1.A 1:1 Matched Case-control Study on Risk Factors for Stomach Cancer in Ganyu County

Objective To explore the risk factors and protective factors for stomach cancer in Ganyu county.Methods Data of 319 pairs of new stomach cancer cases and health controls(1:1 matched)were studied by conditional Logistic regression method on the influential factors.Results The main risk factors for stomach cancer were the habits of eating hot foods(OR= 2.21,95%CI:1.57~3.12),the history of upper gastrointestinal inflammation(4.26,2.59~7.03),family history of cancer(2.29, 1.37~3.82);The main protective factors were often eating raw garlic(OR=0.64,95%CI:0.42~0.96),regular drinking tea(0.37, 0.25~0.54).Conclusion Unhealthy dietary habits,the history of upper gastrointestinal inflammation and family history of cancer are related to stomach cancer,while eating raw garlic and regular drinking tea may be effective in preventing the occurrence of stomach cancer.The results could be applied in stomach cancer prevention activities in Ganyu county.
2.Influence of Flavonoid of Astragalus Membranaceus's Stem and Leaf on the Function of Cell Mediated Immunity in Mice

Objective: To investigate the immune regulation of flavonoid of Astragalus membranaceus's stem and leaf(FAM-sl). Methods: Changes of total T cell count and subsets in mice were determined by monoclonal antibody assay before and after treatment with FAM-sl, and the lymphokine activated killer cell (LAK) activity was tested simultaneously by isotope label method.Results: FAM-sl could promote the proliferation of lymphocytes induced by ConA, raise the total T cell count and regulate the T cell subsets disturbance, and elevate the LAK activity induced by recombinant interleukin-2 (rIL-2).Conclusion: FAM-sl possesses effects of immune stimulation and immune regulation in treating immunosuppressive mice. This study provides experimental evidence for clinical application of FAM-sl.
3.Effects of acupuncture on kidney morphological structure and expression of TGF-beta1 mRNA in rats with spontaneous hypertension.

OBJECTIVETo explore mechanism of acupuncture for renal interstitial fibrosis (RIF) in hypertension rats.
METHODSTwenty-four 24-week-old male spontaneously hypertensive rats were randomly divided into a model group, a perindopril group and an acupuncture group, eight cases in each one. In the acupuncture group, with rats tied up, electroacupuncture was applied at bilateral "Quchi" (LI 11) and "Zusanli" (ST 36) under mild vibration of needle handle for 20 min, once a day. In the perindopril group, perindopril (0.4 mg/kg) suspension liquid was applied for intragastric administration, once a day. In the model group, rats were tied up for 20 min a day without any treatment. Eight same-age same-race Wistar Kyoto (WKY) rats with normal blood pressure were taken as a control group, which was given with free diet but no treatment. The treatment was reuqired for six weeks. The systolic blood pressure of caudal artery was tested. Kidney morphological structure was observed by HE coloration. Deposition optical density of type I and III collagen in kidney was tested by immunohistochemistry. Expression of transforming growth factor-beta1 (TGF-beta1) mRNA was tested by reverse transcription polymerase chain reaction (RT-PCR) method.
RESULTSCompared with the model group, the blood pressure was significantly decreased in the acupuncture group (P < 0.01), the damage of kidney morphology was minor, positive depositional area of type I and III collagen was obviously decreased (both P < 0.05), and the expression of semi-quantitative analysis of TGF-beta1 mRNA was decreased (P < 0.05), which had similar effect as western medication perindopril.
CONCLUSIONAcupuncture at "Quchi" (LI 11) and "Zusanli" (ST 36) probably intervenes the process of RIF by reducing synthesis of kidney type I , III collagen and restraining expression of TGF-beta1.

4.Utilization of ?-Red Recombination System and Balanced Lethal System to Reconstruct the Plasmid with Antibiotic Resistant Gene Using in Live Vaccine

Recombination plasmid pMM085 possessed both immunogens heat-labile enterotoxin(LT) and fimbriae antigen K88 of enterotoxigenic Escherichia coli (ETEC). Althouth vaccine strain MM-3 carrying pMM085 had good effect to protect piglets against diarrhea due to ETEC infections,it was not ideal live vaccine for pMM085 bringing chloramphenicol resistance gene (cat). To solve the problem,the host-plasmid balanced lethal system was introduced which including the replacement of cat gene by asd gene and transformation the new plasmid to the strain X6097 which asd gene was knocked out in its chromosome. Considering pMM085 was a big plasmid (23kb) and traditional genetic manipulations was not easy to carry on,?-Red recombination system was adopt in this work to realize the replacement of cat gene by asd gene. The results indicated that ?-Red recombination system was convenient and efficient to reconstruct big plasmid.
5.Overlapping coverage with bilateral shaft based vascularized dartos pedicle in Snodgrass hypospadias repair

