Objective To study the effect and mechanism of a selective aldosterone receptor blocker, eplerenone (EP), on renovascular hypertensive rats. Method Forty-eight Wistar rats were divided into six groups: sham operation group, control model group, early EP group, late EP group, early losartan (LO) group and late LO group. Systolic blood pressure (SBP) and urinary protein excretion (UPE) were measured before and every two weeks after operation. Histopathology and endothelial cell nitric oxide synthase (ecNOS) gene expression were examined in heart, aorta and kidney. Results SBP obviously increased in control model group as compared to sham group at week 10(132. 1 ?2.4 vs. 115.0?0.6 mmHg, P = 0. 0191), while early treatment with LO or EP could decrease SBP significantly as compared to control group. ObjectiveTo investigate the effects of budesonide inhaled on the levels of interleukin-8 (IL-8), C-reactive protein (CRP) and procalcitonin (PCT) in bronchoalveolar lavage fluid (BALF) of patients with AECOPD treated by using inhalation of budesonide in different doses or by using injection of dexamethasone. MethodsNinety AECOPD patients with mechanical ventilation in ICU ward were enrolled from Jan. 2008 through Sep. 2010. All patients were selected in this study as per the criteria of AECOPD set by the Chinese Medical Association. Ninety AECOPD patients treated with routine therapy were randomly (random number) divided into 3 groups. The patients of group A were given budesonide 2 mg inhaled 1 time/d. The patients of group B was given budesonide 4 mg inhaled 1 time/d. The patients of group C had dexamethasone 2. 5 mg injected 1 time/12h. The changes of IL-8, PCT and CRP in both BALF and serum were respectively detected in three groups at the beginning of treatment, and 3 days and 7days after treatment. The detected data of variables were analyzed by SPSS 13.0 package. ResultsThe level of IL-8 in BALF declined in 3days and 7days after treatment, and the magnitude of decrease in the following order:Group B ( budesonide 4mg/d) ＞ Group A ( budesonide 2 mg/d) ＞ Group C ( intravenous dexamethasone)with significant differences among them ( P ＜ 0. 05 ). The decrease in IL-8 in serum showed the similar trend, but there were no statistical differences among them. The CRP and PCT in both BALF and serum had no significant changes. There was a correlation between the concentration of IL-8 in BALF and the duration of mechanical ventilation.Conclusions The treatment with the inhalation of budesonide could more significantly reduced the IL-8 level in BALF compared with intravenous dexamethasone, whereas the CRP and PCT were not changed. The dynamic changes of IL-8 in BALF might be used as an indicator of prognosis in AECOPD patients with mechanical ventilation.

Objective:To investigate the effects of IL-18BP blockade on apoptosis of synovial cell in IL-18 induced Collagen-induced-arthritis(CIA) in mice and its possible mechanism.Methods: CIA mice were established by freund′s complete adjuvant,and divided into 5 groups:normal group,CIA model group,IL-18-CIA group,MTX group and IL-18BP group,each for 10,total 50 mice.The expression of Fas and FasL in the synovial tissue of the left posterior ankle joint taken from each group were detected by immunohistochemistry analysis.Results: IL-18BP group compared with the model group,the mice of joint swelling was inhibited,the expression level of Fas and FasL increased significantly,the difference was statistically significant (P<0.01);IL-18BP group has a significant curative effect and basically recovered to normal level(P<0.05).Conclusion: IL-18BP can promote the apoptosis of synoviocytes by regulating the expression of apoptosis genes Fas and FasL,which is helpful for the treatment of rheumatoid arthritis.

The immune system is a complex system involving multiple organs,and it is vulnerable to age,gender,environment and other factors.For a variation normal physiological range,it is a great challenge to evaluate drug-induced immunotoxicity in preclinical safety study.Histomorphologic assessment of the immune system is a recognized cornerstone in the identification of immunotoxicity at present.In this paper,the principles of pathological evaluation for immune system,and pathological evaluation for important immune organs including thymus,spleen,lymph nodes are discussed briefly,so that it is intended to assist toxicity pathologists in the accurate and consistent characterization of intended and unintended drug-induced alterations of the immune system.

Objective To retrospectively study pathological characteristics of 1561 patients with colorectal cancer in Nanchang area.Methods Analysis was made regarding the results of colonoscopy and pathological examination in patients from 1990 to 2004.Results Totally 21 853 patients received colonoscopy during the period,and 1561 patients were diagnosed as colorectal cancer.The overall diag- nostic yield was 7.1%.The average age diagnosed was 52 years old.Ratio of male to female was 1.4: 1.The lesion located in rectum accounted for 43.4%.The percentage of cancer in left colon was higher than that in right side (30.5% vs 26.1%,P＜0.05).The proportion (57.5%) of patients with rectal can-cer under 30 years old was the highest in all patients with rectal cancer.Cancer located in sigmoid colon and left side of the colon increased with age.Well and moderately differentiated adenocarcinoma was observed in 83.6% of the patients.Comparing the clinical characteristics of every five years,increasing diagnostic yield was noted from 1990 to 1999,hut the decreased trend was found in last five years.The propor- tions of rectal cancer was also increased in the last fifteen years,and overall proportions of cancer located in left side of colon was decreased.Conclusions The overall diagnostic yield of colorectal cancer in Nanchang area was lower than other province,especially rectal cancer.The proportion of cancer of left side of colon was increased with age.The well and moderately differentiated adenocarcinoma was found increasing in the last fifteen years, while the poorly differentiated adenocarcinoma was decreased.

