3.Two Cases of Acute Coalescent Mastoiditis Complicated by Subperiosteal Abscess.

In the era of antibiotics, acute coalescent mastoiditis is a very uncommon event, but remains a potentially serious condition. Significant and even life-threatening complications beyond the tympanomastoid space may occur, including periauricular subperiosteal abscess, facial paralysis, meningitis, brain abscess, etc. However, there have appeared a few papers in the recent Korean literature on acute coalescent mastoiditis. Recently, we encountered with two cases of acute coalescent mastoidtitis complicated by subperiosteal abscess. Including incision and drainage of the abscess, parenteral antibiotic therapy was done for several days until the acute stage of infection was controlled. After that, simple mastoidectomy with ventilation tube insertion was carried out. We report these cases with review of literature.

4.Two Cases of Isolated Sphenoid Sinus Aspergillosis.

Aspergillus sp. are the most common contaminants found in the paranasal fungal sinusitis, but isolated sphenoid sinus invasion is extremely rare. According to sinus anatomy and depending on its contiguous structures, various symptoms and complications may appear. Of these symptoms and complications, headaches are the most frequently observed in the few documented cases. The treatment is primarily surgical, through either transnasal or transethmoid approach, depending on whether or not the disease is limited to the sphenoid sinus. The transnasal endoscopic technique offers excellent visualization and an atraumatic approach to the sphenoid sinus. It is performed with the superior turbinate used as the key landmark for identification. For this technique, a wide enlargement of the natural sphenoid ostium is also important. Along with a review of literature, we present two cases of sphenoid sinus aspergillosis which were successfully treated by transnasal endoscopic sphenoidotomy.

5.A Case of Shotgun Wound on Head and Neck.

Shotgun injury compared to gunshot injury has been reported to have tendency to make less severe damage due to its characteristic low velocity and fragmentation. Recently, we encountered with a patient who survived from shotgun injury. She shot herself for the purpose of suicide with a hunting gun. The multiple fragmented bullet particles were found on the nasal septum, inferior turbinate, near the sphenoid sinus, and especially in the esophagus. They were removed safely with nasal endoscope, C-arm, and esophagoscope. We report this case with review of literature.

6.A Case of Bilateral Chylothorax Following Neck Dissection without Chyle Leakage of Neck.

A chylous fistula has been reported to occur in 1% to 2% of cases as a complication of neck dissections, but a bilateral chylothorax following neck dissection, without concurrent chylous leakage at operation site, was very rare. In the chylothorax, serious problems may occur in cardiopulmonary function, metabolism, and nutrition of patient. Early diagnosis and proper management is very important. Recently authors experienced a case of bilateral chylothorax following neck dissection and managed with conservative treatment.
