2.Transpedicular balloon kyphoplasty for aged osteoporotic spinal compressive fractures

Objective To investigate the results of transpedicular balloon kyphoplasty for treatement of aged osteoporotic spinal compressive fractures. Methods Ten cases of aged osteoporotic spinal compressive fractures (range 57 to 72 years old), with T11 1 case, T12 4 cases, L1 5 cases were treated with kyphoplasties under “C” armed imaging guiding. The inflatable bone tamp was inserted into the fractured vertebral body transpedicularly in a minimally invasive way. The balloon was inflated, elevating the endplate and restoring vertebral body height and then confirmed by “C” armed imaging. The balloon was deflated and withdrawn, leaving a cavity within the vertebral body. The cavity was then filled with bone cement or other preferred material, creating an “internal cast” which was also confirmed by “C” armed imaging. Results The back pain disappeared immediately after the kyphoplasty in all of 10 cases. In the same day or following day after the procedure, the patients were allowed to get off bed. The height restoration of vertebral body and correction of kyphosis were confirmed by X-ray after the procedure. The average correction of kyphosis was 16?. At the time of the last follow up, all 10 cases retained the life style as before injury, there was no back pain, and deformity correction was maintained well. No complications occurred. Conclusion Kyphoplasty is a safe and effective way to treat aged osteoporotic spinal compressive fractures.
3.Anterior cervical decompression for severe cervical spondylotic myelopathy with ossification of posterior longitudinal ligament

[Objective]To explore the clinical outcome of anterior cervical decompression for severe cervical spondylotic myelopathy with ossification of posterior longitudinal ligament. [Method]Thirty-three patients with severe cervical spondylotic myelopathy and continuous OPLL underwent posterior longitude ligament resection and floating in anterior cervical decompression.The intervertabral space were stabilized by autoilum graft or titanium cage and fixed with windows plates. [Result]All patients were followed up for 8 to 45 months,mean 22 months.Preoperative JOA scores were 6.7,and the postoperative JOA scores were 10.1 and 10.7(evaluated in 3 months and 12 months after surgery),the mean amelioration rate was 72.7% and 78.8%.Of these 33 patients,no severe complications such as cord or vertebral artery injury occurred. [Conclusion]Posterior longitude ligament resection and floating in anterior cervical decompression is a safe and effective treatment for severe cervical spondylotic myelopathy with ossification of posterior longitudinal ligament.
4.The Structural Characteristics and Mechanical Behaviors of the Intramedullary Expand Self-Locking Nail

The intramedullary expand self-locking nail(IESN) is a new type of solid small diameter nail consisting of the dynamic and the static types, designed on the basis of the CAIN. The mechanical experiments showed that its stiffness to resist the bending and twisting momentum was simillar to the Russell -Taylor's nail. The static nail was stable enough to prevent axial displacements of the fractured segments. The main characteristics of the IESN are as follows: 1) It is being able to be inserted without reaming the medullary cavity and without X- ray guidance; 2) a higher limit value to resist fatique and 3) no stress concentration on IESN.

Objective To investigate the effect of intraspinal grafting of brain derived neurotrophic factor (BDNF) ex vivo transgene myoblasts cells and methylprednisolone on caspase-3 expression after spinal cord injury (SCI). Methods A total of 120 experimental rats were divided into Group A (spinal cord contusion injury group), Group B (grafting of BDNF ex vivo transgene myoblasts cells group), Group C (methylprednisolone intravenous injection group) and Group D (grafting of BDNF ex vivo transgene myoblasts cells and methylprednisolone intravenous injection group). At days 1, 3, 7, 14 and 28 respectively after SCI, the expression of caspase-3 was measured immunohistochemically for quantitative analysis via a computer image analysis system. The motion functional recovery of the rats was observed by praxiologic and electrophysiologic examination. To sieve a new type of polyether polyurethane as an injectable prosthetic nucleus pulpous through testing the cytotoxicity in vitro.Methods Five experimental groups were set as following:that samples 1,2,3 groups were polyether-polyurethane polymerized of different ratio of soft and hard segment with different catalysts; group 4 was polyether polyurethane polymerized slowly at 37 ℃ without catalyst; group 5was polyether polyurethane that toxic monomer responded completely.Additionally,a control group of culture medium was set without material extracts.Get 0.6 mg from each sample product and make into material extracts after disinfection,then dilution them by 50% and culture the mouse fibroblasts(cell L929)until 3 d,4d,5 d,by Adopting 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl tetrazolium bromide(MTT)colorimetric method to compare the 490 nm OD between the materials and control groups obtained by enzyme linked immune analysis,calculate the RGR of cells,and then evaluating the cytotoxicity of the materials.Results The RGR of L929 cell in group 4 and group 5 have no significant difference compared to the control group.Cytotoxic levels were 0 to 1; difference of the RGR of L929 cell between group 1-3 and control group were statistically significant.Cytotoxic levels were 1 to 2,but the cell morphology was normal.Conclusion The material polymerized without catalyst does not show cytotoxicity,and has great potential to be used as a substitute for nucleus pulposus.The cytotoxicity of the materials polymerized with catalyst needs to be tested more.

