1. Solitary fibrous tumors: Imaging features and differentiation between benign and malignant tumors

Objective: To investigate the clinical and pathological features and imaging manifestations of SFT (solitary fibrous tumor), and further to define the differiciation between the benign and malignant SFTs. Methods: The imaging findings were retrospectively analyzed and compared with the corresponding pathological results in 18 patients with SFT confirmed pathologically. Results: Of the 18 patients, 2 had postoperative recurrent or metastatic tumor, 3 had malignant SFT. The X-ray and MSCT (multi-slice computed tomography) scan images showed oval or lobulated masses with discrete margins except one malignant tumor. The tumor density was clear or nearly uniform. MRI (magnetic resonance imaging) examination showed that the tumors had intermediate signal intensity on T1WI (T1-weighted image) and heterogeneous signal intensity with flow voids on T2WI (T2-weighted image). All tumors showed heterogeneously intermediate or intense enhancement with geographic pattern. Intralesionally and paraleisonally prominent feeding vessels could be seen. The results of immunohistochemistry demonstrated that CD34 was positive in 18 patients; Bcl-2 and vimentin were positive in 17 patients; S100 was negative in 15 patients; cytokeratin and desmin were negative in 18 patients; Ki-67 expression was less than 5% in 15 benign tumors and more than 5% in 3 malignant tumors. Conclusion: There are some specific imaging features which are helpful to the differential diagnosis for SFT. These specific imaging features include the rich feeding vessels, the signal intensity on T2WI and the different enhancements associated with the tissue compositions of the tumor. However, making a definitive diagnosis still depends on pathology and immunohistochemistry. Copyright © 2013 by TUMOR.
2. Transcatheter hepatic artery infusion with recombinant human endostatin combined with TACE for rabbit VX2 hepatoma and evaluation on angiogenesis by CT perfusion

Objective: Rabbit VX2 hepatoma was treated with transcatheter hepatic artery infusion with recombinant human (rh) endostatin combined with transcatheter arterial chemoembolization (TACE). The therapeutic efficacy was evaluated and the peri-cancerous CT perfusion features as well as the expressions of microvascular density (MVD) and vascular endothelial growth factor (VEGF) were analyzed. Methods: Thirty rabbits with VX2 hepatoma were randomly divided into three groups with 10 rabbits in each group. Group A were treated with TACE and infused with rh-endostatin via hepatic artery. Group B were treated with TACE only. Group C were control group. Routine CT scanning was performed before therapy and CT perfusion imaging were performed at 2nd weeks after treatment to get the data of blood flow (BF), blood volume (BV) and capillary permeability surface (PS) area. The rabbits were sacrified immediately after scanning. The MVD and VEGF expressions in tumor tissue slides were detected by immunohistochemistry. Results: The hepatoma growth rate was significantly lower in group A and B than that in group C (P <0.01). The BF, BV, and PS were significantly increased and MVD and VEGF were significantly decreased in group A than those in group B and C (P < 0.05, P < 0.01). There was no significant difference in the value of BF, BV, PS, MVD, and VEGF between groups B and C (P > 0.05). The expression of MVD positively correlated with the expression of VEGF in linear dependency (P < 0.01). BF, BV, and PS had positive correlation with MVD and VEGF in group A (P < 0.05). BF, BV, and PS had no significant correlation with MVD and VEGF expression in group B (P > 0.05). Conclusion: The combination therapy of TACE and administration of recombinant human endostatin via hepatic artery infusion significantly slows down the growth speed of hepatoma and inhibits tumor angiogenesis. The parameters of CT perfusion imaging can not reflect the changes of pathological parameters such as MVD and VEGF at any therapeutic condition.
3.Data Mining and Analysis of Acupuncture Treatment for Diabetes

