1.Laparoscopic procedures for adnexal cyst torsion in pregnant women:An analysis of 12 cases

Objective To evaluate the feasibility of laparoscopic procedures in pregnant women. Methods A retrospective analysis was made on 12 pregnant women with an acute abdominal emergency resulted from adnexal mass from 1997 to 2002.Results All the 12 patients were laparoscopically diagnosed as having pregnancy associated with torsion or necrosis of adnexal mass. There were 2 cases of unwinding of the twisted fallopian tube and oophorocystectomy, 3 cases of oophorocystectomy, and 7 cases of adnexectomy. All the patients were cured. No miscarriage or infant malformation happened. Follow-up for 8~12 months in 11 infants revealed normal development. Conclusions In pregnant patients the laparoscopic procedure is relatively contraindicated. Satisfactory curative effects can be obtained if the patients are properly selected.
2.Accumulation and Movement of Heavy Metals in the Environment Through Sewage Sludge Using in Agriculture

Objective To know the accumulation and movement of heavy metals in the environment through sewage sluge using in agriculture. Methods The sewage sludge samples were collected from sewage treatment farms of Suzhou, Wuxi and Changzhou in the different seasons of 1998. The content and states of heavy metals were determined by AAS. The accumulation and movement of heavy metals were analyzed as sewage sludge was used in agriculture. Results The content of heavy metals in the sewage sludge was greatly higher than that in the soil background. Especially, the contents of zinc in the sewage sludge in three cities exceeded the related standard, the means were 1 199.00, 784.00 and 1 216.00 mg/kg respectively. A long term using of sewage sludge might induce heavy metal accumulation in the soil. Conclusion Sewage sludge used in agriculture may induce heavy metal accumulation in the soil and the organisms, so the risk of environmental heavy metals accumulation may increase.
3.Clinical value of DR and CT in the secondary pulmonary tuberculosis

Objective To discuss different performance of DR and CT examination on secondary pulmonary tuberculosis,to improve the clinical guidance value of CT in the diagnosis of secondary pulmonary tuberculosis. Methods The image data of 52 cases with secondary pulmonary tuberculosis confirmed by pathology were collected, including chest DR radiography and CT scans.The different image characteristics of two kinds of imaging examination were analyzed and compared.Results In 52 cases of secondary pulmonary tuberculosis,chest X -ray plain film found lesions calcification in 3 cases,2 cases of bronchiectasis,pleural thickening and adhesion in 12 cases,pleural effusion in 1 case,empty cavity in 4 cases,emphysema in 1 case,the lung nodules in 5 cases.Chest CT revealed lesions calcification in 9 cases,16 cases of bronchiectasis,pleural thickening and adhesion in 40 cases,pleural effu-sion in 4 cases,empty cavity in 10 cases,emphysema in 8 cases,the lung nodules in 10 cases,6 cases of pulmonary bullae,pulmonary interstitial change:4 cases of lung reticulocyte shadows,interlobular septal thickening in 4 cases, ground glass sample density in 4 cases.Conclusion DR and CT can be used for inspection of secondary pulmonary tuberculosis,but CT is a better tool,it has important guiding role for clinical diagnosis and treatment.
4.Effect of STAT3 siRNA on proliferation and apoptosis of gastric cancer cells in vitro

Objective To design and construct a vector of signal transducer and activator of transcription 3 (STAT3) small interfering RNA (siRNA),and to investigate the effect of the recombinant plasmid on the proliferation and apoptosis of gastric cancer cell SGC7901. Methods The PAVU6+27-STAT3 siRNA expression vector was constructed, then transfected into the cultured gastric cancer cells SGC7901 by DOTAP method. STAT3 mRNA and protein in SGC7901 were determined by RT-PCR and Western blotting respectively. Cell proliferation was tested by MTT and 3H-TdR incorporation. Cell apoptosis percentage and bcl-2 expression were observed by TUNEL and flow cytometry respectively. Results The PAVU6+27-STAT3 siRNA expression vector was successfully constructed. In the transfected SGC7901 cells, cell proliferation, 3H-TdR incorporation and bcl-2 expression were decreased, and cell apoptosis percentage was increased. Conclusion PAVU6+27-STAT3 siRNA expression vector may be an efficient method to inhibit the proliferation and promote the apoptosis of gastric cancer cells.
5.Long-term result of combined splenorenal shunt plus portaazygous devascularization in the treatment of bleeding esophageal varices in patients of portal hypertension

