2.Clinical trial of levothyroxine sodium on thyroid function in pregnant women with hypothyroidism

Objective To evaluate the clinical effect of levothyroxine sodium on serum lipid levels and thyroid function in pregnant women with hypothyroidism.Methods Fifty-two pregnancy patients with hypothyroidism were randomly divided into control group and treatment group,26 cases in each group.The control group was given conventional therapy,including the amount of added salt,plenty of high quality protein,appropriate low-fat diet.Treatment group was treated with levothyroxine sodium 50 μg,qd on the basis of control group.Levels of serum total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),serum thyroid stimulating hormone (TSH),free thyroid hormone (FT4),free three iodine thyroid acid (FT3) and incidence of pregnancy complications were compared between the two groups.Results After treatment,levels of TC in treatment and control groups were (4.22 ± 0.59),(5.32 _ 0.69) mmol · L-1.Levels of TG in treatment and control groups were respectively (2.11 ±0.25),(2.84 ±0.39) mmol· L-1.Levels of LDL-C in treatment and control groups were (2.18 ±0.31),(3.11 ±0.37) mmol · L-1.Levels of HDL-C in treatment and control groups were (1.82 ±0.26),(1.56 ±0.22) mmol · L-1.Levels of TSH in treatment and control group were (2.03 ± 0.25),(2.45 ± 0.34) pmol · L-1.Levels of FT4 in treatment and control groups were respectively (18.43 ±2.61),(10.84 ± 1.57) pmol · L-1.Levels of FT3 in treatment and control groups were (5.73 ± 0.81),(3.35 ±0.47) pmol · L-1.There were significant difference in TC,TG,LDL-C,HDL-C,TSH,FT4,FT3 between the two groups after treatment (P < 0.05).There were 2 cases of gestational hypertension,1 case of gestational diabetes mellitus,1 case of intrahepatic cholestasis,2 cases of fetal growth restriction,the incidence of pregnancy complications was 23.08% (6/26).There were 1 case of hypertension,1 case of gestational diabetes mellitus,1 case of intrahepatic cholestasis,and the incidence of pregnancy complications was 11.54% (3/26),the difference had no statistical significance (P > 0.05).Conclusion Levothyroxine sodium can significantly reduce the levels of blood lipids in pregnancy patients with hypothyroidism,improve thyroid function,and reduce the incidence of pregnancy complications.
3.Clinical application of low-molecular-weight heparin (Fraxiparine) in rescuing venous crisis of island skin flap.

OBJECTIVETo evaluate the clinical efficacy of low molecular weight heparin (Fraxiparine) in rescuing venous crisis of island skin flap.
METHODSOf the 73 patients with venous crisis of island skin flap, 47 received subcutaneous injection of low-molecular-weight heparin (group I) and 26 were treated with phlebotomy, local compression and topical application of unfractionated heparin solution gauze (group II).
RESULTSThe flap survival ratio was (88.46∓8.64)% in group I and (38.37∓6.53)% in group II (P<0.001). At 0, 2, and 4 h after injection of low-molecular-weight heparin, the activated partial thromboplastin time (APTT) was obviously delayed (24.28∓6.71, 41.35∓7.64 and 32.34∓6.35, respectively, P<0.01), FXa:C level was significantly decreased (152.4∓30.7, 65.8∓24.4 and 83.4∓18.4, respectively, P<0.01), while FIIa:C level underwent no obvious alterations (155.70∓31.61, 143.20∓24.75, and 143.4∓23.35, respectively, P=NS).
CONCLUSIONFraxiparine has good antithrombotic efficacy in rescuing venous crisis of island skin flap without adverse effect on systemic coagulation.

4.Establishment of a rat model of bone marrow mesenchymal stem cell transplantation for repairing full-thickness skin defect.

