1.Effect of metformin on the expression of Cyclin Dl in human gastric cancer Cells

Objective To investigate the effect of metformin on human gastric cancer line SGC-7901 in vitro, trying to explore the mechanism involved. Methods Human gastric cancer SGC-7901 cell was cultured in vitro, MTT was used to study the effects of metformin on cell growth with different concentration or different time. Western blot was used to investigate the influence of metformin on the expression of Cyclin Dl in cell line . Results The result of MTT showed human gastric cancer SGC-7901 cell was cultured in vitro and was exposed to metformin in different concentration (50 mmol/L, 100 mmol/L) for 24 h,48 h,72 h. The inhibitory rates of 50 mmol/L metformin on the effects of metformin on cell growth was 32.93% ,48.64% and 61.40% and the inhibitory rates of 100 mmol/L metformin on the effects of metformin on cell growth was 35.34% , 75.44% and 88.30%. The proliferation of SGC-7901 cell was inhibited by different concentration (50 mmol/L, 100 mmol/L) metformin and,the inhibitory rates were significantly decreasing compared with the control group(P < 0.05).The inhibitory rate treated with 100 mmol/L metformin was significantly decreasing compared with 50 mmol/L metformin group (P < 0.05). The inhibitory rate treated with metformin at different time(24 h,48 h, 72 h) was significantly different(P <0.05). Metformin could inhibit the proliferation of SGC-7901 cells, with dose and time-related effects. Cyclin Dl expressed in high level in SGC-7901 and metformin significantly decreased the expression of Cyclin Dl, with dose and time-related effects. Conclusion metformin could inhibit the proliferation and lower the expression of Cyclin Dl in human gastric cancer line SGC-7901 in vitro.
2.Wuling Capsule promotes hippocampal neurogenesis by improving expression of connexin 43 in rats exposed to chronic unpredictable mild stress.

Objective: To investigate the effects of Wuling Capsule, a compound traditional Chinese herbal medicine, on hippocampal neurogenesis by examining the expressions of brain-derived neurotrophic factor (BDNF) and connexin 43 (Cx43) in rats with depression induced by chronic unpredictable mild stress (CMS), and thereby to explore its antidepressant mechanism. Methods: Forty-five adult male Sprague-Dawley rats were randomly divided into three groups: control group (n=15), untreated group (n=15) and Wuling group (n=15). All rats except those in the control group were subjected to 3-week CMS to induce depression. At the same time Wuling Capsule was daily added to the diet of the rats in the Wuling group at a dose of 100 mg/kg body weight for 21 days. The degree of depression was determined by sucrose preference test. BDNF expression and neurogenesis were tested by using immunohistochemical staining with BDNF and 5-bromodeoxyuridine (BrdU) antibodies; and the mRNA and protein expression levels of Cx43 in hippocampus were examined by semi-quantitative reverse transcription-polymerase chain reaction and Western blotting. Results: The numbers of BDNF-positive neurons and BrdU-positive particles in dentate gyrus (DG) were significantly decreased in CMS rats as compared with the normal rats, and the same changes were found in Cx43 mRNA and protein expressions. After Wuling Capsule treatment, the depressed behaviors were improved. Moreover, the reduced expression levels of Cx43 mRNA and protein and fewer newborn neurons induced by CMS were recovered to the normal levels. However, BDNF-positive cells remained low in DG. Conclusion: Wuling Capsule can improve the low hippocampal neurogenesis in rats subjected to CMS and the antidepressant effects are related to enhancing the Cx43 expression but not through BDNF mediation.
3.The research of SurgiCase CMF software in surgical simulation and prediction for mandibular asymmetry.

OBJECTIVETo evaluate the predictive accuracy of the SurgiCase CMF software in surgical simulation and prediction for mandibular asymmetry with 3-dimensional simulation and measurement.
METHODSCBCT data of 27 patients with mandibular asymmetry were observed in CMF, and postoperative soft tissue physiognomy were predicted by simulating sagittal ramus osteotomy with or without genioplasty. The measurement parameters representing the symmetry of soft tissue were selected and the horizontal, coronal and sagittal planes were established. The results were analyzed by SPSS 19. 0. The overlap compared color grading charts were observed.
RESULTSAngles between cheilions and the horizonta plane (Ch-Ch-FH) in the simulation and postoperative soft tissues are (2. 35 ± 1. 81)° and (1. 44 ± 1. 13)°. The angles constructed among subnasale, upper lip and lower lip (Sn-UL-LL) are (4. 02 ± 3. 05)° and (2. 59 ± 1. 64)°, showing statistically different (P < 0. 01, P < 0. 05), which means that predictive accuracy of the lip canting and lip vertical deviation is relatively low. Distance between gonioi and sagittal plane (Go'-MS), distance between gonion and pogonion (Go'-Pog') and angle betweer subnasale to menton and the horizontal plane (Sn-Me'-MS) are not statistically different, which mean! high predictive accuracy of mandibular angle and chin. By observing the overlap compared color gradin-) charts, the predictive accuracy is not good in the cheek, especially in the deviate side.
CONCLUSIONSThe predictive accuracy of CMF system for patients with mandibular asymmetry is relatively high, but it is not good in the lip and cheek. The software improvement is still necessary.