ObjectiveTo explore the characteristics of and regularities in the application of points for acupuncture treatment of diabetes in clinical literature.MethodOne hundred and two pieces of clinical literature on acupuncture treatment for diabetes published from 1998 to 2013 were sorted out, sifted, entered and statistically analyzed using modern computer and data mining technology to summarize the characteristics of and regularities in the application of acupoints in the literature.ResultSanyinjiao was the most frequently used point for acupuncture treatment of diabetes (68). Two points[Pishu(BL20)and Shenshu(BL23)], three points[Pishu, Shenshu andSanyinjiao(SP6); Weiwanxiashu(Ex-B3), Pishu and Shenshu], four points[Zusanli(ST36),Pishu, Shenshu and Sanyinjiao]and five points[Zusanli, Pishu, Shenshu, Weiwanxiashu and Sanyinjiao]were the most frequently used point combinations. The most frequently used meridian was the Bladder Meridian of Foot-Taiyang (260). The selected points were distributed on the back and the lower limb. The proportion of used specific points reached 74.5%. Five Shu points and Back-Shu points were used most frequently. The proportion of using manual acupuncture was 57.6%. Treatments with the cooperation of Dietary control and exercise conditioning accounted for 64.4%.ConclusionAcupuncture treatment of diabetes is mainly based on regulating the middle Jiao, with the upper Jiao and the lower Jiao taken into account. Specific points are the main part of point combinations. Manual acupuncture is the main treatment and meanwhile, attention is paid to adjusting life style.
4.Pathological Information on Infrared radiation spectrums at the Neiguan acupoint in patients with coronary atherosclerotic heart disease

BACKGROUND: Infrared spectrum technology has potential and important value of application in diagnosing disease and analyzing pathology, as well as providing references for proper treatment. OBJECTIVE: To analyze the distinctive pathological information in the spectrums of infrared radiation at Neiguan (PC 6) acupoint of patients with coronary atherosclerotic heart disease (CAHD). DESIGN: Case-control study.SETFING: Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine. PARTICIPANTS: The study was conducted from April 2003 to April 2005 in patients with CAHD from Out-Patient Clinic in Longhua Hospital (21 males and 29 females, averagely 58.1 years). Forty-seven healthy volunteers were all from advertisement (25 males and 22 females, averagely 50.6 years). All the subjects were voluntary. METHODS: PHE201, a high-sensitive infrared spectrum detective device (Shanghai Institute of Technical Physics of Chinese Academy of Sciences, China) was applied to detect the spectrums of infrared radiation at Neiguan in 50 patients with CAHD and 47 healthy adults were selected as control for comparison. The scanning range was from 1.5μ m to 16μ m,73 detected wavelength spots were scanned at Neiguan of all the subjects, and the scanning spacing was 0.2μ m. MAIN OUTCOME MEASURES: Intensity of infrared radiation at Neiguan between CAHD patients and normal controls at different wavelengths. RESULTS: The intensities of infrared radiation of CAHD patients were significantly different from that of the healthy volunteers at 32 spots of the right Neiguan as well as at 23 spots of the left Neiguan among the 73 detected wavelength spots(P < 0.05-0.001). At 2-2.5μ m wavelength spots that were related to the energy metabolism, the intensities of infrared radiation of CAHD patients were significantly lower than the healthy volunteers (P < 0.05-0.005). Concerning the difference in intensities of infrared radiation between the left and the right Neiguan, the CAHD patients had more wavelength spots than healthy volunteers did (P < 0.001). CONCLUSION: ①Distinctive pathological information exists in the infrared spectrums at Neiguan in CAHD patients. ②CAHD patients have hypo-activities of energy metabolism and hypo-function ofqi and blood in the area of Neiguan.
5.Interference of human tissue kailikrein on renal interstitial fibrosis in rats with 5/6 nephrectomy

