1.On the evaluation and improvement of the amount-based restriction of negotiation in private when resolving medical disputes

The amount-based restriction of negotiation in private when resolving medical disputes is quite com-mon in much local legislation.This kind of provision is legitimate in general, because the rights and interests it pro-tects overweigh those it limits, and the measures it takes are proper.However, there are several flaws.Firstly, as a provision which impairs the rights of citizen, the level of legislation and legal effect need to be upgraded.Secondly, the set of the amount of limitation is arbitrary.Thirdly, differential treatment of public and non-public medical insti-tution in the provision cannot be justified;it actually shows confliction between different government functions.Last-ly, blindly limitation of negotiation in private has neglect something important, that is the self-government functions behind autonomous disputes settlement mechanism.Based on these flaws, several improvement suggestions are pro-vided.
2.The legal safeguard of TCM hospital preparation

TCM hospital preparation, which is only used inside its own orgnations, is an important part of clinical medicine in TCM. In recent years, the lack of a corresponding constitution of legal safeguard seriously hampered the prosperities of TCM hospital preparations. It's a high time to settle a targeted legal safeguard constitution of TCM preparations developed by the hospital. The author takes hospital of TCM preparation as the research object, starting from the current situation and problems of development of hospital of TCM to dig out the reasons. It has great significance to ensure the status of TCM preparation developed by the hospital at first, to combine with other measures. and to put forward about the establishment of comprehensive legal safeguard mechanism for the development TCM hospital preparation.
3.The diagnostic and treated analysis of primary cervical adenocarcinoma

Objective To explore the clinical features of primary cervical adenocarcinoma in order to improve the level of diagnosis and treatment.Methods The clinical data of 25 patients with primary cervical adenocarcinoma were retrospectively analyzed.Results The age was 18-75 (53.23 ± 11.37) years old and 20 (80%,20/25) patients was postmenopause.The gynecologic examination showed that cervical shape was irregular,cervical canals pachynsis,hardening,extension and widen.The gross type included 1 case of erosio lump type,2 cases of fungating type,17 cases of nodus and induration ulcer type,5 cases of cervical canals type.Twenty-two patients received extensive hysterectomy and 2 patients received sub-extensive excision,1 patient was forbiddened and was only received chemical therapy for the abdominal extensively adhesions with little ascites.Twenty-three patients were performed surgery and extracorporeal irradiation or high to midst dose rate after loading program radiotherapy; twelve of them were performed chemoradiation,but eleven patients couldn' t tolerable the chemical side effects and only took radiotherapy.The 5-year survival rate was 76% (19/25).The poorest prognostic patients were the stage Ⅳ women,which died after 3 months and 9 months respectively from first visit the doctor.The mass diameter and the early FIGO stage were correlated with prognosis (P <0.05).But age,gross type,pathological type,treated methods had no correlation with prognosis (P > 0.05).Conclusions Cervical adenocarcinoma would be predilection postmenopause female.The common signs include cervical shape irregular,cervical canals pachynsis,hardening,extension and widen.The 5-year survival rate of the patients who received operation combined modality therapy in early stage and with small mass size is higher.The prognosis would be poor relatively in late FIGO stage and mass diameter over 4 cm.So,early discovery,early diagnosis and early treatment would be the key ways to improve 5-year survival rate.

5.Effect of compatibility between Caulis Aristolochiae Manshuriensis and Cortex Moutan on aristolochic acid Ⅰ

Objective To investigate the thermostability of aristolochic acid Ⅰand the effect of the compatibility on aristolochic acid Ⅰ in Caulis Aristolochiae Manshuriensis decoction and to explore the detoxification mechanism of compatibility for aristolochic acid Ⅰ.Methods Analyzing the contents of aristolochic acid Ⅰ by HPLC in the single decoction of Caulis Aristolochiae Manshuriensis,the concoction of Caulis Aristolochiae Manshuriensis with Cortex Moutan,the residues of decocted Caulis Aristolochiae Manshuriensis and the residues of concocted Caulis Aristolochiae Manshuriensis with Cortex Moutan,respectively.Results Aristolochic acid Ⅰ decreased after heating in pure water,a new peak was found in HPLC spectra and supposed to be the derivate of aristolochic acid Ⅰ,which was also found in the decoction of Caulis Aristolochiae Manshuriensis.The content of aristolochic acid Ⅰ in the concoction of Cortex Moutan with Caulis Aristolochiae Manshuriensis is lower than that in the single Caulis Aristolochiae Manshuriensis decoction.Furthermore,the quantity of aristolochic acid Ⅰ in the residues of Caulis Aristolochiae Manshuriensis after concoction is lower than that in the residues of single Caulis Aristolochiae Manshuriensis decoction.Conclusion Aristolochic acid Ⅰ is unstable in decoction and a part of it was changed into another compound.The stripping of aristolochic acid Ⅰfrom Caulis Aristolochiae Manshuriensis is not inhibited when Cortex Moutan concocted with Caulis Aristolochiae Manshuriensis.It is the chemical reaction of aristolochic acid Ⅰ,which could decrease the toxicity in the decoction or concoction of Caulis Aristolochiae Manshuriensis.