4.Tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract for the treatment of upper ureteral calculi in paediatric patients

Objective To evaluate the efficacy and safety of tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract for the treatment of upper ureteral calculi in paediatric patients. Methods 20 cases of children with upper ureteral calculi (age≤7 years old), in our hospital underline of modular flexible ureteroscope combined with ultramini nephrostomy tract lithotripsy, the stones located on the left side in 12 cases, 8 cases on the right. The average age of children in the 4 years (2 to 7 years), the average diameter of stones 1.0 cm (0.7 ~1.5 cm), multiple 9 (including 3 cases of multiple bilateral, 6 cases of unilateral multiple) single stones 11 cases. Ultrasound-guided percutaneous nephrolithotomy establish supermicro channel (F11), application modular and flexible uretero﹣scope holmium laser lithotripsy systems. Results Tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract calculi Clear Phase 91%(21/23), 2 cases of bilateral stones with phased surgical treatment, surgi﹣cal staging stone clearance rate was 100 % (23/23). Postoperative hemoglobin decline was no significant change. Children do not need a blood transfusion, creatinine, blood urea nitrogen and CRP were no significant differences in change, not placed before and after surgery and renal fistula DJ tube (no tube-based). The mean postoperative hospi﹣tal stay was two days, followed up for 6 months to 1 year, all patients had no long-term complications. Conclusion Tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract for the treatment of Upper ureteral calculi in paediatric patients is an effective and safety method, can shorten hospital stay.
5.Comparison and analysis of advantages and disadvantages of rigid and flexible ureteroscopy in treatment of renal multiple stones or staghorn calculi

Objective Comparing and analyzing the advantages and disadvantages of rigid and flexible ureteroscopy in treatment of renal multiple stones or staghorn calculi. Methods 80 patients of renal multiple stones and staghorn calculi from May 2012 to March 2015 were randomly divided into two groups, group A: flexible ureteroscopy as adjuvant technique of balloon dilatation to establish standards channel of rigid nephrolithotripsy, group B: rigid ureteroscopy as auxiliary to flexible ureteroscopy nephrolithotomy. Clinical data of the two groups including operative time, blood loss, stones clearance rate, hospital stay and other indicators of surgical complications were comparatively analyzed. Results TAll the patients received successful surgical procedures. The operation time in group A was 50.4 min, gravel time was 25.6 min, while operation time in group B was 90.3 min, gravel time was 70.3 min. The differences between the two groups was statistically significant. Other indicators such as blood loss, stones clearance rate, complication rate showed no statistical significance. Conclusion Combined techniques of rigid and flexible ureteroscopy was proceeded widely to improve stone clearance rate when dealing with multiple stones and staghorn calculi, while rigid-ureterscopy-based combination has advantages of shorter operation time and this combination should be promoted in top units.
6.Effects of fluid overload on postoperative outcome of infant following cardiac surgery

Objective To explore effects and cause of fluid overload on infant following cardiac surgery.Methods This is a single center,(ICU,the Great Wall Affiliated Hospital of Nanchang University)retrospective study,between April 2012 and April 2016,176 infants who aged less than 6 months undergoing pediatric cardiac surgery were included.During consecutive postoperative 7 days,comprehensive treatment strategy for infants following cardiac surgery was adopted,collecting clinical outcomes and calculating data of fluid overload;ROC curve analysising postoperative optimal cutoff value of fluid overload.Hospitalization mortality and the second clinical outcome including low cardiac output syndrome,days on mechanical ventilation,ICU length of stay and hospital length of stay were compared to find risk factors of fluid overload.Results 176 cases of infants included in the study.ROC curves for postoperative seven consecutive days time point of Fo determined the optimal cutoff values of 7.8 % for FO (A UC =0.69).The mortality and hospital length of stay is not significant difference between fluid overload above 7.8% and below or equal to 7.8%,however,the incidence of low cardiac output syndrome was higher in infants with fluid overload above 7.8%,and longer duration of mechanical ventilation,ICU residence time.After multivariable analysis,the postoperative adverse outcomes was associated with low cardiac output syndrome and peak serum creatinine (P < 0.05),and cumulative higher fluid overload on the second day after surgery was associated with postoperative acute kidney injury and low cardiac output syndrome(P < 0.05,respectively).Conclusion Postoperative adverse outcomes in infants with congenital heart disease are associated with peak serum creatinine and low cardiac output syndrome;postoperative acute kidney injury and low cardiac output syndrome are the main causes of fluid overload.Shortening cardiopulmonary bypass time and maintaining fluid overload of postoperative day 2 below 7.8% will help to the postoperative complications.
7.Synthesis of related substances of tebipenem pivoxil

