It is of great importance for surgery efficacy and avoidance of complications to reconstruct cervical curve, intervertebral height and cervical stability. Anterior cervical decompression includes discectomy and subtotal corpectomy. The former reconstruction comprises simple bone graft, bone graft with plating, cage implantation, cage with plating and total disc replacement.The latter comprises simple bone graft, bone graft with plating, titanium mesh cage with plating, conbinding cage and titanium mesh cage with plating. The common problem is loss of intervertebral height or nonunion occurring in a part of patients.Total disc replacement may preserve motion of affected levels but narrow indication. Based in the decompression, the best method should be choosen to reconstruct cervical stability and intervertebral height.

[Objective]To explore the optimal time for the surgical operation on patients with ossification of the posterior longitudinal ligament of the cervical spine. [Methods]The clinical symptoms,the persistent time of the symptoms and the interval from exacerbation to operation in 168 patients with OPLL were analyzed.[Results]The average time was 27.6 months from the onset of myelopathy to the operation on patients with OPLL,and 17.4 months from the decrease of muscle strength in extremities to the operation,and 5.3 months from the symptomatic exacerbation of the extremities especially the decrease of muscle strength in lower extremities to the operation.[Conclusion]The patient should be treated earlier by surgical operation after his/her illness has been diagnozed as OPLL.It is the optimal time for surgical operation when the patient has imaging change of hypertrophy or ossification of the posterior longitudinal ligament and severe compression of spinal cord(more than 60% spinal canal stenosis),or the decrease of muscle strength in extremities especially in lower extremities.

Ultrastructural changes of the inner cell mass (ICM) in rabbit blastocysts from 4 to 7 days post coitum (p.c.) were observed with transmission electron microscope. It revealed that ICM of blastocyst on day 5 p.c. began to differentiate after they were arranged into a single layer, and under which the primitive endoderm appeared. It is suggested that the primitive endodermal cells in rabbit blastocysts are derived from the scattered ICM-like cells at the inner surface of the mural trophoblast rather than delaminated from ICM proper. In this paper, disruption and disappearance of polar trophoblast are described and discussed.

Benign symmetric lipomatosis is a rare disease and may appear as a huge tumor in the neck. Four benign symmetric lipomatosis associated with gigantic painless mass or neck motion limitation were reported. Operative technique of one-time radical resection or stage resection was used to remove these tumors. One patient had a postoperative complication of incision effusion and infection, and three patients had no significant complication. At more than one-year follow-up, the motion and appearance of a patient's neck returned to normal, and no recurrence was observed. The etiology, clinical manifestations, diagnosis, and treatment of the disease were discussed.

Objective To explore the distribution and localization of LPGDS (Lipocalin-type prostaglandin D synthase) in rat testis and epididymidis and gain further insight into LPGDS potential function in male reproduction. Methods The expression of LPGDS in rat testis and epididymidis was assessed using RT-PCR and immunobloting. Using immunohistochemistry to explore the distribution of LPGDS in rat testis and epididymidis. Results None of the LPGDS studied was present in the testis of 2 month old SD rat. LPGDS was strikingly expressed in the caput epididymidis, while moderate to weak expressing was observed in the corpus and cauda epididymidis. The result of immunohistochemistry displayed that LPGDS was mainly localized within the cytoplasm and the cilia of the epithelial cells in rat epididymidis. Conclusion LPGDS was mainly distributed in epididymidis and localized with in the cytoplasm and the cilia of the epithelial cells. There was no LPGDS expressed in the testis of 2 month old SD rat, but it was strikingly expressed in the epididymidis, and the dose of expression was different, which suggested that LPGDS may play some kind of role during the process of the spermatozoa maturation.

Objective:To prepare monoclonal antibodies against lipocalin type prostaglandin D synthase and to explore the distribution of lipocalin type prostaglandin D synthase on human sperm.Methods:BALB/c mice were immunized with lipocalin type prostaglandin D synthase,monoclonal antibodies were prepared by hybridoma technique,the biological characteristics of antibodies were analyzed by ELISA and Western blot,the distribution of lipocalin type prostaglandin D synthase on sperm were performed by immunohistochemistry.Results:A strain of hybridoma cell was obtain,and the IgG isotype of the antibody was identified as IgG1,with affinity constat(K) of 2?108 and titer over 103.Western blot demonstrated specific recognition of lipocalin type prostaglandin D synthase by the obtained monoclonal antibody.The result of immunohistochemistry displayed that lipocalin type prostaglandin D synthase was mainly distributed on the surface of human sperm.Conclusion:A hybridoma cell secreting anti-lipocalin type prostaglandin D synthase monoclonal antibodies stably was established.On the surface of human sperm,there is lipocalin type prostaglandin D synthase's distribution. [

Community-based rehabilitation (CBR) is positioned to provide various strategies for people with disabilities for achieving the embracing development of the community. CBR is also the terminal of the three-level rehabilitation service system. This paper analyzed the status of domestic community-based rehabilitation based on the document, discussed the possible problems referred to the international concepts about CBR, and suggested a new service pattern of CBR based on the internet and information technology.

