1.The effect of femoral component design on anterior knee pain after patella reserving total knee arthroplasty

Objective To investigate the effect of femoral design on anterior knee pain after patella reserving total knee arthroplasty. Methods From 15 January to 15 February 2002, 44 patients and 59 knees were followed up. Among these patients, 40 patients and 51 knees were included in this study, the rest cases were excluded for one or more of following factors. 1)Knee flexion was less than 80 degrees;2)Radiography showed that there were surgical errors such as: femoral component flexion more than 30, anterior femoral cortex notching, and patellar baja or alta. According to current generally accepted criteria, femoral components were classified as "Patella Friendly" group and "Patella Unfriendly" group. The percentage and severity of anterior knee pain was measured with patient visual pain index and Knee Society Functional Score System. Results The aforementioned two groups consisted of "Patella Friendly" group 21 knees and "Patella Unfriendly" group 31 knees. The average anterior knee pain score of "Patella Friendly" group and "Patella Unfriendly" group was respectively 1.58 (0-8) and 3.32 (0-6). Objective To investigate the thawing modes on stability of coagulation control products after frozen,looking for a new theoretical basis for cost control and the quality and safety of laboratory.Methods Using ACL TOP 700 automated co-agulation analyzer and supporting the same batch of reagents and quality control materials conduct of the study:after daily QC,recycled the remaining control materials immediately and dispensed into two EP tubes and frozen at-40℃,respectively thawed by room-temperature and 37℃water bath after 24 hours,and examined together with the date of quality control ma-terial,got 20 pairs of data for analysis the financial impact of two alternate ways on coagulation QC parameters.Results For the room-temperature thawing group,FIB high value increasedby an average 0.23 g/L (t=4.026 9,P<0.05);TT normal value average reduction of 0.46 s (t=-3.813 8,P<0.05),TT high value reduced by an average 0.41s (t=-3.972 8,P<0.05);D-Dimer low-value increased by an average of 14.75 ng/ml FEU (t=2.281 6,P<0.05),while APTT,PT normal and high value,FIB normal value,D-Dimer high value were no significant difference after thawing (P>0.05).For the 37℃water bath group,both normal and high value of APTT,PT,FIB,TT and D-Dimer were no significant difference after tha-wing (P>0.05).Conclusion The commercialization of coagulation control materials can be for the second QC,just follow the principle of rapid after melting and timely detection,other laboratories can be used as a reference.
3.Comparison of age-adjusted critical and conventional cut-off values of D-dimer in the diagnosis of pulmonary thromboembolism in elderly patients

Objective To investigate the diagnostic value of D-dimer age-adjusted threshold in elderly patients with Pulmonary thromboembolism(PTE).Methods Collected patients older than 50 years of suspected PTE,who visited Yan′an People′s Hospital and the Hospital Affiliated to Yan′an University from June 2015 to September 2016,using the revised Geneva criteria for clinical probability assessment firstly.The low-risk group was excluded from the study;the patients with moderate-to-high risk were performed D-dimer detection and CTPA.All patients determined both by D-dimer age-adjusted thresholds and traditional thresholds,comparing the diagnostic efficacy of the two methods subsequently.Results 163 patients were diagnosed with PTE by CTPA among the 549 subjects,the positive rate was 29.69%.The diagnostic sensitivity of plasma D-dimer was 83.44%,78.53%,the specificity was 17.88%,31.61% respectively,the missed diagnosis rate was 16.56%,21.47%,respectively.The misdiagnosis rates were 82.12% and 68.39%,respectively,and the Younden index was 0.013 2 and 0.101 4 respectively.All subjects were divided into four groups according to age:50-60 years,60-70 years,70-80 years,and equal or greater than 80 years age group.Compared with the traditional threshold,the misdiagnosis rates of the four groups of age-adjusted thresholds decreased by 4.00%,15.70%,21.36% and 17.39%,respectively and the specificity was increased by 1.23,1.65,2.56 and 3.00 times,respectively.Conclusion The diagnostic accuracy of the age-corrected threshold is higher than the traditional threshold;combined with clinical practice,the optimal threshold is determined by the ROC curve,the clinician can serve as a reference.
4.Effects of DADS on the xenograft growth of NCI-H446 human small cell lung cancer cells in nude mice

