BACKGROUND:Oral insulin iS easy to be degradated by hydrochloric acid in gastric iuice and the enzyme which exists in stomach and intestines,while it is dimcult to through the epithelium cell membrane of gastrointestinal.Even a lasting-long injection iS taken,and it still needs an injection once 36 hours.OBJECTIVE:To prepare oraI insulin sustained-release microspheres.DESIGN,TIME AND SETTING:Contrast observation which was carried out at the Laboratory of Medical Chemistry of Nanjing Information Engineering University from September 2003 to December 2004.MATERIALS:Insulin Was provided bv Nanjing Medicinal Factory of Bio-Chemistry.METHODS:A new method tO prepare the microsphere,I.e.,drying through liquid phase was adopted with the special pH and the acrylic acid polymer which could be dissolved as the medicine'S carrier of the sustained-release microphere.MAIN 0UTCOME MEASURES:The effects of polymer concentration and agitation rates on size of microspheres and drug incorporation efficiency were studied by scanning electronic microscopy(SEM)and ultraviolet-spectrophotometry.RESULTS:The temperature,drug payloads and the content of emulsifier played important effects on microspheres formation.The mean diameter of microspheres decreased with increasing in stirring rate and decreasing in polymer concentration,and drug loading efficiency increased with increasing in stirring rate.CONCLUSION:Insulin-loaded polyacrylate sustained-release microspheres is prepared preferably.
2.Study on abnormal myocardial motion of left ventricle by quantitative tissue velocity imaging

Objective To observe the velocity of left ventricular wall motion features of normal subjects and patients with abnormal regional wall movement of left ventricle with the quantitative tissue velocity imaging(QTVI),and to know whether variances exit between the velocity of left ventricular myocardial motion of the normal subjects and the abnormal subjects with coronary artery disease(CAD) and myocardial ischemia. The practical value of QTVI on myocardium movement of left ventricle was evaluated. Methods The segmental left ventricular wall motion velocity was measured on 27 normal subjects and 21 patients with CAD and myocardial ischemia,including three waves,peak velocity of systolic S wave,diastolic E wave,A wave, and time velocity integrate of S wave(S TVI) was used as measurement parameters.The parameters were analysed between the two groups. Results The velocity and S TVI were gradually decreased from base to apex in the same wall in the control group.There were some abnormal segmental movements in the abnormal group. S wave ,E wave and S TVI of diseased segments in the abnormal group were much lower than those of control group ( P 0.05 ). Conclusions QTVI can quantitatively measure the velocity in abnormal movement segment of ventrical wall of patients with CAD and myocardial ischemia.S wave,E wave and S TVI are better parameters of assessing systolic function of left ventricle.QTVI has clinical application in regional myocardial motion function evaluation.
3.Peroxisome proliferator-activated receptor-gamma coactivator 1 and skeletal muscle adaptation

BACKGROUND:An increase in activity of muscle contraction can induce transcription of a variety of signaling molecules to activate a large number of gene expression within the nucleus through proprietary signaling pathway OBJECTIVE:To review studies related the peroxisome proliferator-activated receptor-gamma coactivator 1(PGC-1)and exercise induced skeletal muscle adaptations.METHODS:A computer-based online search of Pubmed was performed for relevant English articles published from January 1995 to January 2009 with the keywords of"PGC1,skeletal muscle,exercise,adaptations"The relevant articles about PGC-1 and exercise induced skeletaI muscle adaptations were included.and repetitive contents were excluded.PGC1 and mitochondrial oxidative metabolism,and exercise-induced skeletal muscle adaptation and PGC1s changed served as the evaluation indicators.RESULTS AND CONCLUSION:Of 57 collected articles,34 were classified and sorted according to the criteria.Endurance training results in profound adaptations for skeletal muscle,including mitochondrial biogenesis,capillary density,and fiber composition.Transcription factors are highly dependent on coactivator molecules to regulate training physiologic adaptation processes.The majority of transcription factors target genes are involved in mitochondrial biogenesis and metabolism.These transcriptional patterns may provide a basic framework for understanding the integration of mitochondrial biogenesis and function with signaling events that exercise induced energetic properties Post-transcription of PGC-1 alpha protein,there are a variety of protein modifications,with a variety of biological processes closely related to the possible adaptive mechanism of exercise-induced skeletal muscle.
4.Stroke and sleep-wake disturbance

