Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive dermal and subcutaneous mesenchymal neoplasm that often presents as a slow-growing lesion on the trunk, proximal extremities, head and neck. We report a rare case of DFSP arising on the dorsum of the left hand. A 35-year-old male, who presented with a solitary verrucous nodule on the dorsum of the left hand and had experienced two recurrences of the lesion following local excision and electrocautery. Histopathologic examination was consistent with DFSP. CD34 immu-nostain was positive. Wide surgical excision with a three centimeter margin and direct skin closure was done. Although metastasis is rare, DFSP is associated with marked tendency towards local recurrence after surgery hence patient is for follow-up every six to twelve months post operatively.

5.Dermatofibrosarcoma Protuberans on the leg: Pearls in diagnosis and surgical management

Introduction:
Dermatofibrosarcoma protuberans (DFSP) is a rare, slow-growing soft tissue tumor accounting for less
than 0.1% of all malignant neoplasms and approximately 1% of all soft tissue sarcomas1
.
Case Summary:
We report a case of a 27-year-old female who presented with a 1-year history of a gradually enlarging
firm, erythematous to violaceous nodule on the right anterolateral leg. Shave excision biopsy was done, and revealed
spindle-shaped fibroblasts arranged in a storiform pattern around an indistinct vasculature. Histopathologic diagnosis
revealed dermatofibrosarcoma protuberans. There was prominent, diffuse, positivity for CD34 by
immunohistochemistry. The cytogenetic analysis revealed a t(17;22) translocation, confirming our working
impression.The patient underwent wide excision with frozen section. All surgical margins of resection were negative for
tumor. The patient was referred to plastic surgery for post-excision coverage defect. Split-thickness skin graft was
applied over the defect. Patient tolerated the procedure well, and has not experienced recurrence of the tumor.
Conclusion
A protuberant, well-circumscribed tumor that seems benign may incidentally be a sarcoma. Skin biopsy,
immunohistochemistry and cytogenetic studies using Fluorescence In-Situ Hybridization (FISH) are the most essential
laboratory investigations to validate a diagnosis of DFSP. Proper surgical excision with adequate margins will prevent
recurrence of the tumor.

Dermatofibrosarcoma protuberans is an unusual locally aggressive cutaneous neoplasm of low grade malignancy. We report the case of a 47 years- old male who presented with an asymptomatic erythematous firm protruding mass in the left peri-inguinal area. Histopathologically, the tumor showed spindle-shape cells, arranged in a storiform pattern. The tumor cells stained positively for Vimentin and CD34 on immunohistochemical staining. Herein, we report a case of dermatofibrosarcoma protuberans.

Dermatofibrosarcoma protuberans (DFSP) is typically diagnosed at the tumoral stage. But it is clinically important to recognize that DFSP often begins as a non-protuberant plaque and occasionally persists as such. Recently the cases of so-called atrophic DFSPs have been reported, and none of these cases are different from classical DFSP in terms of histopathology and prognosis. Better knowledge of the initial appearance is important for making an early diagnosis and for easier surgical treatment. We report a case of non-protuberant DFSP presenting as a pigmented plaque.
