1.A study on the differences in ERP during sexual arousal elicited by different types of sexual picture in adult men

Objective To observe the differences in event related potential(ERP) during sexual arousal elicited by different types of sexual picture in adult men.Methods An implicitly emotional task and a modified oddball paradigm were used.The differences were recorded in EEG with ERP when sexual arousal was induced by heterosexual couple erotic picture,male erotic picture and female erotic picture in fifteen health men.Results The amplitudes of N2 at Fz and P3 at Pz induced by male-female erotic picture ((2.32 ± 1.70) μV,(16.62 ± 2.16) μV),male erotic picture ((1.58 ± 1.33) μ V,(15.82 ± 1.64)μV) and female erotic picture ((1.23 ± 1.47)μV,(14.54 ± 2.00)μV)were not significant differences (P > 0.05).But the amplitudes of N2 and P3 elicited by neutral human picture((-6.01 ± 1.35)μV,(11.36 ± 1.81) μV) were smaller than each type erotic picture (P < 0.05).There was statistical significance in PSW at Cz elicited by different types of erotic picture.The PSW elicited by male-female erotic picture((12.59 ± 1.13)μV) was significantly higher than female erotic picture((10.38 ±0.76) μV) and male erotic picture((8.80 ±0.89) μV) (P<0.05).However,there was no significant difference in PSW component between male erotic picture and female erotic picture.Conclusion The level of sexual arousal elicited by explicit heterosexual couple erotic pictures is highest and the attention also maintains longest in heterosexual male.
2.Characteristics and causes of complications after internal fixation of femoral neck fractures based on OTA/AO classification

Objective:To investigate the characteristics of complications after internal fixation with hallo nails for femoral neck fractures according to OTA/AO classification and analyze the reasons for differences in complications of different classifications based on the current theories and lever-fulcrum balance theory.Methods:A retrospective case series analysis was made on clinical data of 113 patients with femoral neck fractures treated in Peking University People's Hospital from January 2014 to June 2020. There were 49 males and 64 females, with the age of 21-89 years [61(52, 72)years]. According to OTA/AO classification, there were 5 patients with type B1.1 fractures, 18 with type B1.2, 16 with type B1.3, 44 with type B2.1, 5 with type B2.2, 12 with type B2.3 and 13 with type B3. All patients were treated by percutaneous internal fixation with hallo nails. Complications were recorded during follow-up including femoral head necrosis, nonunion, screw back-sliding and screw cut-out. Causes of internal fixation complications were analyzed based on the lever-fulcrum balance theory, taking the location of fulcrum, screw stress (F1), force arm of pressure (L1), resistance of screw (F2) and resistance side (L2) into consideration.Results:All patients were followed up for 1.1-77.5 months [8.2(3.6, 16.6)months]. In all, 24 patients had complications, including femoral head necrosis in 7 patients, nonunion in 3, screw back-sliding in 12 and screw cut-out in 2. The rate of femoral head necrosis of type B1, B2 and B3 was 15%(6/39), 2% (1/61) and 0%, respectively. The rate of femoral head necrosis of type B1 was higher than type B2 ( P<0.05). The nonunion rate was 3%(1/39) in type B1, 3%(2/61) in type B2, and 0% in type B3 ( P>0.05). The rate of screw back-sliding of type B1, B2 and B3 was 3%(1/39), 2%(1/61) and 0%, respectively. The rate of screw back-sliding of type B2.3 and B3 were higher than that of type B1 ( P<0.05). A trend toward a higher rate of screw back-sliding of type B2.3 and B3 was found when compared to type B2.1/2.2, but there was no significant difference ( P>0.05). Moreover, the rate of screw back-sliding showed no significant difference between type B2.3 and B3 ( P>0.05). The rate of screw cut-out was 3%(1/39) in type B1, 2%(1/61) in type B2, and 0% in type B3, which showed no significant difference between type B1 and B2 ( P>0.05). According to the lever-fulcrum balance theory, the extent of pathologic fulcrum of type B1, B2 and B3 gradually increased, resulting in increased L1 and decreased L2. The L1 of type B1, B2 and B3 was (2.2±0.5)cm, (2.8±0.4)cm and (4.7±0.5)cm, respectively ( P<0.01). The L2 of type B1, B2 and B3 was (5.1±0.5)cm, (4.7±0.5)cm, and (3.6±0.4)cm, respectively ( P<0.01). Therefore, the F1 of type B1, B2 and B3 increased progressively. Conclusions:After internal fixation, the rate of femoral head necrosis of type B1 is significantly higher than that of type B2, and the rate of screw back-sliding of type B1 is significantly lower than that of type B3. The lever-fulcrum balance theory can deepen our knowledge of reasons for complications after operation for femoral neck fractures.
3.A study on differences of cognitive mechanism in processing different types of sexual picture in female university students

