Objective To investigate the risk factors of abdominal infection after orthotopic liver transplantation. Methods Clinical data of 284 recipients undergoing orthotopic liver transplantation were retrospectively analyzed. All recipients were divided into the infection group (n=51) and non-infection group (n=233) according to the incidence of postoperative abdominal infection. Univariate and multivariate logistic regression analyses were used to identify the risk factors of abdominal infection. Nomogram prediction models were constructed and the prediction efficiency of these models was evaluated. The predictive value of continuous variables for abdominal infection was assessed. Results Among 284 recipients, 51 developed abdominal infection with an incidence of 18.0%. Diabetes mellitus before surgery[odds ratio (OR) 2.66, 95% confidence interval (CI) 1.13-6.14, P=0.013], long operation time (OR 1.98, 95%CI 1.03-3.57, P=0.038), low prognostic nutritional index (PNI) (OR 2.18, 95%CI 1.06-4.44, P=0.023), high systemic immune-inflammation index (SII) (OR 2.21, 95%CI 1.06-4.78, P=0.012) and high C-reactive protein/albumin ratio (CAR) (OR 1.90, 95%CI 1.05-3.49, P=0.029) were independent risk factors for abdominal infection after liver transplantation. The area under curve (AUC) of nomogram model for predicting abdominal infection after liver transplantation was 0.761. The standard model yielded high consistency. CAR, PNI and SII were all predictors of abdominal infection after liver transplantation (all P < 0.05), with AUC of 0.648, 0.611 and 0.648, and cut-off values of 2.75, 43.15 and 564.50, respectively. Conclusions CAR, SII and PNI are predictors of abdominal infection after liver transplantation. The nomogram model based on PNI, SII and CAR may effectively predict the incidence of abdominal infection after liver transplantation.

Natamycin is a safe and efficient antimycotics which is widely used in food and medicine industry. The polyene macrolide compound, produced by several bacterial species of the genus Streptomyces, is synthesized by type Ⅰ polyketide synthases using acetyl-CoA, malonyl-CoA, and methylmalonyl-CoA as substrates. In this study, four pathways potentially responsible for the supply of the three precursors were evaluated to identify the effective precursor supply pathway which can support the overproduction of natamycin in Streptomyces gilvosporeus, a natamycin-producing wild-type strain. The results showed that over-expressing acetyl-CoA synthetase and methylmalonyl-CoA mutase increased the yield of natamycin by 44.19% and 20.51%, respectively, compared with the wild type strain under shake flask fermentation. Moreover, the yield of natamycin was increased by 66.29% compared with the wild-type strain by co-overexpression of acetyl-CoA synthetase and methylmalonyl-CoA mutase. The above findings will facilitate natamycin strain improvement as well as development of strains for producing other polyketide compounds.

Acute myeloid leukaemia (AML) is the most common form of acute leukaemia in adults, with increasing incidence with age and a generally poor prognosis. Almost 20% of AML patients express mutant isocitrate dehydrogenase 2 (mIDH2), which leads to the accumulation of the carcinogenic metabolite 2-hydroxyglutarate (2-HG), resulting in poor prognosis. Thus, global institutions have been working to develop mIDH2 inhibitors. SH1573 is a novel mIDH2 inhibitor that we independently designed and synthesised. We have conducted a comprehensive study on its pharmacodynamics, pharmacokinetics and safety. First, SH1573 exhibited a strong selective inhibition of mIDH2 R140Q protein, which could effectively reduce the production of 2-HG in cell lines, serum and tumors of an animal model. It could also promote the differentiation of mutant AML cell lines and granulocytes in PDX models. Then, it was confirmed that SH1573 possessed characteristics of high bioavailability, good metabolic stability and wide tissue distribution. Finally, toxicological data showed that SH1573 had no effects on the respiratory system, cardiovascular system and nervous system, and was genetically safe. This research successfully promoted the approval of SH1573 for clinical trials (CTR20200247). All experiments demonstrated that, as a potential drug against mIDH2 R140Q acute myeloid leukaemia, SH1573 was effective and safe.

Objective:To study the intervention effect of narrative nursing on parent-child conflict in patients with chronic viral hepatitis b complicated with epilepsy.Methods:In 1 case because of parent-child conflicts caused by frequent attacks, depression, and anorexia behavior of chronic hepatitis b patients with viral hepatitis with epilepsy care process, the application of narrative postmodernism theory model of nursing and the nursing of the five core technologies: somatization, rewrite, restore, definition file, ceremony and treatment during the stay in hospital for patients and their parents were conducted three narrative counseling, two telephone follow-up after discharge, psychological intervention to the parent-child conflict problem.Results:Through narrative psychological intervention, no epileptic seizures caused by parent-child conflict occurred, depression was relieved, and no anorexia behavior was observed.Conclusion:Narrative nursing can help to solve the parent-child conflict between patients and their parents and promote physical and mental recovery.

