2.Coblation-channelling for the tongue.

OBJECTIVE:
To investigate the efficacy of Coblation-channelling for the tongue(CCT) treatment of tongue hypertrophy.
METHOD:
The 31 patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS) combining tongue hypertrophy staged as Friedman ll or N were performed CCT after nasal septum surgery, coblation channelling of bilateral inferior turbinate and coblation-assisted UPPP. While the vertical channelling in the base and the body of the tongue, the tilted one parallel side edge and posterior to the tongue were combined. The intraoperative and postoperative complications such as bleeding were observed. The 12 months postoperative follow-up were done by MRI of the tongue.
RESULT:
The retrolingual space were expanded postoperatively. The 2 bleedings occurred immediately after channellings in the tongue body, and they were stopped by local compression. No postoperative tongue paralysis, no tongue hematoma and abscess happened. The tongue sizes changed from preoperative III or IV degree to postoperative I to II degrees. One case received three CCT sessions.
CONCLUSION
CCT treatment tongue hypertrophy is a individually safe, effective and minimally invasive treatment.

3.Application of arginine stimulation test in evaluating function of pancreatic islet beta cell

Objective To investigate the value of intravenous arginine stimulation test (AST) in evaluating function of pancreatic islet beta cell response in patients of diabetes mellitus. Methods Twentyone patients with type 1 diabetes mellitus (DM1, DM1 group) and 113 patients with type 2 diabetes mellitus (DM2, DM2 group) were recruited in this study. DM2 patients were divided into two sub-groups, DM2a group (duration of no more than 1 year, 58 patients) and DM2b group (more than 1 year, 55 patients). The serum levels of C-peptide (CP) were determined at fasting and 2,3,4,5 minutes after intravenous injection of 5 g arginine. Results In DM1 group, the level of CP after injection of arginine was. similar to the fasting level (P> 0.05 ). In DM2 group, the peak level of CP appeared at 3th minute (CP3) during AST, and was significantly higher than fasting CP level(P < 0.01 ). The level of fasting and arginine-stimulated CP in DM2a group was significantly higher than that in DM2b group, and the level of fasting and arginine-stimulated CP in DM2b group was significantly higher than that in DM1 group. The patients of DM1 group whose level of CP3 < 600 pmol/L all needed insulin injection to control hyperglycemia, and the coincident rate was 100.0%.In DM2 group, there were 91 patients whose CP3 ≥600 pmol/L, among which 85 patients could be well controlled with diet or oral hypoglycemic agents, and the coincidentrate was 93.4%; there were 22 patients whose CP3 < 600 pmol/L, among which 19 patients need insulin injection to control hyperglycemia, and the coincident rate was 86.4%. Conclusions AST is valuable in assessing the function of pancreatic islet beta cell in patients with diabetes mellitus. The level of CP3 ≥600 pmol/L can be considered as a reference in diagnosis and treatment of diabetes mellitus.
4.Application of endotracheal silicone stent with silver wire in reconstruction of laryngotracheal stenosis

Objective To investigate the use of the endotracheal silicone stent with silver wire in repair of stricture of laryngotrachea. Methods A retrospective analysis was performed on nine cases with stricture of laryngotracheal stenosis that were treated with endotracheal silicone stent with silver wire that was then decannulated three months later. Results Endotracheal silicone stent with silver wire were successfully decannulated in all cases. The follow up ranging from 11 months to three years showed no stricture happened again. Conclusion Endotracheal silicone stent with silver wire can effectively and minimally invasively treat the stricture of laryngotrachea.

The clinical application of biological immunotherapy such as chimeric antigen receptor T cells (CAR-T) and novel targeted therapy has explored a new therapy for multiple myeloma (MM) treatment. Targeting B-cell maturation antigen (BCMA), allogeneic CAR-T, antibody-drug conjugate (ADC) and bispecific antibody targeting BCMA have achieved remarkable efficacy and safety in many clinical studies. This article introduces the latest immunotherapy for MM at the 62nd American Society of Hematology (ASH) Annual Meeting.
6.Advances in treatment of congenital posterior choanal atresia.

Congenital posterior choanal atresia is a rare congenital deformity. Due to the influence of nose and face and intellectual development, its preferred surgical treatment. But surgical treatment methods and technology are in dispute. The using of high resolution display systems combine with micro tools can effectively reduce postoperative scarring and adhesion formation which is the key point to prevent restenosis. There is also the focus of controversy whether should place an expansive tube after operation. Currently nasal endoscopic approach has become the first choice for otolaryngologist treating congenital posterior choanal atresia.

7.Endoscopic resection for parathyroid adenoma in 5 patients

Objective To investigate the feasibility o f endoscopic resection of parathyroid adenoma. Methods Endoscopi c parathyroidectomy by anterior chest approach was performed in 5 patients with parathyroid adenoma. Results The operation was successfully ac complished in all the 5 patients, requiring no conversions to open surgery. The operating time was 120~150 min (mean, 130 min), and the intraoperative blood los s, 5~15 ml (mean,10 ml). No postoperative complications such as hemorrhage, hoar seness, choke, subcutaneous emphysema and effusion, or infection were seen. All the patients were followed for 6~24 months(mean,14 months), with satisfactory cu rative and cosmetic results. Conclusions Endoscopic resection for parathyroid adenoma gives reliable curative effects and excellent cosmetic o utcomes.
8.A 10-year follow-up study on pulmonary functions in elderly patients with chronic obstructive pulmonary disease

To investigate the changes of cell apoptosis genes in pulmonary tissue of tail suspended rats during simulated weightlessness, 40 Spragno Dawley male rats were divided into 4 groups: suspended for 7d,control of 7d, suspended for 21d, and control of 21d.Tail suspension(-30?) was used as weightlessness simulation.The condition of bcl 2/bax in pulmonary tissues was detected by in situ hybridization. The results showed that the level of bcl 2 mRNA in pulmonary tissue of 7d tail suspended rats decreased significantly( P 0 05). The results also showed that the level of bax mRNA in pulmonary tissue of 7d tail suspended rats increased significantly( P 0 05).It can be concluded that there are certain changes of cell apoptosis genes in pulmonary tissue of 7d tail suspended rats during simulated weightlessness.