ObjectiveTo observe female university students' differences of cognitive mechanism in processing different types of sexual picture from International Affective Picture System (IAPS).MethodsUsing event related potential (ERP),the differences of the amplitudes of N2,P3 and PSW were examined induced by the neutral pictures and different types of erotic pictures.ResultsThe amplitudes of N2 ( (0.99 ±1.69 ) μV) and P3 ( ( 17.27 ± 1.45 ) μV) elicited by heterosexual couple erotic pictures were highest,followed by the male erotic pictures( ( -0.02 ± 1.83 ) μV,( 15.92 ± 1.26 ) μV),and then were the female erotic pictures ( ( -0.22 ± 1.56)μV,( 15.49 ±1.34) μV).There was statistical significance in N2 and P3 between heterosexual couple erotic pictures and female erotic pictures ( P <0.05 ).There was no significant difference in PSW component between heterosexual couple erotic pictures( ( 15.22 ± 1.98 ) μV) and female erotic pictures ( ( 14.53 ± 1.75 ) μV ),but the PSW induced by male erotic pictures (( 10.93 ± 2.60 )μV) was significantly smaller than the other sexual pictures (P<0.05).ConclusionThe level of sexual arousal elicited by explicit heterosexual couple erotic pictures is highest and also the attention maintains longest.
4.The roles of VEGF and C-myc in occurrence, development and metastasis of gallbladder carcinoma.

To investigate the relationship of the expression of vascular endothelial growth factor (VEGF) and C-myc with the occurrence, development and metastasis of gallbladder carcinoma, the expression levels of VEGF and C-myc in gallbladder carcinoma tissue (n = 30) and in normal gallbladder tissue (n = 20) were examined by immunochemistry. Results show that the positive rates of VEGF and C-myc in gallbladder carcinoma tissue were 80% and 63.3% respectively, and 45% and 25% respectively in normal gallbladder tissue. The positive rates of VEGF and C-myc were significantly higher in gallbladder carcinoma than in normal gallbladder tissue. The expression of VEGF and C-myc in gallbladder carcinoma related to the metastasis of gallbladder carcinoma. VEGF and C-myc play important roles in the occurrence, development and metastasis of gallbladder carcinoma.

5.The mediating effect of resilience between perceived stress and insomnia in medical staff

Objective:To explore the mediating effect of resilience between perceived stress and insomnia in medical staff.Methods:A questionnaire survey was conducted among 1 163 medical staff. All subjects were assessed with the insomnia severity index (ISI), simplified Chinese version of Connor-Davidson resilience scale (CD-RISC-10) and perceived stress scale (PSS-10). SPSS 20.0 software was used for common method biases test, descriptive statistics, difference analysis, correlation analysis, hierarchical regression analysis, and SPSS PROCESS V3.4 was used for Bootstrap mediating effect.Results:①The perceived stress (16.28±6.35) was positively correlated with insomnia (6.14±5.80)( r=0.404, P<0.01), and psychological resilience was negatively correlated with insomnia and perceived stress in medical staff ( r=-0.279, P<0.01, r=-0.399, P<0.01). ②Psychological resilience played a partial mediating role between perceived stress and insomnia in medical staff, with the mediating effect as 0.051, accounting of 13.82% of the total effect. Conclusion:Psychological resilience can reduce the negative effects of perceived stress on sleep and is a protective factor for insomnia in medical staff. Clinically, reducing perceived stress and improving psychological resilience can reduce insomnia in medical staff.
6.Correlation of immunoglobulin variable heavy chain gene mutation status with prognosis in patients with mantle cell lymphoma.

OBJECTIVETo study the relationship between immunoglobulin variable heavy chain (IgVH) gene mutation status and clinical features, pathologic findings and biologic behavior of mantle cell lymphoma (MCL).
METHODSIgVH gene was amplified in 60 cases of MCL with FR1-JH and FR2-JH primers in BIOMED-2. The sequence was determined by cloning. The IgVH somatic mutational status was analyzed using NCBI's Ig-Blast tool. The relationship between IgVH gene mutation status and clinicopathologic features was also analyzed.
RESULTSForty percent (24 cases, 28 functional Ig genes) of the MCL cases displayed somatically mutated VH genes (defined as > 2% mutated), whereas 60.0% (36 cases, 40 functional Ig genes) showed unmutated VH genes. The most widely used genes were VH3-21 (27.9%) and VH4-34 (19.1%). The former were mainly used by unmutated cases, while the later mainly by mutated cases.Intraclonal heterogeneity was noted in 19 cases. There was no correlation of VH mutation status and specific VH gene with survival (P > 0.05).
CONCLUSIONSMCL comprises at least two subsets that do not correlate with morphology: one with unmutated VH genes and one with mutated VH genes. The biased use of VH3-21 and VH4-34 is noted. The nonrandom usage of IgVH segments suggests specific antigens may play a role in the pathogenesis and progression of MCL subsets. There is no correlation of IgVH mutation status and specific VH gene with survival.

