1.Pulmonary infection caused by Nocardia otitidiscaviarum a case report and literature review

Abstract: Objective To investigate the clinical and laboratory characteristics of pulmonary infection caused by Nocardia otitidiscaviarum. Methods The clinical data of a patient with pulmonary infection caused by Nocardia otitidiscaviarum were reported, and the clinical characteristics, laboratory characteristics and drug sensitivity of pulmonary infection caused by Nocardia otitidiscaviarum were summarized in combination with the relevant literature at home and abroad from January 2010 to December 2022. Results A 67-year-old female patient was admitted to the hospital on June 30, 2020 because of "repeated chest tightness and shortness of breath for 3 years, aggravated cough, expectoration and fever". The sputum, alveolar lavage fluid and blood of the patient were collected for culture, and the detected pathogenic bacteria were identified. There are pathogenic bacteria growing in sputum and alveolar lavage fluid, which are identified as Nocardia otitidiscaviarum by Autof ms mass spectrometer. According to the results of pathogenic bacteria and the patient's condition, meropenem combined with compound sulfamethoxazole tablets were given anti-infection treatment, and the patient's condition improved and discharged. Conclusion The clinical manifestations and imaging features of nocardiosis are lack of specificity, and are prone to misdiagnosis and missed diagnosis. Etiology is the key to disease diagnosis, and clinical examination and culture should be conducted in time.
2.Short and long-term efficacy of focused ultrasound therapy for vulva dystrophy

3.Effect of evidence-based nursing in preoperative interview for abdominal surgery patients' recovery after general anesthesia

Objective To explore the effect of evidence-based nursing during preoperative interview for abdominal surgery patients' recovery after general anesthesia. Methods 124 patients who accepted selective operation on abdominal region were randomly divided into evidence-based group and control group, 62 cases ineach group. On the base of routine method in the preoperative interview, the evidence-based group established evidence-based information supporting system, which aimed to dredge the psychological stress of the patients'response to surgery individually and intervene by demonstrating simulation of operation process. According to the traditional way, control group accepted routine preoperative interview. To compare patients' indexes of two groups before and after anesthesia. Results Patients' heart rate and the mean arterial pressure lower than control group before anesthesia in evidence-based group. The dosage of midazolam in evidence-based group was less than the control group (t =6. 73,7. 24,8. 17;P <0. 05). The difference of pre-anesthesia anxiety scores was statistically significant between evidence-based group and control group (t =7. 68 ,P <0. 01 ). But the difference of the anxiety scores after operation wasn' t statistically significant between two groups ( t = 0. 62, P > 0. 05 ). Patients in evidence-based group took shorter revival time than control group ( t = 9. 71,10. 45,11.32 ; P < 0. 01 ). Compared the different period pain score of two groups, patients of evidence-based group were lower than those of the control group, and the patients' satisfaction scores also were higher than those of control group, the differences were statistically significant (t =2. 38,5. 22,2. 63,7. 62;P <0. 05). Conclusions Application evidence-based nursing information support in preoperative interview effectively relieves the preoperative anxiety, can reduce the dosage of narcotic and sedative drugs, shorten the revival time, improve patient' s satisfaction and have reached the antieipant effects.
4.The application and effects of BPR in intravenous infusion center for out patients

Objective To explore the application and effects of business process reengineering (BPR) in intravenous infusion center.Methods To analyze and compare retrospectively the effects before BPR with those after BPR for 240 cases.Results Infusion volume per day before BPR was (323.0±8.0) person,and (452.0 ±7.0) person after BPR,and the difference was significant (t =47.000,P<0.01).Degree of satisfaction was 92.08% before BPR and 98.75% after BPR,the difference was statistically significant ( x2 =12.195,P<0.05).Conclusions After nursing business reengineered in intravenous infusion center,nursing efficiency rates were increased and nursing defect rates,infection rates in hospital and nursing cost were significantly decreased,its worthy of spreading in the clinic.
5.Small interfering RNA targeting C-erbB-2 gene increases the radiosensitivity of lung adenocarcinoma cells in vitro.

OBJECTIVETo investigate the effect of small interfering RNA (siRNA) targeting C-erbb-2 oncogene on the radiosensitivity of C-erbb-2-overexpressing lung adenocarcinoma cell line.
METHODSFour pairs of siRNA targeting the coding sequence of C-erbb-2 mRNA were synthesized and their interference effects were evaluated using quantitative real-time fluorescence PCR. The siRNA with the best interference effects was transfected into Calu-3 cells, which were then exposed to 2 or 5 Gy irradiation, with the cells with transfection or irradiation alone as the control groups. The cell apoptosis after the treatment was detected using annexin V-FITC Kit.
RESULTSThe apoptosis rate of the Calu cells was 7.767-/+0.551 in the blank group, 14.400-/+1.114 in the interference group, 11.867-/+0.737 in 2 Gy irradition group, 23.000-/+1.664 in 2 Gy irradiation + interference group, 16.100-/+0.624 in 5 Gy irradiation group, and 27.900-/+1.709 in 5 Gy irradiation+interference group.
CONCLUSIONThe siRNA targeting C-erbb-2 gene can enhance the radiosensitivity of Calu-3 cells to gamma-ray and increase their apoptosis rate following gamma-ray exposure in vitro.

