1.Surgical treatment of congenital artrial septal defect in adults

Objective To summarize the clinical expedence in surgical treatment and prognosis of artrial septal defect(ASD)in adults and to know well the surgical indication of ASD in adults.Methods 32 patients over 20 years old with the left to right shunt and preoperative mean pulmonary artery pressure less than 8kPa underwent direct surgical repair for ASD under extra corporeal circulation.Results All patients survived after surgery,there was no surgical mortality in our group.32 patients were followed-up,29 of them with disappearing symptoms and 3 of them still had various symptoms.Conclusion The following condition ASD in adults who have slight to moderate clinical symptoms should be operated:cardiac enlargement eardiothoracic ratio over 50% and pneumonemia in X-ray films;right ventricular hypertrophy with a right axis deviation of incomplete right bundle branch block on the electrocardiogram;diameter of ASD over 1.5cm in ultrasonic cardiogram.

High altitude polycythemia (HAPC) had become one of the main common chronic diseases, which had seriously threatened the health of Plateau people. In the Tibetan medicine classic bookSi-Bu Yi-Dian, there were recordings on HAPC treatment methods and medications, which had the unique advantages of identified therapeutic effect with little side effect. This article analyzed Tibetan medicine in the prevention and treatment of HAPC from aspects such as etiology and pathogenesis, clinical treatment advantages and modern innovation study. Questions were also raised on lacking of standardization on HAPC clinical effectiveness, as well as Tibetan medicine compound material basis and action mechanisms were unclear. It was proposed that based on the inheritance of Tibetan medicine theoretical basis and clinical therapeutic effect, the Tibetan medicine original thinking should be combined with modern science and technology, in order to strengthen the analysis of ancient literature collection in HAPC treatment and data mining in medication experiences. The clinical treatment standards and medication plan should be standardized. Methods of systems biology, such as metabolomics, can be used in the further study of the scientific connotation of HAPC treatment by Tibetan medicine.
5.Lipase-catalyzed production of biodiesel from high acid value waste oil with ultrasonic assistant.

Biodiesel fuel produced with the enzyme-catalyzed esterification and transesterification of high acid value waste oil through ultrasonic assistant was explored. Propyl oleate, biodiesel, converted from high acid value waste oil and 1-proponal catalyzed with immobilized lipases from Candida antarctica and Aspergillus oryzae in conditions of ultrasonic assistant. Commercial immobilized lipase Novozym 435 from C. antarctica was used as biocatalyst catalyzing high acid value waste oil and 1-proponal esterification and transesterification to propyl oleate under the ultrasonic assistant conditions and different conditions such as lipases amounts, initiatory molar ratio of propanol to oil, frequency of ultrasonic and power of ultrasonic were investigated and optimized. It is revealed that the enzymatic activity of Novozym435 is enhanced and, in particular, enzyme-catalyzed transesterification activity is enhanced obviously under the ultrasonic assistant conditions. Low frequency and mild energy ultrasonic is a key factor for enhancing enzymatic activity, emulsifying oil-propanol system and accelerating the speed of produce diffusing in the system. Under the optimal ultrasonic assistant reaction conditions, such as Novozym435 amounts 8% by oil quantity, initiatory molar ratio of propanol to oil 3:1, frequency of ultrasonic 28 KHz, power of ultrasonic 100 W and temperature of water batch 40-45 degrees C, the conversion ratio to propyl oleate reached to 94.86% in 50 mins in comparison with the highest conversion ratio to propyl oleate 84.43% under the conventional mechanical agitation conditions. Furthermore, it is demonstrated that various short chain linear and branched alcohols (C1-C5) show high conversion ratio to fatty acid alkyl esters (biodiesel) under the optimal ultrasonic assistant reaction conditions. On the other hand, ultrasonic energy is propitious to reduce the adsorption of product propyl oleate, by-product glycerol and other emplastics in system on the surface of immobilized lipase Novzym435 and recyclable Novozym435 possess clean appearances, well decentralizations, no agglomeration and easy washing and well operational stability.

6.Effects of recombinant human erythropoietin on the immune function of premature rats.

