1.Clinical Observation of Prednisone Acetate Combined with Leflunomide in the Treatment of IgA Ne-phropathy

OBJECTIVE:To observe therapeutic efficacy and safety of prednisone acetate combined with leflunomide in the treatment of IgA nephropathy. METHODS:80 patients with IgA nephropathy were randomly divided into observation group and control group,with 40 cases in each group. Control group was given prednisone acetate 1.0 mg/kg,qd,6 weeks later,decreasing gradually,decreasing to 0.5 mg/kg in 12th week;observation group was additionally given leflunomide 50 mg,qd,3 days later de-creasing to 20 mg. Both groups were given 3 months of treatment. Clinical efficacy,24 h urinary protein quantification,Scr and BUN levels were observed in 2 groups after treatment,and ADR were recorded during treatment. RESULTS:Total effective rate of observation group(95.0%)was significantly higher than that of control group(75.0%),with statistical significance(P<0.05). Af-ter treatment,24 h urinary protein quantification,Scr and BUN levels of observation group were significantly lower than those of control group,with statistical significance(P<0.05). The incidence of ADR in observation group(0)was significantly lower than in control group(7.5%),with statistical significance(P<0.05). CONCLUSIONS:Prednisone acetate combined with leflunomide has significant effect on IgA nephropathy,and will not increase the occurrence of ADR.
2.The application of low dose fentanyl combined ropivacaine on subarachnoid block for cesarean section

Objective To explore the clinical efficacy and safety of low dose fentanyl combined to ropivacaine in subarachnoid block for cesarean section.Methods Eighty patients with ASA physical status Ⅰ and Ⅱscheduled to undergo cesarean section for delivery were randomly allocated to receiving spinally mixture none fentanyl ( group R),5 μg fentanyl ( group F5 ), 10 μg fentanyl ( group F10 ), and 15μg fentanyl ( group F15 ), combined with 1%ropivacaine 1 ml and 10% glucose 1 ml respectively,after combined spinal-epidural anesthesia puncture was applied at the L2~3 interspace.Heart rate, blood pressure, the onset and recovery time of the sensory and motor block,time to the highest level and the highest level of sensory block, time to the maximal extent and the maximal extent of the motor block were recorded at regular intervals.The adverse effects were sought during and after surgery.Neonatal outcome was assessed using Apgar score.Results The recovery of sensory block was prolonged significantly in group F10 and group F15 than in group R and group F5.Conclusion Ropivacaine with 10μg fentanyl is safe and effective in subarachnoid block for cesarean section.
3.Magnetic resonance imaging findings of traumatic temporomandibular joint injury induced by type Ⅵ condylar fracture

Objective To investigate the application of MRI in evaluation of the traumatic temporomandibular joint (TMJ) injury induced by type Ⅵ condylar fracture. MethodsMRI was performed in TMJs in 18 patients with type Ⅵ condylar fractures at days 3-14 post-injury and the MRI findings were analyzed. ResultsMRI findings of 18 patients with traumatic TMJ injury with 19 sides of type Ⅵ condylar fractures showed 15 sides of TMJ disk displacement,nine sides of capsule tear,16 sides of retrodiscal tissue tear (double-plate area) and 19 sides of joint effusion change. Conclusions MRI is very important in the diagnosis and evaluation of traumatic TMJ injury,since it can clearly display the TMJ injuries in type Ⅵ condylar fractures.Therefore,the clinical application of MRI is beneficial for selection of the therapeutic schedules.
4.Effects of heat shock protein 70 on mice with traumatic brain injury-related acute gastric mucosal lesions

