1.Study of the Recognition Degree of Adverse Drug Reaction Reports among the Healthcare Professionals in our Hospital

Objective:To investigate the recognition degree and attitude to adverse drug reaction (ADR) reports among the healthcare professionals in our hospital.Method:Through random cluster sampling,the doctors,nurses and pharmacists in our hospital were interviewed on the spot.The questionnaires were input by EPI6.04 and analyzed by soft- ware SPSS11.5.Result:1077 eligible questionnaires were obtained and the recovery was 89.8%.Our bealthcare profes- sionals had a good recognition of basic ADR knowledge.The pharmacists had a better recognition degree than doctors and nurses.The better academic background the healthcare professionals had,the higher their score of recognition was,and there was a significant difference among them.There was no significant difference among the Scores of the healthcare profes- sionals with different working years,and nor was there among the scores of them with different professional titles.The atti- tude to ADR reporting was entirely right,but there were some wrong ideas.Conclusion:It is urgent to provide more train- ing courses for healthcare professionals so as to raise their consciousness of ADR reporting,ability of discrimination and lev- el of monitoring and avoid serious consequences caused by ADR.
2.Retrospective Analysis of New and Severe ADR Case Reports in Our Hospital from 2002 to 2006

0,most symptoms of ADR were reported for the first time with the homogeneous literature ratio being 0.The predominant outcome of severe ADR were hospitalization or delayed hospitalization course,followed by increased risk of death or patients' permanent or distinct disabling.CONCLUSION:New and severe ADR should be given close attention in the clinic to guard against severe ADR.
3.Effects of Prescription Administrative Policy on prescription quality of a tertiary hospital

Objective To discover the effects of the Prescription Administrative Policy in force on the quality of the prescriptions in a tertiary hospital in 2007. Methods The prescriptions of 400 cases were sampled systematically for evaluation, and 60 patients were interviewed. Results The average eligibility rate of the prescriptions was but 37. 2% in this hospital, which was mainly plagued by incompleteness, nonstandard and irrationality found in prescriptions. Implementation of the Policy contributed to a significant improvement of some indicators. For example, the eligibility rate increased by 12. 2% (P=0. 004) ,the percentage of the use of antimicrobial agents dropped significantly (P=0. 001),and the percentage of generic names used rose significantly (P = 0. 000). Conclusions The implementation of the Policy is highly positive for prescription quality.
4.The relationship of congenital choledochus cyst(CCC)with occurence of pancreatitis in adults

Objective To discuss the relationship of congenital choledochus cyst(CCC)with occurrence of pancreatitis in adults and methods of surgical treatment.Methods The clinical data of 17 adult patients with congenital choledochus cyst who underwent surgical treatment from 1997-2005 were analyzed retrospectively.Results Clinical diagnosis was made mainly by B-ultrasound,MRCP,intraoperative cholangiography,ERCP and CT scans.Among 17 cases,10 cases were congenital choledochus cyst typeⅠ,4 cases type Ⅱ,1 case type Ⅲ,1 case type Ⅳ and 1 case type Ⅴ;and associated with cholelithiasis in 14 cases(bile pigment stone in 11cases,cholesterol calculus 3cases),chronic cholecytitis 5 cases,polypoid lesions of gallbladder 1 case,anomalous pancreaticobiliary junction(APBJ)10 cases,and pancreatitis 10 cases.Resection of extrahepatic cyst with Roux-y hepaticojejunostimy was performed in 15 cases,preserving pylorus pancreatoduodinectomy in 1 case,and cholecystectomy and T tube drainage in 1 case.Excellent and good results were achieved on follow-up in 14 out of the 17 CCC cases undergoing surgical treatment,while pancreatitis occurred in 2 cases and unexpected death in 1 case.Conclusions Pancreatitis is apt to occurr in CCC with APBJ and bile pigment stone in choledochus.The incidence of pancreatitis in CCC and APBJ(P-B)can be decreased by resection of extrahepatic cyst and Roux-en-Y hepaticojejunostimy and cholecystectomy.
5.Study on the Effect of Huangqi-Ezhu-Chonglou Combination on Proliferation and Migration of Colorectal Cancer Cells by Regulating Macrophage Polarization

