Objective To investigate the impact of planning parameter settings on plan quality and delivery efficiency of VMAT for nasopharyngeal carcinoma with two treatment planning systems (TPS),as references for clinic plan optimization. Methods 25 patients with nasopharyngeal carcinoma were selected and planned for SIB?VMAT treatment. The same planning aims were used in the two kinds of TPS ( TPS?1 and TPS?2). Multiple planning parameters were set for plan optimization. Dose distribution to the target volumes and organs at risk, monitor unit ( MU) and delivery time were compared. Paired t?test or one?way ANOVA was used for the data which was in accordance to normal distribution;otherwise, nonparametric Wilcoxon signed rank test or nonparametric Friedman test was used. Results More segments lead to better plan quality and less MU but longer delivery time ( Minor impact was observed when segment number was larger than 120) in TPS?1,while it had little impact on both plan quality and delivery efficiency in TPS?2. Comparing to single?arc plans, dual?arc VMAT achieved no significant benefit in plan quality but had more MU and longer delivery time in TPS?1 ( P= 0?000 ) . However, dual?arc VMAT plans had better dose distribution in TPS?2, decreased the maximum and mean dose for spinal cord in 3?9% and 13?7%respectively (P=0?000,0?000).Changing the settings of maximum or minimum dose rate did not affect the plan quality in both of the tested TPSs. Increasing the maximum or minimum dose rate reduced the delivery time but the latter increased the number of MU ( P=0?000,0?000) . Conclusions VMAT plan quality and delivery efficiency is affected significantly and differently by planning parameter settings for two TPSs. Trial test should be conducted for different TPS to determine the optimal parameter settings.

OBJECTIVETo investigate the pathogenic and clinical presentation and laboratory tests of bullous rash in hand, foot and mouth disease (HFMD) in Xi'an from January 2013 to December 2014 by retrospective analysis.
METHODA total of 224 specimens were collected from clinically diagnosed HFMD cases who were characterized by widespread mucocutaneous bullous reactions in Xi'an Children's Hospital from January 2013 to December 2014, the identification and subtyping of the isolates were conducted with real-time fluorescent quantitative RT-PCR. A retrospective analysis was performed to analyze the clinical presentation, laboratory tests and late follow-up problems of the HFMD.
RESULTIn the clinically diagnosed HFMD cases who were characterized by widespread mucocutaneous bullous reactions, 207 were caused by coxsackievirus A6 (CA6), accounting for 92. 4% of all cases with bullous, 4 were caused by enterovirus 71 (EV71), accounting for 1.8%, 10 were caused by coxsackievirus A16 (CA16), accounting for 4. 5%; 4 cases were negative for these viruses. In the cases positive for intestinal virus-nucleic acid, 130 were male, 90 were female; male to female ratio was 1. 44: 1, 203 were <5 years old, accounting for 92. 3%. Leukocytosis was found in 75 cases (34. 1%); high-sensitivity C-reactive protein (hsCRP) increased in 200 cases (90. 9%); elevated myocardial enzyme CK-MB was found in 35 cases (15. 9%), alanine aminotransferase increased in 15 cases (6. 8%); 187 cases had fever (85. 0%). None of the cases had serious complications such as encephalitis or myocarditis. In the course of the critical phase bullous rash or large vesicle-like changes, obvious itching, and facial rash appeared. After the fluid in the bullae was absorbed or the bullae ruptured or became ulcerated, scar formation and large areas of exfoliation occurred, with no effusion on the newly formed epithelium in the base, without significant pigmentation on later follow-up. In the late follow up process, 52 cases in CA6-positive patients (25. 1%) developed onychomadesis within 2-4 weeks after onset, 1 to 8 nails, an average of 4. 3 fell off, new nails grew, the nail bed showed no structural abnormalities and hyperplasia after falling off, the surface was smooth, had no hypertrophy, left no sequelae.
CONCLUSIONThe pathogen in HFMD characterized by widespread bullous reactions was mainly the CA6, this kind of HFMD was mainly mild type, with significant itching, later the bullae may have scar formation and skin exfoliation, in some cases onychomadesis may occur.

OBJECTIVETo observe the clinical effect of acupuncture on acute heart failure (AHF) patients.
METHODSTotally 60 patients who were diagnosed as AHF were assigned to the acupuncture group and the control group, 30 in each group. Those in the control group received inotropic agents, preload and afterload reducing therapy, anti-infection and so on. Besides, those in the acupuncture group received needling at relative points, once daily for 5 consecutive days. The mean arterial pressure (MAP), heart rate (HR), cardiac index (CI), stroke volume index (SI), left ventricle working index (LCWI) were monitored by thermodilution pulse-indicated continuous cardiac output (PiCCO) technique. Changes of the aforesaid data were compared between before and after treatment. The ICU length of stay, readmission rate, and the 28-day mortality were also compared between the two groups.
RESULTSAfter 5 days of the treatment, CI, SI, and LCWI increased more obviously (P < 0.01), HR and MAP decreased significantly (P < 0.05). Of them, CI, SI, and LCWI increased more obviously in the acupuncture group (P < 0.05).There was no obvious difference in HR or MAP between the two groups (P > 0.05). Compared with the first day of admission in the same group, CI, SI, and LCWI obviously increased in the acupuncture group from the second day (P < 0.05). HR started to decrease since the fourth day (P < 0.05), and MAP began to decrease until the fifth day (P < 0.05). CI, SI, and LCWI started to increase in the control group from the third day (P < 0.05); HR and MAP both began to decrease since the fifth day (P < 0.05). Compared with the control group, the ICU length of stay was obviously shortened in the acupuncture group (P < 0.05). The readmission rate and the 28-day mortality rate were lower than those of the control group (P < 0.05).
CONCLUSIONThe combination of acupuncture and Western medical therapy might strengthen acute heart failure patients' heart functions, elevate the therapeutic effect, and improve the prognosis.
