Fat can be prominet around the aortic root and within the right atrioventricular (AV) groove (1). At times, the fat can be globular, suggesting a mass especially on echocardiography. Computed tomography (CT) and masnetic resonance (MR) images are well suited for characterization of fat in and around the heart (1, 2). This report illustrates CT and MR findings of a very unusual case of prominent fat lump in the left AV groove.

3.Optimal clinical dose of rocuronium bromide for laryngeal mask airway insertion during sevoflurane anesthesia in children.

BACKGROUND: The median effective dose, ED(50), of rocuronium bromide for inserting the laryngeal mask airway (LMA) in children was not known. The purpose of this study was to determine the clinical ED(50) (cED(50): median value of rocuronium dose of 6 set of crossing success and failure by Dixon's up-and-down method) of rocuronium to insert a LMA in pediatric patients. METHODS: Children aged 2 to 12 years (n = 21) and scheduled for urologic, orthopedic, plastic or ophthalmic surgery lasting less than 2 hr under general anesthesia were enrolled in this study. After applying a standard cardiorespiratory monitor, all patients received thiopental sodium 5 mg/kg for induction of anesthesia. Insertion of LMA was attempted 90 seconds after administration of rocuronium and manual ventilation with 8 vol% of sevoflurane. We recorded the movement of patients, blood pressure and heart rate. The initial dose of rocuronium was 0.3 mg/kg, and in accordance with Dixon's up-and-down method, the rocuronium concentration for consecutive patients in each patient was varied with increments or decrements of 0.05 mg/kg based on the result of previous patient. RESULTS: The cED(50) of rocuronium required to insert a LMA was 0.09 mg/kg with 8 vol% sevoflurane. With isotonic regression, cED95 of rocuronium was 0.13 mg/kg (95% confidence intervals 0.096-0.14 mg/kg). CONCLUSIONS: This study demonstrated that the cED(50) and cED(95) of rocuronium were 0.09 mg/kg and 0.13 mg/kg in children respectively. It provides satisfactory conditions for LMA insertion in anesthetized children and decreases the excessive hemodynamic change.

4.Effect of Maternal Job Activity on Preterm Delivery, Low Birth Weight and Spontaneous Abortion.

To investigate the effect of maternal job activity on the preterm delivery, low birth weight infant and spontaneous abortion, 1646 delivery data were analyzed from Jan. 1991 to Dec. 1992 in Chungnam University Hospital. The results obtained were as follows; 1. The percentage of subjects who had job during pregnancy was 20.3% and the variables of maternal age, education level, prepregnancy weight, maternal height, number of abortion showed significant difference in the distribution of subjects whether they had job or not. 2. There were no significant difference between the group who had job during pregnancy and those who had no job in the mean gestational period, number of spontaneous abortion, rate of preterm delivery and spontaneous abortion. However mean birth weight and rate of low birth weight showed significant difference between the groups. 3. The rate of preterm delivery of the group who had job were higher than those who had no job activity in the subgroup of 35 years or more age, prematurity history presence, 2 or more abortion history, obstetric complication presence, but there were no significant difference. The rate of low birth weight also higher in the group who had job than those who had no job activity in the same subgroup except obstetric complication presence. 4. The rate of spontaneous abortion of the group who had job were higher than those who had no job activity in the subgroup of 35 years or more age, below high school, height of 163cm or above, but there were no significant difference. 5. Rate of preterm delivery still showed better results in the group who had job than the group who had no job when adjusted for variables related, but there was no statistical significance. 6. Rate of low birth weight still showed better results with statistical significance in the group who had job than the group who had no job when adjusted for variables related. 7. Spontaneous abortion had occured less frequently in the group who had job than the group who had no job when adjusted for variables related. 8. Rate of preterm delivery and low birth weight were lower in the group of teacher, bank clerk, medical personnel, citizen servant than the group who had no job, but higher in the job of company employee, commerce. Spontaneous abortion was most frequent in group of commerce, and the group of teacher, company employee showed higher rate of abortion than the group who had no job.

5.Comparison of emergence agitation between sevoflurane, desflurane, and propofol with bispectral index monitoring in pediatric anesthesia.

BACKGROUND: Sevoflurane, desflurane, and propofol are widely used in pediatric anesthesia because of their rapid recovery. However, emergence agitation is more reported with sevoflurane or desflurane than with propofol. Our clinical experience indicates emergence agitation with propofol is also frequent. We tested the hypothesis that depth of anesthesia could lead to frequent emergence agitation with propofol. METHODS: Sixty children, ASA 1, aged 3-12 years, undergoing general anesthesia for adenotonsillectomy were randomized to receive maintenance anesthesia with sevoflurane, desflurane, or propofol. The bispectral index was monitored and maintained within 40-65. Time to extubation, duration of postanesthetic care units stay, bispectral index just before stopping the anesthetic agent, anesthetic time, early and late postoperative Pediatric Anesthesia Emergence Delirium Scale (PAEDS), and Children's Hospital of Eastern Ontario Pain Scale (CHEOPS) were compared among the 3 groups. Data were analyzed using ANOVA (demographic data, measured time interval, BIS) and Kruskal-Wallis test (PAEDS and CHEOPS). The Spearman correlation coefficient was used to confirm the correlation between the two scales. RESULTS: Although desflurane resulted in the fastest extubation, other scales were not statistically different. PAEDS and CHEOPS for sevoflurane showed a positive correlation in the early recovery period, but desflurane and propofol did not. CONCLUSIONS: With the same depth of anesthesia, emergence agitation among desflurane, sevoflurane, and propofol was not different in children.

