1.Comprehensive clinical evaluation of the impact of a combined stroke unit on severe stroke

目的探讨急性卒中单元联合康复卒中单元对重症脑卒中的影响。方法卒中单元的急性重症脑卒中患者54例为治疗组,神经科常规病房的急性脑卒中患者52例为对照组。比较两组Fugl-Meyger评分、美国卫生研究院脑卒中量表(NIHSS)评分、日常生活活动能力(ADL)评分以及3周感染率、其他并发症发病率、死亡率。结果两组入院时Fugl-Meyger评分、NISHH评分、ADL评分无显著性意义(P>0.05);第3周Fugl-Meyger评分、NISHH评分、ADL评分无显著性差异(P>0.05);第6周出院时或随访两组Fugl-Meyger评分、NISHH评分、ADL评分有非常显著性差异(P<0.01);三周感染率、其他并发症发病率、死亡率有显著性差异(P<0.05)。结论采用急性卒中单元联合康复卒中单元能促进急性重症脑卒中患者运动功能恢复,降低其神经功能缺损程度,提高其日常生活能力。
2.Correlation between plasma homocysteine level and cerebral microbleeds and leukoaraiosis in patients with acute stroke:a retrospective case series study

Objective To investigate the correlation between plasma homocysteine (Hcy) level and cerebral microbleeds (CMBs) band leukoaraiosis (LA) in patients with acute stroke.Methods The clinical and imaging data of patients with acute stroke were analyzed retrospectively.The numbers of CMBs were counted and the severity of LA was graded according to the results of MRI.Fasting venous samples were obtained and the plasma Hey concentration was measured the next day after admission.Results A total of 139 patients with acute stroke were enrolled,67 of them were females and 72 were males (mean age 70.1 ± 10.2 years); 24 had hemorrhagic stroke and 115 had ischemic stroke.The age (76.23 ± 8.74 years vs.64.58 ± 7.42 years;t =4.621,P =0.012) hypertension ratio (89.13% vs.67.74% ;x2 =8.324,P =0.0 370) and plasma Hey level (14.53 ± 4.31 mmol/L vs.11.31 ±3.16 mmol/L;t =6.538,P=0.008| in a severe LA group (n=46) were significantly higher than those in a non-severe LA group (n =93).Spearman correlation analysis showed that there was significant correlation between the plasma Hcy level and the severity of LA (rs =0.365,P =0.002).Multivariate logistic regression analysis showed that the increased Hey level (odds ratio [ OR ],1.366,95%confidence interval [ CI] 1.141 - 1.526; P =0.010) and age (OR 1.093,95% CI 1.031 - 1.162; P =0.016)were the independent risk factors for severe LA.The age (74.37 ± 6.35 years vs.67.56 ± 8.52 years; t =6.628,P =0.038) and hypertension ratio (94.74% vs.62.20%;x2 =8.773,P =0.002) in a CBM group were significantly higher than those in a non-CMB group (n =82).Spearman correlation analysis showed that there was no significant correlation between the plasma Hcy level and the numbers of CBMs (rs =0.038,P =0.813).Multivariate logistic regression analysis showed that hypertension was an independent risk factor for CBMs.Conclusions The elevated plasma Hcy level was associated with LA,but it was not associated with CBMs.
3.Vinorelbine and cisplatin combined with endostatin as the first-line therapy for metastatic pancreatic cancer

Objective Systemic chemotherapy for metastatic pancreatic cancer is still a difficult problem in clinical practice. The standard chemotherapy of pancreatic cancer has been gemcitabine, but the response rate is low. Therefore, it is in urgent need to explore an effective clinical therapy for this cancer. This paper, a case report, is aimed at discussing the effectiveness of vinorelbine and cisplatin combined with endostatin as the first-line therapy for metastatic pancreatic cancer. Methods A 52-year-old female patient was diagnosed with pancreas cancer with liver metastasis at the time of the first visit to our hospital. Systemic chemotherapy with vinorelbine and cisplatin combined with endostatin was conducted. Results Liver metastases almost disappeared after the first cycle of chemotherapy. The primary tumor decreased by one third in size after four cycles and disappeared after the sixth cycle according to the CT scan evaluation. Conclusion Vinorelbine and cisplatin combined with endostatin can be a promising regimen for metastatic pancreatic cancer.
4.The research and expectation on wearable health monitoring system.

Wearable health monitoring systems that use wearable biosensors capturing human motion and physiological parameters, to achieve the wearer's movement and health management needs. Wearable health monitoring system is a noninvasive continuous detection of human physiological information, data wireless transmission and real-time processing capabilities of integrated system, can satisfy physiological condition monitoring under the condition of low physiological and psychological load. This paper first describes the wearable health monitoring system structure and the relevant technology applied to wearable health monitoring system, and focuses on the current research work what we have done associated with wearable monitoring that wearable respiration and ECG acquisition and construction of electric multi-parameter body area network. Finally, the wearable monitoring system for the future development direction is put forward a simple expectation.

5.Studies on expression, purification, crystal growth and optimization of putative transcription factor LytR from Streptococcus pneumoniae.

The aim of the present study was to obtain the crystal of transcription factor LytR of streptococcus pneumoniae for X-ray crystal structure and function analysis. The LytR gene of D39 strains from Streptococcus pneumoniae (S. pn) was cloned into the prokaryotic expression vector pET32a(+), then overexpression was obtained in the E. coli BL21 (DE3) through transformation of the recombinant plasmid that had been verified by colony PCR and sequencing. Soluble fusion protein with His-tag highly expressed by the induction of 0.5 mmol/L IPTG and was purified by a three step procedure, the purity of the purified LytR recombinant protein was over 90%. Preliminary screening of crystallization conditions was performed using the hanging-drop vapour-diffusing method with Hampton Crystal screen and PEG screen kits. The protein crystals X-ray diffraction data were collected from a single crystal and more stick crystals whose X-ray diffraction reached 4.0 A were obtained. These works laid the foundation for further research on the 3D structure of putative transcription factor LytR and its biological aspects.

7. Disrupted network topology in patients with idiopathic rapid eye movement sleep behavior disorder

Objective:
To explore the topological abnormality of brain metabolic network in patients with idiopathic rapid eye movement sleep behavior disorder (iRBD) and compare it with the topology of brain metabolic network in patients with Parkinson′s disease (PD).
Methods:
The 18F-fluorodeoxyglucose (FDG) PET brain images of 19 patients with iRBD diagnosed with polysomnography (PSG) (iRBD group; 15 males, 4 females, average age: 64.9 years), 19 patients with PD (PD group; 12 males, 7 females, average age: 62.2 years) and 19 gender and age-matched healthy controls (HC group; 15 males, 4 females, average age: 63.1 years) in Huashan Hospital from September 2014 to June 2015 were retrospectively analyzed. According to the complex brain network method based on graph theory, the brain metabolic networks of each group was constructed and the network parameters (clustering coefficient, characteristic path length, local efficiency, global efficiency and small-world property, etc) were evaluated quantitatively. The 500 times non-parametric permutation test was used to determine the differences in network parameters between groups.
Results:
The brain metabolic networks of iRBD group and PD group both had abnormal topological structure, which showed that the characteristic path length (for example, when sparsity=34%, HC
