1.Can We Use the Posterior Condylar Off set as a Predictive Factor for Overhang of the Implant in Total Knee Arthroplasty?

PURPOSE: We tried to demonstrate whether the posterior condylar offset (PCO, the distance from the femoral diaphysis posterior cortex to the posterior condylar margin) and ratio (PCOR, dividing PCO by the maximum antero-posterior diameter of the distal femur) could be used as predictive factors for overhang of the implants or using gender implants in total knee arthroplasty (TKA). MATERIALS AND METHODS: One hundred and one women who underwent TKA using NexGen(R) (LPS) implants, were analyzed prospectively. After distal femoral resection, the mediolateral (ML) width was measured at four points (anterior, distal anterior, distal posterior and posterior) and compared with the ML width of the implant. The aspect ratio (AR, ML/AP ratio) and anterior/distal posterior ML width (Ant/DP) were calculated. Preoperative radiographic PCO and PCOR were measured. Differences of PCO, PCOR, AR and Ant/DP according to the size were analyzed and correlations between PCO, PCOR and AR were also analyzed. The patients were classified into two groups according to the presence of overhang, and differences of each parameter were compared between the two groups. RESULTS: The size of the implant was positively correlated PCO, not significantly correlated with PCOR, and negatively correlated with AR and Ant/DP. PCO and PCOR and AR showed no correlation with each other. PCO and PCOR were not significantly different between the two groups. However, AR and Ant/DP were statistically low in the group with overhang. CONCLUSIONS: Preoperative radiographic PCO or PCOR could not be used as a predictive factor for overhang of the implants or using gender implants in TKA.

2.Fibrous Dysplasia in the Epiphysis of the Distal Femur

Fibrous dysplasia is a common benign skeletal lesion that may involve a single bone or multiple bones. Although fibrous dysplasia can affect any bone, monostotic fibrous dysplasia of the long bone typically occurs in the diaphysis or metaphysis. We report a very rare case of monostotic fibrous dysplasia involving the epiphysis of the distal femur in a young man.

3.Malignant Osteoblastoma: A Case Report

Malignant osteoblastoma is very rare tumor which shows histological findings of an aggressive pattern and benign clinico-radiological features, and also called as aggressive or pseudomalignant osteoblstoma. The authors experienced one csse of malignant osteoblastoma involving diaphysis of left humerus in 22-year old man. The patient is healthy and maintains his normal activity without recurrence over five years after complete excision of cystic mass, curettage and bone graft.

4.Comparison of LC-DCP versus LCP for Internal Fixation of Humeral Shaft Fractures in Elderly Patient.

PURPOSE: To compare outcomes of humeral shaft fractures fixed with locking compression plate and those fixed with dynamic compression plate in elderly patients. MATERIALS AND METHODS: Nineteen consecutive elderly patients with a fracture of the humeral diaphysis were evaluated retrospectively. Ten patients had been fixed with LC-DCP, and nine had been fixed with LCP. Radiological and clinical results were compared and comparison of implants was done. RESULTS: Loosening of the plate occurred in one case each from the LCP group and the LC-DCP group. The rest of the patients achieved union uneventfully without any complications. Union rate, clinical score and hardware were not significantly different between the two groups. One patient who developed loosening in the LC DCP underwent reoperation whereas one patient with loosening in the LCP was successfully managed conservatively. CONCLUSION: Principle of fracture fixation was more important than plate selection in humeral shaft fracture of elderly patient.

5.An Anthropolgical Study on the Human Skeleton Excavated from Dugmoe Tomb of the Technopols of Kwang Ju City.

Authors have anthropologically measured the human skeleton from a Dugmoe Tomb of the technopolis of Kwang-ju city. The results obtained were as follows : 1. The human skeleton was thought to be constructed at the beginning of the period of the Chosun, judging from the Dugmoe Tomb. 2. It is female and its stature is estimated as about 163-165cm. The age of the skeleton is estimated to be late 50. 3. The cranial index is 77.09mm and the type is mesocephaly. 4. The cranial length-height index and the cranial breadth-height index were hypsicrane and acrocephaly. 5. The orbital index 95.0mm and the type is hypsiconch. 6. The humerus is comparatively long, slender and has more rounded diaphysis. 7. The femur is similar that of present day, and the type is platyer. 8. The abrasion of the occlusal surface of the teeth was 2.5 point.

