1.Diagnosis and treatment strategy of periodontitis with diabetes.

The number of diabetic patients visiting stomatology for periodontal disease is increasing, and the symptoms are relatively severe, and often complications increase the complexity of periodontal treatment. This article briefly describes the research progress and clinical manifestations of the epidemiology and related pathological mechanisms of periodontitis with diabetes, focusing on the treatment and providing reference for stomatologists in the clinical diagnosis and treatment of patients with diabetic periodontitis.

2.Angiotensin-converting enzyme inhibitors from plants: A review of their diversity, modes of action, prospects, and concerns in the management of diabetes-centric complications.

Angiotensin-converting enzyme (ACE) inhibitors are antihypertensive medications often used in the treatment of diabetes-related complications. Synthetic ACE inhibitors are known to cause serious side effects like hypotension, renal insufficiency, and hyperkalaemia. Therefore, there has been an intensifying search for natural ACE inhibitors. Many plants or plant-based extracts are known to possess ACE-inhibitory activity. In this review, articles focusing on the natural ACE inhibitors extracted from plants were retrieved from databases like Google Scholar, PubMed, Scopus, and Web of Science. We have found more than 50 plant species with ACE-inhibitory activity. Among them, Angelica keiskei, Momordica charantia, Muntingia calabura, Prunus domestica, and Peperomia pellucida were the most potent, showing comparatively lower half-maximal inhibitory concentration values. Among the bioactive metabolites, peptides (e.g., Tyr-Glu-Pro, Met-Arg-Trp, and Gln-Phe-Tyr-Ala-Val), phenolics (e.g., cyanidin-3-O-sambubioside and delphinidin-3-O-sambubioside), flavonoids ([-]-epicatechin, astilbin, and eupatorin), terpenoids (ursolic acid and oleanolic acid) and alkaloids (berberine and harmaline) isolated from several plant and fungus species were found to possess significant ACE-inhibitory activity. These were also known to possess promising antioxidant, antidiabetic, antihyperlipidemic and anti-inflammatory activities. Considering the minimal side effects and lower toxicity of herbal compounds, development of antihypertensive drugs from these plant extracts or phytocompounds for the treatment of diabetes-associated complications is an important endeavour. This review, therefore, focuses on the ACE inhibitors extracted from different plant sources, their possible mechanisms of action, present status, and any safety concerns.

3.Analysis of Compatibility Laws for Acupoint Selection of Acupuncture in Treating Diabetic Gastroparasis.

OBJECTIVETo summarize and analyze the compatibility laws for acupoint selection of acupuncture in treating diabetic gastroparasis (DGP) in modern literatures of clinical researches.
METHODSRetrieved were literatures related to treating DGP by acupuncture or acupuncture combined other therapies from PubMed, CNKI, and WF from 1982 to 2014. Analyzed were frequency of acupoint use, meridians selected acupoints belonged to, regions selected, association laws of selected compatible acupoints.
RESULTSRetrieved were 35 with compatibility frequencies more than 15 listed as follows: compatibility frequency of Zusanli (ST36) and Zhongwan (CV12) was 33 (94.29%); compatibility frequency of Zusanli (ST36) and Neiguan (PC6) was 23 (65.71%), compatibility frequency of Zhongwan (CV12) and Neiguan (PC6) was 22 (62.86%), compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Neiguan (PC6) was 22 (62.86%); compatibility frequency of Zusanli (ST36) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zhongwan (CV12) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Sanyinjiao (SP6) was 15 (42.86%). Meridians selected acupoints belonged to were sequenced as Foot Yangming Stomach channel, Ren channel, Foot Taiyang Bladder channel, and so on. Acupoints selected were mainly in lower limbs, chest and abdomen, waist and back. The compatibility of Zusanli (ST36) and Zhongwan (CV12) was the most often used with the highest frequency.
CONCLUSIONSThe compatibility laws for acupoint selection of acupuncture in treating DGP were mainly dominated as upper-lower selection, three regions selection, local selection, anterior-posterior selection. The compatibility laws for acupoint selection of acupuncture along meridians were mainly dominated as the convergence points and exterior-interior meridian points.

