1.Investigation core emergency response competences of medical staff in public health outbreak of corona virus disease 2019

Objective To investigate the core emergency response capabilities of medical workers for corona virus disease 2019 (COVID-19) public health emergency, in order to provide references for improving emergency level. Methods By means of convenient sampling, the general information questionnaire and the medical staff's 2019-new coronavirus (2019-nCoV) core emergency capability questionnaire were used to conduct this survey, and the status of 2019-nCoV core emergency capability of 629 clinical medical staff was analysed. Results The score of core emergency response capability of medical staff for 2019-nCoV was (135.60±21.73)points, with a score rate of 73.3%; the score of preventive capability dimension was (11.69±1.96)points, with a score rate of 77.93%; the score of preparation ability dimension was (20.79±4.18)points, and the score rate was 69.30%; the score of rescue ability was (103.11±16.93)points, with a scoring rate of 73.57%. Conclusion The core emergency response capability of medical worker for 2019-nCoV and its dimensions are at a medium level. At the present stage, the training of core knowledge of medical personnel 2019-nCoV should be strengthened, effective measures should be taken to improve the alleviation of weakness in rescue, and the self-psychological adjustment of medical personnel should be strengthened to improve the core emergency response capability of medical worker.
2.Investigation on the quality of life and demands of family members of patients with mental disorders in hospice care

ObjectiveTo investigate the quality of life and demands of family members of patients with mental disorders in hospice care, in order to create a better medical care environment for patients to meet their demands. MethodsA total of 205 family members of patients with mental disorders in hospice care were consecutively sampled from two special psychiatric hospitals in Deyang city with convenient sampling method. The Chinese version of WHO Quality of Life-BREF (WHOQOL-BREF) and Critical Care Family Needs Inventory (CCFNI) were used to assess the quality of life and demands of family members. ResultsAmong the patients' family members, WHOQOL-BREF scored (68.08±9.98) in the physiological field, (63.82±9.39) in the psychological field, (70.73±12.61) in the social relations field, and (64.24±11.87) in the environmental field. Compared with the domestic general population, there were significant differences in other fields except the physiological field (t=3.066, 9.845, 16.109, P<0.01). In CCFNI, the score of condition assurance factor was (3.20±0.41), information support was (2.86±0.50), proximity to patients was (2.79±0.46), self comfort was (2.35±0.47), and medical staff support was (2.60±0.44). ConclusionThe quality of life of the family members of patients with mental disorders in hospice care may be higher than that of the general population, and their demands mainly focus on the condition assurance and information support.
3.Moderating effect of psychological resilience in relation to depression and suicidal risk among adolescents

ObjectiveTo investigate the moderating effect of psychological resilience in relation to depressive symptoms and suicidal risk among adolescents. MethodsThe research is a descriptive survey. A total of 71 137 adolescents were selected from 163 schools in Deyang by stratified cluster sampling. Their psychological resilience, depressive symptoms and suicide risks were measured oneline by using 10-item Connor-davidson Resilience Scale (CD-RICS-10), Patients’ Health Questionnaire Depression Scale-9 item (PHQ-9) and Suicidal Behaviors Questionnaire-Revised (SBQ-R). The moderating effect of psychological resilience in relation to depressive symptoms and suicidal risk was examined by multivariate stratified regression analysis. Results① The score of CD-RISC-10 was negatively related to PHQ-9 score and SBQ-R score (r=-0.305, -0.268, P<0.01). ② Psychological resilience significantly moderated the relationship between depressive symptoms and suicidal risk (β=-0.100, t=-31.716, P<0.01). ③ In both male and female adolescents, resilience played a significant role in depressive symptoms and suicide risk (β=-0.086, -0.084, t=-17.502, -18.839, P<0.01). ConclusionPsychological resilience could significantly alleviate the impact of high-level depressive symptoms on suicidal risk among adolescents, and this effects both male and female adolescents.
4.Effect of preoperative waiting time on prognosis of elderly patients with hip fracture.

