1.A Case of Neurofibromatosis without Cafe-au-lait Macule.

Almost always patients with multiple neurofibromatosis show cafe-au-lait macules on their skin. At times the skin macules are seen even before the appearance of the neurofibromata in childhood. The author experienced one case of neurofibromatosis in a 12-year-old boy who has no cafe-au-lait macules. Besides the clinical peculiarity of having no skin pigmentation, the histopathology of the case showed well-formed double-palisading structure, which is not common among the micro-scopical findings of neurofibromata.

2.Follow-up Study of 104 Cases of Blood Exchange Transfusion for Hyperbilirubinemia.

A study was made of 104cases of neonatal hyperbiliruinemia receiving blood exchange transfusion at Kwangju Christian Hospital from January 1969 to December 1975, with the following results: 1. 52 patients(50%) returned for follow-up, 9 patients(8.7%) had died, and 43 patients(41.3%) did not return. 2. Of the 52 patients returning, developmental status was studied by D.D.S.T. 43 patients(82.7%) had normal development, 8(15.4%) were retarded, and 1(1.9%) was questionable. No patients had a failing score. Most of the retarded patients had cerebral palsy, due to kernicterus. 3. There was no retardation among patients exchange-transfused at age of less than 5days, or over 8 days(except 1 pts.) , and with less than 30mg% of Pre-B.E.T. serum bilirubin level. 4. No significant difference of sex incidence was noted. 5. ABO incompatibility (57%) was predominant as the etiology and main cause of retarded development (19.4%), but the idiopathic type of hyperbilirubinemia(41.3%) was interestingly high among these B.E.T. patients. 6. Mean value and standard error of pre-B.E.T. serum bilirubin levels are as follows: Normal development group:30.80+/-1.60 Retarded development group:38.83+/-2.35 The difference of men values between both groups is very significant according to the t-test. 7. Motor disturbance was the predominant handicap in retarded patients at ages less than 3 years, and after this there was a tendency toward recovery, accompanied by mild speech disturbance. Hearing loss, especially of high pitched tone, was noted in 1 patient and confirmed by audiometry.

3.Relationship among Cognitive Style, Perceived Stress and Premenstrual Symptoms.

OBJECTIVES: The cognitive model has been suggested to explain the pathogenesis of premenstrual syndrome. In this model, it is suggested that negative cognitive style may contribute to the experience of severe premenstrual symptoms. We conducted this study to examine the relationship among cognitive style, perceived stress, and premenstrual symptoms. METHOD: Eighty six reproductive aged women older than 18 years were included in this study, to complete the Premenstrual Assessment form, Korean version of automatic thoughts Ques-tionnaire-Positive, Korean version of Automatic Thoughts Questionnaire-Negative, Korean version of Dysfunctional Attitude Scale, Perceived Stress Questionnaire. Among the subjects, twenty one were diagnosed to have a premenstrual dysphoric disorder which was determined by prospective assessment for two months. The relationships among the variables were analyzed by examining Pearson correlation coefficiency. REULTS: In the subjects without premenstrual dysphoric disorder, there exist significant correlation between the premenstrual symptom and the score of Korean-version of automatic thoughts Questionnaire-Negative(r=.36, p<.01). In the subjects with premenstrual dysphoric disorder, premenstrual symptoms had significant correlation to automatic thoughts Questionnaire-Negative(r=.65, p<.01) and Perceived Stress Questionnaire(r=.52, p<0.01). CONCLUSION: Our findings show the significant relationship between negative cognitive style and premenstrual symptoms. This relationship was more prominent in the subjects with premenstrual dysphoric disorder compared to ones without it. Our findings suggest that cognitive style and stress perception may influence on the severity of symptoms of premenstrual syndrome or premenstrual dysphoric disorder, and that cognitive behavior therapy is a possible effective therapeutic strategy for those with severe premenstrual symptoms.

4.The Standardization of the Shortened Premenstrual Assessment Form and Applicability on the Internet.

OBJECTIVES: The purposes of this study are to investigate the reliability and validity of shortened premenstrual assessment form and the applicability to internet interface. METHOD: The total of 55 subjects were included in this study. Among them, 28 subjects had premenstrual dysphoric disorder, and the other 27 subjects did not. All subjects performed the shortened premenstrual assessment form through internet interface in the homepage of Severance Woman Clinic. The diagnosis of premenstrual dysphoric disorder was determined by prospective assessment for 2 months. RESULTS: The shortened premenstrual assessment form consists of 3 factors: affect, pain and water retention. The test-retest reliability of this scale was 0.80, and internal consistency (Cronbach alpha) was 0.91. The correlation coeffeciecy between scores of the premenstrual assessment form and the shortened premenstrual assessment form was 0.92. The difference of the scores of the shortened premenstrual assessment form between the group of premenstrual dysphoric disorder and the group of non-premenstrual dysphoric disorder was significant (t=5.57, p<0.001). Discriminant analysis also revealed that the shortened premenstrual assessment form was useful to differentiate between premenstrual dysyphoric disorder and non-premenstrual dysphoric disorder (eigenvalue: 0.60, canonical correlation coefficiency: 0.61, Wilk's lambda: 0.63). The cut-off point to differentiate between two groups was 27, and sensitivity (75.0%) and specificity (76.9%) of this scale for premenstrual dysphoric disorder was acceptable. CONCLUSION: In summary, the shortened premenstrual assessment form appeared to be reliable and valid for the assesment of premenstrual dysphoric disorder. And the results of this study also suggest that the shortened premenstrual assessment form is a useful tool for internet interface application.

5.Clinical Study of Risk Factors in Patients with Acute Myocardial Infarction.