Sleep-wake disturbance is not only a risk factor for stroke,but also one of the serious complications of stroke.The main clinical manifestations of post-stroke sleep-wake disturbance are insomnia,arousal disorders,sleep-related movement disorders and parasomnias.Its mechanism is mainly associated with the sleep-wake center after stroke injury.
5.Mammography with core needle biopsy for breast lesion

Objective To evaluate the clinical application of mammorgraphy with core needle biopsy in the diagnosis of breast lesions.Methods 36 cases of clinical nonpalpable breast lesions were detected with stereotactic needle core biopsy(SCNB) and 27 cases of ≥2 cm breast masses with direct core biopsy. Results The study showed that breast cancer was 20(31.7%), benign breast lesion 43(68.3%). The diagnostic accuracy, aspiration misplay and false negatigve were 93.6%, 3.2% and 3.2%, respectively. No false positive was found. Conclusion Mammorgaphy with core needle biopsy is a simple, less invasive and accurate localization procedure, very valuable in the diagnosis of nonpalpable lesions that only be visible on mammography.

Objective To evaluate the biocompatibility of nano-hydroxyapatite/polyvinyl alcohol (n-HA/PVA ) hydrogel com-posite for keratoprosthesis .Methods There were four materials to be evaluated ,big-porous and small-porous n-HA/PVA hydrogel composite ,big-porous and small-porous PVA hydrogel .Every kind of materials was digested in F12 nutrient culture for 24 h to get the materials impregnant .Corneal fibroblasts were cultivated in materials impregnant and through methyl thiazolyl tetrazolium col-orimetry to assay the cytotoxicity .Rabbit corneal fibroblasts were seeded onto the four materials in vitro .Three days later the growth of fibroblasts were observed by inverted microscope and scanning electron microscope (SEM ) .Results The cytotoxicity scores of the materials with big-porous were both grade one .The mean corneal fibroblasts counts on culture were no obvious differ-ence between big-porous materials groups and control group under plate inverted microscope 72 hours later(P=0 .608) .Through scanning electron microscope 72 hours later ,corneal fibroblasts grew well on n-HA/PVA hydrogel composite ,and migrated into the porous of composite materials .The corneal fibroblasts on big-porous n-HA/PVA hydrogel composite were more obviously found than the other three materials(F=95 .245 ,P=0 .000) .Conclusion The porous n-HA/PVA hydrogel composite is a kind of good biocompatible material which has no cytotoxicity .The n-HA/PVA hydrogel composite with big porous would be an ideal pe-ripheral skirt of keratoprosthesis .

