By employing the analytic hierarchy process(AHP), the CRITIC method(a weight determination method based on indicator correlations), and the AHP-CRITIC hybrid weighting method, the weight coefficients of evaluation indicators were determined, followed by a comprehensive score comparison. The grey correlation analysis was then performed to analyze the results calculated using the hybrid weighting method. Subsequently, a backpropagation-artificial neural network(BP-ANN) model was constructed to predict the extraction process parameters and optimize the extraction process for Shenxiong Huanglian Jiedu Granules(SHJG). In the extraction process, an L_9(3~4) orthogonal experiment was designed to optimize three factors at three levels, including extraction frequency, water addition amount, and extraction time. The evaluation indicators included geniposide, berberine, ginsenoside Rg_1 + Re, ginsenoside Rb_1, ferulic acid, and extract yield. Finally, the optimal extraction results obtained by the orthogonal experiment, grey correlation analysis, and BP-ANN method were compared, and validation experiments were conducted. The results showed that the optimal extraction process involved two rounds of aqueous extraction, each lasting one hour; the first extraction used ten times the amount of added water, while the second extraction used eight times the amount. In the validation experiments, the average content of each indicator component was higher than the average content obtained in the orthogonal experiment, with a higher comprehensive score. The optimized extraction process parameters were reliable and stable, making them suitable for subsequent preparation process research.

Osteoporosis is a prevalent skeletal condition characterized by reduced bone mass and strength, leading to increased fragility. Buqi-Tongluo (BQTL) decoction, a traditional Chinese medicine (TCM) prescription, has yet to be fully evaluated for its potential in treating bone diseases such as osteoporosis. To investigate the mechanism by which BQTL decoction inhibits osteoclast differentiation in vitro and validate these findings through in vivo experiments. We employed MTS assays to assess the potential proliferative or toxic effects of BQTL on bone marrow macrophages (BMMs) at various concentrations. TRAcP experiments were conducted to examine BQTL's impact on osteoclast differentiation. RT-PCR and Western blot analyses were utilized to evaluate the relative expression levels of osteoclast-specific genes and proteins under BQTL stimulation. Finally, in vivo experiments were performed using an osteoporosis model to further validate the in vitro findings. This study revealed that BQTL suppressed receptor activator of NF-κB ligand (RANKL)-induced osteoclastogenesis and osteoclast resorption activity in vitro in a dose-dependent manner without observable cytotoxicity. The inhibitory effects of BQTL on osteoclast formation and function were attributed to the downregulation of NFATc1 and c-fos activity, primarily through attenuation of the MAPK, NF-κB, and Calcineurin signaling pathways. BQTL's inhibitory capacity was further examined in vivo using an ovariectomized (OVX) rat model, demonstrating a strong protective effect against bone loss. BQTL may serve as an effective therapeutic TCM for the treatment of postmenopausal osteoporosis and the alleviation of bone loss induced by estrogen deficiency and related conditions.

Objective:To investigate the predictive efficacy of brain natriuretic peptide(BNP),angiotensin Ⅱ(An-gⅡ),erythrocyte distribution width(RDW)and neutrophil/lymphocyte ratio(NLR)for postoperative left ventric-ular systolic dysfunction(LVSD)in patients with acute anterior myocardial infarction(AAMI).Methods:A total of 160 AAMI patients treated in our hospital from January 2018 to January 2022 were selected.According to left ventricular ejection fraction(LVEF)after percutaneous coronary intervention,they were divided into no LVSD group(LVEF＞40％,n=110)and LVSD group(LVEF≤40％,n=50).Baseline data were compared between two groups.Multivariate Logistic regression analysis was used to analyze influencing factors of postoperative LVSD in AAMI patients.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of single and combined detection of BNP,AngⅡ,RDW and NLR for postoperative LVSD in AAMI patients.Results:Compared with no LVSD group,there were significant rise in levels of BNP[(347.52±82.66)pg/ml vs.(405.55±105.47)pg/ml],AngⅡ[(238.11±20.43)ng/ml vs.(254.58±22.53)ng/ml],RDW[(22.88±5.25)％vs.(25.52±5.58)％]and NLR[(4.34±1.09)vs.(5.31±1.50)]in LVSD group(P＜0.01 all);BNP,AngⅡ,RDW and NLR were independent risk factors for postoperative LVSD in AAMI patients(OR=2.002～3.692,P＜0.05 or＜0.01);predictive value of combined detection of BNP,AngⅡ,RDW and NLR for postoperative LVSD in AAMI patients was significantly higher than those of single detection[area under the curve(AUC):0.809 vs.0.650,0.696,0.641,0.694,P＜0.01 all].Conclusion:Left ventricular systolic function is closely associated with levels of BNP,AngⅡ,RDW and NLR in AAMI patients.Combined detection of the above indexes can effectively predict postoperative left ventricular systolic dysfunction in these patients.

