1.Study on the Blood Picture of Various Dermatoses.

So far today the blood Picture of specific dermatoses such as leprosy, syphilis, skin tubercuIosis, pemphigus and eczema was studied a good many ways. Author has studied the blood picture especially red blood cells, hemoglobin, hematocrit, white blood cells and its differential counts in 217 cases of various skin diseases such as contact. dermatitis, urticaria, chronic eczema, atoPic dermatitis, seborrheic dermatitis, erythema multiforme, erythemanodosum, drug eruption, prurigo, lichen urticatus, exfoliative dermatitis and purpura. At the same time liver function tests were performed in 46 cases including 25 cases of contact dermatitis, 6 cases of chronic eczema, 5 cases of atopic dermatitis, 5 cases of urticaria and 5 cases of exfolistive dermatitis. Afterwards the correlations of those two groups were investigated. Among all mentioned above dermatose except purpura no changes of red blood cells, hemogiobin and hematocrit were observed according to the authors own control of normal values which were obtained in 62 normal healthy adults. Other findings are as follows; 1. In contact dermatitis leucocytes and eosinophils are increased while neutrophils slightIy decreased. 2. Moderate increase of lymphocytes in urticaria. 3. In chronic eczema leucocytes and lymphocytes slightly increased but neutrophils somewhat decreased. 4. In atopic dermatitis leucocytes and eosinophils increased and there was also moderate increase of lyrnphocytes. 5. In seborreheic dermatitis monocytes and basophils were increased but eosinophils were slightly increased. 6. In erythema multiforme monocytes were decreased and leacocytes increased slightly. 7. In erythema nodosum neitrophils increased and lymphocytes and monocytes were decreased. 8. In drug eruption monocytes were decreased. 9. ln Prurigo eosinophils were increased. 10.:ln lichen uiticatus neutrophils were decreased. 11. In exfoliative dermatitis there are marked increase of leucocytes and eosinophils and decrease of neutrophiIs, lymphocytes and basophils. 12. In purpura red b!ood cells, hemoglobin, hematocrit were decreased. The increase of white blood cells in contact dermatitis and exfoliative dermatitis almost paralleled with the extension of their lesions. Marked liver function disturbarces were found in a good many cases cf exfoliativedermatitis while slight degree of liver function disturbances were found in some cases of contact dermatitis and it was found that some cases of chronic eczema, urticaria and atopic dermatitis gave some influence to liver function. The fact of leucocytes increase in exfoliative dermatitis and contact dermatitis seems more or less go parallels with liver function disturbances.

2.A Statistical Study of Dermatoses in Goyang City (2004~2008).

BACKGROUND: There have been many reports that have focused on the incidence of skin diseases in big cities or provinces. However, there have been no reports concerning the data of a new town such as Goyang City. OBJECTIVE AND METHODS: To study the prevalence of common dermatoses in the new town and to compare this with the previous reports, we reviewed the records of 15,064 outpatients who visited the Department of Dermatology at Myongji Hospital from 2004 to 2008, and we statistically analyzed the findings. RESULTS: The study results are summarized as follows: Of the 15,064 outpatients, the total number of male patients was 6,779 (45.0%), and the number of female patients was 8,285 (55.0%). The most frequent age group was the fourth decade followed by the first decade, third decade and fifth decade, and these 4 age groups comprised 63.7% of the total outpatients. The 15 most common dermatoses were found to be viral warts, dermatophytosis, atopic dermatitis, seborrheic dermatitis, pruritus, urticaria, irritant contact dermatitis, herpes zoster, alopecia, allergic contact dermatitis, acne, other eczema, melanocytic nevus, molluscum contagiosum and postinflammatory hyperpigmentation. For the occurrence of dermatoses as disease groups, 1) viral diseases, 2) diseases resulting from fungi and yeasts, 3) atopic dermatitis, eczema and noninfectious immunodeficiency disorders, 4) contact dermatitis and drug eruptions, 5) pruritus and neurocutaneous dermatosis, 6) seborrheic dermatitis, psoriasis, recalcitrant palmoplantar eruptions, pustular dermatitisand erythroderma, 7) erythema and urticaria, 8) Melanocytic nevi and neoplasm, 9) disturbances of pigmentation, and 10) diseases of skin appendages constituted 82.2% of the total. CONCLUSION: The distinct characteristic of skin diseases of the new town Goyang City showed a high incidence of viral warts and viral skin diseases, as compared to the previous reports.

