1.Investigation core emergency response competences of medical staff in public health outbreak of corona virus disease 2019

Objective To investigate the core emergency response capabilities of medical workers for corona virus disease 2019 (COVID-19) public health emergency, in order to provide references for improving emergency level. Methods By means of convenient sampling, the general information questionnaire and the medical staff's 2019-new coronavirus (2019-nCoV) core emergency capability questionnaire were used to conduct this survey, and the status of 2019-nCoV core emergency capability of 629 clinical medical staff was analysed. Results The score of core emergency response capability of medical staff for 2019-nCoV was (135.60±21.73)points, with a score rate of 73.3%; the score of preventive capability dimension was (11.69±1.96)points, with a score rate of 77.93%; the score of preparation ability dimension was (20.79±4.18)points, and the score rate was 69.30%; the score of rescue ability was (103.11±16.93)points, with a scoring rate of 73.57%. Conclusion The core emergency response capability of medical worker for 2019-nCoV and its dimensions are at a medium level. At the present stage, the training of core knowledge of medical personnel 2019-nCoV should be strengthened, effective measures should be taken to improve the alleviation of weakness in rescue, and the self-psychological adjustment of medical personnel should be strengthened to improve the core emergency response capability of medical worker.
2.Relationship between insomnia and suicidal ideation in adolescents with depressive disorder: the mediating role of rumination

ObjectiveTo explore the mediating effect of rumination on the relationship between insomnia and suicidal ideation in adolescents with depressive disorder, so as to provide guidance for the preventions of suicidal ideation. MethodsA total of 302 adolescents with major depressive disorder in Department of Psychosomatic Medicine of Deyang People's Hospital from January to December 2020 and met the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) clinical significance criterion for depressive disorder were enrolled. All individuals were assessed using Insomnia Severity Index (ISI), Ruminative Response Scale (RRS) and Positive and Negative Suicide Ideation (PANSI). Then the mediating effect of rumination in the relation between insomnia and suicidal ideation in adolescents with major depression disorder was analyzed using Process v3.2 model 4, and testified with the bias-corrected non-parametric percentile Bootstrap method. ResultsISI scores in adolescents with major depression disorder were positively correlated with PANSI score (r=0.400, P<0.01), and were positively correlated with RRS total score, obsessive thinking and introspection factor score (r=0.378, 0.360, 0.333, P<0.01). RRS score was also positively correlated with PANSI score (r=0.292, P<0.01). Rumination exerted a partial mediating effect on the relationship between insomnia and suicidal ideation (β=0.174, 95% CI: 0.098~0.261). ConclusionThe relationship between insomnia and suicidal ideation in adolescents with depressive disorder is partially mediated by rumination, additionally, insomnia can affect suicidal ideation in adolescents with depressive disorder both directly and indirectly through rumination.
3.Moderating effect of psychological resilience in relation to depression and suicidal risk among adolescents

ObjectiveTo investigate the moderating effect of psychological resilience in relation to depressive symptoms and suicidal risk among adolescents. MethodsThe research is a descriptive survey. A total of 71 137 adolescents were selected from 163 schools in Deyang by stratified cluster sampling. Their psychological resilience, depressive symptoms and suicide risks were measured oneline by using 10-item Connor-davidson Resilience Scale (CD-RICS-10), Patients’ Health Questionnaire Depression Scale-9 item (PHQ-9) and Suicidal Behaviors Questionnaire-Revised (SBQ-R). The moderating effect of psychological resilience in relation to depressive symptoms and suicidal risk was examined by multivariate stratified regression analysis. Results① The score of CD-RISC-10 was negatively related to PHQ-9 score and SBQ-R score (r=-0.305, -0.268, P<0.01). ② Psychological resilience significantly moderated the relationship between depressive symptoms and suicidal risk (β=-0.100, t=-31.716, P<0.01). ③ In both male and female adolescents, resilience played a significant role in depressive symptoms and suicide risk (β=-0.086, -0.084, t=-17.502, -18.839, P<0.01). ConclusionPsychological resilience could significantly alleviate the impact of high-level depressive symptoms on suicidal risk among adolescents, and this effects both male and female adolescents.
4.Effect of preoperative waiting time on prognosis of elderly patients with hip fracture.

