4.Surgical treatment for displaced clavicle fracture combined with coracoid process: 9 cases report.

OBJECTIVETo explore clinical effects of internal fixation in treating displaced clavicle fracture combined with coracoid process.
METHODSFrom January 2005 to July 2012, 9 patients with displaced clavicle fracture combined with coracoid process were treated by internal fixation. Among them, there were 6 males and 3 females with an average age of 40.1 (ranged from 20 to 57) years old. According to Eyres classification: 3 cases were type II B, 1 case was type II A, 3 cases were type III B, and 2 cases were type V A. All patients had history of injury, and diagnosed as coracoid fracture X-ray and CT before operation. Herscovici criteria was used to evaluate function of shoulders joint after operation.
RESULTSSeven of 9 patients were followed up from 6 to 18 (averaged 11) months. The incisions were healed at stage I, coracoid process obtained bony healing, and reduction of acromioclavicular joint well. According to Herscovici criteria, 6 patients got excellent results and 1 in good.
CONCLUSIONInternal fixation for the treatment of displaced clavicle fracture combined with coracoid process could restore physiological anatomical position of coracoid process, and benefit for recovery of limb function.

5.Research achievements on biological activities of calycosin.

Calycosin, which is a kind of typical phytoestrogen, can bind with estrogen receptor and produce estrogen-like effects. Calycosin were reported to have antioxidant, anti-osteoporosis, anti-tumor and immunomodulating activities. This review covers biological activities and its mechanism of calycosin. It will provide a useful reference for clinical research and rational utilization of monomericompound.

6.Clinical analysis of 23 patients with Brucellar epididymo-orchitis

Objective Study the epidemiological and clinical findings of patients with Brucellar epididymo-orchitis (BEO),to provide a theoretical basis for clinical diagnosis.Methods A study was carried out to analyze the patients' files of epidemiological,outcome,clinical and laboratory features of 23 cases of BEO and 185 in patients with brucellosis in Hulunbuir People's Hospital during 2011-2013.Results The age of patients with BEO and without epididymo-orchitis (EO) were (34.48 ± 10.32) years old and (42.07 ± 12.19) years old,the difference was statistically significant (t =-2.842,P < 0.05).Patients with BEO and without EO of brucellosis patients with hypogastralgia ratio was 78.3% (18/23) and 8.6% (14/162),the difference was statistically significant (x2 =68.238,P < 0.05).The proportion of BEO cases with abnormal White Blood of Cell (WBC) and C-reactive protein (CRP) levels were 34.8% (8/23) and 91.3% (21/23),which was significantly higher than those in without EO group [9.9% (16/162),63.0% (102/162),x2 =11.066,7.260,all P < 0.05].After 2 to 7 days,BEO patients with scrotal swelling and pain and/or tenderness relieved.After two weeks treatment,11 patients of the urinary system showed normal by ultrasound,and no patients underwent testicular resection.Conclusions Young male patients with Brucellar are prone to BEO.Laboratory abnormalities are mainly leukocytosis and high CRP level.
7.Clinical features of 38 children with brucellosis

Objective To analyze clinical and laboratory results of 38 children with brucellosis for providing theoretical basis for its clinical diagnosis and treatment. Methods The epidemiology, clinical features, laboratory characteristics and therapeutic effect of pediatric patients at HulunBuir People's Hospital from January 2011 to December 2015, were retrospectively evaluated. Results Totally there were 38 child patients with brucellosis, 24 males and 14 females, 36 children had directly contacted with animals, and onset seasons were mainly in Summer and Autumn. The dominant manifestations were fever in 33 (86.8%), anorexia in 21 (55.3%), arthralgiaand myalgia in 17 (44.7%), hepatomegaly in 5 (13.2%), splenomegaly in 11 (28.9%), and lymph nodes enlargement in 10 (26.3%). Abnormal laboratory findings included elevated C-reactive protein (CRP) level in 27 (71.1%) and erythrocyte sedimentation rates (ESR) in 24 (63.2%). Both the Rose Bengal test and serum agglutination test (SAT) were positive in 37 patients, only one patient was negative but the blood culture was positive. After at least 12 weeks of combined, adequate and standardized treatment, there were 30 pediatric brucellosis patients cured, improved in 8 cases, the total effective rate was 100.0%. Conclusions In endemic regions, the presence of persistent fever, joint pain and hepatosplenomegaly should alert clinicians the possibility of brucellosis. Early diagnosis and treatment will reduce the incidence of complications.
8.Research on the mechanism of fluoroquinolone resistance in clinical isolates of Enterococcus faecium

