The present study was carried out to find a simple and safe in. vitro test for phtotoxic drugs. Authors selected two strains of Salmonella typhimurium(TA98 and TA102) which have been used in Ames test for the detsction of mutagenecity af various chemical substances. Both strains are genuine products of genetic enzineering. The etrain TA98 should be highly vulnerable to ultraviolet radition because it lacks normal I)NA excision repair gene. The strain TA102 was chosen as control since it maintained the DNA repair gene. These strains were subjected to increasing dosea of UVA with or without pretreatment of 8 methoxypsoralsn(8-MOP) which is a prototype of photatoxic druge. The authors made use of a perforated stain-less steel template which provided a simple and eosy monitoring of ultraviolet irradiation effects i.e. clear zones due to inhibition of the atrains could be determined. By using this methad, the authors acquired the following results .' I. 8-MOP alone exerted no inhibition on both strains at concentration upto 100mg /ml. 2. UVA irradiation alone showed no growth inhibition at dose upto 5J/cm. 3. UVA irradiation after pretreatment with 8 MOP resulted varying growth inhibition in proportion to irradiation doses. 4. Authors found a suitable concentration of 8-MOP for this test is 10pg/ml. With this ccncentrstion, minimal phatatoxic dose of UVA were O.l J/cm for the strain TA98 and 1.0J/cm for the strain TA102 respectively.

We report a case of doxycycline-induced phototoxicity in a 62-years-old male. The patient, had erythematous macules and patches on sun-exposed areas. A Phototest revealed a marked decreased minimal erythemai. dose (MED) to UVA (MED(UNA)=-10J/cm2). Photopatch tests with 1%, 5% Doxycycline ointment were negative. An oral provocation test was performed, which showed a positive result with a decrease of MED to UVA (MED(UVA)= 20J/cm2). After the cessation of doxycycline, his skin lesioris were improved markedly with complete loss of photosensitivity.

Eccrine squamous syringometaplasia (ESS) is a histologically distinctive skin eruption occurring predominantly in acral or intertriginous areas presenting as erythematous macules, papules or patches. The etiology of ESS remains unclear, but it is usually reported in patients receiving chemotherapy for various malignant neoplasms. To date, only two cases of ESS associated with non-steroidal anti-inflammatory drugs (NSAIDs), which has distinctive clinical features and pathogenesis, have been reported in the literature. Herein, we report a rare and interesting case of ESS associated with pelubiprofen, a recently developed NSAID, which appeared after pelubiprofen therapy and resolved spontaneously after discontinuing the medication.

Interview survey and dermatological examination have been performed to investigate the health problems of workers continuously exposed to coal-tar pitch. The phototoxicity of coal-tar pitch was confirmed by the photopatch tests for six healthy adults. The main results are followings; 1. There was no special history of allergic diseases in both the exposed and non-exposed group. 2. The frequency of the phototoxic dermatosis and the coal-tar acne in the exposed group was significantly greater(p<0.05) than that of the control group. In the exposed group, the phototoxic dermatosis and the coal-tar acne were observed in nine workers(90%) and seven workers(70%), respectively. However, those disease were not observed in the control group. 3. Five results(83%) were positive to the photopatch test for coal-tar pitch 48 hours after UVA irradiation. But the lesion was subsiding 72 hours after UVA irradiation. 4. Malignant cancers were not reported among workers ever exposed to coal-tar pitch. In conclusion, it appears that workers exposed to coal-tar pitch have high risks of phototoxic dermatosis and coal-tar acne. A health policy should be provided to prevent phototoxic dermatosis among coal-tar pitch workers. More studies are required to determine malignancy.

Even though phototherapy is an effective means of treating hyperbilirubinemia, adequate binocular shields are essential to prevent light hazard of retina during phototherapy. The phototherapy to young albino rabbits was performed under various conditions; exposured for 12, 24, and 48 hours, continuity or intermittence of irradiation, and use of shields. Although there was no definite pathologic change in indirect ophthalmoscopic examination, severe damage was seen at electron microscopic level. In all experimental groups, swelling and edema of photoreceptor were seen. at right after phototherapy. Futhermore, more severe damage was seen in case of intermittent irradiated group and prolong exposed group. In mydriatic eyes, severity of damage was similiar to non mydi iatic eyes, but area of retinal damage was more extensive than non-mydriatic eves.. In control group which was protected by binocular shields during phototherapy, no significant abnormality was found.

A 72-year-old woman developed pruritic erythematous patches on sun-exposed areas for 2 months following treatment with hydrochlorothiazide (Dichlozid) for hypertension. A phototest revealed a decreased minimal erythemal dose to UVA (MED(UVA)=2 J/cm²). A photopatch test with 0.5%, 1%, and 5% hydrochlorothiazide ointment revealed no response. Two weeks after discontinuation of hydrochlorothiazide, the skin lesions improved with complete loss of photosensitivity without any therapy. At that time, an oral provocation phototest with hydrochlorothiazide was performed and it showed a decreased MED to UVA (MED(UVA)=2 J/cm²).

