Objective:To investigate the value of NMO-IgG in diagnosis of neuromyelitis optica(NMO).Methods:A total of 120 patients with NMO,including high-risk NMO(HR-NMO),multiple sclerosis(MS),and other neurological diseases were enrolled in the study.The titres of serum NMO-IgG in all patients’ were detected by indirect immunofluorence.Results:The sensitivity and specificity of NMO-IgG were 67%,92% respectively in diagnosis of NMO.There were significantly differences in serum NOM-IgG posieivity among patients with NMO and MS who experienced severe optical neuritis,transverse myelitis,spinal-cord lesion beyond 3 segments as well as the patients with normal MRI scan in brain brain normal in MRI scan,serum and accompanied with other immunological disorders(P

Objective To explore the relations between UA and serum hs-CRP, the effects of early intervention with atovastatin on serum hs-CRP in patients with UA and its clinical significance in the early management of UA. Methods T 60 patients with UA (UA group),53 patients with stable angina (SA group) and 50 healthy controls (control group) were enrolled to the study. The serum hs-CRP levels were measured by particle enhanced immunoturbidimetric assay. UA group were randomly assigned to the atovastatin group and the routine group for a 4-week treatment immediately after admission. Selected coronary artery angiography was performed in 78 patients and 29 healthy controls with Judkin's technique. 3.ULTRASTRUCTURAL STUDY OF THE THYMUS OF THE NEONATAL MOUSE

With light and electron microscope, we studied the morphology of the thymus of the neonatal mouse. The results showed: 1.the lobules of the thymus had not well developed and there was no distinct demarcation between the cortex and medulla; 2.a cyst composed of epithelial cells with microvilli or cilia might be frequently seen in the medulla; 3.occasionally small lymphocytes with some glycogen particles in their cytoplasm were observed; 4.only a few small-sized thymic corpusles existed in the medulla, The article also described the ultrastructure of the lymphocytes, epithelial reticular cells, macrophage, interdigitating cell and blood-thymus barrier in the thymus of the neonatal mouse.
4.Clinical study of the combination therapy with intranasal antihistamine and nasal corticosteroids in the treatment of nasal obstruction of persistent non-allergic rhinitis.

OBJECTIVE:
To determine if greater efficacy could be achieved with the intranasal antihistamine azelastine and the intranasal corticosteroid fluticasone propionate used concurrently in the treatment of nasal obstruction of persistent non-allergic rhinitis.
METHOD:
A total of 162 persistent non-allergic rhinitis cases with moderate to severe nasal obstruction were randomized to treatment with the following: the combination therapy or nasal corticosteroids monotherapy. Efficacy was assessed by change from baseline in nasal obstruction score at week 2 and week 6 visits. The perceptions of global treatment satisfaction(convenience, side effects, cost and effectiveness) in both groups were analyzed.
RESULT:
In both groups, the nasal obstruction score assessment descended significantly at week 2 and week 6 visits versus at baseline (all P < 0.01). At week 2 and week 6 visits, the nasal obstruction score in the combination therapy groups were significantly improved than that in nasal corticosteroids monotherapy groups (all P < 0.01). The perceptions of global treatment satisfaction in the combination therapy groups were significantly better (P < 0.05).
CONCLUSION
Azelastine nasal spray and intranasal corticosteroid in combination may provide a substantial therapeutic benefit for patients with persistent non-allergic rhinitis, especially nasal obstruction. The combination therapy was well tolerated and safety.

5.Comparison of double-channel lfexible ureteroscopy and electronic lfexible ureteroscopy for lithotripsy