Objective To introduce a new technique for urethral coverage in Snodgrass hypospadias repair,and to evaluate its effectiveness and complications.Methods From April 2003 to February 2006, this new procedure was performed in 289 children with hypospadias aged 3 months to 12 years (mean age,2. 4 years).The native meatus of urethra was identified subcoronal in 78 cases,penile/shaft in 136,penoscrotal in 36 and scrotal in 16;and 23 cases had undergoneⅡstage operation and re-operation.The overlapping coverage with bilateral shaft based vascularized dartos pedicle was done in the new urethra by Snodgrass hy- pospadias repair in these children.Results All the cases were followed for 3 months to 2 years.Postoper- atively,urinary fistulas developed in 32 cases (11%).Of them,11 were cured spontaneously within 4 weeks. The incidence of actual urinary fistula was 7% (21/289).Of the 21 fistulas which were not cured,11 (5%) occurred in 214 cases of distal hypospadias;and 10 (13%) in 75 cases of proximal hypospadias,Ⅱstage and re-operation.No dehiscence and diverticulum was found.Combined with mucosal collar technique,the ventral skin of the penis was sewn on the midline.During the follow-up,excellent cosmetic results with normal-ap- pearing circumcised penis were achieved in most patients.Conclusions Bilateral shaft based vascularized dartos pedicle urethral coverage procedure is a reliable and effective method for preventing urethral cutaneous fistulas and dehiscence.This method can reconstruct a satisfactory cosmetic appearance of the penis.

8.Construction of the attenuated Salmonella typhimurium strain expressing Helicobacter pylori conservative region of adhesin antigen.

To construct a non-resistance and attenuated Salmonella typhimurium strain which expresses conservative region of adhesin(AB) of Helicobacter pylori(Hp). The AB gene was amplified by PCR and inserted into the expression vector pYA248 containing asd gene and was introduceded into the delta Cya, delta Crp, delta Asd attenuated Salmonella typhimurium strain by twice transformations, which is a balanced lethal recombinant. Bridged ELISA method was used to measure AB expressed in sonicate and culture supernatant. According to Meacock's way and growth curve, stability of the recombinant is evaluated. Semi-lethal capacity test was used to evaluate the safty of recombinant. Results showed S. typhimurium X4072(pYA248-AB) was constructed successfully, recombinant X4072(pYA248- AB) content of supernatant serum was higher than that of thallus lytic liquor confirmed by bridged ELISA, and after recombinant pYA248- AB cultured 100 generation without selection pressure, all the recombinant germ selected randomly can grow, and the AB antigen was positive by ELISA detection. The growth curve of the recombinant germ showed that the growth state of X4072(pYA248) and X4072(pYA248- AB) were coincidence on the whole, and the survival rate of C57BL/6 was still 100%, 30 days after taking X4072(pYA248- AB) 1.0 x 10(10)cfu. orally. Non-resistance S. typhimurium X4072(pYA248- AB) was constructed successfully. The recombinant plasmid was stable indicated by in vitro experiment. And the recombinant strain was safe confirmed by animal experiment. This live vaccine strain is worthy to be considered as a new live oral vaccine candidate against Hp infection.

9.Evaluation of P-POSSUM scoring system in predicting mortality in patients with hip joint arthroplasty.

OBJECTIVETo evaluate the applicability of Porthsmouth modified physiological and operative severity score for the enumeration of mortality and morbidity (P-POSSUM) in predicting the mortality of the patients undergoing hip joint arthroplasty.
METHODSA total of 141 patients (75 males and 66 females, aged 63.22 years+/-14.45 years on an average) undergoing hip joint arthroplasty during January 2002 and March 2005 were studied retrospectively with P-POSSUM. Their average physiological score and operative severity score were 17.48+/-5.16 and 12.43+/-3.05, respectively. The predicted postoperative mortality with P-POSSUM was compared with the observed value. Subgroup analysis was performed to investigate the predictive capability of P-POSSUM. POSSUM scoring system was used as the control.
RESULTSThree patients died after operation in this study actually. The average physiological scores were 32.33+/-9.87 in the death group and 17.16+/-4.56 in the survival group. The former was obviously higher than the latter, which showed statistical difference between the two groups (Wilcoxon rank sum test, P<0.05). Perfect agreement was found between the observed death number and the predicted death number calculated by P-POSSUM (Cochran-Mantel-Haenszel chi(2) test, P>0.05), though POSSUM overestimated the overall mortality.
CONCLUSIONSP-POSSUM can predict the mortality accurately in the patients undergoing hip joint arthroplasty, which is superior to POSSUM.

10.Therapeutic effect of fibroblast growth factor 21 on NAFLD in MSG-iR mice and its mechanism.

This study is to evaluate the therapeutic effect of fibroblast growth factor 21 (FGF21) on NAFLD in MSG-IR mice and to provide mechanism insights into its therapeutic effect. The MSG-IR mice with insulin resistance were treated with high dose (0.1 micromol.kg-1d-1) and low dose (0.025 micromol.kg-1d-1) of FGF21 once a day for 5 weeks. Body weight was measured weekly. At the end of the experiment, serum lipids, insulin and aminotransferases were measured. Hepatic steatosis was observed. The expression of key genes regulating energy metabolism were detected by real-time PCR. The results showed that after 5 weeks treatment, both doses of FGF21 reduced body weight (P<0.01), corrected dyslipidemia (P<0.01), reversed steatosis and restored the liver morphology in the MSG model mice and significantly ameliorated insulin resistance. Additionally, real-time PCR showed that FGF21 significantly reduced transcription levels of fat synthetic genes, decreased fat synthesis and promoted lipolysis and energy metabolism by up-regulating key genes of lipolysis, thereby liver fat accumulation was reduced and liver function was restored to normal levels. In conclusion, FGF21 significantly reduces body weight of the MSG-IR mice, ameliorates insulin resistance, reverses hepatic steatosis. These findings provide a theoretical support for clinical application of FGF21 as a novel therapeutics for treatment of NAFLD.
