1.EFFECT OF NODAL METASTASIS ON THE PROGNOSIS OF PATIENTS WITH ENDOMETRIAL CANCER

To study the nodal distribution in endometrial carcinoma and to evaluate its effect on recurrence and survival, 230 patients with endometrial carcinoma were reviewed and analyzed. 32(20 25%)cases were found to have lymph node metastasis. Stage Ⅲc disease was defined by positive PLN alone in 22 cases, positive PLN and PALN in 4 cases, and positive PALN alone in 6 cases. Positive peritoneal cytology and adnexal metastasis were present in 10 patients. Only 2/10 of these patients had isolated positive PLN whereas 8/10 had positive PALN ( P 0 05). These findings indicate that PALN involvement is common in patients with FIGO Stage Ⅲc endometrial cancer, and suggest that PLN sampling alone may result in underdiagnosis of disease, and dissection of PALN may have a significant therapeutic role.
2. Selection of integrated PET/MR image reconstruction scheme for small lesions

Objective: To explore the impacts of different reconstruction combinations on image quality of different sized lesions, so as to propose the optimal reconstruction scheme for smaller lesions on integrated PET/MR. Methods: SIGNA TOF PET/MR system was used for scanning of PET image quality (IQ) emission phantom designed taken standards of the International Electrotechnical Commission (IEC) recommendations. Then the list mode raw data of PET were reconstructed using time of flight(TOF)-ordered subsets expectation maximization(OSEM)under different conditions, including the maximum expected value of the ordered subset (the iterative subset: 28, the number of iterations: 1-9 times),the reconstruction matrix (128×128, 192×192, 256×256), Gaussian low-pass filter full width at half maximum (FWHM) (1-6 mm), time-of-flight (TOF) technology alone (TOF+NOTOF), separate point spread function (PSF) (PSF+NOPSF), both TOF and PSF (TOF+PSF), as well as neither TOF nor PSF (NOTOF+NOPSF). Imaging quality was assessed under different reconstruction conditions using parameters including contrast recovery (CR), background variability (BV) and signal-to-noise ratio (SNR). Results: Balls with 10 and 13 mm diameters simulating small lesions in the OSEM (3 iterations, 28 subsets), 192×192 matrix, 2 mm Gaussian low-pass filter full width at half maximum and TOF+PSF reconstruction combination achieved the highest SNR, which was 13.31% and 21.73%, 13.31% and 21.73%, 25.74% and 35.8% as well as 26.25% and 46.01%, respectively. Conclusion: Using reconstruction combination as 3 iterations, 28 subsets, 192×192 matrix and TOF+PSF, the best image quality of small lesions can be obtained under the above mentioned conditions.
3.THE RESEARCH STATUS OF LIPID-MEDIATING ACTION OF CHITOSAN

4.Emergent reconstruction and repair of thumb and finger defect by toes and combined toes transplantation

Objective To explore the method and effect of thumb and finger reconstruction and repair by emergent primary toes transplantation. Methods Three hundred and fourteen fingers of 282 cases including finger thumb defect and complex injury with hyperthena palm radid soft tissue of forearm were reconstructed and repaired by the second toes, wrap-around flap and the second third toes wrap-around flap combined with distal flap of leg or other complex tissue flap transplantation. Results Three hundred and eight reconstructed fingers survived, the survival rate was 98%. After a follow up to 6 months to 6 years, the reconstructed thumbs and fingers have accepted nip, grasp, opponent function. The result was graded as excellent in 214 fingers, good in 63, fair in 26, poor in 5. The excellent and good rate was 88%according to the upper limb functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association. Conclusion Different type of toes and combined toes transplantation can obtain preferable clinical effect on emergent primary thumb and finger defect.
5.Multifactor analysis of postoperative patency of microsurgical vasoepididymostomy