OBJECTIVE:
To investigate the relationship between preoperative waiting time and prognosis of elderly patients with hip fracture.
METHODS:
From January 2014 to December 2018, 333 elderly hip fracture patients undergoing surgery were retrospectively analyzed, including 104 males and 229 females, aged from 60 to 99 years with an average of (77.93±8.49) years, and 183 patients were femoral neck fracture, 150 patients were femoral intertrochanteric fracture. Among them, 269 patients (80.78%) had a clustered preoperative waiting time of 2 to 8 days, and then divided into within 4-day group(91 cases) and over 4-day group(242 cases) according to their preoperative waiting time. The survival situation was followed by telephone, and follow-up time started from fracture admission to the death event, or to the research deadline (December 31, 2019). The Kaplan-Meier method was used for survival analysis, and Cox risk proportion model was used to analyze the independent risk factors of hip fracture in elderly patients.
RESULTS:
All patients were followed up for 12 to 75 months(means 35 months), 59 patients died and the mortality rate was 17.72%(59/333). Compared with within 4-day group, the mortality rate was higher in over 4-day group[20.66%(50/242) vs. 9.89%(9/91), χ2=5.263, P=0.022]. Multiariable Cox regression analysis showed that preoperative waiting time, age, male and Charlson comorbidity index were independent risk factors for the prognosis of hip fracture in elderly patients (all P<0.05), and every 1-day delay was associated with 5% increase of the risk of death[HR=1.05, 95%CI(1.00-1.10), P=0.045]. Subsequent analyse was stratified according to the Charlson comorbidity index (CCI), and found that over 4-day group had a higher mortality rate in patients with CCI<2, with statistically significant difference(P<0.05).
CONCLUSION
For elderly patients with hip fracture, most of hospitals could not complete the hip fracture surgery within 48 hours, we also need to shorten the waiting time before surgery, and thereby improve their prognosis.

5.The study of prevention the gustatory sweating syndrome and facial contour deformity with sternocleidomastoid muscle flaps in the parotidectomy

Objective:To assess the outcome for prevention the gustatory sweating syndrome and facial contour deformity with sternocleidomastoid muscle flaps in the parotidectomy. Method:It was a prospective study. Eighty-one patients who underwent parotidectomy were randomly divided into 2 groups, sternocleidomastoid muscle flaps group and control group. They were tested with the subjective and objective function testing in 3, 6, 9, 12 months after operation. We assessed the result. Result:All of the result of the sternocleidomastoid muscle flaps group was obviously better than the control group. There was significant difference between the 2 groups (P<0.05). Conclusion:It effectively prevented the gustatory sweating syndrome and facial contour deformity that we used the sternocleidomastoid muscle flaps to fill the cavity in the parotidectomy.

7.A cross-sectional survey on the epidemiology of HBV infection among HBsAg-positive people in Deyang Area

Objective To analyze and discuss the epidemiological characteristics of HBV infection in people with hepatitis B virus (HBV) surface antigen (HBsAg) positive in Deyang Area. Methods The health checkup information of the region in 2020 was selected to screen the subjects with the results of HBV serological markers, and 1645 HBsAg-positive patients were selected as the main subjects to analyze the epidemiological characteristics of the population. Use Epidata to enter data, and use SPSS statistical software package to analyze the data. Results Except for the differences in occupational distribution between the sexes, the differences in age, education level, family history, and vaccination history (hepatitis B vaccine) were not statistically significant among men and women in this study. Among the 1645 HBsAg-positive subjects, 41 were double-positive for anti-HBs at the same time, and the double-positive rate was 2.49%. Compared with HBs-negative people, HBs-positive people have a relatively higher proportion of males, junior high school education level, occupation as a farmer, unvaccinated, and positive family history. AmpliPrep/COBAS quantitative detection reagent was used for HBV DNA viral load detection, 92.69% were quantitatively positive (38 cases), of which 23 cases (60.53%) with HBV DNA < 20 IU/mL, and 15 cases (39.47%) with HBV DNA >20 IU/mL). The epidemiological characteristics of the double-positive population suggest that the proportion of males, junior high school education level, occupation as a farmer, unvaccinated and positive family history is relatively higher. Analyzing the potential influencing factors of anti-HBs positive (double positive) in HBsAg-positive people, it can be seen that the increase in age, the occupation is a farmer, the first injection of hepatitis B vaccine is not in time, and the family history of liver disease is positive as potential risk factors for the occurrence of double positive. Conclusion In the future, this region should pay special attention to farmers who are older, have a family history of liver disease, and lack or incomplete hepatitis B vaccination for hepatitis B prevention and control.
8.Risk assessment and screening of common malignant tumors among female urban residents in Deyang

