1.Anti-apoptosis Mechanism of Serum Containing Xin Mai Tong Decoction in Myocardial Cell Under Oxidative Stress

Objective To explore the anti-apoptosis mechanism of serum containing Xin Mai Tong decoction in myocardial cell under oxidative stress.Methods The neonate rat myocardial cell was cultivated and the model of myocardial cell apoptosis was induced with 400 ? mol/L H2O2.The cell viability was determined by MTT method,myocardial cell apoptosis was determined by TUNEL method,and the intra-cellular expression of HSP70,NF-? B and caspase-3 in myocardial cell was determined by immunocytochemistry.Results Administration of 400 ? mol/L H2O2 for 1 hour was harmless to myocardial cell,the expression of HSP70 being slightly increased.However,apoptosis was induced in myocardial cells at the 6 th hour after 400? mol/L H2O2 administration.The serum containing Xin Mai Tong significantly attenuated the decrease of cell viability and apoptosis which was induced by oxidative stress.Increased expression of HSP70 was induced by the serum containing Xin Mai Tong,and the activation of NF-? B and caspase-3 were significantly inhibited.Conclution The serum containing Xin Mai Tong decoction shows a significant inhibitory effect on apoptosis induced by H2O2,and the mechanism may be through increasing protein expression of HSP70,inhibiting the activation of NF-? B and decreasing the expression of caspase-3.
2.Simvastatin Prevents Lipopolysaccharide-induced Septic Shock in Rats

Simvastatin is a hypolipidemic drug that inhibits hydroxymethylglutaryl coenzyme A (HMGCoA) reductase to control elevated cholesterol,or hypercholesterolemia.Previous studies have shown that simvastatin may attenuate inflammation in ischemia-reperfusion injury and sepsis.Herein,we hypothesized that simvastatin may prevent rats from lipopolysaccharide (LPS)-induced septic shock.In our study,rats were divided into a saline group,an LPS group and an LPS plus simvastatin group.Male Sprague-Dawley (SD) rats were pretreated with simvastatin (1 mg/kg) for 30 min before the addition of LPS (8 mg/kg),with variations in left ventricular pressure recorded throughout.Ninety min after LPS injection,whole blood was collected from the inferior vena cava,and neutrophils were separated from the whole blood using separating medium.The neutrophils were then lysed for Western blotting to detect the levels of urokinase-type plasminogen activator (uPA) and plasminogen activator inhibitor-1 (PAI-1).In addition,mesentery microcirculations of inlet diameter,outlet diameter and blood flow rate were measured in all three groups.The results indicated that simvastatin significantly promoted heart systolic function and increased the level ofuPA while simultaneously inhibited the expression of PAI-1 as compared with LPS group.Moreover,simvastatin reversed the LPS-induced inhibition of mesentery microcirculation.Taken together,it was suggested that simvastatin can effectively protect the rats from LPS-induced septic shock.
3.Effects of Dihuangyinzi on behaviors and RAGE/p38 pathway in APP/PS1 mice

Objective To investigate the effects of Dihuangyinzi(DHYZ) on behaviors and RAGE/p38 pathway in APP/PS1 mice.MethodTwenty APP/PS1 dementia mice were randomly divided into model group(n=10) and Chinese medicine group(n=10).The blank group was C57 BL/6 J normal mouse(n=10).The mice in Chinese medicine group were intragastric administration with DHYZ (9.75 g·kg-1·d-1).The mice in model group and blank group were treated with distilled water.After 30 days,the abilities of learning and memory of mice were detected by Morris water maze.The expression of amyloid-beta1-42(Aβ1-42) in the hippocampus and cortex was detected by immunohistochemistry.Reactive oxygen species of brain tissue were detected by DCFH-DA Methods in the brain of APP/PS1 mice.Gene expression level of receptor for advanced glycation end products(RAGE) was measured by real-time polymerase chain reaction (RT-PCR) in the cortex and hippocampus of APP/PS1 mice.The expression of phospho-mitogen-activated protein kinases (p38) was analyzed with Western blot and immunofluorescence analysis in the cortex and hippocampus of APP/PS1 mice.Results Behavioral Results showed that DHYZ significantly increased the distance((23.088±7.083)cm) and residence time((1.961±1.230)s)of effective area in Morris water maze on the fifth day(P<0.05,P<0.01)and remarkably increased the number of effective area crossings((1.607±0.405) times) and plats((0.893±0.283) times) in Morris water maze on the fifth day(P<0.01,P<0.05).DHYZ also significantly reduced the intracelluar ROS level(122.611±7.630) in the brain(P<0.01),and DHYZ could depress the expression of RAGE(1.467±0.081,7.983±0.136) and phosphorylation of p38 (0.376±0.026,0.538±0.016)in the cortex and hippocampus of APP/PS1 mice(P<0.01,P<0.05).Conclusions The Results demonstrate that DHYZ can partly improve memory impairment of APP/PS1 mice by the inhibition of RAGE/p38 pathway.
4.Comparison of the applicatied effect of two kinds of post core porcelain fused to metal crown for restoration of residual crown of first molar