Objective: To investigate the clinical efficacy of daytime continuous blood purification (DCRRT) combined with plasma exchange in the treatment of severe acute pancreatitis. Methods: The clinical data of 49 patients with non-biliary severe acute pancreatitis admitted to the First People's Foshan Hospital from January 2012 to January 2019 were analysed respectively. The enrollees were randomized into DCRRT combined with plasma exchange (combination therapy) group and DCRR only (DCRR) group using a random number table method. All patients received DCRRT therapy [8 hours continuous venous-venous blood purification/day (CVVH/d)] immediately after the diagnosis of non-biliary severe acute pancreatitis was established. The combination group received at least one plasma exchange during the course of treatment. The differences of laboratory examination and prognosis between the two groups before and after treatment were compared. Results: A total of49 patients were enrolled, including 29 males and 20 females, with age of (46.40±17.81) years. There were 24 patients in the combination therapy group and 25 patients in DCRR group. There were no significant differences in the age, gender, body mass index (BMI), and pre-treatment laboratory findings between the two groups. After treatment, the blood glucose, hypersensitive C-reactive protein (hs-CRP), procalcitonin (PCT-u), amylase, lipase, triglyceride, cholesterol, serum creatinine were lower than those before treatment (all P<0.05). The blood hs-CPR, PCT-u, lipase and triglyceride in the combination therapy group were significantly lower than those in the DCRR group (all P<0.05). The acute physiology and chronic health scores (APACHEⅡ) of the two groups were lower than those before treatment, and the combination therapy group was more significant than DCRR group (all P<0.05). There were 5 deaths (20.83%) in the combination therapy group and 7 deaths (28.00%) in DCRR group. There was no significant difference in mortality between the two groups. There was no significant difference in the start time and duration of DCRRT between the two groups. Conclusions DCRRT or combined plasma exchange therapy can effectively treat non-biliary severe acute pancreatitis. DCRRT combines with plasma exchange therapy can more effectively remove inflammatory factors and reduce APACHEⅡ score.

Objective To investigate the effect and mechanism of mitomycin C in reducing hypertrophic scar in rat traumatic osteomyelitis model.Methods A total of 120 Wistar rats were divided into control group (Group A,n =40),traumatic osteomyelitis group (Group B,n =40),traumatic osteomyelitis treated with Mitomycin C group (Group C,n =40),according to the random number table.The model of traumatic osteomyelitis was produced by Staphylococcus aureus.Muscle tissues around the focus were harvested at 15 d and 30 d postinjury.HE staining was used to observe the changes of muscle tissue structure.Immunohistochemistry was used to detect expression of transforming growth factor (TGF)-β1.Masson staining was used for collagen deposition evaluation.Western blot was used for detection of levels of TGF-β1 and collagen Ⅰ.Results HE staining revealed consistent alignment of fibers within the muscle in Group A.Fibrosis with the muscle was observed in both Group B and C,but the degree of muscle fiber disorder was decreased in Group C compared to Group B.Either 15 d or β0 d after injury,expressions intensity of TGF-β1,collagen fraction volume,and activation levels of TGF-β1 as well as collagen Ⅰ were higher in Group B and C than Group A,and all parameters were decreased in Group C compared to Group B (all P ＜ 0.05).Conclusion Mitomycin C can reduce hypertrophic scar formation in traumatic osteomyelitis model,and the potential mechanism relates to downregulated TGF-β1 and collagen Ⅰ.

Objective To understand the epidemiological situation of human parasitic diseases in Jinhu County,so as to pro?vide the evidence for formulating further control strategy. Methods The investigated local residents were sampled by the ran?dom cluster sampling method in 1989,1999 and 2015. The infections of intestinal helminthes were detected by Kato?Katz tech?nique,the eggs of Enterubius vermicularis were examined by cellophane anal swab for children,the intestinal protozoa were ex?amined by the saline smear and iodine staining methods. Results The total rates of parasitic infections were 62.57%,9.32%and 0.49%in 1989,1999 and 2015,respectively. Compared to those in 1989 and in 1999,the infection rate in 2015 was de?creased by 99.22%and 94.74%,respectively. The numbers of detected parasite species were 14,10 and 4 in 1989,1999 and 2015,respectively. The intensities of infections were mainly mild in three investigations,and the intensities of all the infections in 2015 were mild. The species of infected parasites were mainly single,however,multiple infections were observed in 1989,in?cluding 4 parasite species(0.72%)and 3 parasite species(7.02%). Only in 1989,the difference between sexes was significant and the infection rate of the female was higher than that of the male(χ2=18.01,P<0.01). Conclusions The infection rates of human parasites are decreased gradually and stabilized at the low level in Jinhu County. However,the surveillance work still should be strengthened to consolidate the achievement.