Objective:To investigate the effect of recombinant human prourokinase combined with tirofiban on cardiac func-tion and prognosis in patients with acute ST-segment elevation myocardial infarction(ASTEMI)undergoing emergency percutaneous coronary intervention(PCI).Methods:A total of 156 ASTEMI patients undergoing PCI treated in our hos-pital from June 2019 to December 2020 were selected.They were equally divided into control group(received tirofiban in-jection during emergency PCI)and study group(received recombinant human prourokinase injection based on control group)according to random number table method.Two groups received the same medication before and after emergency PCI.Thrombolysis in myocardial infarction(TIMI)flow,TIMI myocardial perfusion grade(TMPG)before and after PCI,cardiac function indexes before and one week after PCI,incidence of major adverse cardiovascular events(MACE)and adverse reactions within 1-year follow-up were compared between two groups.Results:After PCI,compared with control group,there were significant rise in proportions of TIMI flow grade 3(51.28％vs.83.00％)and TMPG grade 3(51.28％vs.84.62％),left ventricular ejection fraction(LVEF)one week after PCI[(44.26±3.45)％vs.(49.17±2.92)％],and significant reductions in left ventricular end-diastolic diameter(LVEDd)[(56.47±2.54)mm vs.(51.32±2.55)mm],left ventricular end-systolic diameter(LVESd)[(41.86±2.49)mm vs.(36.01±1.68)mm]one week after PCI,incidence rates of MACE(24.36％vs.7.69％)and adverse reactions(10.26％vs.1.28％)within 1-year fol-low-up in study group(P＜0.05 or＜0.01).Conclusion:Recombinant human prourokinase combined with tirofiban can significantly improve cardiac function,reduce the incidence rates of MACE and adverse reactions,and improve the prog-nosis in ASTEMI patients after PCI,which is worthy of clinical reference.

Objective:To examine the application of a novel pedagogical approach multidimensional supportive psychological intervention(MSPI)in the clinical practice teaching of andrological nursing care.Methods:Using the Hamilton Depression Scale(HAMD),we assessed the psychology of 100 nursing interns about to enter clinical practice in the Department of Andrology from De-cember 2021 to December 2022.We equally randomized the subjects into an experimental and a control group,the former receiving MSPI and the latter trained on the conventional teaching model without any psychological support intervention.Results:Compared with the baseline,the HAMD scores were significantly decreased in the experimental group after intervention(12.4±2.1 vs 8.9±2.4,P＜0.01),but increased in the controls(13.1±1.8 vs 14.7±1.9,P＜0.01);the skill scores dramatically increased in the experimental group(82.6±4.7 vs 91.2±2.4,P＜0.01),but decreased in the control group after intervention(81.0±3.5 vs 80.4±2.7,P=0.28).Conclusion:MSPI can significantly enhance the learning enthusiasm of nursing students in a short period,re-duce their psychological stress and improve teaching outcomes.This approach,combining psychology with teaching,can also strength-en the mental resilience of nursing students and better confront them with future professional challenges.

Ferroptosis is a new type of programmed cell death different from apoptosis and necrosis.Its mechanism involves iron overload,lipid peroxidation,and reactive oxygen species(ROS)generation caused by disordered iron metabolism.Ferroptosis is associated with primary osteoporosis as well as the secondary osteoporosis such as steroid-induced osteoporosis,diabetic osteoporosis and postmenopausal osteoporosis.The Chinese medicines for the prevention and treatment of osteoporosis through the mechanism of ferroptosis mainly involve Chinese herbal compounds such as herbal pair of Eucommiae Cortex-Dipsaci Radix,Jintiange Capsules,and Bugu Shengsui Prescription,the single Chinese herbal medicine of Achyranthis Bidentatae Radix,and the active ingredients of herbal medicines such as icariin,gastrodin,quercetin,and artesunate.The prevention and treatment of osteoporosis by Chinese medicine monomer and Chinese herbal compound can inhibit ferroptosis through multiple targets and multiple pathways.At present,Chinese medicine has advantages in the prevention and treatment of postmenopausal osteoporosis.The prevention and treatment of osteoporosis by regulating the mechanism of ferroptosis can provide a direction for the future research on the prevention and treatment of osteoporosis by Chinese medicine.

