Objective To investigate the effect of isoflurane preconditioning on the nuclear factor kappa B (NF-kB)activity of leukocytes in patients undergoing cardiac valve replacement with cardiopulmonary bypass (CPB).Methods Twenty ASAⅡorⅢpatients of both sexes aged 20-60 yrs undergoing cardiac valve replacement under CPB were randomly divided into 2 groups(n=10 each):control group(C)and isoflurane preconditioning group(I).Anesthesia was induced with midazolam 0.08-0.12 mg?kg~(-1),fentanyl 5-10?g?kg~(-1) and vecuronium 0.1 mg?kg~(-1).The patients were mechanically ventilated(FiO_2=100%)after tracheal intubation.Anesthesia was maintained with intermittent i.v.boluses of fentanyl and midazolam in group C or with 2 MAC isoflurane and intermittent i.v.boluses of fentanyl and midazolam in group I before CPB.Systolic BP was kept between 90-120 mm Hg,diastolic BP between 50-80 mm Hg and HR between 60-90 bpm in both groups. Isoflurane was discontinued at the initiation of CPB.Arterial blood samples were taken after tracheal intubation and before inhalation of isoflurane(T_0)at 30 min(T_1),1 h(T_2)and 2 h(T_3)after aortic unclamping for determination of NF-kB activity of leukocytes using electrophoretic mobility shift assay(EMSA).The amount of fentanyl,midazolam,dopamine and sodium nitroprusside(SNP)consumed during operation and the rate of recovery of spontaneous heart beat in both groups were recorded.Results The NF-kB activity was significantly increased after aortic unclamping in C group but there was no significant difference in NF-kB activity before CPB (T_0)and after aortic unclamping(T_(1-3))in I group.The NF-kB activity was significant lower at T_(1-3) in group I than in group C.The total amount of fentanyl consumed was 40-60?g?kg~(-1) in C group and 20-30?g?kg~(-1) in group I. Significantly less amount of dopamine was used in group I than in group C.There was no significant difference in SNP consumption between the 2 groups.The rate of recovery of spontaneous heart beat was significantly higher in group I than in group C(P＜0.01).The amount of dopamine consumed was positively correlated with the highest level of NF-kB activity in both group[r=0.962 in group C;r=0.908 in group I(P＜0.01)].Conclusion Isoflurane preconditioning can attenuate the NF-kB activity of leulocytes in patients undergoing cardiac valve replacement under CPB.

Objective: To prepare standard decoction of Alismatis Rhizoma (AR) and establish its quality evaluation system, and provide reference for the development of dispensing granules of AR. Methods: A total of 18 batches of AR decoction pieces were collected to prepare standard decoction of AR according to the standard process. Quality evaluation system of standard decoction of AR was established with pH value, dry extract rate, fingerprint similarity and transfer rate of alisol B 23-acetate as indexes. Results: The mass fraction of alisol B 23-acetate in AR decoction pieces was 0.057%—0.267% with the average value of 0.156%, water content was 9.2%—12.8% with the average value of 10.44%; the pH value of standard decoction of AR was 4.11—5.60, dry extract rate was 10.25%—17.09%; transfer rate of alisol B 23-acetate from decoction pieces to standard decoction was 10.49%—17.49%. Conclusion: The established quality evaluation method is stable and feasible, which is suitable for the development and quality evaluation of standard decoction of AR, which can provide reference for the development of dispensing granules of AR and related classic formulas.

ObjectiveTo observe the effect of integrated traditional Chinese medicine therapy on diabetic peripheral neuropathy.Methods22 patients with diabetic peripheral neuropathy were as the group of integrated traditional Chinese medicine therapy ,who received Chinese herbal medicine treatment(orally),acupuncture,etc. Other 20 cases were as the control group,disease natural course. The effect of two groups were compared with the means.ResultsThe patients in group of integrated traditional Chinese medicine therapy got obviously better results than that of the control group(P<0.05).ConclusionThe integrated traditional Chinese medicine therapy is effective on diabetic peripheral neuropathy.

BACKGROUNDTo evaluate homemade and imported HbsAg ELISA kits on screening blood donors.

METHODSSamples for evaluation included 120 HbsAg serum plates for the golden criteria and 400 sets of serum from blood donors in Dongguan. The samples underwent blind screening with homemade and imported ELISA kits respectively.

RESULTSThe sensitivity of homemade (Xinchuang) and imported (Diasorin) HbsAg ELISA kit were 85.71% (72/84) and 100% (84/84), respectively. Their specificity was 100% (436/436) and 96.55% (421/436) respectively. The consistency of two ELISA kits was 100%.

CONCLUSIONThe imported ELISA kit had the highest sensitivity, but its specificity was not as good as that of homemade ELISA kit. The two kinds of ELISA kits had good repetition. The combination of the two reagents may ensure the safety of blood transfusion.