Objective Diallyl disulfide ( DADS) has achieved remarkable effects in treatment and research of diverserfied cancers.The article was to explore the effects and the mechanism of DADS on the xenograft growth of human small cell lung cancer ( SCLC) cells in nude mice . Methods A total of 25 nude mice were selected to establish xenograft model of NCI-H446 human SCLC cells.The nude mice bearing with SCLC H446 were divided into 5 groups by random selection:positive control group(DDP 66 mg/kg), negative control group(physiological saline), 20 mg/kg DADS group, 60 mg/kg DADS group and 180 mg/kg DADS group, which is 40.6%, 53.1%and 66.4%, respectively.The growth of xenograft tumor in mice was observed after being treated with differ-ent concentrations of DADS .The morphological changes of the tumors were examined under light microscopy .Phase distribution and apoptosis of xenograft cells were analyzed by flow cytometry ( FCM) . Results The growth of xenograft tumor were inhibited signifi-cantly by DADS, resulting in decreased cell density and cellular atypia .Moreover, xenograft cell cycle was blocked in G 2/M and cell apoptosis rate was enhanced . Conclusion DADS can significantly inhibit the growth of NCI-H446 cells and lead to apoptosis .
5.TREATMENT OF CIRRHOTIC ASCITES COMPLICATED WITH PRIMARY PERITONITIS: ANALYSIS OF 55 CASES

Fifty-five cases of cirrhotic ascites complicated with primary peritonitis were treated and analysed clinically. The results showed that high fever and diarrhea were common and severe, and the positive rate of the ascitic culture, rich in B. coli, was higher in the death group, while tenderness and reboun ding pain were prominent and typical in the survival group. The total mortality was 67.27% and the survival rate was 25.45%. 32 out of the 76 cases (some of them had been treated repeatedly) of primary peritonitis were cured (42.11%). The state of restoring health in relation to the drug sensitivity, the mode of administration and the duration of therapy were fully discussed. It was believed that the combined therapy was more effective than the single one, and the duration of the two weeks' treatment seemed to be most suitable. The early diagnosis, effective therapy and prevention were the key points for a good prognosis and better survival rate.
6.Observation of oxidative stress condition and macular edema in patients of early diabetic retinopathy treated by probucol

Objective To study the influence of probucol on the diabetic mellitus patients with non-proliferative diabetic retinopathy (NPDR) about blood lipids,serum level of the total antioxidant capacity (TAOC),malondialdehyde (MDA),superoxide dismutase (SOD) and macular edema,so as to provide clinical basis for the prevention and treatment of early DR by probucol.Methods 66 type 2 diabetes patients with 127 NPDR eyes were included.Patients were random divided into control and treatment groups,the control group was treated by intensive therapy of blood glucose and blood pressure control,and the treatment group was treated with the intensive therapy and probucol 0.375g,2 times a day for 12 months.Before and after treatment,the blood lipids,oxidative stress indicators and fundus fluorescein angiography in both groups had been determined.Results 62 cases with 120 eyes were enrolled in this study.Probucol obviously decreased levels of total cholesterol (TC) [ (3.6 ± 0.58) mmol/L VS (4.71 ± 0.61)mmol/L,t = 7.65,P < 0.01 ],triglyceride (TG) [ (1.07 ± 0.35) mmol/L VS (1.23 ± 0.43) mmol/L,t = 2.02,P < 0.05 ],low density lipoprotein cholesterol (LDLC) [ (2.0 ± 0.47) mmol/L VS (2.55 ±0.56)mmol/L,t =4.18,P <0.01] and MDA[ (10.35 ±2.97)nmol/L VS (14.83 ±2.75)nmol/L,t =6.18,P <0.01] in plasma of the patients.Levels of TAOC [(19.25±4.11)u/ml VS (16.63 ±3.27)u/ml,t =3.57,P <0.01 ]and SOD[ (94.52 ± 10.28)u/ml VS (75.37 ± 9.87) u/ml,t =8.62,P <0.01]were significantly improved in the probucol group,and the macular edema was significantly reduced in patients of the probucol group(x2 =4.219,P <0.05).Conclusion Probucol regulated serum lipids,and it had the apparent action of antioxidant,and it decreased the incidence of macular edema.Probucol had a therapeutic effect in patients with NDPR.
7.Three-Points Approach Ablation for Treatment of Typical Atrial Flutter Guided by CARTO

Objective To evaluate the feasibility and effect of three-points ablation approach in in treatment of typical atrial flutter guided by CARTO. Methods Twenty-six patients with typical atrial flutter diagnosed by ECG and electrophysiological study (EPS) were enrolled in this study. Activation sequence mapping and linear ablation were performed in 11 patients (conventional group) . Three-points guided linear ablation with CARTO system was performed in another15 patients (three-points group) . Results There was no significant difference in the success rate between the two groups. Both the procedure and fluoroscopic time in three-points group were significantly shorter than that in conventional group [(72.66±29.82) vs (102.52±32.61) min;(4.26±2.76) vs (7.32±3.16) min] . Conclusions The three-points ablations approach is as safe and effective as conventional ablation approach in treatment of typical atrial flutter;however,the former can significantly shorten the procedure time and fluoroscopy time.
8.Involvement of MRE11 in inflammasome activation:a preliminary research