To enhance the quality control of tebipenem pivoxil and establish its quality criteria,the synthetic route of tebipenem pivoxil was analyzed and five related substances (P1,P2,P6,P8 and P9)were synthesized and characterized by 1H NMR and MS.The purities of the related substances were over 95% via HPLC detec-tion.The target compounds can be used as the reference of the related substances in the quality control of tebi-penem pivoxil.The starting materials were cheap and easy to obtain;the reaction conditions were mild.
8.Improvement of the Quality Standard for Fuzheng Capsules

Objective: To establish the quality standard for Fuzheng capsules. Methods: TLC was adopted to identify Astragali Radix and Glycyrrhiza uralensis. The method validation for Fuzheng capsules was conducted by microbial limit test as described in the appendixes of Chinese Pharmacopeia (2015 edition). The content of epimedii in Epimedium brevicornu was determined by HPLC. The chromatographic separation was carried out on an Agilent TC-C18(2) (250 mm×4. 6 mm, 5 μm) column. The mobile phase consis-ted of acetonitrile-water( 30: 70) with gradient elution at a flow rate of 1. 0 ml·min-1,and the injection volume was 5 μl. The detec-tion wavelength was 270 nm. Results: The spots in TLC were clear without any interference. The methods of plating and direct inocu-lation could be used for the microbial limit test. The linear range was 0.101-1.008 μg (r =0.999 7). The average recovery was 99. 36% with the RSD of 0. 81% (n=6). Conclusion: The method is simple with high specificity and good repeatability, which can be used as the quality control method for Fuzheng capsules.
9.Predictive value of cardiac biomarkers of low cardiac output in children undergoing congenital heart surgery

Objective:To assess the predictive value of atrial natriuretic peptide, b-type natriuretic peptide(BNP), cardiac troponin I(cTn-I) and central venose oxygen saturation(ScvO 2)as indicators of low cardiac output syndrome(LCOS) in children with congenital heart disease undergoing cardiopulmonary bypass(CPB). Methods:After corrective surgery for congenital heart disease under CPB, 70 children(aged 5 days to 160 months) were enrolled in a prospective observational pilot study during 1-year period from April 2017 to March 2018. The patients were classified according to whether they developed low cardiac output syndrome. Biomarker levels were measured at 2, 12, 24, and 48 h post-CPB. The clinical data and outcome variables were analyzed by a multiple logistic regression model. Results:Twenty-two(31.4%) patients developed low cardiac output syndrome(group 1) and the remaining 48(68.6%) patients were included in group 2. cTn-I levels >13 ng/ml at 2 hours after CPB( OR=40.2, 95% CI: 9.4-171.4, P<0.001) and ScvO 2 levels<0.37 at 12 hours following CPB( OR=124.7, 95% CI: 28.7-538.2, P<0.001) were independent predictors of low cardiac output syndrome. Conclusion:Our results suggest that cTn-I at 2 h and ScvO 2 levels at 12 h post-CPB are evident early predictors of low cardiac output syndrome. This predictive capacity is, moreover, reinforced when cTn-I combined with ScvO 2 following CPB. These 2 cardiac biomarkers would aid in therapeutic decision-making in clinical practice and would also enable clinicians to modify the type of support to be used in the pediatric intensive care unit.
10.Characteristics and risk factors of local recurrence in resected pancreatic cancer

Objective:To investigate the characteristics and risk factors of local recurrence in resected pancreatic cancer.Methods:The clinicopathological data of 190 patients in whom recurrent sites can be identified after radical resection of pancreatic cancer from Sep 2013 to Aug 2020 at the Cangzhou Central Hospital were retrospectively analyzed. The survival time and clinicopathological characteristics of local recurrence were compared with those of other recurrence types. Cox risk regression model was used to analyze the risk factors of local recurrence.Results:The recurrence sites were local (49 cases, 25.8%), liver (53 cases, 27.9%), lung (35 cases, 18.4%), peritoneal (25 cases, 13.2%) and multiple sites (28 cases, 14.7%). Patients mRFS and mOS were 17.8 months and 30.9 months respectively. The clinicopathological features of patients with local recurrence were compared with those of other recurrence types [tumor diameter ( P=0.023), preoperative CA199 level ( P=0.021), peripancreatic nerve plexus invasion ( P=0.031), lymphovascular invasion ( P=0.004), surgical margin state ( P<0.001) and postoperative adjuvant chemotherapy ( P=0.038)]. Tumor diameter ( P=0.018), peripancreatic nerve plexus invasion ( P=0.002) and postoperative adjuvant chemotherapy ( P=0.004) were independent factors for local recurrence in resected pancreatic cancer, and only peripancreatic nerve plexus invasion was not associated with other recurrence types. Conclusions:Local recurrence in resected pancreatic cancer has important impact on the prognosis of patients. Peripancreatic nerve plexus invasion is an independent factor affecting local recurrence.