Objective To analyze the factors which may affect postoperative patency of microsurgical vasoepididymostomy (VE).Methods Ninety-four patients underwent VEs from September 2014 to June 2016 in the Department of Urology,Peking University Third Hospital,with average age of (30.7 ± 4.8) years,and body mass index (BMI) of (25.1 ± 3.0) kg/m2.Semen analyses were performed 1 month,3 months and 5 to 6 months after the operation.The following semen analyses were performed every 3-6 months thereafter.Patency was defined by finding sperms in twice or more analyses during the followup until August 2017.Patients were followed up by face-to-face or telephone interview.Seven factors (age,BMI,bilateral or unilateral anastomosis,anastomosis sites,the adjustment of anastomosis sites,motility and quantity of sperms found in epididymal fluid) were analyzed by Chi-square analysis and multifactor logistic regression analysis.Results Eighty-two patients were followed up (87.2%,82/94) while 12 patients were lost of follow-up.The mean follow-up time was 19 months.Sperms were found in the ejaculate in 59 patients postoperatively.The overall patency rate was 72.0% (59/82),and natural paternity rate was 32.8% (19/58).Patients ≤28 years old and those >28 years old had patency rates of 89.3% (25/28) and 63.0% (34/54,P =0.012),respectively.Patients with BMI <26.0 kg/m2 and BMI≥26.0 kg/m2 had patency rates of 80.4% (41/51) and 58.1% (18/31,P =0.029),respectively.Patency rate of bilateral surgery was 72.1% (44/61) and of unilateral surgery was 71.4% (44/62,P =0.727).Patency rate of caput anastomosis achieved 75.0% (15/20) and of corpus/caudal anastomosis was 71.0% (44/62,P =0.727).Patency rates of patients with and without adjustment of anastomosis sites were 77.8% (7/9) and 71.2% (52/73,P =0.680),respectively.Patency rates of a lot,a few,motile and seldom-motile sperms in epididymal fluid were 74.3% (55/74) vs.50.0% (4/8,P =0.146) and 70.0% (28/40) vs.73.8% (31/42,P =0.701),respectively.Multifactor logistic regression analysis showed that age was well associated with patency rate (OR=4.705,95%CI 1.181-18.742,P=0.028).Conclusions Age ≤28 years is an independent factor leading to higher patency rates.Patients with lower BMIs and younger could have higher patency rates.Factors of anastomosis sides,anastomosis sites,the adjustment of anastomosis sites,motility and quantity of sperms found in epididymal fluid showed no statistical difference in patency rates.
6.Efficacy of three plasma exchange methods in improving renal insufficiency after kidney transplantation and the induction of plasma exchange related adverse reactions: a comparative study

【Objective】 To compare the efficacy of double filtration plasmapheresis(DFPP), centrifugal therapeutic plasma exchange(cTPE) and centrifugation-filtration plasmapheresis(CFPP) in improving renal insufficiency after kidney transplantation, as well as the differences in inducing plasma exchange-related adverse reactions. 【Methods】 Clinical data from 46 patients who underwent plasma exchange after renal transplantation in our hospital were retrospectively collected, and patiens were divided into DFPP group(n=33), cTPE group(n=7) and CFPP group(n=6). Changes in peripheral blood creatinine, albumin, hemoglobin, platelets, fibrinogen levels and urine volume before and after TPE were compared and analyzed among the three groups. 【Results】 Among the DFPP group, cTPE group and CFPP group, the creatinine after TPE decreased by (31.40±25.38)%, (58.91±19.75)% and (39.44±28.64)%, respectively, with cTPE group significantly higher than the DFPP group(P<0.05), but no significant differences between the DFPP group and cTPE group(P>0.05); the urine volume after TPE increased by (49.33±30.03)%, (54.62±39.32)% and (68.89±23.00)%, showing no significant differences(P>0.05); the hemoglobin after TPE decreased by (11.97±5.94)%, (20.17±5.75)% and (9.65±8.75)%, respectively, with the cTPE group significantly higher than the DFPP group and CFPP group(P<0.05), but no significant difference between the DFPP group and CFPP group(P>0.05). The platelet count after TPE decreased by (37.88±18.39)%, (24.56±12.36)% and (21.40±12.51)%, respectively, with no significant differences between the three groups(P>0.05); the fibrinogen after TPE decreased by (0.57±0.20)%, (0.14±0.06)% and (0.26±0.22)%, respectively, with the DFPP group significantly higher than the cTPE group(P<0.05), but the CFPP group had no significant difference with cTPE group or DFPP group(P>0.05); the albumin after TPE decreased by (11.41±5.97)%, (14.67±6.52)% and (25.18±5.10)%, respectively, with cTPE group and DFPP group significantly lower than the CFPP group(P<0.05, P<0.001), but with no significant difference between the DFPP group and cTPE group(P>0.05). 【Conclusion】 The effect of three plasma exchange methods varies on renal function, anemia and coagulation function of patients after kidney transplantation. It is necessary to consider the the patient’s disease characteristics and treatment needs, as well as the laboratory′s technical conditions and plasma supply when selecting TPE methods.
