1.The value of spiral CT and image post-processing in the evaluation of laryngeal carcinoma

Objective To access the value of the combination of axial image,MPR,and VE in theevaluation of laryngeal carcinoma involvement.Methods Twenty patients with laryngeal carcinoma or hypopharyngeal carcinoma were prospectively studied by helical CT,and MPR and VE were subsequently done on the Voxel Q workstation.The axial images findings and the combined image findings of axial image,MPR,and VE were compared with the pathological results by suing a predetermined checklist of 17 regions according to the TNM classification of malignant tumors(UICC and AJCC).the results were studied in a blind way. Results In the evaluation of the neoplastic invasion of ventricular fold,vocal cord,the anterior commissure,subglottic region,thyroid cartilage,and tissue beyond the larynx,the combined image were better than axial image in sensitivity(100% vs 92.4%,P=0.064),specificity(98.5% vs 89.5%,P=0.028),and accuracy(99.2% vs 90.8%,P=0.003).Neoplastic invasion of the arytenoid cartilage was present in 6 patients.The sensitivity and the specificity was 83.3% and 100% respectively when using the criteria of the arytenoid cartilage sclerosing combined with distortion,erosion or lysis.The specificity was only 57.1% when using the criteria of arytenoid cartilage sclerosing for judging parameter.The result was identical when assessing the arytenoid cartilage,PGS,and PES between the two groups.Conclusion Axial image combined with subsequent MPR and VE could improve the diagnosis in the evaluation of the neoplastic invasion of ventricular fold,vocal cord,the anterior commissure, subglottic region,thyroid cartilage,and tissue beyond the larynx.
2.Survey and analysis of dental medical students' concerns before and after their clinical internship

ObjectiveTo survey and analyze the changes of dental medical students' concerns before and after their internship in order to provide practical evidences for intern orientation and junior students education.MethodsAnonymous questionnaires were designed with single choice and multiple-choices and were accomplished by 175 dental medical students of different grades to investigate their' concerns before and after the clinical internship and choices for future employment.Results 47% freshmen,43% junior,45% graduates were willing to work in dental restoration field,30%sophomore and 39% senior were willing to work in orthodontics field.Before the internship,the most concem of 97% students was the teaching quality of the practice hospital; 67% students worried that patients may unsatisfy with the treatment.After the internship,the quality of the practice hospital was considered most important by 79% students; 51% students worried that internship may influence the postgraduate entrance exam.ConclusionTeachers should strengthen their self-cultivation and cope with the key concerns before the practice based on the investigation and survey after the practice.Meanwhile teacher should provide employment guidance for students based on students' interests and practice skill ability,encourage them to broaden their horizon and pay attention to the long-term interests in an aim to pave the way for future employment.
3.The correlation between MCP-1 and cardiovascular complication of diabetes

Objective To explore the significance and correlation between MCP-1 and cardiovascular complication of diabetes. Methods 65 patients with diabetes and 64 patents with IGT and 60 healthy persons as control group are chosen from a population of 1231residents at Jiangnan community in Guangzhou city. Ultrasonic inspection of carotid artery was applied to the intimae media thickness (IMT),and the levels of-MCP-1 were detected by Elisa. Result There was significant difference in the levels of MCP-1 among the normal control group, IGT group and DM group (P=0.000).The levels of MCP-1 in IGT complicated with AS patients were significantly higher than that in IGT alone patients. The levels of MCP-1 in DM complicated with AS patients were significantly higher than that in DM alone patients. The levels of MCP-1 were positively correlated with IMT in these patients. A forward LR Logistic regression analysis showed that IMT was a dependent variable, gender, MCP-1 and age are independently correlated with IMT. Conclusion MCP-1 is correlated with diabetes and its cardiovascular complication and it may be served as the target of therapy.

5.In vitro anti-tumor effect of autologous mixed lymphocyte primed by BCG activated dendritic cells based PANC1 lysate

Objective To evaluate in vitro anti tumor effect of host lymphocyte primed by CalmetteGuerin bacillus (CGB) activated dendritic cells (DC) based PANC1 lysate. Methods DCs were obtained from peripheral blood mononuclear cells of healthy volunteer and cuitured by rhGM CSF and rhIL 4. DC vaccines for pancreatic cancer were loaded with PANC1 tumor lysate (TL) and were further maturated by CGB.CD1a, CD83, CD86 and HLA-DR phenotype was characterized by flow cytometer, and IL-12p70 and TNF-α concentration in DC culture supernatant were measured by ELISA. Autologous mixed lymphocyte proliferation and the cytotoxicity of cytotoxic T lymphocytes primed by activated DCs to PANC1, PaTu8988 and SCG7901 tumor cells was measured by CCK 8 test. Results When DCs based PANC1 lysate were activated by CGB,the expression rates of CD83 and CD86 were increased from (3.7±0.3)% and (38.6±5.0)% to (16.5±0.6)% and (76.6±2.5)% (P <0.05 ). The concentrations of cytokines ILpl2p70 and TNF-α were increased from (20.18±2.06 ) pg/mland (61.38±1.19) pg/mlto (62.48±3.80) pg/mland (592.53±17.96)pg/ml (P<0. 01 ). When co-cultured with CGB activated DCs based PANC1 lysate in proportion of 0.40)% , (3.39±1.05)% , (2.82±0.39)% significantly increased to (55.38±3.58)% , (75.0±2.54) % , (77.07±3.4)% , (99.07±2.4)% (P<0.01) , respectively. The killing effects of lymphocytes 2.77)%, (19.03±3.04) %; but the killing effects on PaTu8988 and SCG7901 were significantly decreased.Conclusions DC vaccines for pancreatic cancer could be more maturated when activated by CGB, and could show a high capability of anti-tumor in vitro.