Fourteen psychrotrophic bacteria were isolated from swamp soil collected in Ruoergai plateau wetland,and their generation time and degrading ability of livestock wastewater CODcr was determined.The results showed that the generation time was within 4.9 h to 11.6 h.Based on the generation time,9 psychro-trophic strains(NLJ1,NLJ6,NLJ7,NLJ9,NLJ10,NLJ11,NLJ12,NLJ13 and NLJ14),whose generation time was within 4.9 h to 5.6 h,were chosen to treat livestock wastewater.The results suggested that these 9 strains had different CODcr disposal ability when treating livestock wastewater singly at 6?C for 6 h,and strains NLJ6,NLJ7,NLJ9,NLJ10,NLJ11 and NLJ13 had good ability to degrade livestock wastewater,the CODcr degrading rate was about 60%~70%,hence,they were used as high efficient strains;However,the CODcr degrading rate of the other strains was less than 50%.After inoculating mixture culture of these six strains into the distilled livestock wastewater,after 6 h's treating,the CODcr degrading rate reached to 85.42%.Furthermore,activated sludge collected from Yaan,Dujiangyan and Chengdu were inoculated by the mixture culture of those six strains,and used to treat livestock wastewater for 6 h.The results showed that the average CODcr degrading rate was 81.67%,76.32% and 70.56%,respectively;Variance analysis showed that there was no significant differentiation between each treatment,which revealed that those six psychrotrophic strains had good adaptability to different source of activated sludge.

Site-saturation mutagenesis is a newly-developed technology in protein engineering.By manipulating the encoding genes,it can rapidly obtain the mutants of desired proteins whose target residues are substituted by 19 other common amino acids.Site-saturation mutagenesis could serve not only as a powerful tool in protein engineering,but also as an important method in exploring the structure-function relationship of proteins.Several techniques were summarized to achieve site-saturation mutagenesis and introduce their application status in the protein engineering.The problem and promising future of its application were also discussed.

Objective To determine the effect of histotype and histodifferentiation on the maximum standardized uptake value (SUV_(max)) of non-small cell lung cancer (NSCLC) ~(18)F-fluorodeoxyglucose (FDG) PET/CT imaging.Methods Two hundred and sixty patients with NSCLc underwent ~(18)F-FDG PET/CT imaging.They were classified according to (1) histotype:as adenocarcinoma (AC),squamous cell carcinoma(SQC),adenosquamous carcinoma (ASC) and other type carcinoma (OTC),and (2) histodifferentiation:as grade Ⅰ (well-differentiated),grade Ⅱ (moderate-differentiated) and grade Ⅲ (poor-differentiated).The SUV_(max) and size(long diameter)of the primary lesions were measured.Multivariate regression analysis was used to analyze the relationship between the SUV_(max) and variable factors including histotype,histodifferentiation,lesion size,age,sex,body height,body weight,body mass index (BMI),blood glucose level,dose,and rate of dose.Results Two hundred and sixty patients had 260 primary NSCLC tumors.There were 161 AC(15 grade Ⅰ,88 grade Ⅱ,58 grade Ⅲ),74 SQC(6 grade Ⅰ,39 grade Ⅱ,29 grade Ⅲ),15 ASC(7 grade Ⅱ,8 gradeⅢ)and OTC(8 large cell,2 carcinosarcoma).Only lesion size (F=87.046.P＜0.001),histodifferentiation (F=87.604,P＜0.001) and histotype (F=66.663,P＜0.001) were included for multivariate regression analysis with SUV_(max).After adjustment for lesion size,the SUV_(max)(mean and 95%confidence interval) in ascending order was AC Ⅰ:3.3(2.1-4.5),ACⅡ:6.0(5.5-6.6),SQCⅠ:6.1(4.2-8,0),ASC Ⅱ:6.6(4.8-8.4),SQCⅡ.7.8(7.0-8.6),OTC:8.1(6.6-9.6),AC Ⅲ:8.3(7.6-8.9),ASC Ⅲ:8.7(7.0-10.4),and SQC Ⅲ:8.9(8.0-9.8).11he SUV_(max) of AC Ⅰ was significantly lower than that of SQC Ⅰ(q=-2.786,P=0.017),same for AC Ⅱ and SQC Ⅱ(q=-1.776,P＜0.001),but no statistically significant differences were found among AC Ⅲ,ASC Ⅲ and SQC Ⅲ(q=-0.593,-0.422,0.171,P=0.288,0.642,0.860,respectively).For the same histotype lesions,the difference of SUV_(max) among AC Ⅰ,Ⅱ and Ⅲ was statistically significant(q=-2.720,-4.943,-2.223,all P＜0.001),as also for SQC Ⅰ and Ⅲ(q=-2.751,P=0.012).Conclusion Histotype and histodifferentiation are significant correlative factors for ~(18)F-FDG uptake of NSCLC,with histodifferentiation being the factor with greater impact.

Objective To study the balance function of normal persons and to explore the differences in gender and age groups.Methods Forty parameters of 445 normal persons(16-70 years old, 205 of male, 240 of female ) were detected by static posturography while they were in seat, double foot standing with opening and closing eyes, and single foot standing with opening eyes. The results of male and female, standing with opening eyes and closing eyes, left foot standing and right foot standing were compared.Results Everyone's center of gravity was distributed in A zone while they were in seat. At standing with opening eyes and closing eyes, the center of gravity in A zone were in 429 subjects( 96.4%) and 436 subjects( 98.0%). Most parameters showed significant difference between the women and the men, opening eyes and closing eyes, left foot standing and right foot standing, different age groups. The stability in closing eyes was better than that in opening eyes, right foot standing was better than left foot standing. Most values of 20—39 years old group were the best, 70—79 years old group were the worst.Conclusion Static posturography can evaluate human balance function objectively and quantitatively. The stability is related to age and gender. To judge the balance function, the subject's age and sex must be taken into account.