Objective To evaluate the function of MRE11 in inflammasome activation.Methods Different stimuli,in-cluding Poly(I∶C), Poly(dA∶dT),E.coli gDNA,293T gDNA,CPPD and HSV,were used to identify the effective inflamma-some activator using ELISA.Then, MRE11 siRNA oligos were sythesized and transfected into THP-1 cells while Western blotting was used to analyze the efficacy of MRE 11 knockdown .Finally ELISA and Western blotting were used to analyze the involvement of MRE11 in inflammasome activation induced by Poly (I∶C), Poly(dA∶dT), E.coli gDNA and 293T gDNA. Results The IL-1βsecretion and pro-caspase-1 activation which induced by Poly ( I∶C) , Poly( dA∶dT) , E.coli gDNA and 293T gDNA were reduced with different degrees in MRE 11-knockdown THP-1 cells.Conclusion These results indicate that MRE11 is required for inflammasome activation induced by genetic materials .
9.Clinical Observation of Xiaoxianxiong Decoction Combined with Metformin in Treating Phlegm-heat Type 2 Diabetic Patients

Objective To observe the effect of Xiaoxianxiong Decoction combined with metformin on glucose and lipid metabolism of phlegm-heat type 2 diabetic patients (T2DM). Methods Totally 104 phlegm-heat T2DM patients were randomly divided into two groups (each contains 52 cases), and given the diabetes lifestyle intervention. Control group additionally took 0.5 g metformin twice daily. And treatment group, besides taking 0.5 g metformin, was given Xiaoxianxiong Decoction, one dose and three times per day warmly. After 12 weeks, FBG, HbA1C, TC, TG, HDL-C, LDL-C, 2 hPG of the two groups were determined, and the therapeutic effect was evaluated. Results The total effective rate in the treatment group was 92.31%, and 76.92%in control group (P<0.01). The difference in symptom score, FBG, 2 hPG, TC, TG, HDL-C and LDL-C between the two group were significant, so as that between before and after treatment of both groups (P<0.05). Conclusion Xiaoxianxiong Decoction combined with metformin can effectively lower the levels of blood glucose and blood lipid in phlegm-heat T2DM patients, enhance the therapeutic effect and improve the metabolism of glucose and lipid.
10.The repair effects of Pim-3 on liver in fulminant hepatic failure rat model

Objective To investigate the liver repair effects of Pim-3 gene in rat with fulminant hepatic failure (FHF). Methods Thirty-two rats were divided into four groups (eight for each group). Three groups of rats were pretreated with Ringer's solution, vector plasmid or Pim-3 gene recombinant plasmid respectively and, one day later, received intraperitoneal injections with lipopolysacchride (LPS) and D-galactosamine (D-GalN). The fourth group served as normal control.Eight hours after the LPS/D-GalN injection, the liver tissues and blood samples were collected. The contents of serum transaminase was tested by automatic blood biochemistry meter. The morphological changes were observed by light microscopy using hematoxylin and eosin (HE) staining. Tumor necrosis factor (TNF)-α and interleukin (IL)-1β gene expression was detected by reverse transcriptionpolymerase chain reaction(RT-PCR). The serum levels of TNF-α and IL-1β were measured by enzyme linked immunosorbent assay (ELISA), and cell apoptosis by TdT-mediated dUTP nick end labeling (TUNEL) assay. Comparisons between groups were done by analysis of variance. ResultsThe over expressions of Pim-3 gene and reporter gene, green fluorescent protein (GFP) were induced by injection with recombinant plasmid solution.In comparison with the rats retreated with Ringer' s solution or vector plasmid, those pretreated with recombinant plasmid had a lower mortality and lower serum transaminase levels. The injection of recombinant plasmid significantly reduced hemorrhage, necrosis and inflammatory infiltration in the liver. Liver apoptotic index (AI) was dramatically lower in rats treated with recombinant vectors compared to the rats treated with Ringer's solution or vector plasmids [(10. 2±6.9)% vs (83. 1±12.6) % and (79.9±13.4) % respectively, P<0. 01]. In addition, the expression of exogenous Pim-3 gene remarkable inhibited the transcriptions and expressions of TNF-α and IL-1β. ConclusionsPim-3 gene can protect rats from LPS/D-GalN-induced FHF possibly by inhibiting expressions and secretions of inflammatory cytokines, such as TNF-α and IL-13, in liver tissues.
