2.Splitted fractions and unoverlapping analysis of chemical constituents of Poria cocos.

With the combined applications of steam distillation, water extraction and alcohol precipitation, liquid-liquid extraction and column chromatography over macroporous resin, a splitted-fractions method of the chemical constituents of Poria cocos was established. The unoverlapping property of the fractions of P. cocos was qualitatively analysed by using principal component analysis and cluster analysis. With angle cosine, squared euclidean distance and the overlapping analysis of peak area of crude herbs, the unoverlapping property of the fractions of P. cocos was half-quantitatively analysed. The chemical components of P. cocos was divided into the fractions of polysaccharide, petroleum ether, ethyl acetate, alcohol eluate from macroporous resin and water eluate from macroporous resin. Non similarity degree among each chemical fraction was above 80% and main chemical components were identified. The established method for splitting fractions of P. cocos has good stability and repeatability and all chemical components in P. cocos could be completely divided into six fractions. It is the first time that the author half-quantitatively analyse the unoverlapping property of the chemical fractions of P. cocos.

3.Screening of Marine Mold Activity and Having Antifungi Primary Research

More than one hundred strains of marine molds have been isolated from the sediment and the sample of seawater collected from the South China Sea. By the first screening, more than 30 strains of marine molds which can inhibit tested fungi such as Candida albicans and Fursarium sp. were obtained.The results of the second screening showed those strains designed as B 4#-6、B 4#-3、1-B 6-6、1-B 6-10-5、1-B 6-22、C 2#-5、A 2-9 and 1-B 6-10 can produced extracelluar antifungi metabolic products and the crude extract of the strains 1-B 6-10-5 and B 4#-3 can inhibit the growth of many other species of fungi.
4.Effect of Medicine Combined with Electric Stimulation Therapy on Acontractile Detrusor

ObjectiveTo investigate the effective approach for the treatment of detrusor underactivity. Methods98 patients with detrusor underactivity were randomly divided into electrotherapy group (electroacupuncture, interference current stimulation, n=32), medicine group (n=32) and electrotherapy-medicine group (n=34). The patients were assessed with examination of urodynamics and clinical parameters before and after treatment. ResultsThe detrusor strength were stronger after treatment in all the 3 groups (P<0.001), but electrotherapy-medicine group was more stronger than the other 2 groups (P<0.01). The cure rate in the 3 groups were 68.75%, 59.38% and 88.23% respectively, the patients in electrotherapy-medicine group gained significantly better results than the other 2 groups (x2=9.976, P<0.01). The difference of cure rate between the electrotherapy group and the medicine group was not significant (P>0.05). ConclusionElectric stimulation therapy combined with medicine is an effective intervention for treatment of detrusor underactivity and is better than electric stimulation therapy or drugs only.
5.Combination of OCT and FFA for the research of pathogenesis of central serous chorioretinopathy

Background The diagnosis of central serous chorioretinopathy (CSC) is mainly dependent onfluorescine fundus angiography (FFA). However, the combination of optical coherence topography (OCT) with FFA offers a new approach to the research of the pathogenesis of CSC. Objective This clinical study was designed to study the combined application of the FFA and OCT for the research of the pathogenesis of central serous chorioretinopathy (CSC). Methods Forty-four eyes of 44 patients with CSC were included in this study with 36 cases of males and 8 cases of female. The patients were aged 39.3 ± 5.3 years and the visual acuity was 0. 64 ±0. 27. FFA and OCT examinations were performed in all patients and the FFA images were imported into the Topcon 3D OCT 1000 device to locate the conformity of OCT lesions with the leakages of FFA. The neuroepithelial layer thickness at the fovea and the height of the neuroepithelial layer detachment were measured using 3-D OCT. Results OCT showed serous REP detachment in 34 eyes (77.3%) and rough surfaces of RPE in 10 eyes (22. 7% ). In thirtyfour eyes with RPE detachment, the OCT lesions and FFA leakage spots conformed to the same locations in 31 eyes, but the other three eyes did not. The mean foveal neuroepithelial thickness was (138.5±19.4) μm in CSC eyes and that of normal eyes was ( 131.35±5. 01 ) μm ,showing a significant difference between them( t=0. 39 ,P>0. 05 ). The mean height of neuroepithelial detachment was (263.3 ± 126.7 ) μm in CSC eyes. Conclusion RPE detachment occurs in CSC eyes and further induces macular neuroepithelial detachment. Leakage lesion of fluorescine corresponds to RPE detachment. CSC without RPE detachment may be related to the increase in RPE permeability. OCT can accurately measure the thickness of the macular neuroepithelial layer and the height of the neuroepithelial detachment.
6.Clinical observation on effect of modified banxia houpu decoction in treating patients with protracted heroin abstinence syndrome.