Objective To compare the efficiency and safety of double-channel flexible ureteroscopy and electronic flexible ureteroscopy.Methods From April 2007 to 2016 July, clinical data of 836 cases flexible ureteroscopic holmium laser lithotripsy were collected. All the patients were randomly divided into electronic lfexible ureteroscopy group (n = 427) and double-channel lfexible ureteroscopy group (n = 409), the operation time, success rate of surgery, complications, residual stone rate of the two groups were compared.Results The success rate of surgery was 92.5% and 83.6% of the two groups respectively, the difference was significant (P < 0.05); postoperative systemic inlfammatory response syndrome (SIRS) were occurred in 10 cases and 13 cases respectively, which were cured and the difference was not statistically significant (P > 0.05); the average operation time was (81.1 ± 7.9) min and (95.3 ± 7.6) min respectively, the difference was significant (P < 0.05); The formation of stone street of electronic lfexible ureteroscopy group and double-channel lfexible ureteroscopy group were 17 cases and 25 cases. The residual stone rate of electronic flexible ureteroscopy group and double-channel flexible ureteroscopy group were 6 cases and 8 cases, of them were cleared after extracorporeal shockwave lithotrips (ESWL). There were no difference between the two groups (P > 0.05).Conclusion Both double-channel lfexible ureteroscopy and electronic lfexible ureteroscopy are effective and safe therapeutic modalities. Electronic flexible ureteroscopy is better than double-channel lfexible ureteroscopy in success rate of surgery because of its high deifnition and lfexible operation.
6.Effect of schistosomiasis control in Bianmin River of Nanjing City

Objective To evaluate the effect of comprehensive control for schistosomiasis with emphasis on environmental modification in the Bianmin River water system of Nanjing City.so as to provide scientific evidence for making up further control measures in this water system.Methods Schistosome infections of Oncomelania snails in the waterway.sentinel mice in water and neighbouring human were investigated longitudinally from 1998 to 2007,and the changes of huaman infection rates in differentyears,the infection rates of sentinel mice and snails in different settings were analyzed and compared.Results A total of 77 395 snails collected from the Bianmin River water system were dissected from 1998 to 2007,and among them,27 snails were infected with Schistosoma japonicum,with a total snail infection rate of 0.03%.A total of 61 039 snails collected from the neighbouring marshland which connected to the Yangtze River wore dissected,and among them,257 were infected with S.japonicum,with a total snail infection rate of 0.42%,and there was a significant difference compared with that in the water system(χ~2=248.55,P<0.01).After the protection works in the waterway,the infection rates of sentinel mice in the water system decreased from 69.68% in 1998 to 17.50% in 2001.with a reduction rate of 74.89%.Two years afterthe clearance ofmarshlandinthewaterway,no infected sentinel mouse was found.The infection rates of residents from 1998 to 2007 were 1.96%,1.37%,1.34%,1.60%,0.30%, 0.26%,0.16%,0.10%,0.04% and 0,respectively,andthe rates declined year by year afterthecomprehensive control.Conclusions The control measures based on the elimination of snail habitats in the waterway that is connected to the Yangtze River have achieved obvious effect.However,the clearance of the re-emerging snail habitats should be carried out termly to consolidate the control effect.
7.Diagnosis and treatment of perirenal hematoma associated with ureteral flexible lithotripsy

Objective To explore the risk factors ,prevention and treatment of the occurrence of perirenal hematoma after flexible lithotripsy. Methods We retrospectively analyzed the clinical data of 18 patients with symptomatic perirenal hematoma from 1259 who had undergone ureteral flexible lithotripsy in our hospital during the period of April 2007 to April 2016. Of the 18 patients,7 were complicated with diabetes mellitus,11 had urinary tract infection;15 were female,and 3 were male. Results Perirenal hematoma was confirmed by B ultrasound and CT,which situated on the Posterolateral side of the kidney in 8 patients,and on the lower pole and abdominal side in 10. Hematoma depth was 2.6-15.3 cm(average was 5.2 cm). The hematoma?related symptoms gradually disappeared in 11 patients 7 to 14 days after they received conservative treatment. 5 recovered gradually 15 days after undergoing hematoma puncture and drainage ,and 2 recovered 36 days after receiving open surgery for removal of hematoma. Conclusions Female,diabetes,urinary tract infection,bigger stone size,prolonged surgical duration,and infected stones were the risk factors for perinephric hematoma related to ureteroscopic lithotripsy. Full preoperative preparation ,effective anti?infection ,intraoperative improvement of calculus?breaking skills and use of large caliber semirigid through sheath,low pressure perfusion,shorter surgical duration,and staging surgery are effective ways to reduce the occurrence of perirenal hematoma.
8.Analysis and strategy of family caregiving behavior in children with recurrent lower respiratory tract infection