7.Thyroid Hormone Reference Intervals among Healthy Individuals In Lanzhou, China

Background:
The common reference intervals (RIs) for thyroid hormones currently used in China are provided by equipment manufacturers. This study aimed to establish thyroid hormone RIs in the population of Lanzhou, a city in the subplateau region of northwest China, and compare them with previous reports and manufacturer-provided values.
Methods:
In total, 3,123 individuals (1,680 men, 1,443 women) from Lanzhou, an iodine-adequate area of China, perceived as healthy were selected. The Abbott Architect analyzer was used to determine the serum concentration of thyroid hormones. The 95% RI was estimated using the 2.5th and 97.5th percentiles as the lower and upper reference limits, respectively.
Results:
The serum levels of thyroid-stimulating hormone (TSH), total triiodothyronine (TT3), antithyroglobulin (ATG) antibody, and antithyroid peroxidase (ATPO) antibody levels were significantly correlated with sex (P<0.05). TSH, total thyroxine (TT4), and ATPO levels were significantly correlated with age (P<0.05). The serum levels of TSH, ATG, and ATPO in men were significantly lower than in women; in contrast, the serum TT3 level was significantly higher in men than in women (P<0.05). Serum TSH, TT3, TT4, and ATG levels differed across age groups (P<0.05), but no such variation was observed for ATG levels (P>0.05). The established RIs of TSH, ATG, and ATPO in this study differed between sexes (P<0.05). The thyroid hormone RIs established herein were inconsistent with the manufacturer-provided values.
Conclusion
The RIs of thyroid hormones in the healthy population of Lanzhou were inconsistent with those in the manufacturer’s manual. Validated sex-specific values are required for diagnosing thyroid diseases.
8.Effect of continued nursing support on the quality of life of postoperative patients with gynecologic malignancies

Objective To evaluate the effect of continued nursing support on the quality of life of postoperative patients with gynecologic malignancies.Methods A total of 125 patients with gynecologic malignancies were randomly divided into two groups according to random number method:63 patients in the intervention group and 62 patients in the control group.All patients received the same preoperative routine nursing following doctors' orders.At the same time,the patients in intervention group received the continued nursing intervention of rehabilitation after discharge.Intervention effects were observed in two groups by using Self-rating Anxiety Scale (SAS) and Trait Coping Style Questionnaire (TCSQ).Results There were not statistically significant difference between the two groups of patients in the scores of anxiety,positive response and negative response before intervention (P > 0.05).The scores of anxiety,positive response and negative response after intervention in the intervention group were (23.52 ±4.15),(47.85 ± 3.52) and (18.15 ± 3.18),which were (38.92 ± 4.22),(33.59 ± 3.57) and (25.70 ± 3.25) in the control group,respectively,and these difference were statistically significant (t =20.570,22.486,13.127,respectively; P <0.01).The postoperative recurrence rate in patients in the intervention group was significantly lower than in the control group,1 patient with recurrence in the intervention group and 7 patients in the control group,and the difference was statistically significant (x2 =3.425,P < 0.05).There were statistically significant difference between the two groups of patients in the life quality score at 6 months after operation and the treatment adherence score at one year after operation (P < 0.01).Conclusions The implementation of continued nursing support can promote early recovery of physical function in patients,reduce the recurrence rate,improve the quality of life of postoperative patients,and increase the effect of surgical treatment,which should be worthy of clinical promotion and application.
9.The effect of improved inhalation therapy as adjuvant treatment for infants with severe pneumonia

Objective To investigate the effect of improved therapy-trace intermittent inhalation therapy as the adjuvant treatment for infants and children with severe pneumonia. Methods A total of 148 cases of infants and children with severe pneumonia were randomly divided into test group and control group, each group contained 74 patients. We used trace-intermittent inhalation therapy for the test group and selected the appropriate inhalation time and intermittent time according to children' s age and the degree of choke gasp. In the control group, we used the traditional continuous inhalation method. Breathing, heart rate, oxygen saturation, indicators of blood gas analysis and the extent of choke before and after inhalation were compared. Results The two group showed no differences in breathing, heart rate, oxygen saturation, indicators of blood gas analysis and the degree of choke before inhalation (P > 0. 05) , while significant difference were showed after inhalation (P < 0. 01), and the difference of the choke degree was statistically significant (P < 0. 05). This study showed that the trace-intermittent inhalation treatment was significantly superior to the traditional continuous inhalation treatment (P <0. 01). Conclusions Trace intermittent inhalation therapy can promote the effective oxygen supply of the organization, improve ventilation and oxygenation function and blood gas results in children, relieve the different degrees of choke gasp, shorten the course of disease and improve the therapeutic effect significantly.
10.Effect on breastfeeding by early position intervention in women after cesarean section

Objective To explore the effect on breastfeeding by early position intervention in women after cesarean section. Methods 126 mothers after cesarean section under epidural anesthesia were randomly divided into two groups, including 64 cases and 62 cases. Mothers in two groups were given regular health education,breast-feeding early, sucking early. The mothers in observer group were instructed breastfeeding in different time with position intervention. The mothers in control group were used traditional methods. Results There were significant difference between the two groups in postoperative pain blade, immobility time in early lactation,breastfeeding success rates, feeling after operation, the baby with access position, defecation, enough time of feeding up(P < 0. 05). Conclusions Early position intervention after cesarean section can increase the success rate of breastfeeding and the health of the mothers and children.