Objective To investigate the mechanism of fluoroquinolone resistance in clinical isolates of Enterococcus faecium. Methods The MICs of six fluoroquinolones(norfloxacin,ciprofloxacin,ofloxacin,levofloxacin,gatifloxacin and moxifloxacin) against 35 clinical isolates of E.faecium from eight hospitals in Tianjin were determined by agar dilution method in the absence or presence of multidrug resistance efflux pump inhibitor reserpine.The quinolone-resistance determining region(QRDR)of parC and gyrA were amplified and sequenced.Results No less than twofold decrease in MIC values of the six fluoroquinolones in the presence of reserpine was observed in 35,29,1,0,6 and 2 of the 35 strains of E.faecium respectively.One fluoro- quinolone-susceptible isolate and five fluoroquinolone-resistant isolates were selected randomly to analyze the QRDR of parC and gyrA.All five fluoroquinolone-resistant isolates had single amino acid alteration in both GyrA and ParC.Ser-80 in ParC was substituted by lie(4 isolates)or Arg(1 isolates).Glu-87 in GyrA was replaced by Lys(2 isolates)or Gly(2 isolates). The other one had an Ser-83-to-Ile substitution.The one fluoroquinolone-suseeptible isolate had no alteration in the QRDR of either ParC or GyrA.Conclusions Both target alteration and active efflux are responsible for the resistance to fluoroquinolone in clinical isolates of E.faecium.
9.Analysis for surgical treatment of hepatolithiasis and the efficacy

Objective To study the relationship between the choice of operation and the efficacy on hepa- tolithiasis.Methods From Januray of 1995 to December of 2006,89 patients with hepatolithiasis underwent surgical treatment were retrospectively analyzed.Of them 33 cases underwent hepaticoplasty,hepatolobectomy in 7 cases, cholangiojejunostomy in 22 cases,choledocholithotomy with T-tube drainage in 27 cases.Results Out of the 89 cas- es,follow-up was completed in 81 cases for 6 months to 12 years.The postoperative stone residual rate of the group which underwent hepaticoplasty was 15.15 %(5/33)and cholannitis recurrence rate was 12.50 %(4/32),hepa- tolobecromy was 14.29%(1/7)and 16.67%(1/6),cholangiojejunostomy was 18.18%(4/22)and 30%(6/20), choledocholithotomy with T-tube drainage was 33.33 %(9/27)and 29.17 %(7/24).Conclusion Hepaticoplasty and hepatolobecromy were superior to cholangiojejunostomy and choledocholithotomy with T-tube drainage for treat- ment of hepatolithiasis.
10.Effects of metformin on the levels of serum retinol-binding protein 4 and inflammatory factors in patients with impaired glucose tolerance and metabolic syndrome

ObjectiveTo investigate the effects of mefformin on the levels of serum retinol-binding protein 4 (RBP-4),high sensitive C reactive protein (hs-CRP) and tumor necrosis factorα (TNF-α) in patients with impaired glucose tolerance(IGT) and metabolic syndrome(MS). MethodsSixty patients with IGT and MS were divided into mefformin treatment group (30 cases) and life-style intervention group (30 cases) by random digits table. Body mass index (BMI),the levels of HbAic, HOMA-IR, blood fat, RBP-4,hs-CRP, TNF- α were measured both before and 16 weeks after treatment in the two groups and compared.ResultsThe levels of HbA1c, HOMA-IR, RBP-4, hs-CRP and TNF- α were significantly lower in mefformin treatment group than those in life-style intervention group [(5.09 + 0.26 )% vs. (5.69 ± 0.49 )%, 2.95 ± 0.63vs. 3.49 ± 0.78, ( 18.69 ± 6.50) mg/L vs.(26.20 ± 6.97) mg/L, (2.37 ± 0.53) mg/L vs.(2.99 ± 0.57) mg/L,(9.49 ± 2.37) μ g/L vs. ( 14.33 ± 2.62) μ g/L] (P < 0.01 ). The results of multiple linear regression showed that correlations were found between the changes of RBP-4 and BMI,HOMA-IR,hs-CRP,TNF-α(β =0.284,0.506,0.274,0.230,P <0.01 ),and HOMA-IR was the most important limiting factor. Conclusions Mefformin can improve insulin sensitivity of the patients with IGT and MS, and depress the levels of RBP-4,hs-CRP and TNF- α. Meanwhile mefformin has anti-inflammatory effect.
