1.Clinical features and confocal microscopic imaging characteristics of 466 cases with infectious keratitis

AIM: To observe the role of confocal microscopy in infectious keratitis management.
METHODS:Totally 466 patients (467 eyes) diagnosed as infectious keratitis from January 2010 to December 2013 were retrospectively studied. the corneas were examined early by in vivo confocal microscopy. The characteristics of their images and clinical features were studied and summarized.
RESULTS:All patients were recorded, the average age was 54. 4±13. 0 years, in which 264 cases (56. 7%) were male, and 202 cases ( 43. 3%) were female. In the 466 patients, 190 (40. 8%) were fungal keratitis, 148 (31. 8%) were viral keratitis, 125 (26. 8%) were bacterial keratitis and 3 ( 0.6%) were acanthamoeba keratitis. There were fungal hyphae in the images of fungal keratitis. Amebic cysts were found in acanthamoeba keratitis.
CONCLUSION:Confocal microscope can help the early diagnose and treatment of infectious keratitis. It is a noninvasive imaging technique that provides high resolution images of ocular structures at a cellular level and infectious keratitis represents one of its most important clinical uses.
2.Observation on therapeutic effect of heat needle combined with herb iontophoresis and western medicine on rheumatoid arthritis.

OBJECTIVETo search for a therapeutic program for increasing the therapeutic effect of western medicine on rheumatoid arthritis (RA).
METHODSNinety-six cases of RA were randomly assigned to a treatment group and a control group, 48 cases in each group. The treatment group were treated with heat electroacupuncture instrument with Chinese herb iontophoresis, combined with Meloxicam, Salazosulfamide, and Methotrexate. The control group with simple the western medicine. After treatment of a month, changes of the morning stiff duration, grasp strength, number of tenderness joints, tenderness index, joint rest pain, number of swelling joints, swelling index, assessment of the patient and doctor, and blood rheumatism factor (RF), C-response protein (CRP), erythrocyte sedimentation rate (ESR), the white blood cell (WBC) and platelet counts (PLT) were investigated.
RESULTSThe effective rate was 79.2% in the treatment group and 52.1% in the control group, the treatment group being significantly better than the control group (P < 0.01). After treatment of one month, morning stiff duration, average grasp strength of the both hands, tenderness of joint and swelling of joints significantly improved (P < 0.01), with the treatment group being superior to the control group (P < 0.01). After treatment, blood RF, CRP, ESR, WBC and PLT decreased in the two groups (P < 0.01), and the decreases of blood CRP, ESR, PLT in the treatment group were more significantly as compared with the control group (P < 0.01).
CONCLUSIONThe therapeutic effect of heat needle combined with herb iontophoresis and western medicine is superior to simple medicine for treatment of rheumatoid arthritis.

3.Right ventricular desynchronization in patients with pacemaker syndrome.

OBJECTIVETo observe the incidence of ventricular desynchronization in patients with or without pacemaker syndrome (PMS).
METHODSThe systolic peak velocity, the acceleration and the time to peak velocity of the interventricular septum (IVS), left ventricular (LV) and right ventricular (RV) lateral wall were detected by tissue Doppler imaging (TDI) in 14 atrial fibrillation patients without pacemaker implantation (control), 18 atrial fibrillation patients without PMS and 16 atrial fibrillation patients with PMS. All patients were free of valve disease, myocardial infarction, severe pulmonary hypertension, low left ventricular eject fraction (< or = 50%), significant segmental hypokinesis of ventricular wall or complete bundle branch block.
RESULTSCompared to the control patients, the systolic peak velocity and the accelerations on lateral walls of the LV and RV reduced significantly in patients with implanted pacemakers (P < 0.05). The intervals to peak velocity of the IVS and LV lateral walls were significantly prolonged [PMS group (80.13 +/- 26.92) ms vs. (25.60 +/- 4.30) ms, P < 0.01; without PMS group (76.22 +/- 23.32) ms vs. (25.60 +/- 4.30) ms, P < 0.01] and the intervals to peak velocity of the IVS and RV lateral walls significantly shortened [PMS group (16.33 +/- 6.85) ms vs. (40.70 +/- 7.60) ms, P < 0.01; without PMS group (21.20 +/- 7.34) ms vs. (40.70 +/- 7.60) ms, P < 0.01]. The systolic peak velocities, the accelerations of the IVS and bilateral walls and the intervals to peak velocity of the IVS and LV lateral wall were similar in patients with and without PMS (P > 0.05), however, the intervals to peak velocity of the IVS and RV lateral wall was significant shorter in patients with PMS compared to that of patients without PMS [(16.33 +/- 6.85) ms vs. (21.20 +/- 7.34) ms, P < 0.01].
CONCLUSIONRV desynchronization but not LV desynchronization might play an important role in patients with PMS.

4.Effect of sinomenine on adjuvant arthritis and its mechanisms.