7.The Present State of Diabetes Mellitus in Korea.

Diabetes mellitus has approached to us as a social epidemic in Korea. The prevalence of diabetes in Korea has increased five to six-fold from 1.5% to 7~9% during the past 30 years. This increasing rate is remarkably high in comparison with those of developed countries such as the USA- doubled during the past 30 years. In addition, diabetic complication is very common in Korean diabetic patients. For example, a total of 70.5% among new patients who were started with renal replacement therapy had diabetes mellitus. Furthermore, diabetes-related mortality has rapidly increased over the last 20 years. The age-sex adjusted mortality rate of diabetic patients was about three-fold higher than those of general population. As a result, medical cost of diabetes mellitus covered by the national health insurance corporation was 3.2 trillion won and accounted for 19.2% of all medical costs. On the other hand, the rate of awareness and treatment in diabetic patients has improved from 1998 to 2005. However, the proportion of adequate glycemic control (HbA1c < 7%) among the treated diabetic patients was only 40%. The comprehensive and integrated health intervention including public approach is urgently needed to control the increasing prevalence of diabetes mellitus and its related undesirable outcomes.

8.Pharmacotherapy of Diabetes Focused on Stroke

Cardiovascular disease, including stroke, is one of the major causes of death in diabetes. Numerous studies have long suggested reducing macrovascular complication such as ischemic vascular disease through intensive glycemic control, but none was successful proving the effect of glycemic control. Recently, new possibilities in cardiovascular disease reduction have been proposed through cardiovascular safety trials of newly developed anti-hyperglycemic agents. The purpose of this review is to introduce the traditional and newly developed anti-diabetic medications and review their effects regarding cardiovascular outcomes mainly focusing on stroke.

9.A Health Education Program for Home Emergency Management of Acute Complications of Diabetes in the Elderly.

Objective To establish a health education program for home emergency management of acute complications of diabetes in the elderly.Methods The program was drafted by literature review and panel discussion.The final draft was formed after two rounds of correspondence from 13 experts.Results The recovery rate of the two rounds of expert correspondence was 100%,and the expert authority coefficient was 0.98.The Kendall's harmony coefficients of the two rounds of correspondence were 0.263 and 0.212 respectively(both P<0.001).The established health education program included indicators of three categories:early stage of acute complications of diabetes at home(understanding the inducing factors),emergency warning(quick and early identification in case of emergency),and emergency treatment at home.Conclusion The contents of the health education program are systematic and reliable and meet the needs of health education for home emergency management of the elderly with diabetes.

10.Observation on therapeutic effect of turtle probing the cave needling method on diabetic gastroparesis.

OBJECTIVETo compare therapeutic effects of acupuncture therapy and Motilium on diabetic gastroparesis.
METHODSSixty cases were randomly divided into a needling method group and a medication group, 30 cases in each group. The needling method group were treated with turtle probing the cave (a kind of needling manipulation) as main, with pair-acupoints, Zhongwan (CV 12) and Zusanli (ST 36), Neiguan (PC 6) and Sanyinjiao (SP 6) selected, once a day, two weeks constituting one course. The medication group were treated with oral administration of 10 mg Motilium, thrice each day, 30 minutes before meals, two weeks constituting one course. The therapeutic effects were observed after 2 courses.
RESULTSThe total effective rate was 93.3% in the needling method group and 73.3% in the medication group, with a significant difference between the two groups (P<0.05). The needling method group in improvement of epigastric distention and pain, eructation, nausea and vomiting was better than the medication group.
CONCLUSIONThe therapeutic effect of turtle probing the cave needling method on diabetic gastroparesis is significantly better than that of Mutilium.