Objective To observe the effect of l-3-n-butylphthalide(l-NBP)on the of nuclear factor-?B and I?B-? in primary cultured murine brain glial cells following oxygen-glucose deprivation/ reoxygenation(OGD/R).Methods Cultured mouse brain glial cells in vitro and established OGD/R model.Glial cells were divided into five groups: normal control(NC)group,OGD 24 h/R 24 h group,l-NBP 20 ?mol/L group,l-NBP 100 ?mol/L group,l-NBP 500 ?mol/L group.The protein expression of NF-?B in nuclear and I?B-? in endochylema were analyzed by Western blot.Results Comparing with NC group,OGD 24 h/R 24 h group showed a significantly higher expression of NF-?B(P
7.Interpretation and discussion about the Chinese physician group model and the prevention and control of its legal risks

This paper introduces the development situation and mode of physician group in China, and analyzes its main legal risks and countermeasures.The main legal risks include: the nature of the Physician group is fuzzy and specific laws and regulations lack a clear stipulation;the legitimacy of the cooperative mode of the physician group is difficult to define and lacks legal protection for its contract effectiveness and implementation;the practice is driven by the interests and loopholes in legal norms and physicians may illegally transfer the patients;the multipoint practice policy is not perfect and brought legal risks for the development of the physician group.Therefore, the following measures are proposed to take adequate precautions: firstly, the nature of the physician group and its standards of supervision and taxes should be identified and clarified;secondly, the legitimacy standard for the physician group should be established for its cooperation mode to standardize the doctors` behavioral standards;and finally, we should make sure the multipoint practice policy and physician practice law are coherent during implementation.
9.Treatment of pancreatic pseudocysts by therapeutic ERCP

Objective To determine the effect of therapeutic ERCP(endoscopic retrograde cholan-giopancreatography) on endoscopic drainage of pancreatic pseudocysts and patency of main pancreatic duct after dilation or placement of stent. Methods Eight patients with pancreatic pseudocyst were selected after abdominal ultrasonography and computed tomography. All patients have had be operated two times because of acute pancreatitis and /or cholangitis, and were hard to receive further operation. These patients underwent endoscopic transpapillary drainage by main pancreatic duct dilation or placement of stents after ERCP, and were follow up by abdominal ultrasonography and / or computed tomography 6 months after drainage. The stent would be pulled out after the complete disappearance of the cyst. Results Diagnostic ERCP revealed that cysts communicated with main pancreatic duct in 3 cases and obstructive jaundice was present in 1 case. All patients were managed by transpapillary main pancreatic duct dilation (3 cases) and placement of stents (5 cases). Both forms of endoscopic drainage were effective in treating pancreatic pseudocyst and in 7 cases the cysts were completely disappeared within 4 months, while the rest one required surgery as the cyst merely decreased in size. Only 2 cases the levels of amylase in serum and urine were higher than normal but no severe complications occurred. Conclusion Endoscopic transpapillary drainage through main pancreatic duct dilation or placement of stents is quietly an effective, painless and safe mode in treating pancreatic pseudo-cyst. This method may be an ideal choice for the treatment of pancreatic pseudocysts in experts.
10.Sequential changes of HIF-1αprotein and mRNA in hypoxic bovine retinal microvessel endothelial cells

● AIM: To investigate the sequential changes of HIF-1 α Protein and mRNA in hypoxic bovine retinal microvessel endothelial cells.● METHODS: The bovine retinal microvessel endothelial cells were cultured in normoxic and CoCl2-induced hypoxic conditions respectively. Expressions of HIF-1 α Protein were measured with immunohistochemical staining, and RT-PCR was used to determine the HIF-1 α mRNA.● RESULTS: HIF-1 α began to increase 1h after hypoxia,and reached the peak at 4h. After 16h, it declined significantly. Compared with the normoxic group, the expression of HIF-1 α protein in the hypoxic groups had significant difference (P<0.01), and HIF-1 α mRNA expression was unchanged under hypoxia.● CONCLUSION: HIF-1 α participates in the hypoxic procedures in retinal microvessel endothelial cells, and hypoxia induce time-dependent changes of HIF-1 α protein expression, which is not modulated on the transcription level. Analysis of HIF-1 α expression revealed a temporal and spatial changes with regard to the hyperoxic repression, indicating that HIF-1 may play a major role in the development of retinopathy of prematurity and other ischemic retinal disorders such as diabetic retinopathy.