Afloqualone is a centrally acting muscle relaxant and has been used in therapy for musculotensive diseases. A 35-year-old female had been taking afloqulaone for a left hemifacial spasm for one month. The patient visited our hospital with erythematous papules and patches on the sun exposed area. A photopatch test with afloqualone ointment with the concentration of 0.1%, 1%, 5% turned out negative. An oral provocation test was also done and it showed a positive result with a decrease of MED to UVA (10 J/cm²).

BACKGROUND: Although dermatologists may encounter photosensitivity patients commonly, most dermatologists are not familiar with diseases manifesting photosensitivity and there is still no clinical study on photosensitivity disease in Korea. OBJECTIVE: We tried to investigate incidences and sorts of photosensitivity diseases with clinical informations. METHODS: We performed a retrospective clinical review of patients who had photosensitivity as a chief complaint in the department of dermatology, Gyeongsang National University hospital from 1991 to 2000. RESULTS: There were a variety of diverse photosensitive diseases in this clinical study. Drug-induced photosensitivity including phototoxicity and photoallergy were fairly common (19.7%), and causative drugs were various including antibiotics, diuretics and even medicinal herb. We could find co-occurrence of some diseases in photosensitivity patients (39.3%), most of which were chronic degenerative diseases. CONCLUSION: Further clinical studies will be needed to get additional and standardized information in Korea, and precise clinical work-ups would be necessary to differentiate variable spectrums of photosensitivity diseases we showed in this study.

BACKGROUND: Solar ultraviolet (UV) radiation induces sunburn, immune suppression, and various pigmentary disorders. Sunscreens are widely used to protect those untoward effects by UV but there are reports of phototoxicity or stability problems of sunscreens after exposure to UV. OBJECTIVE: We tried to compare sunscreens with different photostability in terms of their protection against various biologic responses like sunburn, immune suppression or pigmentation. METHODS: Three different sunscreens with SPF around 30 were used; Sunscreen-A (Sc-A) was photochemically inert, sunscreen-B (Sc-B) showed intermediate level of photostability, and sunscreen-C (Sc-C) was the least stable. To observe their in vivo effects, we measured sunscreen-protection against sunburn by back-skin swelling and sunburn cell formation, against immune suppression measured by depletion of Langerhans cells, local and systemic suppression of contact hypersensitivity (CHS), and against pigmentation by irradiation with mixed light source with UVA and UVB lamps that mimic solar UV spectrum. RESULTS: Back skin swellings by 5 kJ/m2 of UVB were protected well by sunscreens, but protection of Sc-C against 50 kJ/m2 of UVB was worse than Sc-A or Sc-B. Sunburn cells were increased significantly in mice irradiated with 5 kJ/m2 of UVB and it was protected by sunscreens, and the effect of photostability was minimal. Depletion of epidermal Langerhans cells by 5 kJ/m2 of UVB was protected completely by sunscreens. Local suppression of CHS by 5 kJ/m2 of UVB was protected by sunscreens, and Sc-A had better protection. But, in the experiment with 50 kJ/m2 of UVB, the protective efficacy was reversed; Sc-A showed worse protection. Systemic suppression of CHS by 10 kJ/m2 of UVB was protected well by sunscreens, and Sc-A had better protection and Sc-C had worse protection. In the experiment irradiated with 100 kJ/m2 of UVB, the protection of sunscreens was decreased, and Sc-B showed better protection, whereas Sc-C showed worse protection. In UV-induced pigmentation, all three sunscreens showed significant protection both by L* value and individual topographic angle (ITA) with the best protection by Sc-A and the worst protection by Sc-B. CONCLUSION: These data showed sunscreens can protect various in vivo responses and photostability of sunscreens played important roles particularly in the back-skin swelling and systemic suppression of CHS by high dose of UVB.

Background: If phototoxic:ity can be predetermined in vitro, the information will aid in prognosticating whether or not caimpounds have a photosensitizing pczrlial. OBJECTIVE: For the evaluation of the phototoxic potentials of reral drugs, we performed the Candida albicans test and the photohemolysis test. METHODS: The Candida altiicans test is based on growth inhibtion of Candida albicans after application of the drug and ultraviolet light A( UVA ) irradiatior and the photohemolysis test is based on hemolysis of red blood cells caused by irradiation ir the presence of photosensitizing compounds. In the Candida albicans test, clear zones around the drug patches were evaluated, which means positive results for the phototoxic potential of the drugs. In the photohemolysis test, the amounts of hemolysis were evaluated by measuring the relar absorbance at 540nm using a spectrophotometer. RESULTS: In the Candida albicans test, ibuprofen, naldix acid, chlorpromazine and thiodiphenylamine showed positive results, whereas others did not the photohemolysis test, griseofulvin, ibuprofen and nalicdixic acid showed increased amounts hemolysis at UVA and ultraviolet light B(UVB) irracliation, and chlorpromazine, thiodiphenylaiair, chlorothiazide and piroxicam showed increased amounts of hemolysis at UVA irradiation only. CONCLUSION: The results showed that both methods were goodness screening tests for demonstrating the phototoxicity of therapeutic drugs.
