female malignant tumors.Professor LIN Yi,the first master of traditional Chinese medicine(TCM)in the field of breast diseases who founded the theory of'treating breast diseases from the perspective of six stagnations',believes that six stagnations are the core pathogenesis of breast cancer,and six stagnations are closely related to the body's stress response system.TCM has unique advantages in adjusting the balance of stress homeostasis.By reviewing the classical TCM theory,the theory of'treating breast diseases from the perspective of six depressions'and the modern stress theory,this paper put forward the theory of'stress-based breast cancer prevention & treatment',and expounded the pathogenesis features,pathogenic characteristics and molecular mechanism of stress factors in mediating the development and progression of breast cancer.Moreover,the core idea and treatment principle based on the regulation of stress homeostasis were proposed,and the prescription and medication guided by the theory of'stress-based breast cancer prevention & treatment'were summarized.The establishment of the theory of'stress-based breast cancer prevention & treatment'enriches the theoretical system of breast cancer in TCM,and is expected to provide a new approach for improving the clinical prognosis of breast cancer patients.
5.Application of the theories of materiomic release kinetics to the evaluation of the sustained release kinetics and synchronicity of yinqiaojiedu honeyed pills.

Yinqiaojiedu honeyed pills were equally divided into 1/4, 1/8, 1/12, and 1/16 parts. The materiomics release rates within 12 h of the intact Yinqiaojiedu honeyed pills and the divided granules were determined by the paddle method with a rotate speed at 100 r x min(-1), and the materiome was quantified by UV-scan and Kalman filter methods. The intact Yinqiaojiedu honeyed pills behaved typical sustained release profiles, while the well-divided portions also maintained a sustained release profile over 2-4 h. The release rates were well correlated with the extents for the divisions of the pills. The Weibull distribution parameters, Td and T50, were reduced in line with the particle size, indicating that the ways of administration of the pills may play a role in the in vivo pharmacokinetics of the pills. The visualization results showed obvious difference of materiomic release synchronicities between the intact pills and the equally divided particles, and the divisions enhanced the asynchronization. Therefore the novel theory of materiomic release/dissolution kinetics of traditional Chinese medicines (TCMs) quantitatively proved the traditional dosage form, namely, honeyed pills, as a prototype of the sustained-release dosage form with a visualization of the scientific connotation to the old saying in the classics of TCM, Pills, the moderate ones in action. In terms of materiome increase for each period of the release profiles, the materiomic release synchronicity was visually demonstrated. The novel theories provided methodological basis for the evaluation of traditional dosage forms and the design of the modern drug delivery systems for TCMs.

6.Correlation between serum anti-P53 and familial clustering of hepatocellular carcinoma in Guangxi.

OBJECTIVETo assess the correlation between familial clustering of hepatocellular carcinoma (HCC) and the level of anti-P53 in human serum in Guangxi.
METHODSEnzyme-linked immunosorbent assay (ELISA) was used to detect anti-P53 in 164 members from 20 HCC families and 164 members from non-cancer control families. Univariate analysis was performed to assess the correlation between seral level of P53 antibody and familial clustering of HCC.
RESULTSThe level of P53 antibody was significantly higher in the members of HCC families than controls (Z=-3.04, P=0.002). After eliminating the interference of hepatitis B virus infection, this tendency still remains (P=0.011). And there was a significant difference between relatives of different degrees from HCC families (chi-square=11.593, P=0.021), with the expression of anti-P53 declining along with decrease in relationship coefficient. Furthermore, the number of individuals with high anti-P53 expression was also significantly greater in HCC families (95/164) than controls (71/164) (P=0.006). And the expression was rising along with the increasing HCC numbers (chi-square=16.068, P=0.000). Anti-P53 level was also greater in HCC families featuring sibling affection than parental affection (chi-square=12.679, P=0.002). Univariate analysis indicated that high expression of anti-P53 is a risk factor for development of HCC (OR=2.087, 95%CI: 1.270-3.431).
CONCLUSIONHigh level of anti-P53 expression may be a factor for the clustering of HCC families in Guangxi, China.

7.Association of Visit-to-Visit Variability of Blood Pressure with Cardiovascular Disease among Type 2 Diabetes Mellitus Patients: A Cohort Study

BACKGROUND: Increasing evidence has shown that visit-to-visit variability (VVV) of blood pressure (BP) is associated with an increased risk of cardiovascular disease (CVD). The objective of this study was to evaluate the impact of VVV of systolic blood pressure (SBP) and diastolic blood pressure (DBP) on the risk of CVD among patients with type 2 diabetes mellitus (T2DM) in China. METHODS: We conducted a retrospective cohort study of 10,163 T2DM patients who were not previously diagnosed with CVD from January 2008 to December 2012 in Ningbo, China. The VVV of BP was calculated using five metrics, including standard deviation (SD), coefficient of variation (CV), variation independent of mean, average real variability, and successive variability (SV) of measurements, obtained over a 24-month measurement period. Hazard ratios and 95% confidence intervals (CIs) were estimated by Cox proportional hazards regression models for the associations of variability in BP with risk of CVD. RESULTS: A total of 894 CVD events were observed during a median follow-up of 49.5 months. The hazard ratio in the highest quintile of SD of SBP was 1.24 (95% CI, 1.01 to 1.52) compared with patients in the lowest quintile. The association between higher VVV of DBP and risk of CVD was not consistent across different metrics and sensitivity analyses. CONCLUSION: Higher VVV of SBP was associated with an increased risk of CVD, irrespective of the mean SBP level. Future studies are needed to confirm these findings.