Objective To investigate the interference and associated mechanism of hnman tissue kallikrein (HK) gene on renal interstitial fibrosis in rats with 5/6 nephrectomy. Methods Human kallikrein cDNA was packed in a recombinant adeno-associated virus(rAAV)-based plasmid vector. The rAAV-HK was produced by transfection in 293 cells. Twenty-four male Wistsr rats were divided into sham operation and operation groups. The rats with 5/6 nephrectomy were randomly divided into simple operation, control and experiment groups. The rats in experiment group received single dose rAAV-HK via the tail vein with 1×1011 pfu. Before nephrectomy and every month after surgery until the rats were sacrificed, the caudal arterial pressure was measured using tail cuff blood pressure determinator. Three months after HK gene delivery, the rats were sacrificed. The expression of HK in rats was assessed by RT-PCR, Western blot and enzyme-linked immunosorbent assay (ELISA). The pathological changes of renal interstitium were evaluated by Masson stainning, and the distribution of bradykinin B2 receptor (BKB2R) and angiotensin Ⅱ typel receptor (ATIR) was examined by immunohistochemistry. The expressions of BKB2R, AT1R, p-MAPK protein in renal tissue were detected by Western blot. Results Three months after HK gene delivery, the systolic blood pressure of experiment group was significantly decreased compared with the control group [(163±13) nun Hg vs (217±16) mm Hg, P<0.01](1 mm Hg=0.133 kPa). Compared with sham rats, the rats in simple operation group and control group had much more renal interstitial collagen deposition and more serious fibrosis performance, but renal interstitial collagen deposition and fibrosis were significantly ameliorated in the rats of experiment group. In addition, the tubulointerstitial injury index of HK transgenic rats was significantly lower than that of the rats in control group (1.33±0.73 vs 3.01±0.62, P<0.01). Up-regnlating expression of bradykinn B2 receptor protein and down-regulating expression of AT1 receptor and p-MAPK protein were found in renal tissues of experimental group after three months (P<0.05). Conclusion HK gene delivery significantly alleviates renal interstitial fibrosis in rats with 5/6 nephrectomy through regulating the expression of bradykinin B2 receptor, AT1 receptor and p-MAPK in renal tissue.

7.Effect of infrared laser on pituitrin-induced bradycardia in rabbits

OBJECTIVE: To observe the effect of infrared laser stimulation on Neiguan (PC 6) for bradycardia of rabbits induced by injection of pituitrin. METHODS: 10.6 microm CO(2) infrared laser, 650 nm semiconductor red laser and the compound laser of their aggregation were sent out by a compound laser sanatory instrument to stimulate Neiguan (PC 6) respectively for the purpose of analyzing the differences of the effects exerted by infrared laser and red laser as well as the changes of the effects after their aggregation. RESULTS: After 30 min and 40 min of the injection of pituitrin, the heart rate showed no significant difference between 10.6 microm CO(2) infrared laser group and the normal control group (P>0.05). However, the heart rate showed significant difference between the 650 nm semiconductor red laser group and the normal control group (P<0.05). After 30 min of the injection of pituitrin, the heart rate of the compound laser group were significantly different to that of the untreated group (P<0.05), and it was not significantly different to that of the normal control group (P>0.05). However, the heart rate of non-acupoint stimulated by the compound laser group showed no significant difference to that of the untreated group (P>0.05), and it was significantly different to that of the normal control group (P<0.05). CONCLUSION: Stimulation on Neiguan with 10.6 microm CO(2) infrared laser can evidently improve the bradycardia induced by injection of pituitrin, while the 650 nm semiconductor red laser has no such function, but they have coordinating effect. The thermal effect produced by CO(2) infrared laser stimulating acupoint is probably beneficial for semiconductor laser penetrating skin as well as the biochemical reactions it carries. The improvement of heart rate produced by the laser stimulation on Neiguan (PC 6) has relative specificity.
8.Application of emergency bedside focused assessment with sonography for trauma together with regional organ focused ultrasonography in closed abdominal trauma