3.Clinical experience with Fluocinonide cream.

Lidex(Fluacinonide) 0.05% in FAPG base is a new fluorinated topical corticosteroid. 30 patients with different steroid responsive dermatoaes were treated with 0. 05% Lidex. The diagnosis included atopic dermatitis, contact dermatitis, neurodermatitis, seborheic dermatitis, nummular eczema, psoriasis vulgaris, chronic eczema and pustulosis pahmaris et plantaris. In the majority of cases Lidex cream produced excellent to good result(70%) after unoccluded application 3 times daily for 10 days. Two patients complained burning or itching sensation after the initial application of Lidex. 0.05% Lidex was compared with 0. 25% Desoxymethasone cream and 0. 25% Fluocortolone cream and was found slightly more effective in Lidex.

4.Clinical Trials of Frason on Various Dermatosis.

Esperson(R), a new topical steroid ointment containing 0. 05% 17-Desoxymethasone,. was used in several kinds of dermatoses. Framycetin which was isolated and uientified by the research organisation of Roussel Laboratories (SOFRAPEN) in 1950 from culture broths of a strain of Streptomyces species is a wide spectrum antibiotics on various bacterial infectious diseases. Total 50 patients including 10 cases of 2ndary infected Atopic dermatitis, 10 of infected Contact dermatitis, 6 of infected Seborrheic dermatitis, 7 of infected Neurodermatitis, 3 of infected Chronic dermatitis, 5 of lmpetigo, 2 of infected Nummular eczema, 2 of infected Infantile ecze;na, 1 of infected Intertrigo, 1 of Eczematoid Otitis extema, 1 of 2nd degree Bum, 1 of infected Pruritis ani, 1 of infeeted 1nsect bite. were treated with Frason(R) (0.05% 17-desoxymethasone & 0.5% Framycetin combination). Among these 50 patiants, we observed excellent effects in 7 patients (14%), good effect in 29 patients (58%), fair effect in 10 patients (20%), Totally 36 patients (72%) revealed excellent to good therapeutic effects on their skin conditione after unoccluded application of Frason(R). 3 times daily for about 14 days. There was no sid ffect in all subjects after application of Esperson-F.

5.Clinical Trials of Demacre on Varioys Dermatoses.

Demacre', a new topical steroid cream containing 0.025% beclomethasone dipropionate, was used in several kinds of dermatoses. Total 35 patients including ll cases of contact dermatitis, 5 of atopic dermatitis, 3 of hand eczema, 3 of seborrheic dermatitis, 2 of neurodermatitis, 2 of nummular eczema, 2 of autosensitization dermatitis, 1 of chronic eczema, 3 of psoriasis, 1 of insect bite, 1 of miliaria rubra, and 1 of intertrigo were treated by Demacre'. Among these patients, we observed excellent effects in 4 patients (11%), good effects in 22 patients (63%), and fair effects in 3 patients (9%). Namely, among these 35 patients, 26 patients (74%) revealed excellent to good therapeutic effects on their skin conditions with Demacre'. No untoward side effects were noted in all subjects treated with Demacre.

6.Clinical Trials of Esperson on Various Dermatoses.