Aim To find the material foundation of treatment for diabetes in Coptidis Rhizoma ( RC ) . Methods The antihyperglycemic effect of RC alka-loids ( berberine, coptisine, palmatine, epiberberine, and jatrorrhizine) was evaluated in spontaneity diabe-tes KK-Ay mice. Results After 40 days′ oral admin-istration ( 225 mg · kg-1 · d-1 , ig ) , berberine and coptisine significantly suppressed the elevated fasting blood glucose level and ameliorated the glucose toler-ance . Body weight gain of KK-Ay mice was significant-ly decreased in the epiberberine-treated group. Berber-ine improved insulin resistance and jatrorrhizine in-creased the SOD activity, decreased the MDA level. Conclusions These results indicate that the main an-tihypoglycemic effect constituents are berberine and coptisine, while they show different mechanisms. Pal-matine, epiberberine and jatrorrhizine display different potential roles in the treatment of diabetes. The meth-ylene-dioxy groups at the C-2 , C-3 , C-9 and C-10 po-sitions are indispensable for antihyperglycemic effect of RC alkaloids.

Aim To study the hypnotic effect of five alkaloids extracted from Rhizoma Coptidis (berberine , coptisine,palmatine,epiberberine,jatrorrhizine )in mice,and preliminarily explore its underlying mecha-nism.Methods The experiments of locomotor activity and hypnosis induced by suprathreshold and subthresh-old doses of pentobarbital sodium were used to evaluate the effect of drugs on sleep behavior in mice.Then, HPLC-FLD was used to detect the contents of NE,DA and 5-HT on PCPA mice model.Results Compared with control group,berberine and coptisine notably in-hibited spontaneous activity in behavioral experiments (P <0.05),and increased the sleeping percentage of mice under subthreshold dose of pentobarbital sodium. Berberine and coptisine shortened the period of sleep latency,and prolonged the sustained period of sleeping at suprathreshold dose in mice (P <0.05 or P <0.01 ).Other alkaloids had no significant differences in sleep latency and period of sleep observed in this current experiment.Compared with PCPA mice model group,berberine and coptisine remarkably increased the contents of NE and 5-HT (P <0.01 ),but they had no effects on DA.Conclusions Berberine and coptisine may play a sedative and hypnotic role in PC-PA mice by increasing contents of 5-HT and NE in hy-pothalamus,and the sedative and hypnotic effects of berberine are stronger than those of coptisine.Other alkaloids have no effects on sleeping in mice.

Objective To evaluate the effect of integrated control measures on soil-transmitted nematodiasis so as to provide the evidence for formulating the appropriate control strategies and measures in Jinhu County,Jiangsu Province. Methods Since 1995,the comprehensive control measures were carried out for soil-transmitted nematodiasis,and the measures included deworm-ing,health education,safe water,sanitation and environmental remediation. The effects of the comprehensive control measures were evaluated by the investigations of the prevalence of soil-transmitted nematodiasis,awareness of health knowledge,and behav-iors of residents. Results From 1995 to 2012,646 437 person-times were administrated in deworming medication with 2.48 times per capita;the benefit rate of safe water was 97.90%;the popularity rate of harmless toilets was 86.89%. The awareness rate of health knowledge increased from 54.05%in 1996 to 95.60%in 2012,the difference between them were statistically significant (χ2=230.92,P<0.01);the rate of correct health behaviors increased from 59.07%in 1996 to 96.40%in 2012,the difference between them had statistical significance(χ2=202.69,P<0.01). The prevalence of soil-transmitted nematodiasis decreased from 62.57%in 1989 to 1.21%in 2012,the difference had statistical significance(χ2=1 016.92,P<0.01). The infection rates of Asca-ris lumbricodes,hookworm and Trichuris trichiura were 0.58%,1.12%and 0,respectively in 2012,and compared with the rates of those infections in 1989,the decline rates were 94.96%,97.28%and 100%respectively,the differences between them were sta-tistically significant(χ2A sc aris =129.50,χ2hookworm=544.62,χ2Tri ch uris =254.19,all P<0.01). Conclusion The comprehensive control strategies and measures are effective and soil-transmitted nematodiasis has been controlled in Jinhu County.