7. Clinical efficacy of ultrasound-guided subacromial drug injection in the treatment of subacromial impingement syndrome

Objective:
To evaluate the clinical effects of ultrasound-guided subacromial injection of two drugs in treatment of subacromial impingement syndrome.
Methods:
This was a randomized controlled trial study, prospective collection of patients′ data who were diagnosed as subacromial impingement syndrome at Orthopedic of Tianjin Fifth Central Hospital from January 2015 to August 2017. To ensure the randomness of the study, patients were randomized into two groups using a random number table. Pain(visual analogue scale(VAS)) and shoulder function (University of California Shoulder Score System(UCLA) and shoulder abduction) were performed before and after each injection.For continuous endings with multiple points in time, firstly, the normality, the homogeneity of variance, and the sphericity test of Mauchly were performed. After the condition was satisfied, the variance analysis of the two-factor repeated measurement data was performed.
Results:
Fifty-two patients completed the injection within 5 minutes and 41 patients got the final follow-up. Twenty-two patients in the corticosteroid group, 14 males and 8 females, aged (44.7±9.5) years old(range: 35-68 years old). The lesion involved 13 cases of superior shoulder, more than 1 time injection in 4 cases. There were 19 patients in the NSAID group, 7 males and 12 females, aged (37.2±10.1) years old(range: 27-63 years old), 10 cases with lesions involving superior shoulders, and 1 case with more than one injection. 8.Diffuse large B-cell lymphoma with aberrant expression of CD56: a clinicopathologic and immunohistochemical study.

OBJECTIVETo study the clinicopathologic features and significance of aberrant CD56 expression in diffuse large B-cell lymphoma (DLBCL).
METHODSThe clinical and pathologic profiles of 10 cases of DLBCL with aberrant expression of CD56 were investigated. Immunohistochemical staining, in-situ hybridization for Epstein-Barr virus encoded RNA and gene rearrangement for IgH and Igκ were carried out.
RESULTSThere were 6 male and 4 female patients. The medium age of patients was 46 years. All of them presented with extranodal lymphoma involvement, with gastrointestinal tract being the commonest site (5/10). Histologic examination showed that most of the atypical lymphoid cells were centroblast-like and demonstrated a diffuse growth pattern. Apoptosis and necrosis were identified in some cases. Immunohistochemical study showed that the tumor cells were positive for CD20 or CD79α and aberrantly expressed CD56. Five cases had the GCB phenotype while the remaining cases had the non-GCB phenotype, according to Hans classification. Bcl-6 was positive in most cases (9/10). All cases showed a high proliferation index by Ki-67. The tumor cells were negative for CD3ε, CD138 and granzyme B. In-situ hybridization for Epstein-Barr virus encoded RNA was performed in 7 cases and none of them showed positive signals. IgH gene rearranged bands were detected in 4 cases (4/6) and Igκ was detected in 3 cases (3/6). Follow-up data were available in 8 patients. Two patients died of disease progression within 5 to 13 months after diagnosis and the other 6 patients were alive 8 to 60 months after therapy.
CONCLUSIONSDLBCL with aberrant expression of CD56 is rare. Most of them present with extranodal involvement, show high frequency of bcl-6 expression and high proliferation index. The patients often have good response to chemotherapy.

9.Clinicopathologic features of mantle cell lymphoma and the significance of cyclin D1 in diagnosis.

OBJECTIVETo investigate the clinicopathologic features of mantle cell lymphoma (MCL) and the significance of immunostaining for cyclin D1 in diagnosis.
METHODSClinicopathologic observation and immunohistochemical staining for CD20, CD45RO, cyclin D1, bcl-2, Ki-67, CD5 for 8 cases of mantle cell lymphoma were performed.
RESULTSThe 8 cases of mantle cell lymphoma consisted of 6 males and 2 females, aged from 43 to 78 years (mean 57 years). Histopathologically, MCL demonstrated architectural destruction by a vaguely nodular monomorphic lymphoid proliferation with vaguely nodular, diffuse or mantle zone growth patterns. Analogous to centrocytes, the lymphoma cells with slightly to markedly irregular nuclear contours showed moderately dispersed chromatin and a low mitotic figure. Three cases were transformed into highly aggressive blastoid variants. The tumor cells were positive for CD20, CD5, bcl-2 and cyclinD1 in all 8 cases and negative for CD45RO.
CONCLUSIONSThe clinicopathological features and special immunophenotypes were present in mantle cell lymphoma. This tumor can be differentiated from other small B-cell lymphomas on the basis of histopathologic features and positive cyclin D1 immunophenotype. The blastoid variant should also be differentiated from other variants.

10.Research progress in pathogenesis of neurologic heterotopic ossification

Neurologic heterotopic ossification (NHO) is a pathological phenomenon that involves the formation of ectopic bone in soft tissues, which is associated with central nervous system injury. NHO may cause severe complications such as disorder of limb's activity, severe pain, and skin ulcer. The pathogenesis of NHO is not clear. In the past decades, there's better understanding of NHO pathogenesis, and potential predictive and therapeutic targets were found. The authors overview the NHO pathogenesis based on osteogenic factors, osteogenic precursor cells and microenvironments, soas to provide references for clinical diagnosis and treatment.