OBJECTIVE:
To investigate the relationship between preoperative waiting time and prognosis of elderly patients with hip fracture.
METHODS:
From January 2014 to December 2018, 333 elderly hip fracture patients undergoing surgery were retrospectively analyzed, including 104 males and 229 females, aged from 60 to 99 years with an average of (77.93±8.49) years, and 183 patients were femoral neck fracture, 150 patients were femoral intertrochanteric fracture. Among them, 269 patients (80.78%) had a clustered preoperative waiting time of 2 to 8 days, and then divided into within 4-day group(91 cases) and over 4-day group(242 cases) according to their preoperative waiting time. The survival situation was followed by telephone, and follow-up time started from fracture admission to the death event, or to the research deadline (December 31, 2019). The Kaplan-Meier method was used for survival analysis, and Cox risk proportion model was used to analyze the independent risk factors of hip fracture in elderly patients.
RESULTS:
All patients were followed up for 12 to 75 months(means 35 months), 59 patients died and the mortality rate was 17.72%(59/333). Compared with within 4-day group, the mortality rate was higher in over 4-day group[20.66%(50/242) vs. 9.89%(9/91), χ2=5.263, P=0.022]. Multiariable Cox regression analysis showed that preoperative waiting time, age, male and Charlson comorbidity index were independent risk factors for the prognosis of hip fracture in elderly patients (all P<0.05), and every 1-day delay was associated with 5% increase of the risk of death[HR=1.05, 95%CI(1.00-1.10), P=0.045]. Subsequent analyse was stratified according to the Charlson comorbidity index (CCI), and found that over 4-day group had a higher mortality rate in patients with CCI<2, with statistically significant difference(P<0.05).
CONCLUSION
For elderly patients with hip fracture, most of hospitals could not complete the hip fracture surgery within 48 hours, we also need to shorten the waiting time before surgery, and thereby improve their prognosis.

5.Safety of modified radical prostatectomy by transperineal injection of sodium hyaluronate to the Dirichlet gap: an animal experiment

【Objective】 To explore the safety of transrectal ultrasound-guided transperineal injection of sodium hyaluronate to expand the Dirichlet gap in laparoscopic radical prostatectomy. 【Methods】 A total of 14 healthy male purebred beagle dogs were selected and randomly divided into 2 groups, with 7 in either group.The control group was treated with conventional laparoscopic radical prostatectomy, while the experimental group was treated with laparoscopic radical prostatectomy after 2.5 mL sodium hyaluronate was injected into the Dirichlet gap under the guidance of transrectal ultrasound.The total operation time, prostate separation time, intraoperative blood loss and rectal status of the 2 groups were observed. 【Results】 After the injection of sodium hyaluronate into the Dirichlet gap between the prostate and the rectum, no rectal tissue was found in the prostate, and no obvious damage was found in the posterior rectum in either groups.The postoperative hemoglobin (HGB) was [(118.70±2.56) g/L vs.(122.10±2.19) g/L, P=0.02]; the total operation time was [(141.40±9.80) min vs.(119.10±9.16) min, P<0.05]; the prostate separation time was [(24.99±1.75) min vs.(16.64±2.34) min, P<0.05]; the amount of bleeding was [(47.43±4.32) mL vs.(34.86±5.18) mL, P<0.05] in the control group and experimental group. 【Conclusion】 Laparoscopic radical prostatectomy performed after 2.5 mL of sodium hyaluronate injection into the Dirichlet gap under the guidance of transrectal ultrasound can shorten the total operation time, the separation and resection time of the prostate, and reduce the amount of bleeding, which can improve and reduce the incidence of rectal injury, and prove the feasibility of this approach for prostatic cancer.
6.Ultrasonographic characteristics of renal artery involvement in acute Stanford type A aortic dissection and its relationship with renal function: A retrospective cohort study