8.Establishment and evaluation of pathologic model of CD1 mice infected with Streptococcus suis serotype 7

Objective To study the pathogenicity of Streptococcus suis type 7 in CD1 mice and to explore the feasibility of CD1 mice as an animal infection model of Streptococcus suis type 7. Methods CD1 mice were infected with Streptococcus suis type 7 isolated from Guangdong Province by intraperitoneal inoculation. The pathological model of CD1 mice was evaluated by the observation of clinical symptoms, pathological autopsy, histopathological examination of liver and brain tissue sections, determination of LD50, and identification of bacteria and detection of bacterial load in tissues and organs. Results The pathological incidence of infected CD1 mice showed good regularity and repeatability. The clinical symptoms, pathological changes and tissue damage of infected CD1 mice were basically consistent with the previous experimental results of Streptococcus suis type 2 and 9 infection in mice and were similar to the natural disease of pigs. Live bacteria could be isolated from blood, heart, liver, spleen, lung, kidney and brain of infected mice, and the recovered bacteria were consistent with the original bacteria. The detection of bacterial load in tissues and organs showed that the bacterial load in heart was the highest, followed by liver, kidney, spleen, lung and blood, and the bacterial load in brain was the lowest. Conclusion In this study, we demonstrate that Streptococcus suis type 7 strain can cause sepsis and brain inflammation in CD1 mice, resulting in tissue damage and death. CD1 mice are sensitive to the experimental strain and can be used as an animal model of streptococcus suis type 7 infection.
9.Application of ventilator cluster nursing for mechanically ventilated patients in Craniocerebral Department

Objective To study the effect of ventilator cluster nursing in mechanically ventilated patients in Craniocerebral Department.Methods Totals of 60 cases of mechanically ventilated patients admitted during January 2010 to July 2011 in Craniocerebral Department wards were the experimental group,which received ventilator cluster nursing,including raising bed 30-45 °,prevention of peptic ulcer,trying to stop sedatives everyday,prevention of deep vein thrombosis,oral care,attraction of subglottic secretions and hand hygiene.A total of 58 cases of mechanically ventilated patients from June 2008 to December 2009 in the same department were the control group,receiving conventional methods of nursing.Incidence,duration of mechanical ventilation and mortality of ventilator-associated pneumonia (VAP) in two groups were compared.Results The incidence,duration of mechanical ventilation and mortality of VAP were respectively 40.0%,(189.7 ± 64.5) h and 25.0% in the experimental group,72.4%,(331.3 ± 86.7) h and 51.7% in the control group,and the differences were statistically significant (x2/t =12.571,10.089,8.928,respectively; P <0.05).Conclusions Ventilator cluster nursing can reduce the incidence and mortality of VAP,as well as shorten the duration of mechanical ventilation in mechanically ventilated patients in Craniocerebral Department.
10.Effect of therapies for kidney-tonifying and blood-activating in treatment of anovulatory infertility in eugenics.

In the context of the new era, paying attention to maternal and child health and advocating prenatal and postnatal care can effectively improve the quality of the birth population. Traditional Chinese medicine has a long history of prenatal and postnatal healthcare with rich content, which is the theoretical basis of modern related services. With the social development and the improvement of people's awareness of prenatal and postnatal healthcare, people have gradually shifted the focus of prenatal and postnatal healthcare to the peri-pregnancy stage at present, namely that couples of childbearing age are guided to prepare for pregnancy under the premise of solving their basic diseases. Infertility is a common and refractory disease for women of childbearing age. Ovulation disorder is one of its common pathological mechanisms. Traditional Chinese medicine believes that kidney deficiency is the main cause and pa-thogenesis of anovulation infertility and blood stasis is an important factor throughout the disease course. In clinical practice, therapies for invigorating kidney and activating blood are safe and reliable to treat anovulatory infertility mainly by adjusting the hypothalamus-pituitary-ovarian axis, improving ovarian function, uterine environment and gamete quality and increasing endometrial volume. Under the guidance of the thought of prenatal and postnatal healthcare, the authors tried to explore the effect of therapies for kidney-tonifying and blood-activating in the treatment of anovulatory infertility in eugenics, with the purpose of providing ideas and basis for subsequent relevant clinical studies and contributing to prenatal and postnatal healthcare services.