Objective To detect the genetic toxicity of a neotype in situ polymeric injectable artificial prosthetic nucleus pulposus.Methods The artificial prosthetic nucleus pulposus was soaked for preparing the leaching liquor which was used for tests of genetic toxicity.Salmonella typhimurium reverse mutation test(Ames Test),mammalian cell chromosome aberration(CA) test utilizing Chinese hamster ovary cells(CHO),and mouse micronucleus(MN) test were performed to detect the genetic toxicity of the extraction,including the effects on DNA,chromosome aberration and genetic mutation.Results The number of reverse mutation strains,from five strains of every dosage group,was all lower than half of quantum of control group,with or without the addition of S9,in the Ames Test,which assessed as negative.In CA test,no significant difference of chromosome aberration rate existed among the high,medium and low concentration group,while the CA rate of all the three groups was lower than that of the negative group(P0.05),but significantly lower than that of the positive control group(P
8.Quality of vision related survival among primary and secondary school students with different myopia correction status

Objective:
To analyse the influencing factors of myopia correction among primary and secondary school students with myopia and to compare the quality of visionrelated survival of students with different correction conditions.
Methods:
A total of 3 649 students from eight primary and secondary schools in a district of Chongqing were selected for myopia screening and vision related survival quality questionnaires using a stratified cluster random sampling method.
Results:
Univariate analysis showed that there were statistically significant differences in the rate of wearing eyeglasses among myopic students with different gender, school, educational stage, academic stress, and parenting style ( χ 2=10.98, 31.63, 31.86, 11.28, 9.59, all P <0.05). The fully vision correction rate among students wearing eyeglasses differed in educational stage, academic stress and parenting style ( χ 2= 11.77, 9.92, 8.69, all P <0.05). Multivariate Logistic regression analysis showed that gender, school and educational stage were all influencing factors of the rate of wearing eyeglasses (all P <0.01). Myopic students total scores on the Quality of Survival Scale were significantly higher than those of non myopic students ( t =20.07, P <0.01). There were significant differences in the scores of physical, emotional, visual and physical functioning and the total score of quality of survival scores among the three groups of myopic students without wearing eyeglasses, students with undercorrection and students with full vision correction ( F=49.93, 49.38, 28.14, 67.31, P <0.01).
Conclusion
Myopic students have low rates of wearing eyeglasses and fully vision correction. The rate of wearing glasses is different in urban and rural areas, gender and educational stage. The quality of life among myopic students is decreased by wearing eyeglasses, and the quality of life of undercorrected students is lower than that of fully corrected students. We should pay attention to the correction of myopia and improve the quality of life among myopic students.
9.Experimental study of online teaching based on reflective experience in the cultivation of professional core competence of nursing

Objective:To evaluate the practical effect of online teaching based on reflective experience to promote the cultivation of professional core competence of nursing.Methods:A total of 160 intern nurses who entered the clinic from July 2019 to December 2019 in The Affiliated Hospital of Inner Mongolia Medical University were selected as the research subjects, and they were randomized into a control group and an experimental group, with 80 nurses in each group. Both groups were given the creation of clinical practice teaching online classroom under the existing nursing online teaching framework. The control group conducted general online course education, and the experimental group created a problem-solving reflective experience course task and teaching process from the core competence training content of nursing. After the internship, the core competence assessment of the standardized training for nursing major was conducted, and the difference in teaching was evaluated using the "Nurse Core Competency Scale" and the self-designed "Teaching Acceptance Questionnaire". SPSS 22.0 was used for t-test and Chi-square test. Results:The excellent rates of the professional theory and practical skills assessments of the students in the experimental group were 58.75% (47/80) and 51.25% (41/80), which were higher than those in the control group, 48.75% (39/80) and 31.25% (25/80). The differences between the two groups were statistically significant ( P<0.05). In the comparison of the "Core Competency Scale" of nurses between the two groups of nurse students, the total score of the experimental group and the score of each dimension were significantly higher than those of the control group ( P<0.05). They survey of "Teaching Acceptance Questionnaire" showed that the nursing students of the experimental group had higher recognition of the teaching methods of this group, and there were significant differences between the two groups ( P<0.05). Conclusion:Carrying out online training content of clinical practice teaching based on reflective experience is conducive to the establishment of core competence of nursing students and the improvement of clinical practice ability.