OBJECTIVETo observe the therapeutic effect of Modified Banxia Houpu Decoction (MBHD) in treating heroin abusers with protracted abstinence syndrome.
METHODSOne hundred and eighty-seven heroin abusers were randomly divided into three groups, the 58 patients in the control group, 62 in the treated group A and 67 in the treated group B. All were detoxified with lofexidine hydrochloride (LFX) tablet for 12 days. MBHD was given to the two treated groups, the medication started from the beginning of detoxification in the group B, and from the end of detoxification in group A for 60 days. To the control group, an imitate preparation was given. The observation was carried out 10 days after withdrawal of medication, and the protracted abstinence related symptoms were observed and scored. And the condition of re-abusing in patients were investigated through urinary examination one year later.
RESULTSThe heroin abusers' protracted abstinence symptom score in the treated group was significantly lower than that in the control group (P < 0.01), and comparison of the scores between the two treated groups also showed significant difference (A < B, P < 0.01). The 1-year re-abusing rate in treated group B was significantly lower than that in the control group and in the treated group A (P < 0.05).
CONCLUSIONMBHD could improve the heroin abusers' protracted abstinence symptoms after detoxification. In spite of the complexity of various factors, to effectively control the early stage abstinence symptoms and median stage protracted abstinence symptoms is one of the effective measures to prevent drug re-abusing.

8.Detection of the mutation in HBV polymerase gene by RFLP PCR method in hepatitis B patients treated with lamivudine.

BACKGROUNDTo investigate the mutation of HBV polymerase gene in chronic hepatitis B patients treated with lamivudine.
METHODSThe restriction-fragment-length-polymorphism (RFLP) assay for HBV DNA sequence determination at the codon 528 and 552 in the HBV polymerase gene associated with lamivudine resistance in vitro. HBV DNA samples extracted from sera of 240 patients were subjected to PCR amplification with primer pairs F2/R2 (552), F3/R2 (528). Each PCR product was digested with Nde I or Nla III.
RESULTSSerum HBV DNA mutation was found in 51/240 patients (38/51M552V, 26/38L528M, 13/51M552I) after therapy for 52 weeks. DNA sequence analysis was performed on samples of 3 patients, and the results were consistent with those of RFLP assay.
CONCLUSIONThe RFLP assay was able to detect the mutation of HBV DNA at codon 552 and 528 which are the principal site of HBV DNA resistant to lamivudine. The specific PCR method for HBV DNA mutation is rapid, simple and specific.

9.Influence of different antibiotic strategies on outcomes of hospitalized neonates.

OBJECTIVETo observe the outcomes of hospitalized neonates who were managed with two different antibiotics strategies, namely, the risk factor based antibiotic strategy and the combination antibiotic strategy that is based on risk factors, infection screening and monitoring.
METHODSA cohort study was performed on a control group of 4 406 cases of neonates hospitalized between January 2010 and May 2011 and an observed group of 4 476 neonates hospitalized between July 2011 and October 2012. The control group adopted the risk factor based antibiotic strategy and the observed group received a combination antibiotic strategy based on risk factors, infection screening and monitoring. The rate of antibiotic use, average length of stay, readmission rate and mortality rate were compared between the two groups.
RESULTSWith the change from the risk factor based antibiotic strategy to the combination antibiotic strategy, the total rate of antibiotic use decreased from 79.6% to 50.5% (P<0.01). There were no differences in the average length of stay, readmission rate, and mortality rate between the two groups.
CONCLUSIONSThe combination antibiotic strategy based on risk factors, infection screening and monitoring can reduce antibiotic use substantially and has no adverse effects on treatment outcomes in hospitalized neonates.

10.Circumcision affects glans penis vibration perception threshold.

OBJECTIVETo evaluate the effect of circumcision on the glans penis sensitivity by comparing the changes of the glans penis vibrotactile threshold between normal men and patients with simple redundant prepuce and among the patients before and after the operation.
METHODSThe vibrotactile thresholds were measured at the forefinger and glans penis in 73 normal volunteer controls and 96 patients with simple redundant prepuce before and after circumcision by biological vibration measurement instrument, and the changes in the perception sensitivity of the body surface were analyzed.
RESULTSThe G/F (glans/finger) indexes in the control and the test group were respectively 2.39 +/- 1.72 and 1.97 +/- 0.71, with no significant difference in between (P > 0.05). And those of the test group were 1.97 +/- 0.71, 2.64 +/- 1.38, 3.09 +/-1.46 and 2.97 +/- 1.20 respectively before and 1, 2 and 3 months after circumcision, with significant difference between pre- and post-operation (P < 0.05).
CONCLUSIONThere is a statistic difference in the glans penis vibration perception threshold between normal men and patients with simple redundant prepuce. The glans penis perception sensitivity decreases after circumcision.