OBJECTIVETo establish a rat model of full-thickness skin defect to receive bone marrow mesenchymal stem cell transplantation for wound repair.
METHODSA full-thickness skin defect measuring 4 cmx4 cm in 36 F344 rats, which were divided into 3 groups with the wound covered with alloskin graft, acellular dermal matrix, or petrolatum gauze. In vitro cultured BMSCs in the 5th passage were transplanted into the skin defect, and the time of wound dressing dissociation and number of transplanted Brdu-positive cells in the wound were observed 14 days later.
RESULTSThe alloskin graft resulted in significantly longer time before dressing dissociation, with greater number of Brdu-positive cells in the wound than the other two wound dressings (P<0.001). The acellular dermal matrix showed better effect than petrolatum gauze in terms of the dressing dissociation time and the viable transplanted cell number in the wound.
CONCLUSIONAlloskin graft can be ideal for covering the wound surface to protect the transplanted BMSCs in rats.

5.Performance of Fasting Plasma Glucose and Postprandial Urine Glucose in Screening for Diabetes in Chinese High-risk Population.

BACKGROUNDThe conventional approaches to diabetes screening are potentially limited by poor compliance and laboratory demand. This study aimed to evaluate the performance of fasting plasma glucose (FPG) and postprandial urine glucose (PUG) in screening for diabetes in Chinese high-risk population.
METHODSNine hundred and nine subjects with high-risk factors of diabetes underwent oral glucose tolerance test after an overnight fast. FPG, hemoglobin A1c, 2-h plasma glucose (2 h-PG), and 2 h-PUG were evaluated. Diabetes and prediabetes were defined by the American Diabetes Association criteria. The area under the receiver operating characteristic (ROC) curve was used to evaluate the diagnostic accuracy of 2 h-PUG, and the optimal cut-off determined to provide the largest Youden index. Spearman correlation was used for relationship analysis.
RESULTSAmong 909 subjects, 33.4% (304/909) of subjects had prediabetes, and 17.2% (156/909) had diabetes. The 2 h-PUG was positively related to FPG and 2 h-PG (r = 0.428 and 0.551, respectively, both P < 0.001). For estimation of 2 h-PG ≥ 7.8 mmol/L and 2 h-PG ≥ 11.1 mmol/L using 2 h-PUG, the area under the ROC curve were 0.772 (95% confidence interval [CI ]: 0.738-0.806) and 0.885 (95% CI: 0.850-0.921), respectively. The corresponding optimal cut-offs for 2 h-PUG were 5.6 mmol/L and 7.5 mmol/L, respectively. Compared with FPG alone, FPG combined with 2 h-PUG had a higher sensitivity for detecting glucose abnormalities (84.1% vs. 73.7%, P < 0.001) and diabetes (82.7% vs. 48.1%, P < 0.001).
CONCLUSIONFPG combined with 2 h-PUG substantially improves the sensitivity in detecting prediabetes and diabetes relative to FPG alone, and may represent an efficient layperson-oriented diabetes screening method.

Objective To investigate the epidemiological,genealogic characteristic,familial history of the families with fatal familial insomnia,its clinical and pathological features as well as the heredity rule of related genes.Methods 135 familial members of 7 eras were studied.Vein blood samples from patients as well as from some familial members were collected.PRNP gene was studied with PCR,its serial was determined and then authenticated with Nsp I.Brain tissue was obtained for neuropathological test and PrPSc test with Western blot method.Results Clinical symptoms of the 2 diagnosed cases were typical.11 familial members died of similar neural disease.32 samples of their familial members,codon at D178N of PRNP of 11 members was mutated,with mutation rate as 34.38% while D129N showed as methionine.Brain tissue of both probands denaturalized into spongiform and the nerve fiber was absent but PrPSc protein was identified.Conclusion Genealogy was described in the family with fatal familial insomnia since the patients had typical clinical symptoms and pathological characteristics.It seemed necessary to confirm cases of fatal familial insomnia and their genealogy with epidemiological data and to investigate its gene characteristics as well as with neuropathological and Western blot tests.