OBJECTIVETo discuss the anti-inflammatory mechanism of sinomenine on inflammatory media in joint of adjuvant arthritis rats.
METHODRats were randomly divided into the normal group and the model group, the prednisone group, the small, medium, large of sinomenine group (30, 60, 120 mg x kg(-1)). Except for the rats in the normal group, animals were modeled to adjuvant arthritiswith freund's complete adjuvant. The arthritis index (AI) and the swelling degree of paws were recorded, and the activity of IL-1, TNF and the levels of NO, PGE2 in joint fluids of secondary arthritis were determined.
RESULTCompared with the normal group, the activity of IL-1, TNF and the levels of NO, PGE2 in joint fluids of secondary arthritis were increased significantly in the model group (P < 0.05). Compared with the model group, it was shown to exert a dramatic inhibitory effect on secondary reaction of freund's adjuvant arthritis of rats, and the activity of IL-1, TNF and the levels of NO, PGE2 in joint fluids of secondary arthritis were significantly decreased in the sinomenine group (P < 0.05).
CONCLUSIONSinomenine has a remarkable treatment effect on RA. It is via NO to inhibit the activity of cytokines and decrease the level of inflammation mediators, which may be one of its curing RA mechanism.

5.Optimization of Dry Granulation Technology of Typha Pollen Granules by Box-Behnken Design and Response Surface Method

Objective To optimize the dry granules process of Typha Pollen. Methods The particle size and particle friability were selected as evaluation indexes. The inspect factors were water content, compression frequency, and granulation frequency. Influence of inspect factors on evaluation indexes was investigated by single factor test, the influence of inspect factors on OD value was investigated by Box-Behnken design, and response surface method was adopted to predict, analyze and choose optimal process. Results The optimal dry granulation technology was as follows: the water content was 35.0%; the frequency of tabletting was 27 Hz; the granulating frequency was 15 Hz. Conclusion The selected process is stable, feasible and reproducible, which can be used for granulation of Typha Pollen granules.
6.Investigation on the selenium and T-2 toxin level in Kaschin-Beck disease relative active regions in Aba state of Sichuan province in 2008

Objective To investigate the children's body environmental Se and T-2 toxin level in their staple food in Kaschin-Beck disease(KBD)relative active regions in Aba state of Sichuan province in 2008.Methods We took X-ray photograph of the right hand on children aged 7-13 years in 48 villages from 11 counties in Aba state.The relative active regions of KBD were chosen according to the X-ray result and historical status of KBD.The children's urine and hair,drinking water and their staple food werr sampled.Selenium contents in urine,hair,water and food samples were determined by naphthalene fluorescence,and T-2 toxin in staple food samples were detected by ELISA kits.Results In 2145 X-ray films,66 films were positive,and the children's KBD positive rate was 3.08%(66/2145).The KBD positive rate was respectively 10.98%(29/264)and 8.52%(19/223)in Maerkang county,Jinchuan county and it was 0.75%(3/400)in Rangtang county,historically serious endemic area.The selenium content in urine of children aged 7-13 years in Maerkang county,Jinchuan county and Rangtang county was (10.41±4.67), (10.11±3.65), (8.42±2.68)μg/g Cr, respectively, there was no statistical difference among three counties(F=0.901, P>0.05). The selenium content in hair of children aged 7-13 years in Maerkang county[(0.18±0.04)mg/kg] was lower than that in Jinchuan county[(0.21±0.04)mg/kg, P<0.05].The selenium content in water in Jinchuan county [(0.225±0.124 )μg/L ] was lower than that in Maerkang county and Rangtang county[(0.320±0.092), (0.339±0.105)μg/L, all P<0.05]. The selenium content in staple food in Jinchuan county(0.0033 mg/kg) was lower than that in Maerkang county and Rangtang county(0.0258,0.0137mg/kg, Z=-6.146,-3.042, all P<0.017). The T-2 toxin level in flour in three counties was 19.60,17.95,26.25 ng/g,respectively,there was no statistical difference among three counties(X2=5.623, P>0.05).The T-2 toxin level in grain Maerkang county (10.72 ng/g) was higher than that in Jinchuan county and Rangtang county (3.74,3.30 ng/g, Z=-6.315,-4.407,all P<0.017). T-2 toxin contamination in flour was more severe than that in grain (Z=-6.690,-5.493,-3.676, all P<0.05). Conclusions In 3 relative active KBD regions of Aba state,the children's selenium nutritional status and the T-2 toxin contamination level in their staple food is consistent with the distribution of KBD.
7.Clinical study of use of laser fluorescence for detecting occlusal caries in deciduous teeth.