The detection rate of solitary pulmonary nodule (SPN) is significantly increased with the widespread application of chest computed tomography (CT) scans. Therefore, there is rising demand and expectation for more accurate diagnostic tests to characterize SPN. The different diagnostic methods currently used in clinical practice have their advantages and disadvantages. This article reviews the literature pertaining to SNP diagnosis and treatment strategy and above mentioned concerns according to Fleischner society, American College of Chest Physicians (ACCP) and National Comprehensive Cancer Network (NCCN) screening guideline.

OBJECTIVE:
To observe the self-developed horn type of titanium clamp used for inferior turbinate resection from filling effect.
METHOD:
Choose the cases of inferior turbinate resection of 152 cases randomly selected 92 cases (group) in 2-4 angle type titanium clip head-tail closed wound middle turbinate, and therefore more than nasal passages in the surgical wound, just as in the nasal passages above micro tamponade, bare breathing zone, keep the ventilation, 1- 3 days to take out the angle titanium clamp; 60 cases (control group) with vaseline oil gauze or postoperative Merocel hemostatic sponge tamponade nasal bleeding. Observation of 1-3 days after nasal ventilation, headache, nasal bleeding, dry mouth, tolerance is painful, aural fullness tinnitus, a total of 7 indicators of sleep.
RESULT:
The team outside the there was no difference in blood loss and the control group, the rest of the indicators is better than the control group.
CONCLUSION
The angle of titanium clamp used in inferior turbinate resection from stuffing, patients get better comfort, avoid drawn yarn of pain, improve the quality of perioperative patients with life.

9.Serum level and clinical significance of macrophage inflammatory protein-1α and interferon ;gamma inducible protein-10 in acute myelogenous leukemia

Objective To study the serum level of macrophage inflammatory protein-1α(MIP-1α) and interferon gamma inducible protein-10 (IP-10) in acute myelogenous leukemia (AML) and clarify their clinical significance. Methods Enzyme-linked immunosorbent assay was used to detect the level of MIP-1α and IP-10 in serum samples from 34 AML patients(observation group) and 20 volunteers (normal control group). Results The levels of MIP-1αand IP-10 in observation group before induction chemotherapy were significantly higher than those in normal control group (P<0.05). The levels of MIP-1αand IP-10 in observation group after induction chemotherapy were decreased, and significantly lower than those before induction chemotherapy (P<0.05). After treatment for one course, 21 patients reached complete remission (CR), and 13 patients did not reach CR. The levels of MIP-1αand IP-10 in CR group had no significant difference compared with those in normal control group (P<0.05), but the levels of MIP-1αand IP-10 in none CR group were significantly higher than those in normal control group and CR group (P<0.05). The drop percentage of MIP- 1αlevels in CR group and none CR group was (32.51 ± 10.34)% and (10.57 ± 10.39)%, and there was significant difference (P<0.05). The drop percentage of IP-10 levelsin CR group and none CR group was(45.94 ± 13.68)% and (31.17 ± 11.85)%, and there was significant difference (P<0.05). Liner correlation analysis revealed that the levels of MIP-1αand IP-10 had significantly positive correlation in AML patients (r=0.652, P<0.05). Conclusions Different expressions of serum MIP-1α and IP-10 are found before and after induction chemotherapy AML patients, and there is significant correlation. Combined detection of serum MIP-1αand IP-10 may be used as an index to monitor clinical stages and prognosis.
10.Predicting various outcomes of post-resuscitation comatose survivors: PRCSs Prognostication Score

Objective To develop a tool capable of early and exactly predicting various outcomes in comatose survivors who restore spontaneous circulation after cardiopulmonary resuscitation (CPR) and validate its performance.Methods Variables that were both readily available and predictive of outcomes were identified by systematically reviewing published literature on resuscitation.A value was assigned to these variables.We used these variables in combination with APACHE Ⅱ/score to devise a multifactorial prediction score system,which we called PRCSs Prognostication Score (PRCSs-PS).Outcomes in 115 hospitalized comatose survivors after CPR were retrospectively reviewed using PRCSs-PS.Score of patients with different outcomes was compared.The area under the receiveroperating characteristic (ROC) curve was determined to evaluate performance of this tool to identify patients with a poor outcome (CPC4 and 5) and other outcomes (CPC1,2,and 3).Results There were differences of PRCSs-PS score among multiple groups with five different outcomes (CPC1-5)(F=65.91,P=0.000).Pairwise groups with different CPC were compared:no significant difference was noted between CPC 1 and CPC2 (12.41±6.49 vs 17.38±6.91,P=0.092),but difference between other pairwise CPC groups was statistically significant (CPC2 vs CPC3:17.38±6.91 vs 24.50±5.80,P=0.041,CPC3 vs CPC4:24.50±5.80 vs 32.29±5.24,P=0.006).The performance of PRCSs-PS to discriminate patients with a poor outcome from patients with other outcomes went as follows:it had 100% sensitivity,78.6% specificity,and 178.6 diagnostic index at the score cut-off22.5; it had 77.8% sensitivity,100% specificity and 176.4 diagnostic index at the score cut-off32.5.Score 23 and 33 were two key cut-offpoints.The area under the ROC curve was 0.968,showing excellent discrimination.Conclusions The final outcomes in post-resuscitation comatose survivors can be accurately predicted using PRCSs-PS Score.