OBJECTIVE: To observe the clinical efficacy of transcutaneous electrical acupoint stimulation (TEAS) combined with warm acupuncture in treating breast cancer associated with upper limb lymphedema (BCRL). METHODS: This was a retrospective cohort study using a paired control design. Fifty-two BCRL patients were assigned to the control group (27 cases) and the treatment group (25 cases). The patients in the control group were treated with lymphedema comprehensive detumescence treatment (CDT) for 4 weeks, including systematic therapy composed of manual lymphatic drainage, compression bandage, skincare, and functional exercise. The patients in the treatment group were treated with TEAS combined with warm acupuncture based on the control group methods. Each treatment lasted for 30 min and was applied twice a week for 4 weeks. The arm circumference (AC) of different positions of the affected limb and the degree of swelling of the affected limb were evaluated before the first treatment and after the last treatment. The clinical efficacy was evaluated according to the degree of edema before and after treatment. All adverse events during treatment were recorded. RESULTS: The patients' AC and the swelling feeling of the affected limb in the treatment group and the control group were both reduced compared with those before treatment. Compared with the control group, AC of the wrist joint transverse stria, the midpoint between the wrist joint transverse stria and the elbow joint transverse stria in the treatment group were significantly reduced (P<0.05). The decrease in AC diameter at the midpoint between the elbow joint transverse stria and the axillary transverse stria was the most significant (P<0.01). The swelling degree of the affected limbs in the treatment group was significantly lower than before treatment, and was significantly lower compared with the control group after treatment (P<0.01). The total effective rate was 72% in the treatment group, significantly higher than that in the control group (55.56%, P<0.05). No serious adverse events occured in either group. CONCLUSIONS TEAS combined with warm acupuncture can effectively reduce AC and swelling feeling of the affected limb in patients with BCRL. The effect is better than that of CDT therapy alone. According to the theory of 'Xingben Dazao' of Psoralea corylifolia Linn. (BL), the susceptible syndromes and biomarkers of liver injury caused by BL were searched. Rat models of kidney-yin deficiency syndrome (M_yin) and kidney-yang deficiency syndrome (M_yang) were established, and all animal experimental operations and welfare following the provisions of the First Affiliated Experimental Animal Ethics and Animal Welfare Committee of Henan University of Traditional Chinese Medicine (No. YFYDW2020017). The results showed that BL significantly decreased the body weight, water intake, and urine weight of M_yin rats and increase the organ indexes of the liver, testis, adrenal gland, and spleen and the expression of alanine aminotransferase (ALT). Meantime, BL significantly increased the urine weight of M_yang rats and decreased the expression of ALT and aspartate aminotransferase (AST). Hematoxylin and eosin (HE) staining showed that BL could aggravate inflammatory infiltration of hepatocytes in rats with M_yin and alleviate liver injury in rats with M_yang. Metabolomics identified 17 BL co-regulated significant differential metabolic markers in M_yin and M_yang rats. Among them, 8 metabolites such as glutamine, quinolinate, biliverdin, and lactosylceramide showed opposite trends, mainly involving cysteine and methionine metabolism, tyrosine metabolism, tryptophan metabolism, purine metabolism, sphingolipid metabolism, glycerol phospholipid metabolism, glutamine metabolism, and other pathways. M_yin/M_yang may be the susceptible constitution of BL for liver damage or protection, which may be related to the regulation of amino acid metabolism and sphingolipid metabolism. The study can provide some experimental data support for the safe and accurate use of BL in the clinical practice of traditional Chinese medicine.

OBJECTIVE: To access the efficacy and safety of the double-ProGlide technique for the femoral vein access-site closure in cryoballoon ablation with uninterrupted oral anticoagulants (OAC), and its impact on the electrophysiology laboratory time as well as hospital stay after the procedure in this observational study. METHODS: Patients with atrial fibrillation undergoing cryoballoon ablation with uninterrupted OAC at Department of Cardiology, Beijing Anzhen Hospital, Capital Medical University, Beijing, China from May 2019 to May 2021 were enrolled in this study. From October 2020, double-ProGlide technique was consistently used for hemostasis (ProGlide group), and before that conventional manual compression was utilized (manual compression group). The occurrence of vascular and groin complications was accessed during the hospital stay and until the three-month follow-up. RESULTS: A total of 140 participants (69.30% of male, mean age: 59.21 ± 10.29 years) were evaluated, 70 participants being in each group. Immediate hemostasis was achieved in all the patients with ProGlide closure. No major vascular complications were found in the ProGlide group while two major vascular complications were occurred in the manual compression group. The incidence of any groin complication was obviously higher in subjects with manual compression than patients with ProGlide devices (15.71% vs. 2.86%, P = 0.009). In addition, compared with the manual compression group, the ProGlide group was associated with significantly shorter total time in the electrophysiology laboratory [112.0 (93.3-128.8) min vs. 123.5 (107.3-158.3) min, P = 0.006], time from sheath removal until venous site hemostasis [3.8 (3.4-4.2) min vs. 8.0 (7.6-8.5) min, P < 0.001], bed rest time [8.0 (7.6-8.0) h vs. 14.1 (12.0-17.6) h, P < 0.001] and hospital stay after the procedure [13.8 (12.5-17.8) h vs. 38.0 (21.5-41.0) h, P < 0.001]. CONCLUSIONS Utilization of the double-ProGlide technique for hemostasis after cryoballoon ablation with uninterrupted OAC is feasible and safe, which has the clinical benefit in reducing the total electrophysiology laboratory time and the hospital stay length after the procedure.