Objective To investigate the ultrasonographic characteristics of acute Stanford type A aortic dissection (ATAAD) involving the renal arteries and their relationship with renal function. Methods Patients with ATAAD admitted to Deyang People's Hospital from February 2013 to May 2023 were selected for the study. Based on whether the renal arteries were involved in the dissection, the patients were divided into two groups: a renal artery involvement group and a renal artery non-involvement group. General data and ultrasound characteristics of the two groups were compared. Logistic regression analysis and model correction were performed to analyze the relationship between ultrasound characteristics and renal function involvement in ATAAD patients. Receiver operating characteristic (ROC) curves were used to evaluate the predictive value of ultrasound characteristics for renal artery involvement in ATAAD patients. Additionally, patients in the renal artery involvement group were divided into normal renal function and abnormal renal function subgroups based on serum blood urea nitrogen (BUN) and serum creatinine (Scr) levels. Clinical data of the two subgroups were compared, and a log-binomial model was used to analyze the risk effects of ultrasound characteristics for abnormal renal function. Pearson correlation analysis was performed to assess the correlation between ultrasound characteristics of renal artery involvement and renal function indicators. Results A total of 163 patients were included, consisting of 106 males and 57 females, with a mean age of (50.06±10.46) years (ranging from 20 to 85 years). Significant differences in gender, Scr, and BUN were observed between the renal artery involvement group and the renal artery non-involvement group (P<0.001). Compared to the renal artery non-involvement group, the renal artery involvement group had an increased ascending aorta diameter, a greater proportion of ascending aortic dilation and poor renal perfusion (P<0.05). Logistic regression analysis indicated that ascending aorta diameter, ascending aortic dilation, and poor renal perfusion were independent factors for renal artery involvement (P<0.05). Ultrasonographic characteristics showed good predictive ability for renal artery involvement in ATAAD patients. Furthermore, the combination of the three characteristics yielded a higher predictive value for renal artery involvement. Compared to the normal renal function group, the abnormal renal function group had higher BUN and Scr levels, increased ascending aortic diameter, a greater proportion of ascending aortic dilation and poor renal perfusion (P<0.05). The log-binomial model analysis revealed that the risk ratios for ascending aortic diameter, ascending aortic dilation, and poor renal perfusion were statistically significant both before and after adjustment (P<0.05). Pearson correlation analysis revealed that ascending aortic diameter, ascending aortic dilation, and poor renal perfusion were strongly correlated with renal function parameters (P<0.05). Conclusion Ultrasound characteristics of ATAAD involving the renal arteries are associated with renal function. Ascending aorta diameter, ascending aortic dilation, and poor renal perfusion are independent risk factors for abnormal renal function.
7.Relationship between occupational coping self-efficacy and health-related productivity loss in ICU nurses: path analysis of perceived social support

BackgroundCompared with absenteeism, health-related productivity loss has a long-lasting negative effect, and poses a greater harm and loss. The health-related productivity loss is mediated by self-efficacy, and perceived social support has been shown to have an impact on health-related productivity loss, whereas the interaction mechanism among the three remains unclear. ObjectiveTo investigate the status of perceived social support, occupational coping self-efficacy and health-related productivity loss among ICU nurses, and to test the mediation role of perceived social support in the relationship between occupational coping self-efficacy and health-related productivity loss. MethodsFrom September to November, 2021, purposive sampling technique was adopted to select 468 ICU nurses in 8 tertiary hospitals in Sichuan Province. Subjects were assessed using self-made general information questionnaire, Perceived Social Support Scale (PSSS), Occupational Coping Self Efficacy Scale for Nurses (OCSE-N) and Stanford Presenteeism Scale-6 (SPS-6). Pearson correlation analysis was used to examine the correlation among variables. Amos 24.0 was utilized to test the mediation role of perceived social support in the relationship between occupational coping self-efficacy and health-related productivity loss. ResultsA total of 415 ICU nurses completed the valid questionnaire survey. ICU nurses scored (63.13±11.62) on PSSS, (22.24±6.15) on OCSE-N, and (16.83±4.24) on SPS-6. Health-related productivity loss was detected in 279 ICU nurses (67.23%). Correlation analysis denoted that PSSS total score was positively correlated with OCSE-N total score (r=0.348, P<0.05), and negatively correlated with SPS-6 total score (r=-0.274, P<0.05). OCSE-N total score was negatively correlated with SPS-6 total score (r=-0.421, P<0.05). The direct effect value of occupational coping self-efficacy on health-related productivity loss was -0.401, and perceived social support showed a mediation role in the relationship between occupational coping self-efficacy and health-related productivity loss (the indirect effect value was -0.052, accounting for 11.48% of the total effect). ConclusionThe occupational coping self-efficacy of ICU nurses may affect the health-related productivity loss through the action path of perceived social support.[Funded by Scientific Research Project of Sichuan Provincial Health Commission (number, 19PJ042)]
8.Study on the Mechanism of Resveratrol Attenuating LPS-induced Acute Lung Injury in Mice Based on TLR 4/ NF-κB Pathway