Objective To analyze the screening results and risk assessment of common malignant tumors among female residents in Deyang from 2017 to 2019, so as to provide basis for cancer prevention and control. Methods Women who underwent physical examinations and completed the survey of common malignant tumors related risk factors from 2017 to 2019 were selected. The high risk rate, screening rate and detection rate of common malignant tumor among female urban residents in Deyang were analyzed. Results From 2017 to 2019, a total of 49,871 female urban residents completed the survey of common malignant tumor related risk factors. The risk rates of breast cancer, lung cancer, cervical cancer and colorectal cancer were 13.24%, 7.65%, 12.92% and 5.53%, respectively. Among all the age groups, women at the age of 50~59 years old were at the highest risk of breast cancer, lung cancer, cervical cancer, colorectal cancer; The screening rates of breast cancer, lung cancer, cervical cancer and colorectal cancer were 24.68%, 11.72%, 34.62% and 14.15%, respectively; The screening rates of breast cancer, lung cancer, cervical cancer and colorectal cancer among women with different educational levels showed significant difference (P<0.05). The screen rates of breast cancer, lung cancer, cervical cancer and colorectal cancer among high risk groups were 24.68%, 11.75%, 26.56% and 14.35%, respectively; The detection rates of breast cancer, lung cancer, cervical cancer and colorectal cancer among the screening groups were 9.33%, 2.68%, 9.82%, and 2.02%, respectively (P<0.05). Conclusion Female residents in Deyang from 2017 to 2019 have high risk and detection rates of breast cancer and cervical cancer, so active screening of malignant tumors among urban female residents can effectively improve the residents' quality of life, and is of vital importance for the early diagnosis and treatment of malignant tumors.
9.Effect of enhanced recovery after surgery on postoperative function and pain in total hip arthroplasty patients with high comorbidity.

OBJECTIVE:
To investigate the effects of enhanced recovery after surgery (ERAS) on postoperative function and pain in total hip arthroplasty (THA) patients with high comorbidity.
METHODS:
Patients with THA who were admitted between January 2020 and January 2022 were selected as the study objects, and a total of 223 patients with high comorbidity met the selection criteria. Patients were randomly divided into two groups using the random envelope method. During perioperative period, 112 cases in the ERAS group were treated according to the ERAS protocol and 111 cases in the control group with the traditional protocol. There was no significant difference in gender, age, body mass index, Charlson comorbidity index, preoperative diagnosis, the type and number of the comorbidities, preoperative visual analogue scale (VAS) score between the two groups ( P>0.05). However, the Harris score of ERAS group was significantly lower than that of control group before operation ( P<0.05). Preoperative and postoperative hospital stays were recorded. The VAS score was used to evaluate the pain before operation, at 1 day after operation, at the leaving bed time, at the day after discharge, and at 2 weeks after operation. Harris score was used to evaluate hip function before operation and at 2 weeks, 1 month, 3 months, 6 months, and 12 months after operation. The incidence of complications, 30-day readmission rate, mortality rate, and patient's satisfaction were recorded.
RESULTS:
The length of preoperative hospital stay in ERAS group was significantly shorter than that in control group ( P<0.05). But there was no significant difference in the length of postoperative hospital stay between groups ( P>0.05). All patients in the two groups were followed up 12 months. The VAS score in the two groups after operation was lower than that before operation, and showed a gradually trend with the extension of time, with significant differences between different time points ( P<0.05). VAS scores of ERAS group were significantly lower than those of control group at different time points after operation ( P<0.05). The postoperative Harris scores in both groups were higher than those before operation, and showed a gradually increasing trend with the extension of time, with significant differences between different time points ( P<0.05). Harris scores of ERAS group at 2 weeks, 1 month, and 3 months after operation were significantly higher than those of control group ( P<0.05). Complications occurred in 2 cases (1.79%) of the ERAS group and 6 cases (5.41%) of the control group, with no significant difference in incidence ( P>0.05). In the control group, 1 case was readmitted within 30 days after operation, and 1 case died of severe pneumonia within 1 year of follow-up. There was no readmission or death in ERAS group, and there was no significant difference in the above indexes between the two groups ( P>0.05). At last follow-up, the satisfaction rate of patients in ERAS group was slightly higher than that in control group, but the difference was not significant ( P>0.05).
CONCLUSION
For THA patients with high comorbidity, ERAS protocol can shorten preoperative waiting time, better reduce pain, and improve hip function.

10.Clinical observation of Guilong decoction combined western medicine in the treatment of rheumatoid arthritis with cold-dampness syndrome

Objective To observe clinical effects of Guilong decoction combined conventional western medi-cine in the treatment of rheumatoid arthritis with cold-dampness syndrome.Methods 106 cases of rheumatoid arthri-tis with cold-dampness syndrome were randomly divided into the treatment group and the control group,with 53 cases in each group.The control group was orally given methotrexate and hydroxychloroquine,while the treatment group was given Guilong decoction based on the treatment of the control group.The clinical effects of the two groups were observed after a treatment course of 3 months.Results In the treatment group,35 cases were markedly effective, which was significantly higher than the control group(P <0.05);12 cases were effective,and there was no statistically significant difference with the control group (P >0.05).The total effective rate of the treatment group was 88.68%, which was significantly higher than the control group (χ2 =4.810,P =0.028).There were no significant differences of the adverse reaction ratio between the two groups(P >0.05).Conclusion Guilong decoction combined with con-ventional western medicine can obviously improve the rheumatoid arthritis disease severities,the curative effect is re-markable.It is worth popularizing in clinical application.
