1.Effect of embossed new dental pick on periodontitis.

Since bacterial plaque is known as the major cause of every oral disease, it is necessary to control plaque to maintain periodontal health. Although tooth brushing has been the typical method of plaque control, additional use of plaque control devices are necessary for cleansing interdental areas. Dental floss has been used for a long time as the additional plaque control device and proved to be effective. The newly developed embossed new dental pick is composed of elastic plastic material in form with its tip longer than other toothpick. In addition, as thickness of the tip gradually increases, it can be adapted easily with minimal trauma to interdental gingiva. The embossed tip can remove dental plaque easily, and the opposite end can approach lingual and palatal areas through triple angulation. Therefore, the authors studied the effect of embossed new dental pick and dental floss on periodontally involved 15 patient. The following results were obtained. 1. For embossed new dental pick, probing depth at baseline, 1 week, 4 weeks were 3.6+/-0.6mm, 3.4+/-0.5mm, 3.2+/-0.5mm, respectively, and 4.2+/-0.9mm, 4.1+/-0.9mm, 4.0+/-0.9mm, respectivly for clinical attachment level. 2. In case of embossed new dental pick, bleeding on probing at baseline, 1 week, 4 weeks were 43.0+/-4.4%, 28.5+/-5.1%, 22.2+/-5.6%, respectively, and, at 1 week and 4 weeks, they were significantly different from that of dental floss. 3. Gingival index of toothpick at baseline, 1 week, 4 weeks were 1.7+/-0.5, 1.0+/-0.3, 0.9+/-0.2, respectively, and, 1.7+/-0.5, 1.1+/-0.4, 1.0+/-0.4, respectively for plaque index, and 3.0+/-0.5, 1.7+/-0.5, 1.4+/-0.4, respectively for proximal plaque index. proximal plaque index at 4 weeks was significantly different from that of dental floss. 4. The result of questionnaire showed that the two types of device were both satisfactory, however, embossed new dental pick had higher preference as well as satisfaction. As a result, embossed new dental pick is thought to be clinically effective for its simplicity and good accessibility.

2.Comparing the efficacy of a high pressure spray oral hygiene appliance and a sonic vibration toothbrush in reducing dental plaque.

OBJECTIVES: This study aims to compare home care dental devices for their dental plaque removal ability. METHODS: A single blinded randomized crossover clinical study design was used to measure plaque index (Turesky Modification Quigley-Hein Plaque Index). RESULTS: All 3 groups showed a statistically significant decrease in the plaque index after usage (P<0.001). In manual tooth-brushing group, the teeth on the right dental arch showed a greater decrease in the plaque index than on the left arch (P=0.041). All appliances showed greater plaque reduction in the upper right posterior teeth than in the lower right posterior teeth (P=0.009, 0.004, 0.007). CONCLUSIONS: The results of this study support development of more effective oral hygiene appliances and emphasis on oral hygienic education.

3.The survey on the infection control of noncritical instruments used in dental treatment

PURPOSE: The aims of this study were to evaluate the dentist's awareness and the actual status of infection control of noncritical dental instruments. MATERIALS AND METHODS: 40 dental clinics in Daejeon, South Chungcheong, North Chungcheong and North Jeolla provinces were surveyed. The questionnaire was delivered to the dentists belonging to those clinics, and the awareness and the practice of infection control were examined. The microbial contamination on the surface of five noncritical instruments (impression gun, light curing unit, 3-way syringe, shade guide, and dental floss dispenser) used by them was measured with an ATP luminometer. Correlation analysis between the awareness and the actual state of infection control was conducted. RESULTS: Awareness and frequency of infection control was highest in the 3-way syringe. Surface disinfection using disinfectant was most frequent in all instruments. 3-way syringes and shade guides were less contaminated than impression guns, light curing units, and dental floss dispensers. CONCLUSION: 3-way syringes had a significant correlation between user awareness of infection control and surface contamination, and the higher awareness, the lower the contamination measurement was shown.

4.Elucidating the relationship of gingivitis and dental calculus with the periodontal health of 12-year-old children

OBJECTIVES: This study aimed to investigate factors related to the periodontal health of 12-year-old children.METHODS: In 2015, the Korean Children's Oral Health Survey from the Ministry of Health & Social Welfare conducted a nationwide representative sample comprised of 23,702 12-year-old children. The calibration-trained dentists examined the gingivitis and dental calculus of the children taking into consideration of the Löe and Silness gingival index to diagnose gingivitis with a modified gingivitis scale. We used questionnaires to collect data from the children on dental treatments, the experience of dental pain and gingival bleeding, self-perceived oral health, and oral health behaviors. Data were analyzed using a complex samples Chi-square test, general linear model, and logistic regression. Significance was determined at P < 0.05.RESULTS: The prevalence of gingivitis was higher among males (OR 1.57), among children with poor perception (OR 1.19), dental calculus (OR 3.68), or gingival bleeding experience (OR 2.00), and among children not using dental floss (OR 1.69) or tongue cleaner (OR 1.90). The prevalence of dental calculus was higher among children with gingivitis (OR 3.82) and among children who had not visited a dental clinic in the preceding year (OR 1.31). However, dental calculus was lower among children with a higher frequency of daily toothbrushing (OR 0.75), intake of cariogenic foods (OR 0.90), or a higher DMFT index (OR 0.91).CONCLUSIONS: Children with dental calculus and gingival bleeding who did not visit a dental clinic in the preceding year also had a higher prevalence of gingivitis and dental calculus. The prevalence of children's dental calculus was lower among children with a high frequency of daily toothbrushing.

5.Beneficial Effects of Lactobacillus casei ATCC 334 on Halitosis Induced by Periodontopathogens.