Objective To summarize the clinical results of posterior fusion plus pedicle screw fixation in treatment of dens fracture combined with recoverable atlantoaxial dislocation. Methods Twenty-seven patients with dens fractures combined with recoverable atlantoaxial dislocation were treated with posterior pedicle screw fixation, reduction and fusion. In this series of patients, skull traction was made to restore the normal atlantoaxial joint before the operation. Results Atlantoaxial alignment or stability were restored, without complication due to instrumentation. A follow-up for 12-48 months (average 24 months) showed osseous union. Conclusion Posterior aflantoaxial pedicle screw and rod fixation provides immediate three-dimensional rigid fixation of aflantoaxial joint and is a more effective technique compared with previously reported techniques.

Objective To identify the differences of 3D geometry of internal carotid artery (ICA)siphon between the controls and patients with ICA siphon stenosis. Methods The clinical and imaging data of the inpatients underwent carotid artery MRA in the past three years were collected. All patients were divided into the control group ( 17 males and 14 females with mean age of 67. 5 years) and ICA siphon stenosis group (20 males and 9 females with mean age of 58.6 years). There were 5 smokers and 9 smokers in two groups, respectively. The atherosclerotic predisposing factors were compared between the two groups using chi-square test and paired t-test. In order to extract the 3D geometry of ICA siphon, the MRA data were transferred to PC and processed with the software of Mimics. The average curvature radius (ACR) was calculated and paired t-test was applied to determine the bilateral differences in the controls. According to the stenotic site of ICA siphon, ICA siphon stenosis group was divided into C2 segment stenosis group and C4 segment stenosis group. The differences of ACR among the control group, C2 segment stenosis group and C4 segment stenosis group were compared. In addition, the values of ACR in the stenotic and normal sides were compared with paired t-test in patients with unilateral C2 segment stenosis. Results No significant differences were found in gender and smoker between the control group and the ICA siphon stenosis group ( χ2 = 1.63, P ＞ 0. 05; χ2 = 1.86, P ＞ 0. 05 ). The systolic blood pressure was ( 146. 6 ± 21.3 ) mm Hg ( 1 mm Hg =0. 133 kPa) and ( 140. 3 ± 17. 3) mm Hg respectively in the ICA siphon stenosis group and the control group. The serum glucose level was ( 5.94 ± 1.89 ) mmol/L and ( 6. 79 ± 3.57 ) mmol/L respectively in two groups. The serum cholesterol level and triglyceride level were (4. 57 ± 0. 87 ) mmol/L,( 1.34 ± 0. 63 ) mmol/L and (4. 75 ± 1.70) mmol/L, ( 1.54 ± 0. 72) mmol/L respectively in two groups.There were no differences in the atherosclerotic predisposing factors between two groups (t = 1.24, 1.16,0. 71 and 1.16 respectively,P＞ 0. 05). In the control group, the ACRs on the left and right were (3. 82 ±0. 69) mm and (4. 08 ± 1.04) mm respectively and no difference was found ( t = - 1.44, P ＞ 0. 05 ).Kruskal-Wallis test revealed that there were significant differences among the controls, C2 segment stenosis group and C4 segment stenosis group ( χ2 = 6. 67, P ＜ 0. 05 ). The ACR was significantly different only between the controls and C2 segment stenosis group ( t = 2. 63, P ＜ 0. 05 ). The stenotic side had a tendency to have less ACR than the normal side (t =2. 09, P =0. 05) in patients with unilateral C2 segment stenosis. Conclusion Patients with smaller ACR are more susceptible to have the stenosis of C2 segment in ICA siphon, and 3D geometry may be a possible reason for the development of the ICA stenosis.

Further research on the target antigens of antisperm antibodies may help to discover the causal relationship of antisperm antibodies to infertility. This paper summarizes the structure and function of the six target antigens of antisperm antibodies found recently, so as to discover the causal relationship of the antibodies to infertility and provide a basis for screening a vaccine for immunological contraception.