OBJECTIVE:To observe the p rotective effects of resveratrol (Res)on LPS-induced acute lung injury (ALI)model mice,and to explore its possible mechanism based on TLR 4/NF-κB pathway. METHODS:Kunming mice were divided into normal group,model group ,positive control group (dexamethasone,0.5 mg/kg),Res low-dose ,medium-dose and high-dose groups (50, 100,200 mg/kg),with 10 mice in each group. Normal group and model group were given normal saline intragastrically ,once a day,for 7 days;positive control group were intraperitoneally injected with Dexamethasone sodium phosphate injection ,once a day,for 3 days;Res groups were given relevant medicine intragastrically ,once a day ,for 7 days. After last administration ,all the mice except the normal group were dripped LPS (5 mg/kg)into the nose to induce ALI model. The apoptosis of neutrophils in BALF was observed by Hoechst 33242 staining;the apoptosis rate of neutrophils were detected by flow cytometry. The contents of IL- 6 and TNF-α in the plasma were detected by ELISA. After wet mass to dry mass (W/D)ratio of lung was E-mail:492234709@qq.com calculated,the morphological characteristics of lung tissue were observed by HE staining. Western blotting assay was used to detect the expression o f TLR 4 and NF-κ B in lung tissue. RESULTS :In normal group ,there were few apoptotic neutrophils in the BALF ,and the lung tissue structure was intact , without edema,hyperemia,exudation,inflammatory cell infiltration or other inflammatory manifestations. In model group ,the number of apoptotic neutrophils in BALF increased ,and the apoptotic rate of neutrophils were enhanced significantly (P<0.01); edema and hyperemia of lung tissue were significantly increased ,and the red consolidation area was observed ;the contents of IL- 6 and TNF-α in plasma,the ratio of lung W/D,the relative expression of TLR 4 and NF-κB in lung tissue were significantly increased (P<0.01). Compared with model group ,the number of apoptotic neutrophils in BALF were increased ,and the apoptotic rate of neutrophils were enhanced significantly (P<0.05 or P<0.01)above symptoms of lung tissue were improved to different extents ; the contents of IL- 6 and TNF-α in plasma,lung W/D ratio as well as relative expression of TLR 4 and NF-κB in lung tissue(except for Res low-dose group )in administration groups were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS :Res has a protective effect on ALI model mice ,the mechanism of which may be related to reducing the generation of inflammatory cytokines TNF-α and IL-6 by inhibiting TLR 4/NF-kB expression.
10.Analysis of medical insurance access policy for rare disease drugs in the United Kingdom and enlightenment to China

OBJECTIVE To provide reference for the access to medical insurance for rare diseases in China based on the existing access pathway and framework by analyzing the access policy of medical insurance for rare diseases in the United Kingdom (UK). METHODS After collecting relevant guidelines and policy documents related to drug use for rare diseases in the UK, content analysis method was used to analyze the evaluation mechanism of drug use for rare diseases, reimbursement decision- making standards, stakeholder participation, coping strategies for dealing with uncertainties and risks, and policy implementation effects, and extract the key points of medical insurance access for drug use for rare diseases in the UK, to provide some suggestions for the establishment of medical insurance access system for rare diseases in China. RESULTS & CONCLUSIONS From the perspective of access, the UK had adopted a separate approach and clear criteria to assess and reimburse drugs for rare diseases. From the perspective of evaluation mechanism, multi-stakeholders such as doctors, patients and applicants participated in the decision-making process in the UK. The UK addressed uncertainty and risk by gathering better clinical evidence and using the patient access programme. After the implementation of the policy related to drug use for rare diseases, the UK had achieved remarkable results in terms of funding for drug use for rare diseases, the reimbursement rate of drug application, and the number of funded patients. It is suggested that in the process of establishing and improving the evaluation and reimbursement system for rare diseases drugs in China, the availability of rare diseases drugs should be improved by establishing a separate access assessment path for rare diseases drugs and involving more stakeholders.