Objective To explore and analyze the influencing factors of family caregiving behavior and protective strategies in children with recurrent lower respiratory tract infection. Methods By reviewing the literature, a self-designed questionnaire for family caregiving behavior related to recurrent lower respiratory tract infection were adopted, including feeding behavior, hand hygiene, environmental factors, time of outdoor activities and family health-seeking behavior. Totally 206 cases with recurrent lower respiratory tract infection (the study group) and 206 cases with acute lower respiratory tract infection (the control group) were included and all cases were investigated by family caregiving behavior questionnaire. The influencing factors of family caregiving behavior of two groups were analyzed and compared. Results The feeding behavior in the study group was worse than that in the control group(χ2=5.14-14.76, P<0.05). There were significant differences in family health-seeking behavior (χ2=4.76, P=0.03), 49.50%(102/206) in the control group,38.8%(80/206)in the study group and passive smoking (χ2=5.70, P=0.02) between two groups. There was no significant difference between two groups in hand hygiene, time of outdoor activities, history of contacting with patients with respiratory tract infection, cold history (χ2=0.48-2.63, P>0.05). Conclusions We should guide parents to establish the right and reasonable family care behavior to effectively enhance children's physical fitness and disease resistance and to avoid exposure to infectious agents and harmful substances, reduce the occurrence of Recurrent Lower Respiratory Tract Infections.
9.Investigation of B-cell activating factor in serum and cerebrospinal fluid of patients with neuromyelitis optical.

OBJECTIVETo study the expression of B-cell activating factor (BAFF) in the serum and cerebrospinal fluid of patients with neuromyelitis optical (NMO).
METHODSClinical data were collected from 44 patients with NMO and 38 patients with multiple sclerosis (MS). Thirty healthy controls and 15 controls with noninflammatory neurological diseases were also recruited. The concentration of BAFF in the serum and cerebrospinal fluid were measured by enzyme-linked immunosorbent assay (ELISA).
RESULTSThe mean serum BAFF level was 250.2 ± 126.9 pg/ml in NMO patients, 249.6 ± 130.7 pg/ml in MS patients, and 222.9 ± 126.1 pg/ml in the control subjects, showing no significant difference among the 3 groups (P>0.05). The mean BAFF level in the CSF was significantly higher in NMO patients than in MS patients (525.8 ± 230.0 pg/ml vs 298.4 ± 141.9 pg/ml, P<0.05), and higher in MS patients than in the control subjects (141.4 ± 76.2 pg/ml, P<0.05). Both NMO and MS group showed a positive correlation between EDSS scores and CSF BAFF level. But in NMO patients, CSF BAFF level was not associated with AQP4-antibody titer.
CONCLUSIONBAFF in the CSF may be a useful biomarker for a differential diagnosis of NMO from MS and has an important value in evaluating the disease severity.

10.In vitro reconstruction of malformed renal blood vessels prior to kidney transplantation: 2 years follow-up

OBJECTIVE: Anatomic variation of renal artery existed in donor kidney, and whether the variation would affect clinical effects is unconfirmed. This paper is aimed to study the outcomes of in vitro reconstruction of malformed renal blood vessels on outcome of kidney transplantation. METHODS: Patients treated with kidney transplantation at Department of Urology, Lanzhou General Hospital of Chinese PLA were selected; including 27 cases underwent kidney transplantation with malformed blood vessels. Seventeen of them had accessory renal artery, 10 of them with two or three renal arteries. Additional 22 patients transplanted with normal kidney during the same period were selected as the control group. Bench surgery with microsurgical techniques was employed for the repair of the vessels, which included 11 cases of end to end anastomosis of arteries, 6 cases of end to side anastomosis of arteries, 6 cases of side to side conjoined anastomosis of arteries, and 4 cases of renal artery lengthening with an interposition of donor or recipient iliac artery. The hemorrhage, hypertension, renal arterial stenosis, delayed graft function, incidence rate of acute rejection, and the serum creatinine level were followed up.RESULTS: Totally 49 cases were received a mean 2-year follow-up, and no death occurred in both groups. Two cases in the experimental group, and 3 cases in the control group were suffered hypertension or hypertension aggravated, the difference had no significant (P =0.673). No arterial stenosis occurred during the follow up. In the experimental and control groups, the incidence of delayed graft function was 20% and 14%, the incidence of acute rejection was 13% and 5%, and mean serum creatinine at 2 year was (119±11) and (127±8) μmol/L, respectively, the difference was not significant between two groups (P=0.179, 0.385, 0.658).CONCLUSION: Donor kidneys with malformed vascular can be used for transplantation after bench reconstruction, which do not influence the outcome of kidney transplantation.