Esperson, a new topieal steroid ointment containing 0. 25% 17-desoxymethasone, w as used in several kinds of dermatoses. Total 32 patients including 8 cases of contact dermatitis, 2 of atopic dermatitis, 5 of seborrheicdennatitis, 3 of neurodermatitis, 4 of nummular eczema, 2 of hand eczema, 1 of chronic eczema, 1 of perioral dermatitis, 5 of psoriasis and 1 of pityriasis rosea were treated with Esperson. Among these 32 patients, we observed excellent effects in 11 patients (34.4%), good effects in 16 patients (50%) and fair effects in 4 patients (12. 5%). Namely, among these 32 patients, 27 patients (84. 4%) revealed excellent to good therapeutic effects on their skin conditions with Esperson. In some details, 22 out of 26 patients (84.6%) with eczema and 4 out of 5 patients (80%) with psoriasis revealed excellent to good therapeutic effects respectively. No untoward side effect was noted in all subjects treated with Esperson.

7.A Statistical Study of Dermatoses in the Gangnam Area of Seoul.

BACKGROUND: There has been no statistical study of dermatoses in the Ganganm area of Seoul which has been rapidly developing in the last 10 years. OBJECTIVE: We evaluated the recent distributions of dermatoses in the Gangam area of Seoul and compared them with the previous reports in Seoul and other provinces of Korea. METHODS: 47,015 new outpatients who visited the Yongdong Severance Hospital from 1983 to 1992 were analyzed statistically RESULTS: The study results are summarized as follows: 1. Among the 47,015 outpatients, the total number of number of male patients were 20,180(42.9%) and female patients 26,835(57.1%). 2. In age distribution, the most frequent age groups were the 3rd decade(21.3%), the lst decade(19.5%), and the 4th decade(19.2%) amounting to 60% of the total outpatients. 3. The most common 15 dermatoses were dermatophytosis, atopic dermatitis, urticaria, irritant contact dermatitis, acne vulgaris, allergic contact dermatitis, seborrheic dermatitis, other eczemas, warts, vitiligo, drug eruption, herpes zoster, psoriasis, bacterial infection, and syphilis. 4. Those dermatoses which showed an increasing tendency annually were seborrheic dermatitis, warts, vitiligo, and syphiiis. Dermatophytosis and bacterial infection showed a decreasing tendency. 5. Seasonal distribution showed that those diseases occurring most frequently during the summer were dermatophytosis, urticaria, allergic contact dermatitis, vitiligo, herpes zoster, and bacterial infection. The incidence of atopic dermatitis and psoriasis increased during the winter season. 6. In sexual distribution, acne, allergic contact dermatitis, and irritant contact dermatitis were more frequent in females, and the remainding common dermatoses did no show any difference. 7. In age distribution, atopic dermatitis and bacterial infection were most frequent in the lst decade. Urticaria, allergic contact dermatitis, acne, drung eruption, psoriasis, and syphilis were most frequent in the 3rd decade, with warts and vitiligo in the 2nd decade, and herpes zoster in the 6th decaded. 8. The distributions of dermotoses as disease groups were eczema(24.9%), urticaria-drug eruption (13.2%), dermatophytosis(10.7%), skin appendage disorder(10.1%), viral infection(10.1%), pigmentary disorders(4.5%), and papulosquamous diseases(4.0%) which constituted 78% of the total. 9. In patients dwelling in apartments, atopic dermatitis and molluscum contagiosumn showed a more frequent tendency in comparison to those living in house. 10. The most common 10 dermatoses among children were atopic dermatitis, urticaria, contact dermatitis, molluscum contagiosum, bacterial infection, dermatophytosis, vitiligo, other viral infections, and nevo-cellular nevi. On the other hand, the most common 10 dermatoses among the elderly were drmatophytosis, other eczemas, urticaria, herpes zoster, pruritus, contact dermatitis, drug eruption, lichen simplex chronicus, seborrheic dermatitis, and vitiligo. CONCLUSION: In contrast to earlier published report from Seoul, the infectous dermatoses, especially parasitic infestation, showed a decreasing tendency. The distribution pattern of the skin diseases did not differ fro other provinces of Korea in general.

8.Clinical Features of Dermatology-Consulted Inpatients: Focus on the Differences between Individual Departments.