Objective To compare the applicatied effect of two kinds of post core porcelain fused to metal crown for restoration of residual crown of first molar .Methods 52 patients with residual crowns of first molars (98 teeth) were selected.According to the random numerical table method ,they were divided into group A[post core crown of metal casting in 27 cases ( 49 teeth ) ] and group B [ post core crown of screw thread amalgam in 25 cases (49 teeth)].After a year of follow -up,the success rate of the two groups was compared .Results Metal casting core crown pile was broken at 0 in group A and at 3 in group B,the success rate of group A was 93.88%,and the success rate of group B was 79.59%,the difference was statistically significant between the two groups (χ2 =4.346, P<0.05).Conclusion The retention,resistance and durability of post core crown of metal casting is far superior to post core crown of screw thread amalgam ,and it is a more ideal repair method .
5.Use of the ultrasound-based total malignancy score in the management of thyroid nodules.

PURPOSE: The purpose of this study was to validate the role of the total malignancy score (TMS) in identifying thyroid nodules suspicious for malignancy through the sum of their ultrasound features. METHODS: The local ethical committee approved this prospective observational study. We examined 231 nodules in 231 consecutive patients (164 females and 67 males; age range, 20 to 87 years; median age, 59 years; interquartile range, 48 to 70 years) who underwent ultrasound followed by fine-needle aspiration cytology (FNAC). The nodules were further classified using the TMS, which considers ultrasound features (number, echogenicity, structure, halo, margins, Doppler signal, calcifications, and growth), and the Bethesda System for Reporting Thyroid Cytopathology (TBSRTC), which considers cytological features. Patients with non-negative nodules (TBSRTC categories III to VI) underwent histological analysis, repeated FNAC, or 2 years of regular ultrasound follow-up. The associations between the final diagnosis, each of the ultrasound features, and the TMS were estimated using the chi-square test, the Mann-Whitney U test, and multivariate logistic regression. A receiver operating characteristic (ROC) curve was used to evaluate the diagnostic accuracy of the TMS. RESULTS: On ultrasound, 47% of the nodules (108 of 231) had a TMS < 3, 18% (42 of 231) had a TMS of 3, and 35% (81 of 231) had a TMS >3. The FNAC results of 85% of the nodules (196 of 231) were benign, while 15% (35 of 231) had non-negative results. Hypoechogenicity, solid structure, the presence of microcalcifications, and the number of nodules were independent predictors of the final diagnosis, and the diagnostic accuracy of the TMS was good (area under the ROC curve, 0.82). CONCLUSION: The TMS system is simple to use, reliable, easily reproducible, and closely reflects malignancy risk. Based on our results, FNAC could be limited to nodules with a TMS ≥3 without missing any cases of carcinoma.