Objective To investigate the value of emergency bedside focused assessment with sonography for trauma (FAST) in combination with regional organ focused ultrasonography in patients with closed abdominal trauma.Methods Two hundred and thirty-seven patients with closed abdominal trauma underwent bedside ultrasonography in the Emergency Department at General Hospital of Chengdu Military Command.In patients with unstable hemodynamics or severe craniocerebral injury,ultrasonography was ended immediately after FAST screening.In patients with stable vital signs,the focused regional organ injury after FAST screening was determined based on the mechanism of trauma,action location and direction of external force,abdominal symptoms,and signs.The results of operation and CT scanning were considered as golden criteria and used to compare with those of ultrasonography.Results (1) The diagnostic sensitivity of FAST to abdominal organ rupture and ascites due to abdominal organ rupture calling for emergency operation was 79.3% and 100%,respectively.The total specificity of FAST was 98.2%,positive predictive value 93.9%,and negative predictive value 86.7%.(2) The diagnostic accordance rate of single visceral injury examined by regional organ focused ultrasonography was 91.2% and that of multiple visceral injury was 63.6%.The missed diagnoses were predominantly contusion and microlaceration.Conclusions FAST can quickly screen out the patients with unstable hemodynamics due to abdominal organ rupture calling for emergency management.Regional organ focused ultrasonography can increase the rate of correct diagnosis of visceral injury and decrease the rate of missed diagnosis.
9.Dynamic observation of GVHD induced by chemotherapy and Th17/Treg in a relapse T-ALL patient after allo-HSCT

Objective To explore clinical features of chemotherapy-induced graft-versus-host disease and to observe the dynamical changes of Th17/Treg ratio and related cytokines in one relapsed acute T-lymphoblastic leukemia after allo HSCT.Methods Twenty health volunteers were healthy controls.One patient achieved complete haematologic remission after two courses of chemotherapy and underwent matched HLA identical sibling allogenic peripheral blood stem cell transplantation,donor cell implanted state,disease recurrence and graft-versus-host disease were observed and Th17/Treg cells and its related factors in the peripheral blood were detected in different periods dynamically changes by methods of flow cytometry and ELISA.Results The patient achieved complete donor chimerism (FDC) at day +27 after transplantation.Hematopoietic function was fully recovered at day +34.Chronic GVHD (cGVHD) occurred at day +110.Thereafter,extramedullary relapse occurred in cGVHD state at day +264.After one course of Hyper-CVAD chemotherapy,patient complicated overlap syndrome with cGVHD of oral cavity ulcer and degree Ⅳ intestinal aGVHD at day +294 and day +347,respectively.Th17 cell ratio in CD+4 T cells (1.7 %) in the overlap syndrome slightly increased,compared with control group [(0.56±0.35) %].The ratio of Treg cells in CD+4 T cells (4.66 %) with cGVHD increased compared with normal control group [(0.59±0.33) %],recurrence (0.39 %),and hematopoietic stem cell implantation (1.15 %),but the ratio of Treg cells increased significantly when patient complicated overlap syndrome (7.83 %).The ratios of Th17/Treg were more than 1 at relapse stage and less than 1 at GVHD stage.The IL-17A level in serum significantly increased in cGVHD (6.114 pg/ml)and overlap syndrome (6.805 pg/ml) stage compared with normal control group [(5.19±0.77) pg/ml].TGF-β1 levels were significantly higher at different periods after transplantation compared with control group [(48.81±4.9) ng/ml].Conclusion Chemotherapy can induce GVHD in relapsed acute leukemia patients after hematopoietic stem cell transplantation,and the imbalance of the Th17/Treg cells and its cytokines maybe related with disease relapse and GVHD.
10.Application of Target-guided Teaching Method in the Practice of Teaching Meridian and Acupoint Science

TheMeridian and Acupoint Science is a basic course in the major of acupuncture-moxibustion and tuina. We applied target-guided teaching method in teaching this course to evoke student's interest in studyingMeridian and Acupoint Science through initiative guidance, and boost student's initiative and enthusiasm by exhibiting the teaching targets. During the practice, we emphasized the specification of each unit target, around which we adopted various methods; meanwhile, we also focused on prompt evaluation, based on which necessary adjustments would be made to approach the targets. Teachers should act as guider, maintainer and promoter in class, rather than transmitter, to help students to study positively rather than passively listen. Via this kind of teaching reform, students not only obtained knowledge, but also enhanced their capability and feelings.