BACKGROUND: The major risk factors such as hypercholesterolemia, hypertension, diabetes melitus and cigarrete smoking have not been studied in an attempt to provide a composition of major risk factors in Korea. Also we have met with some patients without major risk factors. METHODS: A Retrospective Study was done on 160 patients of acute myocardial infarction who had been admitted to National Medical Center within 48 hours from onset, from January 1948 to December 1992 and studied on major risk factors of acute myocardial infarction. RESULTS: 1) Among major risk factors, smoking was found in 50%, hypertension in 49%, diabetes in 25% and hypercholesterolemia in 21%. 2) The percentage of patients without major risk factors was 19%. 3) Among major risk factors, hypertension, hypercholesterolemia and diabetes were increasing tendencies, recentely. CONCLUSION: Some patients with acute myocardial infarction don't have any major risk factors. Also hypertension, hypercholesterolemia and diabetes were increasing tendencies. So further study for other risk factors is needed and preventive management should require a commitment to behavior modification and alteration in life-style.

6.The Effect of Non-Ionic Contrast Media on Q-T Interval and ST-T Wave of ECG during Coronary Angiography.

BACKGROUND: During coronary angiography, some electrocardiographic changes occured due to contrast media, which do life threatening influences. METHODS: We compared the electrocardiographic changes which were induced by injection of three radiopaque contrast media during selective coronary angiography in 49 patients with chest pain. One of the contrast media was high osmolar ionic(Urografin_76) and the another was low osmolar ionic(Hexabrix) and the last was non-ionic(Ioversol). Electrocardiograms were obtained before, during and after selective coronary angiography. RESULTS: The changes of S-T segment or T were decreased in non-ionic group rather than high osmolar or ionic group. And there was significant Q-Tc interval prolongation among all three groups except comparision of low osmolar ionic contrast dye and non-ionic contrast dye in left coronary angiography. CONCLUSION: Non-ionic low osmolar contrast media was safer than high osmolar or ionic contrast medial because of lesser change of Q-Tc interval during selective coronary angiography.

7.Effects of Recombinant Human Growth Hormone Treatment in Elderly Patients Undergoing Surgery for Hip Fracture.

PURPOSE: To investigate whether early recombinant human growth hormone (hGH) treatment in elderly patients undergoing surgery for hip fracture improves biochemical and functional recovery. MATERIALS AND METHODS: From September 2007 to December 2008, a total of 25 patients older than 60 years who underwent a gamma nailing surgery for an accidental hip fracture were randomized to be treated with either hGH (Declage(R), 3mg/week) (Group I: 15 patients) or placebo (Group II: 10 patients) at a postoperative 1, 2, 3 weeks, starting within 24 hour after the hip fracture. Mean age was 73.5 years. 6 cases were male and 19 cases were female. Serum IGF-I was measured by radioimmunoassay. Subjective and objective parameters were analyzed such as the mean mid-thigh circumference, change of weight, hand grip strength, functional recovery by Modified Barthel Index of activities of daily living (ADL). RESULTS: There were significant differences in the mean serum IGF-I, mid-thigh circumference, hand grip strength, Modified Barthel Index of activities of daily living between two groups. There were no significant differences in the change of weight. There were three nausea symptom after hGH injection, but no any general adverse effect. CONCLUSION: Early recombinant human growth hormone (hGH) treatment in elderly patients undergoing surgery for hip fracture showed clinical safety and good biochemical and functional recovery.

T cell large granular lymphocytic leukemia (T-LGL leukemia) is defined as a clonal proliferative disorder of CD3+ cytotoxic T cells. T-LGL leukemia usually expresses CD3+, CD4-, CD8+, CD16+, CD56- and CD57+ cell markers, and this represents a rearrangement of the T cell receptor (TCR) gene. The clinical course is indolent in most cases, but on rare occasions, when CD3+ and CD56+ are expressed on the leukemic cells, it can be more aggressive. We experienced a patient with T-LGL leukemia and the disease was indolent at the time of diagnosis, and so any specific treatment was not required. Two years after the initial diagnosis, her clinical course became quite aggressive as the CD 56+ cell surface antigen was expressed. We report here on the first case of T-LGL leukemia in Korea and we review the relevant literature.

10.Inhibitory Activity of Cordyceps bassiana Extract on LPS-induced Inflammation in RAW 264.7 Cells by Suppressing NF-κB Activation.

Cordyceps bassiana has long been used as an oriental medicine and reported to possess diverse biological activities. The fruiting bodies of Cordyceps bassiana was extracted with ethanol and then further fractionated with n-hexane, ethyl acetate, n-butanol and water. The butanol fraction from Cordyceps bassiana (CBBF) exhibited the most effective in anti-inflammatory activity in RAW 264.7 macrophages and the roles of CBBF on the anti-inflammation cascade in LPS-stimulated RAW 264.7 cells were studied. To investigate the mechanism by which CBBF inhibits NO, iNOS and COX-2, the activation of IκB and MAPKs in LPS-activated macrophage were examined. Our present results demonstrated that CBBF inhibits NO production and iNOS expression in LPS-stimulated RAW 264.7 macrophage cells, and these effects were mediated through the inhibition of IκB-α, JNK and p38 phosphorylation. Also, CBBF suppressed activation of MAPKs including p38 and SAPK/JNK. Furthermore, CBBF significantly suppressed LPS-induced intracellular ROS generation. Its inhibition on iNOS expression, together with its antioxidant activity, may support its anti-inflammatory activity. Thus Cordyceps bassiana can be used as a useful medicinal food or drug for further studies.
