1.Development and Evaluation of the Scale for Assessing the Report Quality of Clinical Trials

Objective:To develop the instrument for the evaluation of the report quality of clinical trials with the modified Delphi method in combination with panel discussions, and assess its validity and reliability. Method: For the development of the instrument, the modified Delphi method in combination with a panel discussion was applied. The consulting panel included pharmacologists, clinical doctors, epidemiologists, statisticians and editors of journal. 30 reports for the therapy of hypertension published in 2000 were selected for the estimation of the instrument of the validity and reliability. The interclass correlation coefficient (ICC) was used to test the reliability. The CONSORT statement was used as a control, and a Kendall correlation was worked out to estimate the validity. Result:A total of 10 specialists took part in the consultation. During the first round of consultation, the expert harmonious coefficient (W) reached to 0. 505 and 19 items were identified. A total of 100 scores were distributed to the 19 items in the second round, and finally the scale was completed. After scoring the 30 reports on hypertension therapy by two readers independently, the interclass correlation coefficients (ICC) of the total score were 0.921,and 0.987 in the introduction part, 0.875 in materials and methods, 0.902 in results, and 0.966 in the discussion and others, P

Objective:To develop and evaluate the improved Chinese hearing questionnaire for school children(CHQS)for mass epidemiology study on hearing impairment in China.Method:Using the probability proportion to size(PPS) method, 8 412 residents were investigated in 40 clusters in Jiangsu province with the WHO ear diseases and hearing disorders survey protocol.87.9% of the residents aged 7 years and over answered the questionnaire and accepted the pure tone audiometry.Result:The prevalence of hearing impairment was 12.9% by the questionnaire. Compared with golden standard(pure tone audiometry), Sen=58.5%, Spe=96.7%, PV+=78.9%, PV-=91.7%, overall accuracy=90.0% . The sensitivity for women was higher than men.Conclusion:The questionnaire produced high efficiency and specificity values.It could be used in mass hearing screening, particularly in remote and rural area, although the sensitivity was as low as most questionnaires.

5.Retrospective study of a novel anti-rheumatic drugs to reduce the panel reactive antibody of kidney transplant recipients

Objective:To observe the efficacy and safety of Iguratimod in reducing the level of panel reactive antibodies in renal transplant recipients.Methods:The clinical data of 35 patients with PRA-positive renal transplant recipients were retrospectively analyzed. All recipients were treated with Iguratimod for PRA-positive. The changes in PRA levels before and after treatment and the adverse events were observed.Results:Of the 35 recipients, 4 of them were discontinued due to pulmonary infection, and 2 patients were discontinued during the observation period. 3 patients were lost to follow-up. A total of 26 recipients were included. When Iguratimod was taken to 9 months, the PRA was reviewed. 71.5 % of the 207 sites showed a downward trend, 69.9 % of the 107 class I sites and 75.9 % of the 41 class II site showed a downward trend, and there was no difference in renal function before and after treatment. There were no significant changes in blood routine, liver function, blood lipids, and blood glucose. There were no other adverse events.Conclusions:Iguratimod can effectively reduce the level of PRA in renal transplant recipients with less adverse events.
6.Preparation of monoclonal antibodies with blocking activity against cat TNF-α

In order to prepare monoclonal antibodies with blocking activity against feline TNF-α,this study successfully constructed,expressed and purified the recombinant plasmid pET-28a-sTNFα based on the soluble feline TNF-α(sTNFα)gene,and further investigated the induced ex-pression.The conditions were explored and optimized to identify its biological activity;secondly,the feline TNF-α recombinant protein was used as an immunogen for mouse immunization,after cell fusion,screening of blocking active hybridoma cells and ascites preparation,the obtained mon-oclonal antibodies were tested.The results showed that the pET-28a-sTNFα plasmid was success-fully constructed and the bioactive feline TNF-α recombinant protein was expressed in E.coli sys-tem.The molecular weight was 34 kDa and the 50%inhibitory concentration was 1.22 pg/L.Three monoclonal antibodies(A6-B7-9,H5-E2-94 and C8-A10-100)with blocking activity were success-fully screened out.The results of Western blot showed that all the three mAbs could specifically bind to TNF-α with a titer of 1:512 000.When the concentration of the three mAbs was 100 mg/L,the inhibitory effect on TNF-α was the strongest.In this study,we screened antibodies that can block the activity of cat TNF-α,in order to provide novel,safe and effective candidate drugs for the treatment of TNF-α mediated diseases in cats.