6.A Neuromedin B Receptor Blockade Inhibits the Growth of Human Oral Cancer Cells.

Neuromedin B (NMB) acts as a growth factor or a morphogen and plays a role in cancer progression. Indeed, the NMB receptor (NMB-R) is overexpressed in different types of tumors. In our current study, we investigated the involvement of NMB-R in the proliferation of oral cancer cells. Human oral squamous cell carcinoma (SCC) and human oral cancer cells, SCC-25 cells were found to be NMB-R-positive. The NMB-R antagonist PD168368 inhibited the proliferation of SCC-25 cells and reduced their colony formation capacity. We also found that PD168368 induced the cell cycle arrest and apoptosis of SCC-25 cells in a dose-/time-dependent manner. Overall, this antitumor activity of PD168368 in human oral cancer cells suggests that NMB-R is a potential target for the future prevention and treatment of human cancers.

7.Influence of beverage type and ingestion time on tooth corrosion

This study was conducted to analyze the effects of beverages on tooth corrosion by selecting drinks that are enjoyed by modern people. Drinks were selected for carbonated beverages (Coca Cola), energy drinks (Red Bull), orange juice, beer (Hite) and water (Evian). Bovine tooth was immersed for 1 hour, 24 hours and 48 hours. The root mean square (RMS) values were obtained by scanning the bovine tooth at pre-immersion and immersion time after impression making. Two-way ANOVA, one-way ANOVA, and Tukey-test were used to compare differences between groups (α=0.05). The RMS value tended to increase with longer immersion times in all beverages, and there was a significant difference (p < 0.05) between before immersion-1hr and before immersion-48 hr. The mean of RMS according to the type of beverage was significantly different and there was a significant difference according to the change of immersion time (p < 0.05). In order to prevent tooth corrosion, it is necessary to change the method of ingestion or to reduce the time of ingestion.

8.A Case of Extranodal Histiocytic Sarcoma of Stomach Mimicking Gastric Adenocarcinoma.

Histiocytic sarcoma is a rare malignant neoplasm that originates from a histiocytic hematopoietic lineage characterized by histiocytic differentiation and its corresponding immunophenotypic features. Patients with histiocytic sarcoma usually have a poor prognosis due to its aggressive clinical behavior. Here we report a rare case of extranodal histiocytic sarcoma of the stomach which was confirmed through immunohistochemical staining. A 71-yearold man was presented with epigastric pain. Gastroscopy, abdominal CT, and EUS revealed a mass located on the posterior wall of upper body and fundus of the stomach. Grossly, grayish white solid masses were seen extending down to the submucosal layer. Microscopically, the tumor cells had eosinophilic cytoplasm, abundant vacuole, and mitosis. Immunohistochemical staining revealed that the tumor cells were positive for LCA, CD68, and lysozyme. Early detection and accurate diagnosis of this rare neoplasm is important because it can make a great difference in prognostic outcomes. To make an accurate and definitive diagnosis, immunohistochemical staining is essential in the confimation of histiocytic orign.

9.The Effect of Intraoperative Autotransfusion using a Continuous Autotransfusion System (CATS(R)) on for Homologous Transfusion in Children.

BACKGROUND: Until recently, it was not easy to conduct intraoperative autotransfusion in children due to technical limitations, however, due to advanced technology this is now possible. This study was conducted to determine if the intraoperative use of the continuous autotransfusion system (CATS(R)) can reduce homologous transfusion during pediatric orthopaedic surgery. METHODS: Fifty-five children scheduled for elective orthopaedic surgery were reviewed and divided into two groups according to the availability of the CATS(R). The control group (n = 29) had surgery without the cell saver, whereas the CATS(R) group (n = 26) had surgery with the cell saver. The amounts of perioperative homologous transfusion were then compared between the two groups. RESULTS: The amount of homologous blood transfusion required during the operation was significantly less in the CATS(R) group 5 +/- 10 (ml/kg) than in the control group 15 +/- 13 (ml/kg) (P < 0.01). There was no difference in the amount of homologous blood transfusion required after operation between the groups. CONCLUSIONS: CATS(R) can reduce the need for intraoperative homologous transfusion during pediatric orthopaedic surgery.

10.A case of the primary malignant melanoma arising from uterine endometrium.

Malignant melanoma arising in the uterine endometrium is extremely rare. Only 12 cases of malignant melanoma of the uterine endometrium have been previously reported to date. All of them were metastatic cases. The most common presenting symptom was abnormal uterine bleeding. We report a case of primary malignant melanoma arising from the uterine endometrium. A 63-year-old multigravid woman presented with uterine bleeding. The pathologic review of an endometrial curettage specimen suggested an undifferentiated malignant tumor. A total abdominal hysterectomy, bilateral salpingo-oophorectomy, and pelvic lymph node dissection were performed. Final pathologic examination revealed malignant melanoma. Immunohistochemical studies demonstrated reactivity of tumor cells for HMB-45 and S-100 protein. She had no previous history of malignancy. Whole body PET scan was performed in an effort to find the primary site of malignant melanoma and showed no demonstrable abnormal FDG uptake suggesting malignancy elsewhere. This case is the first report of primary malignant melanoma involving uterine endometrium in Korea.