Objective To explore the mechanism of protecting cells from ischemic reperfusion injury by constructing specific small interference RNA (siRNA) to inhibit Na+-H+exchanger-1 (NHE-1) expression in human renal tubular epithelial cell (HKC). Methods The siRNA was designed and synthesized based on human NHE-1 complete sequence,and was transfected into HKC.The irrespective siRNA transfected group was used as control.The cells were treated with 10 μmol/L antimyein A to induce ischemia and anoxyaemia environment.NHE-1expression was examined by RT-PCR and Western blot.The intraeellular pH (pHi),Ca2+ or Na+ concentrations were detected by BCECF/AM,Fluo-3/AM and SBFI-AM,respectively,combining with laser eonfocal assay system.Nucleic morphology was determined by Hoechst 33342.Cellular apoptosis was examined by Annexin V/PI staining and flow eytometry.Fluorescent probe JC-1 was used to detect the change of mitechondrial transmembrane potential. Results The specific siRNA could efficiently inhibit NHE-1 expression in HKC.Compared with the irrespective siRNA transfected group,the mRNA and protein expression of NHE-1 was significantly down-regulated in NHE-1 siRNA transfeeted group (all P＜0.05).After treatment with antimyein A,the mRNA and protein expression of NHE-1 was significantly up-regulated in both groups,however,it was less than that in irrespective siBNA transfected group.At the same time,the ratio of apoptosis decreased (8.9% +2.9% vs 18.8%±3.2% , 17.4%±3.6% ,P＜0.05) and mitochondrial transmembrane potential rose significantly in NHE-1 siRNA transfected group as compared to irrespective siRNA transfected group and antimycin A group.The intracellular Na+,H+ and Ca2+concentrations increased in NHE-1 siRNA transfected group treated with antimyein A,but their levels were lower than those in irrespective siRNA transfected group with the same treatment(P＜0.05). Conclusions The synthesized siBNA can inhibit the expression of NHE-1 and can protect HKC from isehemia reperfasion injury induced by antimyein A.The mechanism might be via suppressing the expression of NHE-1 to delay intracelluar Na+ accumulation,attenuate intracellular Ca2+ overloading,and inhibit the decrease of mitechondrion transmembrane potential and reduce cellular apoptosis.

Objective To evaluate the application of Mammotome minimally invasive biopsy(MMIBS) in the diagnosis of non-palpable small lesions in breast. Methods A total of 42 patients with non-palpable small breast lesions and abnormal imaging findings were performed MMIBS.After MMIBS,21 patients received open operations,and the pathological diagnosis was compared and analysed. Results All these 42 patients were successfully performed MMIBS,with no severe complications,and the pathological diagnosis of MMIBS were breast cancer(8 patients) and benign pathological changes(34 patients).Among the 21 patients with open operations,the post-operative pathological findings of 20 patients were same as the results of MMIBS,with the sensitivity of 88.9% and specificity of 100%.The other 21 patients without operations were followed up for 6 to 12 months,and no abnormal mammographic and ultrasonographic findings were observed.Conclusion MMIBS boasts the characteristics of minimal invasion,safety,high sensitivity and high specificity,which can serve as the first choice in the diagnosis of non-palpable small lesions with abnormal imaging findings.

OBJECTIVETo observe the effect of Xuebijing injection on serum protein level in the early phase of septic rats and explain the mechanism from the perspective of molecular biology.

METHODFifty-four healthy wistar rats were randomly divided into control group, sepsis group and Xuebijing treatment group. The rat model of sepsis was established with injecting lipopolysaccharide (LPS) through caudal vein. Serum total protein (TP) and albumin (ALB) were measured at the point of 6, 12 and 24 h with the established model. The expression of AMPK, eEF2 protein in liver in the three groups were detected by Western blot analysis.

RESULTCompared with control gruop, the concentrations of TP and ALB of sepsis group and Xuebijing group were no significant difference with 6, 12 h treatment TP and ALB in sepsis group was lower (P<0.01) than control group after 24 h, and the expression of phos-AMPK, pho-eEF2 protein in livers was increased (P<0.01) simultaneously. All measured indexes in Xuebijing group has no difference with control group. Compared with sepsis group, TP and ALB of Xuebijing group was significantly higher (P<0.05), but litle lower than control group, the expression of phos-AMPK, pho-eEF2 protein in livers was decreased (P<0.05) simultaneously.

CONCLUSIONThese data suggest that Xuebijing injection prohibits catabolising serum albumin and inhibit liver protein catabolism by method of AMPK way.

Toxicological pathology is a morphological elucidatet ion of tissue damage caused by drug toxicity,including damage type,location,severity grade and prognosis.It provides objective and accurate data to support preclinical drug safety evaluation.Therefore,toxicological pathology plays an important role in drug safety evaluation,and the pathologic data or conclusion often determines the termination or continuation in many drug development projects.In this paper,the characteristic of toxicological pathology,standardized management,the development of new techniques including in situ hybridization,laser scanning cytometry and laser capture microdissection,and related problems in pathology are discussed briefly.
