The purpose of this study was to evaluate 6 years cumulative survival rate (CSR, %) of mandibular posterior single tooth implants replaced with Branemark TiUnite(R) implant system. The findings from this study were as followed ; 1. The 112 (111 persons) single implants that were placed in the mandibular posterior region were successful except 4 cases and showed 96.42% CSR. 2. The 55 (55 persons) single implants that were placed in the mandibular first molar region were successful except 2 cases and showed 96.36% survival rate. And, among the 57 (56 persons) single implants replacing the mandibular second molar, 2 failed showing 96.49% survival rate. There was no significant statistical difference. 3. Among the total 112 implants, 5.0mm wide diameter implants were placed in 96 cases(85.7%) showing 96.9% survival rate. 4.0mm standard diameter implants were placed in 16 cases showing 93.8% survival rate. There was no significant statistical difference. 4. Long implants above 10.0mm length were placed 103 cases(91.0%) and showed 96.1% survival rate. Short implants within 8.5mm length were placed 9 cases and showed 100% survival rate. There was no significant statistical difference. 5. 37 implants placed in typeI,II bone quality were showed higher survival rate(100%) than that of 52 implants placed in typeIII, IV bone quality(92.3%). But, there was no significant statistical difference. In conclusion, Branemark TiUnite(R) implant showed successful results when replacing manbibular single molar.

OBJECTIVES: To evaluate the clinical effect of integrated angulated screw channel (ASC) abutment crown in implant-supported rehabilitation of the aesthetic area. METHODS: Sixteen patients who received single implant-supported rehabilitation using integrated ASC abutment crown in the aesthetic area were included in the study. After one-year follow-up, the cumulative survival rate, aesthetic effectiveness, bone resorption around implants, and patient satisfaction were analyzed through periapical film, clinical examination, and the visual analog scale (VAS). RESULTS: The implant cumulative survival rate of the integrated ASC abutment crown in the implant-supported rehabilitation of the aesthetic area was 100%. The average pink esthetic index scores reached 9.5, and the white esthetic index scores reached 9.4. The mean marginal bone loss of implants was (0.439±0.123) mm at the mesial side and (0.341±0.118) mm at the distal side. The average VAS satisfaction score was 8.9, which showed that all patients were satisfied with the final restorative effect. CONCLUSIONS Using integrated ASC abutment crown for implant restoration is an ideal implant restoration design in the aesthetic zone and can be applied clinically.

OBJECTIVE: This study aimed to evaluate the clinical effect of the socket-shield technique in the maxillary anterior region at one year after implant placement. METHODS: Ten patients with maxillary anterior teeth that cannot be reserved were enrolled. Implants were installed following the socket-shield technique and restored six months after the surgery. The thickness of the labial maxillary bone of the implant was compared before and one year after the surgery. At one year follow-up, the reten-tion rate of the implants, Jemt classification, and pink and white aesthetic scores were evaluated. Postoperative complications and patient satisfaction were also analyzed. RESULTS: One year after installation, the retention rate of the implant was 100%. The thickness of the maxillary bone at the neck, central, and root section of the implant reduced to (0.27±0.21), (0.19±0.20), and (0.28±0.29) mm, respectively, compared with the values immediately after the operation. The thickness of the labial maxillary bone at the three measurement points immediately after and one year after the operation was statistically significant (P<0.05). No significant difference (P>0.05) of the difference thickness between immediate and one year after operation at the three measurement points. The pink esthetic scores of the implant prosthesis was 9.10±0.54, and the white esthetic scores was 9.00±0.63. No complications were observed, and the patients had a high degree of satisfaction. CONCLUSIONS The socket-shield technique could provide acceptable treatment results but cannot completely avoid the reconstruction of the labial maxillary bone of the implants. The technique exhibits favo-rable short-term aesthetic result, but its long-term clinical effect and aesthetic problems need to be further studied.

STATEMENT OF PROBLEM: A phenomenon of screw-loosening in implant abutment is frequently occurred in a single and multiple implant restoration. PURPOSE: This study was performed to evaluate an effect of abutment material on screw-loosening before and after a cyclic loading. In a single-tooth implant , different materials of abutment, Type III Gold alloy and Zirconium composite(ZrO2/Al2O3) were used. MATERIAL AND METHOD: The Gold alloy(Type III) and Zirconium composite(ZrO2/Al2O3) were used to make a superstructure of implant, the one of types of UCLA, Each group was constituted of 5 sample with a 30-degree offset angulated loading platform. The external hexagonal fixture was rigidly hel d in a special holding zig to ensure solid fixation without rotation during the tightening and a cyclic loading. A Titanium-alloy screw was used to connect and controlled to be tighten in 20Ncm torque by a digital torque gauge. A 20 times of consecutive closing/opening cycle were performed to evaluate the immediate torque loss. In 5 sample of each material group, an initial opening torque was recorded during 3 closing/opening cycle, then 2Hz, 200N, 1,000,000 cyclic loadings were performed, then a opening torque was evaluated. RESULT & CONCLUSION: 1. In this limited study, titanium alloy screw tightened in 20Ncm, a cold-welding phenomen on was not observed during the 20 times of closing/opening cycle(p=0.11, p=0.18). 2. In titanium alloy abutment screw, repeated opening and closing of the screw caused to progressive decrease of opening torque(p=0.014). 3. The difference in preload of screw between gold alloy abutment and ceramic(ZrO2/Al2O3) abutment was not significant(p=0.78). 4. The difference in torque loss of screw between gold alloy abutment and ceramic(ZrO2/Al2O3) abutment was not significant after 2Hz,200N, 1,000,000 cyclic loading(p=0.92). 5. In titanium alloy abutment screw tightened by 20Ncm, the screw loosening was not significant on each group after 2Hz, 200Ncm, 1,000,000 cyclic loading(p=0.59).