Objective To evaluate the therapeutic effect of combined splenorenal shunt and portaazygous devascularization on bleeding esophageal varices.Methods We summarized 120 cases suffering from esophageal variceal bleeding treated by splenorenal shunt plus portaazygods devascularization,from April,1978 to April,1997.Results The operative mortality was 4.2%,no short-term postoperative bleeding,and long-term postoperative bleeding rate was 9.5%,postoperative hepatic encephalopathy rate was 4.8%.Five,ten and fifteen years survival rate were 83.6%,64.5% and 54.5% respectively. The portal hemodynamics was observed by means of Doppler flowmeter,DSA,and intraoperative portal pressure measurement in some of the patients.Conclusion The result showed that combined shunt and portaazygous devacularization not only preserve the advantage of devacularization but also decrease the portal pressure and blood volume,alleviating portal hyperhemodynamics while preserve hepatopetal blood flow to some extent.
6.Cervical adenoid basal cell carcinoma:a clinlcopathologic study of 12 patients

Purpose To investigate the clinicopathologic features of cervical adenoid basal cell carcinoma. Methods Clinical and pathological data were analyzed in 12 cases of cervical adenoid basal cell carcinoma and the relevant literatures were reviewed. Results The age of 12 cases of with cervical adenoid basal cell cancer patients ranged from 36~70 years ( mean:53. 3 years) , with a medi-an age of 51. 5 years. Amongst 12 patients who contained no gross definite lesion, 5 patients (41. 7%) had the symptoms of vaginal bleeding. 8 cases of patients underwent hysterectomy, while the other four received cervical conization. Cervical intraepithelial neopla-sia ( CIN) lesions were observed in all 12 cases. The tumor cells were small and uniform, with dark oval nuclei without conspicuous nucleoli and scanty cytoplasm. Tumor infiltrated into the stroma in nests and cords. Glandular differentiation within tumor nests were seen in 12 cases, including two cases of squamous cell differentiation accompanied by partial and minor interstitial edema. Peripheral palisading cells around tumor nests were also found. The infiltrating depth of tumor ranged from 0. 5~10 mm with the average 4. 12 mm. Cancer embolis were available in 2 cases with no lymph node metastasis. 10 cases followed up for 3~78 months with no recur-rence and metastasis, but 2 cases were lost. Conclusion Adenoid basal cell carcinoma is a rare uterine cervical tumor found in post-menopausal women, which are often accompanied with CIN. This tumor has a favorable prognosis and should be clearly separated from adenoid cystic carcinoma and other tumors which also have infiltrative growth pattern.
7.Detection of serum HCV RNA in patients with chronic hepatitis C by transcription mediated amplification and real-time reverse transcription polymerase chain reaction

Objective:To observe the stability and sensitivity of transcription mediated ampliifcation (TMA) system, and to compare it with real-time reverse transcription polymerase chain reaction (RT-PCR) in amplifying serum HCV RNA in HCV infected patients. Methods: TMA system was established by moloney murine leukemia virus (MMLV) reverse transcriptase, T7 RNA polymerase and 2 speciifc primers ifrstly,and then its stability and repeatability were compared at different storage temperatures by the correlation change of HCV RNA amplification curve. The sensitivity difference between TMA and RT-PCR was evaluated by amplifying a group of 10-fold diluted HCV RNA samples which were transcribed in vitro. A total of 101 serums of chronic HCV infected patients were measured by TMA system and RT-PCR to observe the positive rate and their correlation. Linear correlation and linear regression were used to observe the correlation of the two methods. Results:TMA system was successfully established. TMA system was not stable when stored at 20℃ (placed for 24 hours only), but it was stable for 6 days when stored at 4℃ or within 6 months when stored at-20 ℃. Compared with RT-PCR whose reagent was made by Hunan Sansure Biotechology Corporation, TMA system showed 20 positive samples and 11 negative samples in a total of 31 samples. So was the RT-PCR kit of the Sansure Biotechology Corporation, and the concordance rate of the two methods was 100%. Advanced quantitative study of the 20 positive samples found that the two methods had good correlation and consistency (r=0.91,P<0.01). Compared with the results of RT-PCR whose reagent was made by Shanghai Kehua Bio-engineering Corporation, TMA system had 34 positive samples and 36 negative samples, while the RT-PCR technology had 32 positive samples and 38 negative samples out of 70 samples. hTe concordance rate of the two methods was 97.1%, with no statistical difference in the positive rate of the two methods (P>0.05). Advanced quantitative study of 29 positive samples found that the two methods had good correlation and consistency (r=0.96,P<0.01). Conclusion:The stability and repeatability of TMA system are good within 6 months when stored at-20 ℃ storage temperature. Both TMA and RT-PCR HCV RNA can detect serum HCV RNA well, and the two methods have good correlation and consistency.
8.PATHOGENIC AND PATHOLOGICAL OBSERVATIONS OF PNEUMOCYSTIS PNEUMONIA IN RATS