Objective To explore the effects of impaired glucose metabolism and impaired blood lipoprotein metabolism on heart rate variability(HRV) in aged male hypertension patients and to have an insight into the correlation between the years of impaired glucose metabolism and the patients’ HRV. Methods 120 male subjects were divided into three groups: simple hypertension patients (group A, 40 people), hypertension patients with diabetes mellitus (group B, 40 people), and hypertension patients with diabetes mellitus and impaired blood lipoprotein metabolism (group C, 40 people). Objective To observe the effects of one kind of angiotensin converting enzyme inhibitor (ACE1) drugs fosinopril (FOS) on transforming growth factor β1 (TGF-β1)and β1- integrin( Itg-31 ) expression in rat glomerular mesangial cells (GMC)induced by lipopolysacchatide (LPS).Methods We established the cultured glomerular mesangial cells of rat in vitro and passages 3 ~ 10 of cells were used in the experiment after identification.The experiment included the following groups:Control group,LPS induced group (LPS group) and FOS intervened group.According to the different concentrations of FOS,FOS intervened group was divided into high,middle and low dose FOS groups,which were FOS1 group,FOS2 group and FOS3 group respectively.The changes of TGF-β1 protein secretion was detected by the enzyme-linked immunosorbent-assay; The changes of TGF-β1 and Itg-β1 mRNA expression was detected by quantitative real-time RT-PCR.Results (1) TGF-β1protein secretion in rat GMC at 6h,12h,24h three time points:They were 958.55 ± 34.67 ( ng/L),1052.05 ±48.59( ng/L),1166.06 + 35.39 (ng/L) respectively in Control group.They were 1342.12 + 39.87 ( ng/L),1432.31 + 39.33 (ng/L) and 1 537.77 + 43.79 (ng/L) respectively in LPS group,which were higher significantly than those in Control group ( all P < 0.01 ).They were 779.58 ± 48.64 ( ng/L),878.33 ± 29.50 (ng/L) and 962.57 ±31.94( ng/L) in FOS1 group,989.311±73.56(ng/L),1073.29±66.89(ng/L) and 1210.75 ±61.68(ng/L) in FOS2 group,1 253.78 ±45.32( ng/L),1 348.18 ±45.81 (ng/L) and 1450.06 ±46.24( ng/L) in FOS3 group respectively,which were lower significantly in all FOS intervened groups than that in LPS group (all P<O.01).(2)TGF-β1 mRNA expressions in rat GMC at6h,12h,24h three time points were higher significantly than that in Control group.TGF-β1 mRNA expressions were lower significantly in all FOS intervened groups than that in LPS group.( 3 ) Itg-β1 mRNA expressiones in rat GMC at 6h,12h,24h three time points were higher significantly than that in Control group.Itg-β1 expressions were lower significantly in all FOS intervened groups than that in LPS group.Conclusions LPS can induce the increase of TGF-β1 secretion and mRNA expression.FOS can inhibit the TGF-β1 secrection and mRNA expession in GMC as dose-dependent manner,at the same time down regulated the Itg-β1 mRNA expression iuduced by LPS.All above supply the theoretical evidence for the renal protection of FOS by non-hemodynamics mechanism.
10.The inhibitory effects of rh-endostatin (YH-16) in combination with radiotherapy on lung adenocarcinoma A549 in mice and the underlying mechanisms.

In order to investigate the inhibitory effects of Endostar (rh-endostatin, YH-16) in combination with radiotherapy on lung adenocarcinoma A549 in mice and the interaction mechanisms of combined therapy, the transplantation tumor models of A549 lung adenocarcinoma were established. When the largest diameter of tumor reached 1.0 cm, all nude mice were randomly divided into 4 groups: Endostar group, radiotherapy group, radiotherapy plus Endostar (combined treatment) group, and control group (n=6 in each group). The largest diameter and the vertical diameter of tumor were measured at different time points. At the 16th day, mice were executed, and the tumors were applied to analysis of rate of tumor cell apoptosis, and the expression levels of basic fibroblast growth factor (bFGF) mRNA were detected by reverse transcription-polymerase chain reaction (RT-PCR) and those of vascular endothelial growth factor (VEGF) by immunohistochemistry. The results demonstrated that the rate of tumor inhibition in combined treatment group was higher than that in other groups. And the rate of tumor cell apoptosis in combined treatment group was also higher than that in other groups. Meanwhile, the levels of bFGF mRNA and VEGF expression in combined treatment group were lower than those in other groups. It was concluded that Endostar obviously enhanced the curative effectiveness of radiotherapy on lung adenocarcinoma A549 in mice. The underlying mechanisms may involve the down-regulation of bFGF mRNA and VEGF expression to inhibit angiogenesis by Endostar and the cooperative effect of Endostar and radiotherapy to synergistically promote tumor cell apoptosis. And Endostar inhibits angiogenesis by down-regulating the expression of bFGF mRNA and VEGF.