OBJECTIVE To investigate the effect of Huangqi-Ezhu-Chonglou combination on macrophage polarization and its mechanism of inhibiting colorectal cancer(CRC)cells proliferation and migration.METHODS THP-1 cells were stimulated with phorbol 12-myristate 13-acetate(PMA)and interleukin-4(IL-4)to establish M2 macrophage polarization model.The experiment was divided into M0 group(PMA treatment),M2 group(PMA+IL-4 treatment),and M2+ Huangqi-Ezhu-Chonglou combination group(PMA+IL-4+Huangqi-Ezhu-Chonglou combination treatment).The effect of Huangqi-Ezhu-Chonglou combination freeze-dried powder on the viability of macrophage was detected by CCK-8 method.The expression of macrophage polarization markers,glu-taminase(GLS)mRNA and protein was detected by qPCR and Western blot.The levels of interleukin-10(IL-10),transforming growth factor-β(TGF-β)and tumor necrosis factor-α(TNF-α)in cell supernatant were detected by ELISA.CCK-8 method and Tr-answell assays were used to detect the proliferation and migration of HCT116 cells intervened by the supernatant of macrophage culture treated with Huangqi-Ezhu-Chonglou combination,namely conditioned medium(CM).RESULTS Compared with the M0 group,the expression levels of IL-10,mannose receptor(CD206),arginase 1(ARG1),and GLS mRNA and protein in the M2 group were significantly increased(P<0.01,P<0.001),the levels of IL-10 and TGF-β secreted by macrophages were significantly increased(P<0.01,P<0.001);compared with the M2 group,the M2+ Huangqi-Ezhu-Chonglou combination group had significantly reduced IL-10,CD206,ARG1,and GLS mRNA and protein expression(P<0.05,P<0.01),the mRNA and protein levels of TNF-α and in-ducible nitric oxide synthase(iNOS)were significantly increased(P<0.05,P<0.01,P<0.001),the interleukin-1β(Interleukin-1β,IL-1β)mRNA expression significantly increased(P<0.01),and the contents of IL-10 and TGF-β in the cell supernatant sig-nificantly decreased(P<0.05,P<0.01),while TNF-α content significantly increased(P<0.01).CCK-8 and Transwell results showed that compared with the M0-CM group,the M2-CM promoted the proliferation and migration of HCT116 cells(P<0.01,P<0.001),the M2+ Huangqi-Ezhu-Chonglou-CM group significantly inhibited HCT116 cell proliferation and reduced cell migration compared to the M2-CM group(P<0.01,P<0.001).CONCLUSION Huangqi-Ezhu-Chonglou combination can inhibit colorectal cancer cells proliferation and migration by regulating macrophage polarization,and its mechanism may be related to the changes in the expression of GLS,a key enzyme in glutamine metabolism.
6.Establishment and application of intravenous drug resolvent generation system and solvent incompatibility rules library

OBJECTIVE To standardize the use of sterile powder for injection redissolution solvent and intravenous drugs diluent solvent, and ensure the safety of clinical drug use. METHODS The intravenous drug resolvent generation system and the intravenous drug solvent incompatibility rules library were constructed and operated by prescription-checking pharmacist team in our hospital by formulating a list of intravenous drugs needed to be dissolved with sterilized water for injection and a contraindication list of intravenous solvent compatibility. The unreasonable medical orders reviewed and intervened in our hospital from January to April 2023 (before the implementation of the intravenous drug solvent incompatibility rules library) and from May to August 2023 (after the implementation) were analyzed statistically to compare the number of unreasonable solvent selection orders, interception rate, and the success rate of pharmacists’ interventions for unreasonable solvent selection orders before and after the implementation. RESULTS Before the implementation of the solvent incompatibility rules library, a total of 5 229 groups of medical orders with unreasonable solvent selection in our hospital were identified, among which there were 1 204 groups of seriously unreasonable medical orders, with interception rate of 23.03% and the success rate of pharmacists’ intervention of 15.90%. After the implementation of the solvent incompatibility rules library, the total number of medical orders with unreasonable solvent selection was 3 258 groups, among which there were 1 148 groups of seriously unreasonable medical orders, with interception rate of 35.24% and success rate of pharmacists’ intervention of 24.83%, being significantly higher than before implementation (P<0.01 or P<0.001). CONCLUSIONS The establishment and application of the intravenous drug resolvent generation system and the solvent incompatibility rules library can significantly increase the interception rate of unreasonable solvent selection orders and the success rate of pharmacists’ interventions, and ensure the safety of clinical drug use.