Objective To explore the protective mechanism of HSP70 protein in traumatic brain injury (TBI)‐related acute gastric mucosal lesions in mice .Methods Forty adult male Balb/c mice were randomly divided into sham (A) ,TBI (B) ,TBI+ geranylgeranylacetone (GGA) (C) ,and TBI+saline (D) groups .TBI was induced via the Feeney impact model .GGA (800 mg/kg) was administered via oral tube beginning before the model was built in group C .The expressions of HSP70 protein in brain and gastric mucosa were determined by immunohistochemistry , and the apoptotic index was detected by TUNEL method .Results The injury area in mouse brain and gastric mucosa was greater in group B than in groups A and C (P<0 .05) .After model induction ,the content of HSP70 protein in group B was markedly higher in the brain and gastric mucosa ,which was notably higher than in group A (P<0 .05) .Obviously apoptotic cells were observed in groups B and D ,which were significantly higher than in groups A and C .GGA pretreatment enhanced the up‐regulated expression of HSP70 and decreased the apoptotic index distinctly ;HSP70 expression was higher in group C than in groups B and D ,but the apoptotic index was lower (P<0 .05) .Conclusion GGA can induce HSP70 protein expression in mouse brain and gastric mucosa .HSP70 is involved in the process of apoptosis inhibition .GGA can be used in the prevention and therapy of TBI‐related acute gastic mucosal lesions .
5.Effects of grasp seed procyanidins on homocysteine-induced proliferation and migration in vascular smooth muscle cells

AIM:To investigate the effects of grasp seed procyanidins(GSP) on homocysteine-induced proliferation and migration in vascular smooth muscle cells(VSMC) and related molecular mechanisms.METHODS: Cell count and -TdR assay were used for detecting cell proliferation and DNA synthesis,ELISA assay was used for detecting inflammatory response,DCFH-DA assay for examining the levels of reactive oxygen species(ROS),Western blotting for detecting protein expression.RESULTS: Homocysteine(0.1-1 mmol/L) increased VSMC proliferation and migration,and the levels of ROS were in a dose-dependent manner.The results of Western blotting showed that homocysteine significantly increased the expression of MCP-1,IL-6 and TNF-?.However,Compared with control group,in GSP(5-20 g/L) group,the increased VSMC proliferation,migration and the production of ROS and the expression of MCP-1,IL-6 and TNF-? mediated by homocysteine were markedly suppressed.EMSA showed that in GSP treatment group,the NF-?B activation was also almost completely inhibited.CONCLUSION: GSP inhibits homocysteine-induced VSMC proliferation,migration and inflammatory response through interfering with ROS dependent on NF-?B signal pathway.

7.Comprehension of Chinese spoken garden path sentences for children with high-functioning autism

ObjectiveTo explore the psychological and behavioral characteristics of understanding Chinese garden path sentences in children with high-functioning autism (HFA). MethodsFrom April to August, 2019, 35 children with HFA and 35 age-matched typically developing (TD) children were recruited by social recruitment method in the Key Laboratory of Speech and Hearing of East China Normal University in Shanghai. The Garden Path Sentence Comprehension Test was designed based on the psycholinguistic method, and the mental process and the ability of understanding garden path sentences were tested by the agent task experiment paradigm. Finally, the processing strategies and error types of Chinese spoken garden path sentences for HFA children were explored. ResultsThe score in understanding garden path sentences was lower (t = -2.941, P < 0.01), and the reaction time in processing garden path sentences was significantly longer (t = 6.132, P < 0.001) in HFA children than in TD children, however, there was no significant difference in the number of childern mastering garden path sentences between two groups (χ2 = 2.954, P > 0.05). In terms of the error types of "animate noun + verb + animate noun + 's + noun" (AVA) and "animate noun + verb + inanimate noun + 's + noun" (AVI) garden path sentences, both groups showed more errors in "pursuit of correct word order" than in "pursuit of semantically correct" (t > 2.503, P < 0.05). In "inanimate noun + verb + animate noun + 's + noun" (IVA) sentences, HFA children showed less erros in "pursuit of correct word order" than in "pursuit of semantically correct" (t = -6.523, P < 0.001), however, no difference was found between them in TD children (t = 2.024, P > 0.05). ConclusionThe processing and comprehension ability of Chinese spoken garden path sentences in HFA children aged five to seven yeas are poor. HFA children are more sensitive to semantics, and garden path sentences with semantic violations are more likely to lead to HFA children's understanding errors. In the garden path sentence patterns of AVA and AVI, HFA children appear similar sentence understanding and processing strategies as TD children. In the garden path of IVA, HFA children use a sentence processing strategy combining word order and semantics, but semantics play a more prominent role, while TD children use a more stable word order processing strategy.