The aim of this study was to systematically investigate the complications of single implant-supported restorations followed more than 5-year. Thirty-five studies were selected for the systematic review. A total of 3932 single implants were included at the beginning of studies. Thirty-one implants were removed before loading and 91 implants after loading. The overall implant loss rate was 3.1 %. Implant losses were concentrated on the period between loading and 2-year follow-up, and, after a stable period, increased after 5-year follow-up. The mean marginal bone loss at single-tooth implants was well within 0.2 mm/year, i.e., acceptable annual bone loss by the implant success criteria. However, considerable amounts of single implants suffered a marginal bone loss at implant more than 0.2 mm/year. Fistula was a frequent biological complication in the early studies. The most frequent technical problem was a screw loosening, but its frequency was reduced after the use of a gold screw and torque controller. Within the limits of this study, the complications of single implants might be underestimated due to the lack of information about the biological and technical complications available in the relevant literature.

PURPOSE: The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIAL AND METHODS: Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS: All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION: Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

OBJECTIVE: To compare the clinical effects of a modified socket shield technique (SST) and the conventional immediate implant placement in the esthetic zone. METHODS: Twenty-four patients with one hopeless upper incisor were consecutively enrolled in this study. All patients were scheduled for immediate implant placement. Twelve patients underwent a modified SST (experimental group) whereas the others underwent conventional procedures (control group). Radiographic and clinical data were obtained preoperatively, immediately after surgery, and 12 months after surgery. The success rate of the implants, the pink esthetic score (PES) and patient satisfaction were analyzed. Statistical analysis was conducted using SPSS 21.0 software. RESULTS: One year after operation, the success rate of the implants was 100% in both groups. The PES and patient satisfaction of the experimental group were significantly higher than those of the control group (P<0.05). The buccal bone plate resorbed less in the experimental group than in the control group (P<0.05). CONCLUSIONS The modified SST is advantageous over the conventional immediate implant placement in terms of buccal bone and soft tissue maintenance.

Objective: To evaluate the pink and white esthetics of conventional and socket shield technique (SST) immediate implant restoration. Methods: Thirty volunteers were recruited according to preset criteria, and were assigned to 3 groups. Natural teeth group (ten undergraduates or postgraduates from Peking University School and Hospital of Stomatology in January 2020, n=10): all volunteers' maxillary anterior teeth were natural teeth with healthy gingiva, and none of the teeth were restored by crowns or composite. Conventional group and SST group (patients had a maxillary central incisor immediate implant placed in Department of Prosthodontics, Peking University School and Hospital of Stomatology during October 2016 to January 2021, n=10 for each group): the volunteer had an unrestored natural maxillary central incisor, and the contralateral maxillary central incisor was restored by conventional or SST immediate implant placement, temporization and all ceramic final restoration, photos were taken 12 months post-surgery. Three groups of evaluators namely layperson (staff from Second Clinical Division of Peking University School and Hospital of Stomatology and 2 family members, n=10), dental students (class 2015 undergraduates from Peking University School and Hospital of Stomatology, n=10) and prosthodontists (from Department of Prosthodontics, Peking University School and Hospital of Stomatology, n=10) were invited to assess the esthetics using pink esthetic score (PES) and white esthetic score (WES). The results were statistically analyzed. Results: PES and WES of natural teeth group [9(8, 10) and 8(7, 10)] were significantly higher than conventional group [7(6,8) and 7(6,9)] (H=287.08, 132.79,P<0.01) and SST group [7(6, 9) and 8(7, 9)] (H=216.01, 101.21, P<0.01). SST group yielded higher PES than the conventional group (H=-71.06, P<0.01), yet had similar WES (H=-31.57, P>0.05). Dental students had significant lower PES and WES than prosthodontists (H=-120.90, -218.86, P<0.01) and layperson (H=-109.55, 134.97, P<0.01). Prosthodontists and layperson got similar PES (H=-11.36, P>0.05), however yielded different WES (H=-83.89, P<0.01). Conclusions: SST immediate implant placement obtained better pink esthetics than conventional protocol 12-month after implant surgery, profession may have significant impact on evaluators during pink and white esthetic evaluation.
