1.Development of a novel antigen capture-ELISA using IgY against porcine interleukin-6 and its application.

Interleukin-6 (IL-6) is introduced as a marker of disease. At present, a variety of method may be used to quantify expression of this protein. Antigen capture-ELISA is a sensitive and accurate quantification method previously used with ovine, rat, and human IL-6 proteins. However, it has never been reported to quantify porcine IL-6 protein using capture ELISA. In this study, we generated and characterized a set of IgY and mono-specific polyclonal antibodies to recombinant porcine IL-6 (rpIL-6), and combining these with a sensitive and specific capture-ELISA for a diagnostic purpose. cDNA encoding the mature protein coding region of porcine IL-6 was cloned and expressed with pQE-30UA expression vector. rpIL-6 was then expressed and purified by using Ni-NTA resin. Protein mass of 24 kDa was found with SDS-PAGE and the identity of the protein was confirmed by Western-blot. Production of polyclonal antibodies against rpIL-6 was performed using the purified rpIL-6 in mice and hens. An antigen capture-ELISA was developed with the antibodies after their extraction. To compare the IL-6 level in the different sanitary state of farms, pig sera were randomly collected and concentration of IL-6 in the sera was measured with the antigen capture-ELISA. The capture-ELISA with the optimal concentration of antibodies, in this study, was able to detect about 10 ng/ml of rpIL-6. IL-6 levels determined with the capture-ELISA in pig sera showed positive correlation with the sanitary states of the farms. These results suggested that the developed antigen capture-ELISA could be a good tool for the screening of microbial infection in pig farms.

5.Ender Nailing of the Tibial Shaft Fractures under the Local Anesthesia.

PURPOSE: To evaluate the clinical result of ender nailing under local anesthesis was done to patient with tibia shaft fracture who had high risk for general anesthesia or spinal anesthesia. MATERIALS AND METHODS: 10 cases with ender nailing procedure under local anesthesia due to high anesthetic risk were selected from total of 20 cases with tibia shaft fracture operated with ender nailing. In each patient, hepatic, diabetic, cadiopulmonary complication and thromboembolism which can be initiated or aggravated by general or spinal anesthesia, were evaluated. Radiologic and clinical evaluation were used to check bone union. RESULTS: There were no complication of local anesthesia. Bone union were acquired in all cases with average bone union period of 18 weeks. There were no evidence of flexion deformity, limb shortening or joint contracture. CONCLUSION: Ender nail fixations under local anesthesia enable close reduction and intramedullary nailing and is effective in patient care but has no problem with bone union. For this reason, ender nail fixation under local anesthesia seem to be effective method if general of spinal anesthesia is difficult.

6.Evaluation of Left Ventricular Diastolic Function Using New Doppler Time Index.

BACKGROUND: There is a clinical need for a simpler measurement of global cardiac function incorporating elements of both systole and diastole. Doppler time index is theoretically regarded as a sensitive index of global left ventricular perfomance and defined as the sum of isovolumetric contraction time (IVCT) and isovolumetric relaxation time (IVRT) divided by ejection time (ET). This study was designed to determine the clinical usefulness of the Doppler time index in patients with left ventricular diastolic dysfunction as well as systolic dysfunction. METHODS: The study population consisted of 23 patients with hypertension as a diastolic dysfunction group, 16 patients with low ejection fraction as a systolic dysfunction group and 31 subjects with normal LV function. The ejection fraction (EF) was measured using M-mode echocardiography. Doppler profiles such as IVCT, IVRT and ET were obtained from Doppler echocardiography. The Doppler time index [ (IVCT+RT)/ET] was calculated from each Doppler velocity profiles. RESULTS: IVRT, IVRT/ET and (IVCT+VRT)/ET were significantly increased in the diastolic dysfunction group (120.5+/-19.5 msec, 0.45+/-0.1, 0.64+/-0.2, respectively:p<0.001, p<0.001, p<0.001, respectively) compared with normal subjects (66.1+/-17.4 msec, 0.25+/-0.0, 0.41+/-0.1). IVCT and IVRT were significantly increased and ET was significantly shortened in systolic dysfunction group (75.4+/-25.7, 144.0+/-39.5 msec, 242.7+/-46.5 msec respectively:p<0.001, p<0.05, p<0.05, respectively) compared with diastolic dysfunction group (50.4+/-23.0 msec, 120.5+/-19.5 msec, 276.8+/-44.6 msec, respectively). IVCT/ET, IVRT/ET and (IVCT+IVRT)/ET also were increased in patients with systolic dysfunction group (0.32+/-0.1, 0.61+/-0.2, 0.93+/-0.2 respectively:p<0.01, p<0.01, p<0.001, res-pectively) compared with diastolic dysfunction group (0.19+/-0.1, 0.45+/-0.1, 0.64+/-0.2). Ejection fraction calculated by M-mode parameters was significantly correlated with (IVCT+IVRT)/ET (correlation coefficient - 0.605, p<0.001). CONCLUSION: The Doppler time index was significantly difficient from normal subjects in patients with isolated LV diastolic dysfunction as well as in those with systolic dysfunction. Thus, this index can be used as a sensitive indicator of myocardial performance.

7.A Study on Air Pollution and Thermal Factors in Underground Shopping Center of Pusan Area.

