Objective To study and analyze the efficacy and adverse reactions of beraprost sodium combined with aspirin in the treatment of diabetic foot. Methods 90 cases with diabetic foot were selected and they were randomly divided into the observation group and control group,35 cases in each group. Two groups were both treated with basic treatment. On the basis of the basic treatment, the control group was given aspirin, while the observation group was treated with beraprost sodium combined with aspirin. The clinical efficacy, ulcer healing rate, ankle back index, transcutaneous oxygen pressure, average healing time and the occurrence of adverse reactions were compared between the two groups. Results The clinical effective rate of the observation group was 93.3% (42/45),which was significantly higher than 84.4% (38/45) of the control group, the difference between the two groups was statistically significant (x2 =8.030,P < 0.05).The wound healing rate, ankle dorsal index, average healing time, the degree of improvement of transcutaneous and oxygen pressuer of the observation group were (81.4 ± 2.7) %,(11.2 ± 2.1),(39.1 ± 2.1) d and (26.3 ± 1.8) mmHg,which were significantly better than (69.3 ± 1.9) %,(8.4 ± 2.9) %,(52.3 ± 8.2) d and (16.3 ± 2.6)mmHg of the control group. The differences of these indicators between the two groups were statistically significant(t =12.72,7.92,12.06,9.97,P < 0.05).The difference of adverse reactions between the two groups was not statistically significant (P > 0.05).Conclusion Beraprost sodium combined with aspirin has better clinical efficacy in the treatment of diabetic foot.It can improve the patients'ulcer healing rate, the ankle back index, transcutaneous oxygen pressure, shorter the average healing time, and it has few adverse reactions.

Objective To explore the clinical features of Shenzhen service worker with upper gastrointestinal bleeding and association with medical insurance. Methods 169 patients(service workers in Shenzhen city)with upper gastrointestinal bleeding were retospectiveng analyzed, the treatment time, the severity of bleeding, the cause of the bleeding and bleeding incentive, diagnosis and treatment compliance, and other characteristics were analyzed; the differences between the case of group health insurance and the case of group no-health insurance, were observedResults For the case of non-insurance group the treatment time was delayed( P<0. 01 ) ;the non-standard pre-hospital diagnosis and treatment was higher( P<0. 01 ). The rate of moderate and severe cases was higher( P<0. 05 ). The rate of the patients do not comply with gastroscopy and refused hospitalization, the non-insurance group was higher than the insurance group(P<0. 01 and P<0. 05 respectively). In compliance with the treatment of the cases,ly the efficiency and the total effective rate were sigmficantly higher in the insurance group than non-insurance. Conclusion Public participation in health insurance services enable the upper gastrointestinal bleeding in patients with timely medical treatment fixed medical institutions in order to get more early diagnosis and treatment, access to better efficacy and prognosis.

As an emerging tumor marker, the rarity and heterogeneity of circulating tumor cells (CTCs) increase the difficulty of detection. In recent years, CTCs enrichment technology based on biophysical, biochemical and microfluidic properties has been continuously developed, which has promoted the research and application of CTCs in malignant tumors diagnosis, clinical treatment and prognosis evaluation. Although there are still some deficiencies in the detection of CTCs, with the interdisciplinary integration, CTCs will play increasingly important roles in the diagnosis and treatment of malignant tumors.

Objective To explore the characteristics of operation, curative effects and operative management in elderly patients of age older than 70 years with coronary heart disease receiving coronary artery bypass grafting. Methods 108 elderly patients of age older than 70 years with coronary heart disease were divided into two groups:OPCAB group(n = 76) and CCABG group (n = 32) The clinical curative effects, early postoperative mortality and complications of the two groups were compared and analyzed respectively. Results OPCAB group was better than CCABG group in these series(P < 0.05): The early postoperative mortality (5.8%, 11.2%)、 myocardial infarction (2.9%, 10.6%), respiration failure(8.7%, 17.5%), pulmonary complications: (11.8%, 31.5%) 、complication of CNS:(1.8% ,9.8%) 、acute renal failure(1.8% ,6.2%) ,the time of intubation: (9.3 ±4.5), (25.4 ±7.5) h,ICU stay(3.1 ± 1.8) ,(7.1 ±2.9) d,hospital stay(15.5 ±8.6) ,(26.4 ±8.6)d. Conclusion OPCAB could reduce operative mortality and complication, it should be the first option for the surgery of elder patients with coronary heart disease;surgical skills and correct perioperative management were the key factors to assure surgical outcome.

The incidence of gastric cancer ranks the fourth in all kinds of cancers in the world, ranking the second among patients with cancer-related deaths. The 5-year survival rate of gastric cancer patients is less than 30%.About 50% of gastric cancer patients have recurred or metastasized after curative resection.Metastasis and recurrence are the major causes of death in cancer patients.CTCs play an important role in tumor metastasis. At present, there are more than 10 kinds of detection methods for CTCs in gastric cancer. The most widely used methods are RT-PCR,FCM,CelLTracks(R) AutoPrep(R) system and ISET.Recent studies have shown that CTCs in peripheral blood of patients with gastric cancer can be used to determine the clinical stage of patients, assess the prognosis and guide the individualized treatment of cancer.

There are many ways to identify circulating tumor cells in the current.Fluorescence has a wide range of applications in the identification of circulating tumor cells. The labeled cells can be observed and counted more intuitively by labeling the tumor cells with fluorescent group containing antibodies, probes and aptamers, and cytokine,CD45 and fluorescence in situ hybridization are widely used in the identification of various circulating tumor cell related tests. In recent years, people have explored the feasibility of using fluorescent probes to directly identify circulating tumor cells. With the development of biopsy probes and optical sectioning imaging technology of confocal microscopy,it is possible to directly identify circulating tumor cells with fluorescent probes in the future.