6.Research on the development process of the knowledge system of medical communication discipline

In the process of achieving the"Healthy China 2030"goal,the importance of health knowledge popularization behavior is increasingly prominent.Medical communication has emerged to meet the growing demand for medical popularization and improve the shortage of professional medical staff in medical knowledge popularization.At present,literature is relatively scarce on the development process of medical communication in the academic community.By analyzing the development process of this discipline,this paper divided the development process of the knowledge system of medical communication into three stages,including the"initial stage",the"practical exploration stage",and the"rapid development stage".After summarizing the different characteristics exhibited in the three stages,it can be concluded that the number of colleges and universities offering medical communication courses is gradually increasing,and social organizations such as the Society of Medical Communication have been established one after another,expanding the influence of the discipline.The development of medical communication not only helps to enhance the popularization and communication abilities of medical students,but also accelerates the development of China's health industry through its extensive influence at the practical level.
7.Case classification in measles surveillance system under the Two-level Logistic Model

Objective To study the prevalence rates of Rash and Febrile Illnesses (RFIs) including measles,rubella,scarlet fever,exanthema subitum and the differences among measles and other RFIs to tentatively formulate the logistic regression model through clinical manifestation.Methods All the suspected cases of measles,rubella,scarlet fever,exanthema subitum reported by the county/prefecture lever hospitals at four counties were collected during March 2011 to February 2012.When setting laboratory confirmed measles as dependent variable and existed symptoms as independent variable,a logistic regression model was formulated and optimal operational point (OOP) chosen,according to the ROC curve.Results A total number of 551 cases were collected but the consistency of measles diagnosis between clinical and laboratory was not satisfied,with Kappa value=0.349,same to the diagnosis of rubella.As for the result from the two-lever logistic regression model,symptoms that related to the confirmation of measles would include cough (OR=5.75),conjunctivitis (OR=3.00),Koplik spot (OR =7.52),lymphadenectasis (OR =0.07),rash after fever (OR=0.07).The area under ROC curve was 0.97 and the optimal operational point was 0.249.Conclusion A logistic regression model was formulated using the clinical symptoms which was resulted in better performance on prediction.As the sample size of this survey was small,the expansion on the scale of investigation and laboratory testings were needed before the types and components of measles-related RFIs be clarified.
8.Arterial Stiffness in Patients with Deep and Lobar Intracerebral Hemorrhage.

BACKGROUND AND PURPOSE: Intracerebral hemorrhage (ICH) accounts for approximately 10% of stroke cases. Hypertension may play a role in the pathogenesis of ICH that occurs in the basal ganglia, thalamus, pons, and cerebellum, but not in that of lobar ICH. Hypertension contributes to decreased elasticity of arteries, thereby increasing the likelihood of rupture in response to acute elevation in intravascular pressure. This study aimed to evaluate arterial stiffness (using the arterial stiffness index [ASI]) in patients with deep (putaminal and thalamic) ICH in comparison with patients with lobar ICH. METHODS: We enrolled 64 patients (mean+/-SD age: 69.3+/-10.7 years; 47 men and 17 women) among 73 who referred consecutively to our department for intraparenchymal hemorrhage and underwent brain computed tomography (CT) and cerebral angio-CT. In all the subjects, 24-hour heart rates and blood pressures were monitored. The linear regression slope of diastolic on systolic blood pressure was assumed as a global measure of arterial compliance, and its complement (1 minus the slope), ASI, has been considered as a measure of arterial stiffness. RESULTS: In the patients with deep ICH, ASI was significantly higher than in the patients with lobar ICH (0.64+/-0.19 vs. 0.53+/-0.17, P=0.04). CONCLUSIONS: Our results suggest that in deep ICH, arterial stiffening represents a possible pathogenetic factor that modifies arterial wall properties and contributes to vascular rupture in response to intravascular pressure acute elevation. Therapeutic strategies that reduce arterial stiffness may potentially lower the incidence of deep hemorrhagic stroke.

Contrast-enhanced computed tomography colonography (CE-CTC) is a useful guide for the laparoscopic surgeon to avoid incorrectly removing the colonic segment and the failure to diagnose of synchronous colonic and extra-colonic lesions. Lymph node dissection and vessel ligation under a laparoscopic approach can be time-consuming and can damage vessels and organs. Moreover, mesenteric vessels have extreme variations in terms of their courses and numbers. We describe the benefit of using an abdominal vascular map created by CE-CTC in laparoscopic colorectal surgery candidates. We describe patients with different diseases (colorectal cancer, diverticular disease, and inflammatory bowel disease) who underwent CE-CTC just prior to laparoscopic surgery.