For the purpose of preparing the fundamental data on air pollution in underground shopping center and also contributing to the health improvement of resident, the authors measured the level of SO2, NO2, TSP, CO, CO2 and also some related factors as air temperature, air movement, relative humidity and mean radiation temperature at inside and outside of underground shopping center in Pusan from January to February and from July to August 1994. The results were as follows: 1. The mean concentration of CO within the underground shopping center was 3.1+/-1.3ppm in winter and 2.1+/-0.9 ppm in summer. There was a negative correlation (p<0.01) between inner CO concentration and temperature in summer and no correlation between inner CO concentration and outer CO concentration in underground shopping center 2. The mean concentration of C02 within the underground shopping center was 876+/-353 ppm in winter and 757+/-125 ppm in summer. There was a negative correlation (P<0.01) between inner CO2 concentration and air movement in summer and positive correlation (p<0.05) between inner CO2 oncentration and outer CO2 concentration in underground shopping center. 3. The mean concentration SO2 within a underground shopping center was 0.036+/-0.019ppm in winter and 0.040+/-0.013ppm in summer. There was a positive correlation(p<0.01) between inner SO2 concentration and positive correlation between inner SO2 concentration and outer SO2 concentration in summer and winter in Underground shopping center. 4. The mean concentration of NO2 within a underground shopping center was 0.052+/-0.038ppm in winter and 0.042+/-0.016ppm in summer. There was a no correlation between inner NO2 concentration and thermal factors in summer and winter and low correlation between inner NO2 concentration in underground shopping center. 5. The mean concentration of TSP within a underground shopping center was 430+/-214 microgram/m3 in winter, 366+/-73 microgram/m3 in summer, and very in excess of the atmospheric environmental quality standards of Korea(150 microgram/m3). There was low correlation between inner TSP concentration and temperature in summer and high correlation between inner TSP concentration and outer TSP concentration in underground shopping center.

8.The Effects of the decortication on pulmonary function in tuberculous empyema.

BACKGROUND: The purpose decortication is to eliminate the infection focus and to improve the decreased lung function due to chronic empyema. However, lung function is not improved in all cases. It would be clinically useful it we could predict preoperatively whether lung function would improve after decortication. The purpose of this study is to find useful indices for predicting the possible improvement of lung function after decortication. METHOD: The medical records of 37 tuberculous empyema patients who underwent pleural decortication were analyzed retrospectively from 1990 to 1996. The measurements of preoperative and postoperative forced vital capacity(FVC) were used for evaluating the effects of decortication. RESULTS: The sex ratio was 29 : 8 (male to female), and the median age was 34 years. The time interval between the formation of empyema and operation was 1 month to 30 years. Postoperative pulmonary function test was performed 5.4±2.6 months later. FVC(forced vital capacity) was significantly increased from 2.77±0.67(L) to 2.95± 0.81(L). Interestingly, postoperative pulmonary function was significantly improved in patients who were less than 40 years old, within 4 months after diagnosis of tuberculous empyema, in the group with FVC of less than 60% of the predicted value and in the absence of calcification. CONCLUSION: The improvement of lung function after decortication was expected in patients younger than 40 years old, within 4 months after diagnosis of tuberculous empyema, in the group having less than 60% of the predicted FVC, without calcification.

9.Comparison of Train-of-four Response between Adductor Pollicis and Orbicularis Oculi to Determine the Optimal Time for Endotracheal Intubation after Vecuronium Administration.

BACKGROUND: Ideal condition of endotracheal intubation after administration of non-depolarizing muscle relaxants like vecuronium is the time when the diaphragm and upper airway muscles are completely relaxed. But these muscles are difficult to determine the degree of relaxation. Neuromuscular response of these muscles are similar to that of orbicularis oculi (OO), but adductor pollicis (AP) is different. However, it is sometimes difficult to monitor OO response. The purpose of this study was to monitor the upper airway muscle relaxation using AP other than OO. METHODS: Fourty-four adult patients of ASA class 1 schaduled for elective surgery under general anesthesia were examined. Anesthesia was induced with fentanyl 2 mcg/kg, and 2 minutes later followed by thiopental sodium 5 mg/kg. After supramaximal stimulation for control twitch height, vecuronium 0.1 mg/kg was intravenously injected and applied continuous train-of-four (TOF) facial nerve stimuli. The TOF response of OO was closely observed with examiner's naked eyes. When complete relaxation of OO achieved, TOF ratio of AP and the time after vecuronium injection were recorded. Thereafter, tracheal intubation was performed and the intubating condition scores was recorded. Ulnar nerve stimuli were continuously applied until complete relaxation of AP was achieved. The time of complete relaxation of AP after vecuronium injection was also recorded. RESULTS: The onset time of complete relaxation was significantly faster in OO (181.3+/- 47.4 secs) as compared with that of AP (265.0+/-67.8 secs). The average TOF ratio of AP was 47.3+/-17.2% and the condition of intubation performed after complete relaxation of OO was satisfied. CONCLUSION: The optimal time for endotracheal intubation was about 3 min after vecuronium 0.1 mg/kg injection, when TOF ratio of AP was about 50%.

10.A Case of Viral Myocarditis Presenting as Acute Extensive Myocardial Infarction.

Viral myocarditis is a inflammatory process of the heart caused by virus. Its manifestation ranges from asymptom to acute fulminent congestive heart failure and often mimics acute myocardial infarction. A 22 year old man was admitted to the hospital because of prolonged anterior chest pain. 15 days before entry, anterior chest pain with cough and fever brought him to the another hospital. At that time, the LDH level was 1160U/L, the CPK level was 659.7U/L and MB band was 16.1%. The ECG revealed acute inferior wall infarction. And 3 days later, extensive anterior wall infarction findings appeared. 2D-Echo findings showed proximal septal hypokinesia. On admission time to this hospital, physical examination was negative. The ECG showed no interval change. 2D-Echo findings showed increased echogenicity of anterior and inferior wall. Treadmill test did not evoked chest pain or change of ECG. The LDH, CPK levels returned to normal range. The antibody titers of Coxsackievirus type A-16 was positive. Coronary angiography showed normal findings. 3 months later, antibody's titer of Coxsackievirus A-16 was increased. A diagnosis of acute viral myocarditis was made.