6.Malignant Fibrous Histiocytoma in Spine.

Malignant fibrous histiocytoma is a rare malignant tumor of probably histiocytic origin. It is more common in soft tissue than bone. Usually it involves metaphysis or diaphysis of long bone. Primary spinal malignant fibrous histiocytoma is exceedingly rare. Authors report a case of primary malignant fibrous histiocytoma which showed rapid spinal cord compression in thoracic spine with review of literatures.

Atypical femoral fracture related to a long-term bisphosphonate therapy has commonly been reported; however, a fracture at the site other than the femur has rarely been reported to date. Herein, we report a case of a patient on long-term bisphosphonate therapy who presented atypical tibial insufficiency fracture at the anterolateral aspect of diaphysis, without trauma. We, for the first time in Korea, present this case with a literature review.

8.Biomechanical test study of rat femurs growing under different stress environment.

By creating two kinds of stress environment in the same animal model, we performed a three-point bending test and a compressing test on the rat femurs growing under different stress conditions to characterize the effect of stress on bone mechanical properties. The right hindlimbs were subjected to sciatic nerve resection to become cripple and were used as unloading group; the left hindlimbs bore excess load and made up the overloading group; the normal rats were used as control group. The animals were encouraged to exercise for half an hour everyday in the morning, noon and evening. The experiment observation finished in four weeks. The biomechanical parameters of femur diaphyses were measured. The experiment results showed that stress environment may change several mechanical parameters of rat femurs. This study indicated that bone tissues can adapt to its stress environment by changing its mechanical properties. The experimental model in this article is practical and reliable.

9.Responsible Factors for Femoral Shortening in Piglet Legg-Calve-Perthes Disease Models.

PURPOSE: To know the proportional contribution of causative factors to the femoral shortening in Legg-Calve-Perthes disease. MATERIALS AND METHODS: In twenty piglets, 4 to 5 weeks old, vascular supply to the capital femoral epiphysis was interrupted by ligating the femoral neck containing the epiphyseal artery. RESULTS: Mean femoral shortening in piglets sacrificed at 4 weeks after devascularization was 7.4 mm. In detail, the shortening was 2.5 mm (34%) in the epiphysis and 4.9 mm (66%) in the neck and diaphysis. Mean femoral shortening in piglets sacrificed at 8 weeks after operation was 12.1 mm. The distribution of shortening was 4.6 mm (38%) in the epiphysis and 7.5 mm (62%) in the neck and diaphysis. Mean femoral shortening in piglets sacrificed at 20 weeks after operation was 20.7 mm. The distribution of shortening was 4.0 mm (19%) in the epiphysis and 16.7 mm (81%) in the neck and diaphysis. CONCLUSION: We determined the proportional contribution to the residual shortening according to the causative factors.

10.Evaluation of bioactivity and osseointegration for ti-6al-4v alloy implant modified by anodic oxidation and cyclic precalcification treatments

The aim of this study was to examine the bioactivity and osseointegration of Ti-6Al-4V alloy implant which was modified by an anodic oxidation and a cyclic precalcification treatments. After blasting treatment using HAp (Hydroxyapatitie; HAp) powder which is resorbable blasting media (RBM) on the surface of Ti-6Al-4V alloy implants, the anodic oxidation treatment and the cyclic precalcification treatment were conducted to form nanotube TiO2 layer and HAp precipitation respectively. The surface morphology of the surface-treated Ti-6Al-4V alloy implant was investigated after immersion in the simulated body fluid(SBF) for 3 days to investigate the bioactivity. To investigate the effect of surface treatment on bonding between the implant and bone, RBM treated implant and RBM-anodization-cyclic precalcification(RACP) treated implant were placed on the distal side of both tibia diaphysis of rats, and then the removal torque of the implant was measured after 4 weeks. On the surface of RACP treated group, bone-like apatite precipitation was observed after immersion in SBF for 3 days. The removal torque was significantly higher in the RACP treated group than in the RBM treated group. The interfacial fracture between the implant and the new bone was observed in the RBM treated group, but both the cohesive fracture at the new bone and the interfacial fractures between the implant and the new bone were observed in the RACP treated group.