OBJECTIVETo evaluate the distribution of DIAGNOdent value according to varying clinical caries severity.
METHODSA total of 541 deciduous molar teeth in children aged from 5 to 6 were examined using DIAGNOdent by one trained dentist. The most severity site in every tooth was recorded. The same sites were examined visually by another dentist. The distribution regular of DIAGNOdent value was analyzed according to the clinical severity score by Ekstrand index.
RESULTSThe higher the visual score, the higher the mean DIAGNOdent value. The variation of values in each visual category was larger than that of values recommended by manufacturers. When clinical severity score was 0, 1, 2, 3 and 4, the median of DIAGNOdent value was 0, 5, 19, 49 and 99 respectively. The mean value for sound surfaces was lower in primary teeth than the cut-off points recommended by manufacturers, but the value of DIAGNOdent was increased obviously when the transparency change of enamel was detected visually.
CONCLUSIONDIAGNOdent is useful in detecting occlusal caries in deciduous teeth, but the cut-off levels is not coinciding perfectly with manufacturer suggesting.

8.Correlation between symptoms of pollen allergic rhinitis and pollen grain spreading in summer and autumn.

OBJECTIVETo analyze the correlation between airborne pollen concentrations and symptoms in patients with pollen allergic rhinitis.
METHODSDurhum sampler was used to collect the pollen concentration and species from June to September in 2011. The clinical skin prick test (SPT) data were analyzed. The patients with pollen allergic rhinitis were divided into pure pollen allergic rhinitis group (pollen group) and pollen combined perennial allergens allergic rhinitis group (combined group). Symptom scores of patients were assessed, and correlation between pollen concentration and onset of symptoms of patients were analyzed. SPSS 16.0 software was used to analyse the data.
RESULTSWhile the peak of Summer-Autumn pollen concentration appeared from August 20 to September 15, the major pollen included Artemisia L, Chenopodium album and Humulus scandens. The peak of pollen concentration in one day reached 638/1000 mm(2). The patients taken SPT from June to September accounted for 51.9% of the patients in whole year, among which SPT pollen positive patients were 1509, 60.7% of all SPT positive patients. The amount and rate of SPT positive patients showed significant correlation with pollen concentration(r value were 0.90 and 0.99, both P < 0.05). Onset of symptoms in two groups was correlated with pollen concentration in Summer-Autumn. Symptoms of cough in combined group showed more severe compared with patients with pollen group (t = 2.36, P < 0.05).
CONCLUSIONSPollen concentration has a major effect on onset of symptoms of allergic rhinitis. Airborne pollen monitoring has important preventive and therapeutic significance on patients with allergic rhinitis.

9.Investigation of state and influence factors of children's Kaschin-Beck disease in Xinghai county of Qinghai province in 2009

Objective To investigate the present prevalence state of children's Kaschin-Beck disease (KBD) in Xinghai county of Qinghai province,a relative active KBD area in 2009,and to investigate their nutritional selenium level of local children and the T-2 toxin contamination level in their staple food.Methods Right hand X-ray photographs of children aged 7 - 12 in Shang,Zhong and Xia villages of Tangnaihai countryside in Xinghai county were taken.X-ray diagnosis was carried out according to the Diagnostic Criteria of Kashin Beck Disease (GB 16003-1995 ).Selected samples (children's hair,drinking water and their staple food) were collected according to X-ray film taken.Selenium contents in hair,drinking water and staple food samples were measured by 2,3-diaminonaphthalene fluorescence,and T-2 toxin in staple food sample was detected with enzyme-linked immunosorbent assay(ELISA) kits.ResultsTotal X-ray detection rate of children KBD was 12.20%(31/254) and KBD positive rate of children in Xia village was up to 14.97%(22/147),Shang village was up to 9.52%(6/63),and Zhong village was up to 6.82% (3/44).The selenium level in children's body and outer environment was very low,namely,the selenium content in hair,drinking water,wheat and flour was (0.250 ± 0.136)mg/kg,(0.156 ± 0.046)μg/L,(0.0045 ± 0.0030)mg/kg,and (0.0067 ± 0.0116)mg/kg,respectively.The T-2 toxin level was relatively high in children's staple food,which was (78.91 ± 46.17)μg/kg in wheat and (47.47 ± 46.47)μg/kg in flour.Conclusions In relative active KBD areas of Xinghai county of Qinghai province,the children's selenium nutritional level is low,and the T-2 toxin contamination level in their staple food is relatively high,which is consistent with the distribution of local children's KBD.
10.Heterogeneity of HIV strains isolated from different tissues of 3 AIDS patients.

BACKGROUNDTo reveal the characteristics of genotype and phenotype of HIV strains in blood and some tissues of AIDS patients.
METHODSThe virus was isolated from peripheral blood mononuclear cell (PBMC),cerebrospinal fluid (CSF)and lymph nodes of 3 AIDS patients by coculture with PBMC stimulated by PHA for 72 hours from uninfected donor. The cytopathic effect of the HIV isolates was determined in cultured MT2 cell line. The env gene sequences form proviral DNA were analyzed by GCG software.
RESULTSIn one patient,there were differences between the strains from blood and different tissues both in genotype and phenotype. The biological phenotypes of two strains from CSF were non syncytium (NSI) type, their env sequences were similar to standard CNS tropic strain (SF162).
CONCLUSIONSThe viral heterogeneity exists in different body compartments within an infected individual. The neurotropic isolate which is similar to international standard strain exists in some AIDS patients in China.