OBJECTIVETo observe the effect of recombinant human erythropoietin (rHuEPO) on immune function of premature rats.
METHODSRHuEPO of 250 IU/(kg.t) or 500 IU/(kg.t) was administered to premature rats every other day for nineteen days. The control premature rats were received normal saline. The changes of hemoglobin (Hb), serum erythropoietin (EPO), red blood cell (RBC) immune function, T lymphocyte proliferative responsiveness, and production of tumor necrosis factor alpha (TNF-alpha) were observed.
RESULTSPremature rats showed lower levels on Hb, RBC immune function, T cell responsiveness and production of TNF-alpha compared with mature rats at birth. The postnatal declines of Hb and RBC immune function were lessened in the treated groups of premature rats, the higher dosage group of 500 IU/(kg.t) was more significant than the lower dosage group of 250 IU/(kg.t). When experiments were over, Hb of control premature rats was (7.72 +/- 0.89) g/dl, Hb of premature rats received 500 IU/(kg.t) was (10.08 +/- 0.90) g/dl (P < 0.01). C3b-R% of control premature rats was (11.00 +/- 0.95)%, C3b-R% of premature rats received 500 IU/(kg.t) was (17.75 +/- 1.04)% (P < 0.01). IC-R% in control premature rats was (12.83 +/- 1.33)%, IC-R% of premature rats received 500 IU/(kg.t) was (10.50 +/- 1.67)% (P < 0.01). The postnatal rise of T cell responsiveness and the production of TNF-alpha in premature rats increased in the treated groups, which was more significant in the higher dosage group of 500 IU/(kg.t) than in the lower dosage group of 250 IU/(kg.t). The OD index of control premature rats was 0.159 +/- 0.014, the OD index of premature rats received 500 IU/(kg.t) was 0.354 +/- 0.050 (P < 0.01). TNF-alpha in control premature rats was (0.270 +/- 0.014) ng/ml, TNF-alpha of premature rats received 500 IU/(kg.t) was (0.415 +/- 0.010) ng/ml (P < 0.01).
CONCLUSIONS(1) Premature rats had lower RBC immune function and T cell responsiveness and underproduction of TNF-alpha at birth. (2) Premature rats had an improvement with the RBC immune function after rHuEPO administration. (3) Premature rats had improvements with T cell responsiveness and TNF-alpha after rHuEPO administration, and there was a positive correction between the RBC immune function and T cell responsiveness with the production of TNF-alpha.

8.Analysis on occupational exposure levels and control effectiveness of dust in cement production line of new dry method.

OBJECTIVETo investigate the occupational exposure levels of dust in new suspension preheated dry process (NSP) cement production line and put forward rectification measures for dust-exposed posts, and to provide ideas for the modern cement production enterprises in dust control and occupational health management.
METHODSOccupational health field investigation combined with field test were used to measure the time-weighted average concentration (C(TWA)) of the dust in the workplace. Rectification measures were taken for the dust-exposed posts with unqualified dust concentration, and the protective effects of dustproof facilities in the rectified workplace were evaluated.
RESULTSThe field investigation revealed incompletely closed dustproof facilities, improperly set dust hoods, excess of dust leakage points, and other problems in the dust-exposed posts of an NSP cement production line before rectification, and the dustproof facilities could hardly exert dust removal effect. The field test showed that the vast majority of dust-exposed posts had the dust concentrations exceeding the occupational exposure limits (OELs), with a qualified rate as low as 31.8%. A series of rectification measures were taken for these posts. After the rectification, the dust-exposed posts demonstrated dramatically dropped C(TWA), and the qualified rate of dust concentration in the dust-exposed posts rose to 90.9%.
CONCLUSIONThe dust hazards in NSP cement production line cannot be ignored. Taking appropriate protective measures are critical for curbing dust hazards in modern cement production.

9.Preparation of curcumin-EC sustained-release composite particles by supercritical CO2 anti-solvent technology.

Curcumin-ethyl-cellulose (EC) sustained-release composite particles were prepared by using supercritical CO2 anti-solvent technology. With drug loading and yield of inclusion complex as evaluation indexes, on the basis of single factor tests, orthogonal experimental design was used to optimize the preparation process of curcumin-EC sustained-release composite particles. The experiments such as drug loading, yield, particle size distribution, electron microscope analysis (SEM) , infrared spectrum (IR), differential scanning calorimetry (DSC) and in vitro dissolution were used to analyze the optimal process combination. The orthogonal experimental optimization process conditions were set as follows: crystallization temperature 45 degrees C, crystallization pressure 10 MPa, curcumin concentration 8 g x L(-1), solvent flow rate 0.9 mL x min(-1), and CO2 velocity 4 L x min(-1). Under the optimal conditions, the average drug loading and yield of curcumin-EC sustained-release composite particles were 33.01% and 83.97%, and the average particle size of the particles was 20.632 μm. IR and DSC analysis showed that curcumin might complex with EC. The experiments of in vitro dissolution showed that curcumin-EC composite particles had good sustained-release effect. Curcumin-EC sustained-release composite particles can be prepared by supercritical CO2 anti-solvent technology.