BACKGROUND: A variety of dermatological disorders develop in hospitalized patients and the need for dermatological consultations is on the rise. OBJECTIVE: We analyzed 2 years of dermatological consultation data from a tertiary medical center in Korea and compared dermatological problems among individual departments. METHODS: We reviewed 3,102 dermatological inpatient consultations by performing a retrospective chart review. RESULTS: Contact dermatitis (11.4%), drug eruption (9.6%), tinea pedis (5.5%), xerotic eczema (5.3%), and seborrheic dermatitis (5.2%) were the 5 dermatological disorders that were most commonly referred to dermatologists. Patients in the allergy and pulmonology departments had higher rates of drug eruptions. Endocrinology-admitted patients, especially diabetic patients, frequently complained of foot dermatitis. The cardiology, gastroenterology, and hemato-oncology departments referred many patients with purpuric dermatosis to dermatologists. Patients from the surgical departments consulted more frequently consulted contact dermatitis and drug eruptions. The neurology and rehabilitation medicine departments referred more seborrheic dermatitis patients than other departments. Pediatric patients commonly consulted for atopic dermatitis, viral exanthemata, and seborrheic dermatitis. Regardless of department, patients who suffered from severe illness and underwent major surgery developed herpes zoster and cutaneous fungal infections more frequently than other patients. Personal hygiene was closely related to the development of follicular disorders. CONCLUSION: Dermatologists should be familiar with the diverse dermatological complaints of patients admitted to various departments and be actively involved in the diagnosis, treatment, and education of doctors and patients to improve the quality of inpatient care.

9.Clinical Study of Dermatologic Disorders in Patients with Breast Cancer.

BACKGROUND: In Korea, breast cancer is the most common cancer in female and its incidence is increasing recently. Moreover, breast cancer is the most common cutaneous metastasis in female. However, dermatologic disorders manifested in patients with breast cancer have not been investigated yet. OBJECTIVES: Our purpose was to study the clinical characteristics of dermatologic disorders in patients with breast cancer. METHODS: A clinical observation and review of medical records were made by analyzing 359 cases of breast cancer who visited Our Lady of Mercy hospital from 1994 to 2003. Among them, 91 cases of the patients with breast cancer showed dermatologic disorders. Their photographs of cutaneous findings and histologic specimens were reviewed by more than 2 dermatologists. RESULTS & CONCLUSIONS: 1. The mean age at the diagnosis of breast cancer was 48.74+/-11.28 years old. 2. The stages of patients with breast cancer were as follows: stage 0 (4 cases), stage 1 (48 cases), stage 2 (196 cases), stage 3 (94 cases), stage 4 (2 cases), and others (15 cases). 3. The treatments of patients with breast cancer were as follows: surgery (358 cases), radiation therapy (115 cases), chemotherapy (256 cases), combination of chemotherapy and radiation therapy (82 cases), and others (9 cases). 4. The numbers of patients developing cutaneous infections among the patients with breast cancer were as follows: bacterial infection (18 cases), fungal infection (18 cases), and viral infection (14 cases). 5. The numbers of patients developing pruritus and eczema among the patients with breast cancer were as follows: contact dermatitis (7 cases), seborrheic dermatitis (3 cases), neurodermatitis (2 cases), and others (9 cases). 6. The numbers of patients developing treatment-associated dermatologic disorders among the patients with breast cancer were as follows: urticaria (11 cases), radiodermatitis (8 cases), drug eruption (6 cases), and lymphedema (1 case). 7. The number of patients developing skin metastasis among the patients with breast cancer was 10 cases. 8. The numbers of patients developing other dermatologic disorders among the patients with breast cancer were as follows: psoriasis (4 cases), erythema multiforme (2 cases), and others (16 cases). 9. The numbers of patients developing postoperative dermatologic complications were as follows: seroma (28 cases), arm swelling and lymphedema (9 cases), wound infection (6 cases), and others (5 cases).