Halitosis is caused by consumption of certain foods or drinks and production of volatile sulfur compounds (VSCs) by periodontopathogens. VSCs-related halitosis is not easily removed using mechanical or chemical therapies such as dental floss, plaque control and mouth rinse. Lactobacillus are known to be probiotics and stimulate immune systems of human. Furthermore, L. casei ATCC 334 and L. rhamnosus GG have an effect on protection of dental caries in vitro studies. The aim of this study was to investigate effect of Lactobacillus on halitosis by Fusobacterium nucleatum- and Porphyromonas gingivalis- producing VSCs and to analyze inhibitory mechanism. The periodontopathogens were cultivated in the presence or the absence Lactobacillus, and the level of VSCs was measured by gas chromatograph. For analysis of inhibitory mechanisms, the susceptibility assay of the spent culture medium of Lactobacillus against F. nucleatum and P. gingivalis was investigated. Also, the spent culture medium of Lactobacillus and periodontopathogens were mixed, and the emission of VSCs from the spent culture medium was measured by gas chromatograph. L. casei and L. rhamnosus significantly reduced production of VSCs. L. casei and L. rhamnosus exhibited strong antibacterial activity against F. nucleatum and P. gingivalis. The spent culture medium of L. casei inhibited to emit gaseous hydrogen sulfide, methyl mercaptan and dimethyl sulfide from the spent culture medium of periodontopathogens. However, the spent medium of L. rhamnosus repressed only dimethyl sulfide. L. casei ATCC 334 may improve halitosis by growth inhibition of periodontopathogens and reduction of VSCs emission.

6.Risk indicators of periodontal disease in Korean adults.

OBJECTIVES: The purpose of this study was to evaluate the risk indicators of periodontal disease in a representative sample of Korean adults aged between 18 and 44 years. METHODS: Data of 6,485 adults were derived from the Fourth Korean National Health and Nutrition Examination Survey, which was conducted from 2007 to 2009. Demographic, socioeconomic, and oral health-related behavioral data were collected as independent variables. The Community Periodontal Index (CPI) was used as a dependent variable. The chi-square test and logistic regression analysis were performed to identify the relationship between the CPI and the other variables. RESULTS: Multiple logistic regression analysis revealed that periodontal pocket formation (4 mm or more) was related with sex, age, and income level, irrespective of whether the mother engaged in economic activities in childhood, smoking experience, dental flossing, and use of interdental brushes. The subjects in the lowest-income quartile and those who were current smokers had a higher risk of pocket formation (odds ratios, 1.43 and 1.74, respectively) than those in the highest-income quartile and those who had no smoking experience, respectively. CONCLUSIONS: This study confirmed various risk indicators of periodontal disease in Korean adults, using Korean national survey data. Smoking cessation, dental flossing, and efforts to decrease socioeconomic inequality should be encouraged to decrease the risk of periodontal disease.

7.Association of Anxiety and Depressive Symptoms with Periodontal Disease in Korean Women.

Periodontal diseases occur from the interplay between increased bacterial response and the response of the host immune system over time. Anxiety and depression can impair immunological defense mechanisms, causing accumulation of periodontopathogens and thus exacerbating periodontal disease. We investigated the relationship of anxiety and depression to periodontal diseases in Korean women. In this study, 3,551 women aged ≥19 years were evaluated based on data from the first year (2010) of the Fifth Korea National Health and Nutrition Examination Survey. The analysis of the factors that caused periodontal diseases revealed that dental floss or interdental toothbrush nonuse behaviors have been shown to increase the risk of periodontal disease (odds ratio [OR], 1.49; 95% confidence interval [CI], 1.14~1.95). After adjusting for conditions such as age, marital status, income, educational level, economic activity, diabetes mellitus, smoking, drinking, and frequencies of toothbrushing and interdental cleaning, we found that anxiety and depression increased the risk of developing periodontal diseases (OR, 1.47; 95% CI, 1.04~2.09). People with anxiety and depression have a higher prevalence of periodontal diseases than people without anxiety and depression. Thus, periodic periodontal care and effective self-care education are needed to manage periodontal diseases.

8.Menopause experience and the relationship between metabolic syndrome components and periodontitis

OBJECTIVES: The purpose of this study was to investigate the correlation of periodontitis with metabolic syndromes and menopause status. METHODS: The 2013 Korea National Health and Nutrition Examination Survey data of 645 postmenopausal and 663 premenopausal women were analyzed using the Pearson's chi-squared test and multiple regression analysis with the Statistical Package for the Social Sciences (PASW statistics) software version 21.0. Metabolic syndromes and community periodontal index (CPI) were analyzed. RESULTS: CPI 3, CPI 4 (P<0.001), and the prevalence of periodontal disease were significantly higher in the postmenopausal group (P<0.001). Periodontal disease was significantly higher (P<0.001) in postmenopausal women not using dental floss (2.69 times), hypertensive postmenopausal women (1.60 times), and premenopausal women with high blood glucose levels (1.67 times). In the multiple logistic regression analysis, after adjusting for age, smoking habit, and alcohol consumption, significant risk factors for periodontal disease were the lack of dental floss use (odds ratio [OR]: 1.83; 95% confidence interval [CI]: 1.26–2.67), menopause (OR: 1.71; 95% CI: 1.22–2.41), and number of metabolic syndromes (when 1 or 2, OR: 2.22 and 95% CI: 1.50–3.29; when 3 or more, OR: 2.22 and 95% CI: 1.58–3.13). CONCLUSIONS: In summary, menopause status is associated with periodontal disease. Therefore, oral health programs for middle-aged women with menopause and metabolic syndromes should be conducted to improve their oral health-related quality of life.