In order to induce heavy Pneumocystis pneumonia, rats were treated with dexame-, thasone twice a week for 14 weeks. Mature cysts, immature cysts and ruptured cysts were, identified on lung imprints. By phase-contrast microscopy mature cysts were spherical in, shape containing intracystic bodies both spherical and irregular, small to large trophozoites having polymorphic shapes, one nucleus and many vacuoles. Elcctro-microscopically, large amount of irregular trophozoites adhered to the surface of the type I alveolar epithelial cells in the alveolar cavity were observed. The above histological findings were typical features of Pneumocystis pneumonia (Figs. 1-8).
9.Hepatitis B virus X gene transforms human hepatocyte QSG7701 in vitro

Objective To observe the effects of hepatitis B virus(HBV)X gene muhifunctional protein(HBx)on the biological characteristics of QSG7701 and the transformational effects on QSG7701 cell.Methods QSG7701 ceils were stably transformed by recombinant plasmid pCMV/X and eukaryotic expressed plasmid pRc/CMV2 by liposome-based assay,respectively.Non-transfeeted QSG7701 cells were employed as controls.The expressions of HBx,c-Myc and Bel-2 proteins in QSG7701 cells were detected by Western blot.MTT colorimetric analysis,flow cytometry and soft agar clone-forming assay were performed tO detect the biolo~dcal activity of cells.Results HBx Drotein was highly expressed in pCMV X/QSG7701 cells.The expression level of c-Myc protein in the pCMV X/QSG7701 cells was much higher than those in the other tWO groups of cells.The expression of Bcl2 protein was detected in the three groups of cells,but the expression levels were similar.Percentage of S stage cells in pCMV X/QSG7701 ceils was significantly higher than those in pRcCMV2/QSG7701 and non-transfected QSG7701 cells E(28.80±2.32)%,(15.5±2.64)%and(21.5±3.66)%,LSD 0.05=3.95%,LSD 0.01=5.47%,P<0.01].While percentage of G1 stage cells in pCMV X/QSG7701 cells was significantly lower than those in pRcCMV2/QSG7701 and non-transfected QSG7701 cells[(62.30±3.85)%,(78.70±4.12)%and(78.10q±4.45)%,LSD 0.05=5.63%,LSD 0.01-7.79 %,P<0.01].The apoptosis rate of pCMV X/QSG7701 cells was much higher than those in pRcCMV2/QSG770t and non-transfected QSG7701 cells[(14.90±1.01)%,(8.91±0.48)%and (4.03±0.47)%,LSD 0.05=O.94%,LSD 0.01=1.31%,P<0.01].The population doubling time of pCMV X/QSG7701 cells was shorter than those in pRcCMV2/QSG and non-trarmfected cells(14 h,29 h,38 h,respectively).The cloning ratio in soft agar of pCMV X/QSG7701 cells WaS significantly higher than those of pRcCMV2/QSG and non-transfected QSG7701 cells[(19.83±1.96)%,(1.76±0.03)%and (1.33±0.18)%,LSD 0.05=1.53%,LSD 0.01=2.11%,P<0.01].Conclusion HBx may transform human non-tumor hepatoeyte QSG7701,which makes the cell growth malignizeA.
